PURPOSE:
The protease inhibitor bortezomib attenuates the action of NF-κB and has shown preclinical activity alone and in combination with chemotherapy.

DESIGN:
A Phase I dose-escalation study was performed administering bortezomib (0.7, 1.0, 1.3 and 1.6 mg m(-2) on days 1 and 8 from cycle 2 onwards) in combination with Epirubicin 50 mg m(-2) intravenously on day 1, Carboplatin AUC 5 day 1 and Capecitabine 625 mg m(-2) BD days 1-21 every 21 days (VECarboX regimen), in patients with advanced oesophagogastric adenocarcinoma. The primary objective was to define the maximum tolerated dose (MTD) of Bortezomib when combined with ECarboX.

RESULTS:
18 patients received bortezomib 0.7 (n = 6), 1.0 (n = 3), 1.3 (n = 6) and 1.6 mg m(-2) (n = 3) and a protocol amendment reducing the capecitabine dose to 500 mg m(-2) BD was enacted due to myelotoxicity. Common treatment-related non-haematological adverse events of any grade were fatigue (83.3 %), anorexia (55.6 %), constipation (55.6 %) and nausea (55.6 %). Common Grade 3/4 haematological toxicities were neutropenia (77.8 %) and thrombocytopenia (44.4 %). Objective responses were achieved in 6 patients (33.3 %) and a further 5 patients (27.8 %) had stable disease for >8 weeks.

CONCLUSIONS:
The addition of Bortezomib to ECarboX is well tolerated and response rates are comparable with standard chemotherapy.

Using a prism-air gap-sample (Otto) configuration we have optically excited surface plasmon polaritons at the Ag-air interface of passive Al-Al oxide-Ag tunnel junction structures at wavelength 632.8 nm. It is found that the internal damping of this excitation is more than a factor of 2 greater for samples with a very thin (approximately 15 nm) Ag electrode than for samples with a thicker (approximately 40 nm) Ag electrode. This observation is explained by the fact that the fields of the surface plasmon polariton penetrate more substantially into the lossy Al base electrode when the Ag top electrode is very thin.

On présente dans ce mémoire la mesure de la statistique de photons des fluctuations électromagnétiques micro-ondes d’une jonction tunnel placée à ultra-basse température. En particulier, on déduit cettedite statistique en mesurant les cumulants des fluctuations de courant générées par une jonction tunnel polarisée en courant continu et photoexcitée par un signal haute fréquence. On utilise un amplificateur paramétrique Josephson en tant que premier maillon de la chaîne d’amplification en raison de son bruit près de la limite quantique. On développe aussi un modèle pour tenir compte de sa saturation, celle-ci étant apparente à des puissances de signal incident très faibles. C’est, à ma connaissance, la première fois qu’un tel amplificateur est utilisé pour étudier la physique au sein du bruit émis par un conducteur mésoscopique. Les résultats mettent en évidence l’apparition d’un excès dans le quatrième cumulant lorsque la photoexcitation s’effectue au double de la fréquence de mesure. Un développement mathématique simple nous permet d’associer cet excès à la statistique de photons, sous la forme d’une augmentation – à nombre de photons fixe – de la variance du nombre moyen de photons contenue dans le signal. C’est en fait une signature de l’émission de paires de photons corrélés par la jonction tunnel photoexcitée. Ces résultats font le pont entre ceux obtenus précédemment par notre groupe de recherche quant aux cumulants d’ordre supérieur du bruit et ceux associées aux parallèles entre l’optique quantique et les fluctuations de courant des dispositifs mésoscopiques. PRL 113, 043602 (2014)] Ils dressent un portait intéressant du bruit photoexcité en termes normalement associés à l’optique quantique.

OBJECTIVES: To investigate the development of the ureterovesical junction in rats. METHODS: A total of 110 albino rats (50 prenatal and 60 newborn) with a gestation of 21 days were studied at the age of 17 days after conception until 5 days after birth. The lower urinary tract was microdissected. Microphotography (110 animals), histologic examination (44 animals), and scanning electron microscopy (66 animals) of the ureterovesical junction were performed. Urea and creatinine from the amniotic fluid of 20 fetuses and from the urine of 10 neonates were measured. RESULTS: At day 17 after conception, separate penetration of the mesonephric duct and ureter into the wall of the urogenital sinus was observed. Continuity between the lumen of the ureter and the urogenital sinus was established on day 19 after conception. The straight passage of the intramural ureter into the urogenital sinus at day 17 after conception changed to the definitive L-shape with a vertical entry into the bladder on day 5 after birth. In the distal ureter, the change of the mesenchymal tissue into immature smooth muscle was first observed at birth, and the muscle became mature on the fifth postnatal day. At birth, Waldeyer's sheath was recognized. The creatinine and urea levels were stable prenatally (average 22.4 micromol/L and 6.88 mmol/L, respectively) and rose significantly postnatally (average 133 micromol/L and 32.65 mmol/L, respectively). CONCLUSIONS: The attachment of the ureter to the urogenital sinus and later to the bladder, the modification of its passage, and its mobility within Waldeyer's sheath may be essential in preventing vesicoureteral reflux. The production of urine and its flow does not seem to be the trigger of ureteral smooth muscle formation.
