Objectif : Évaluer les effets sur la croissance des maxillaires, ainsi que les effets dentaires, de l'utilisation du Forsus versus celle des élastiques de classe II. Matériel et méthode : Un échantillon de 30 patients a été traité sans extraction et a eu une phase d'alignement préliminaire avec appareillage orthodontique fixe complet. Vingt-trois (23) patients (14 filles, 9 garçons) ont été traités à l'aide de Forsus (âge moyen : 13,4 ans) et 7 patients (4 filles, 3 garçons) ont été traités avec des élastiques de classe II (âge moyen : 14,3 ans). Une radiographie céphalométrique a été prise à la fin de la phase d'alignement, juste avant la mise en place de l'appareil myofonctionnel choisi, et une autre au moment de l'enlèvement de l'appareil (temps de port moyen : 0,5 an). Les radiographies ont ensuite été tracées à l'aveugle et 17 mesures ont été sélectionnées pour évaluer l'effet des appareils sur les maxillaires (ANS-PNS, SNA, SNB, ANB, Go-Pg, Ar-Go, Co-Gn, axe Y, Ar-Gn, Ar-Go-Me, FMA, POF/FH, PP/FH, B-Pg(PM), 1/-FH, 1/-/1, /1-PM). Un questionnaire pour évaluer le confort face à leur appareil a été remis aux patients à environ la moitié du temps de port estimé. Résultats : Il n'y a aucune différence statistiquement significative entre les deux traitements sur la croissance du maxillaire (ANS-PNS p = 0,93, SNA p = 0,12). De façon générale, il n'y a pas non plus de différence significative entre les deux traitements sur la croissance de la mandibule (Ar-Gn p = 0,03, SNB p = 0,02 et pour les 6 autres mesures p > 0,05). Pour la composante dento-alvéolaire, les deux traitements proclinent les incisives inférieures et rétroclinent les incisives supérieures, le Forsus causant une plus forte rétroclinaison des incisives supérieures (1/-FH p = 0,007, /1-PM p = 0,10). Pour les changements angulaires des plans, le Forsus cause de manière significative une augmentation de l’inclinaison du plan occlusal (POF/FH p = 0,001). Pour le questionnaire sur l'évaluation du confort, il n'y a pas de différence entre les deux traitements en ce qui concerne la gêne face aux activités quotidiennes (p = 0,19). L'hygiène est plus facile avec les élastiques (p = 0,03). Le sommeil n’est perturbé par aucun des appareils (p =0,76). La différence entre le groupe «élastiques» et le groupe «Forsus» pour le confort en général n'est pas significative (p = 0,08). Conclusions : Le but de l’étude étant de vérifier l’efficacité des élastiques de classe II bien portés versus celle des Forsus, on peut conclure que leurs effets sont relativement similaires sur les maxillaires. Cependant, le Forsus cause de manière statistiquement significative une augmentation de l'angle du plan occlusal et une rétroclinaison plus importante des incisives supérieures.

Introduction : La croissance maxillo-mandibulaire des enfants avec une séquence de Pierre Robin (SPR) est controversée dans la littérature. Certains auteurs croient que la croissance mandibulaire est accélérée après la naissance, mais peu se sont penchés sur la croissance du maxillaire supérieur. Cette étude rétrospective sur dossier vise à analyser la croissance maxillo-mandibulaire des enfants atteints de la SPR. Dans un deuxième temps, nous aurions aimé évaluer la sévérité et l’évolution de l’apnée du sommeil en lien avec la croissance des maxillaires, mais un manque de données a empêché l’atteinte de cet objectif. Matériel et méthode : Les dossiers médicaux et orthodontiques de 93 patients (82 volet apnée et 40 volet croissance) du CHU Ste-Justine avec une SPR isolée ont été révisés puis comparés au groupe contrôle composé d’enfants normaux de l’Université du Michigan. L’analyse statistique de modèle mixte pour mesures répétées de même que celle de Brunner-Langer furent effectuées. Résultats : L’évaluation orthodontique a montré un changement statistiquement significatif pour la relation molaire droite, la présence de chevauchement et de diastème au maxillaire et le surplomb vertical. L’analyse des données céphalométriques nous montre que le maxillaire supérieur, la branche montante et le corps de la mandibule sont tous réduits par rapport à la normale. Ce dernier montre une diminution significative avec l’âge (p = 0,03). L’angle gonial, le SNA, SNB, ANB, l’angle de convexité faciale et l’inclinaison de l’incisive supérieure par rapport à FH sont tous normaux. Par contre, on remarque une augmentation statistiquement significative de cette dernière avec l’âge (p = 0,04). L’angle Y est augmenté tandis que les hauteurs faciales supérieure (HFS) et inférieure (HFI) sont diminuées bien que cette dernière montre une tendance à s’approcher de la normale avec l’âge (p ≤ 0,001). Discussion : Les dimensions des maxillaires sont similaires à plusieurs études. En ce qui concerne la mandibule, la croissance est soit plus lente, soit diminuée. Cette observation est plus marquée lorsque l’on s’approche du pic de croissance puisque l’écart par rapport à la normale s’agrandit. On voit une tendance à la croissance hyperdivergente qui pourrait expliquer l’augmentation de la HFI avec l’âge. Le fait que SNA et SNB soient dans la normale pourrait s’expliquer par une diminution de la longueur de la base crânienne. Conclusion : Il n’y a pas de rattrapage de croissance maxillaire et mandibulaire. Les maxillaires restent micrognathes quoique proportionnels l’un envers l’autre et le profil est convexe tout au long de la croissance. La comparaison des données céphalométriques et des traitements orthodontiques avec ceux des patients présentant une fente palatine isolée devrait se faire sous peu. Nous n’avons pas été en mesure d’atteindre nos objectifs concernant l’apnée du sommeil. Une étude prospective serait à prévoir pour y arriver.

Aim To evaluate ex vivo the accuracy of two electronic apex locators during root canal length determination in primary incisor and molar teeth with different stages of physiological root resorption. Methodology One calibrated examiner determined the root canal length in 17 primary incisors and 16 primary molars (total of 57 root canals) with different stages of root resorption based on the actual canal length and using two electronic apex locators. Root canal length was measured both visually, with the placement of a K-file 1 mm short of the apical foramen or the apical resorption bevel, and electronically using two electronic apex locators (Root ZX II - J. Morita Corp. and Mini Apex Locator - SybronEndo) according to the manufacturers` instructions. Data were analysed statistically using the intraclass correlation (ICC) test. Results Comparison of the actual root canal length and the electronic root canal length measurements revealed high correlation (ICC = 0.99), regardless of the tooth type (single-rooted and multi-rooted teeth) or the presence/absence of physiological root resorption. Conclusions Root ZX II and Mini Apex Locator proved useful and accurate for apex foramen location during root canal length measurement in primary incisors and molars.

The current study evaluated the influence of two endodontic post systems and the elastic modulus and film thickness of resin cement on stress distribution in a maxillary central incisor (MCI) restored with direct resin composite using finite element analysis (FEA). A three-dimensional model of an MCI with a coronary fracture and supporting structures was performed. A static chewing pressure of 2.16 N/mm(2) was applied to two areas on the palatal surface of the composite restoration. Zirconia ceramic (ZC) and glass fiber (GF) posts were considered. The stress distribution was analyzed in the post, dentin and cement layer when ZC and GF posts were fixed to the root canals using resin cements of different elastic moduli (7.0 and 18.6 GPa) and different layer thicknesses (70 and 200 mu m). The different post materials presented a significant influence on stress distribution with lesser stress concentration when using the GF post. The higher elastic modulus cement created higher stress levels within itself. The cement thicknesses did not present significant changes.

Here we report on the clinical and genetic data for a large sample of Brazilian patients studied at the Hospital de Reabilitacao de Anomalas Craniofaciais-Universidade de Sao Paulo (HRAC-USP) who presented with either the classic holoprosencephaly or the holoprosencephaly-like (HPE-L) phenotype. The sample included patients without detected mutations in some HPE determinant genes such as SHH, GLI2, SIX3, TGIF, and PTCH, as well as the photographic documentation of the previously reported patients in our Center. The HPE-L phenotype has been also called of HPE ``minor forms"" or ""microforms,"" The variable phenotype, the challenge of genetic counseling, and the similarities to patients with isolated cleft lip/palate are discussed. Objective: To radiographically evaluate the prevalence of tooth abnormalities of number and position in the permanent dentition of individuals with complete bilateral cleft lip and palate. Design: Cross-sectional retrospective. Setting: Hospital for Rehabilitation of Craniofacial Anomalies, University of Sao Paulo, Bauru, Brazil. Patients: Two hundred five individuals with complete bilateral cleft lip and palate. Interventions: Analysis of patient records and panoramic radiographs. Main outcome measures: Evaluation of hypodontia and supernumerary teeth and analysis of the position of the permanent maxillary lateral incisor in relation to the alveolar cleft. Results: Hypodontia was observed in 144 patients (70.2%), and the highest prevalence was observed for the maxillary lateral incisor. When both lateral incisors were present (43%), they were primarily located on the distal side of the cleft (25%). Supernumerary teeth were observed in 11.7% of individuals. Conclusion: Patients with cleft lip and palate presented high prevalence of hypodontia and supernumerary teeth. The prevailing characteristics of their location may suggest the presence of a similar genetic component for the occurrence of hypodontia and cleft.

Objective: To evaluate the presence of enamel alterations in deciduous maxillary central incisors of infants with unilateral cleft lip and alveolar ridge, with or without cleft palate, and to compare the occurrence and location of these alterations between the central incisor adjacent to the cleft and the contralateral incisor. Design: Intraoral clinical examination was performed after tooth cleaning and drying by a single examiner with the aid of a dental mirror, dental probe, and artificial light, with the child positioned on a dental chair. The defects were recorded in a standardized manner according to the criteria of the Modified Developmental Defects of Enamel Index. Setting: Hospital for Rehabilitation of Craniofacial Anomalies (HRAC) at Bauru, Sao Paulo, Brazil. Patients: One hundred one infants were evaluated. All were white, of both genders, aged 12 to 36 months and had at least two thirds of the crowns of maxillary incisors erupted. Results: Demarcated opacity was the most common defect at both cleft and noncleft sides, followed by diffuse opacity. The occurrence of hypoplasia at the cleft side was 11.8%. Most defects affected less than one third of the crown. Conclusion: The occurrence of enamel defects in deciduous maxillary central incisors of patients with unilateral cleft lip was 42.6%, mainly affecting the cleft side as to both number and severity.

Morphogenesis and cytodifferentiation are distinct processes in tooth development. Cell proliferation predominates in morphogenesis; differentiation involves changes in form and gene expression. The cytoskeleton is essential for both processes, being regulated by Rho GTPases. The aim of this study was to verify the expression, distribution, and role of Rho GTPases in ameloblasts and odontoblasts during tooth development in correlation with actin and tubulin arrangements and amelogenin and dentin sialophosphoprotein (DSPP) expression. RhoA, Rac1, and Cdc42 were strongly expressed during morphogenesis; during cytodifferentiation, RhoA was present in ameloblasts and odontoblasts, Rac1 and its effector Pak3 were observed in ameloblasts; and Cdc42 was present in all cells of the tooth germ and mesenchyme. The expression of RhoA mRNA and its effectors RockI and RockII, Rac1 and Pak3, as analyzed by real-time polymerase chain reaction, increased after ameloblast and odontoblast differentiation, according to the mRNA expression of amelogenin and DSPP. The inhibition of all Rho GTPases by Clostridium difficile toxin A completely abolished amelogenin and DSPP expression in tooth germs cultured in anterior eye chamber, whereas the specific inhibition of the Rocks showed only a partial effect. Thus, both GTPases are important during tooth morphogenesis. During cytodifferentiation, Rho proteins are essential for the complete differentiation of ameloblasts and odontoblasts by regulating the expression of amelogenin and DSPP. RhoA and its effector RockI contribute to this role. A specific function for Rac1 in ameloblasts remains to be elucidated; its punctate distribution indicates its possible role in exocytosis/endocytosis.

The people of Ceará state are descended from miscegenation between the Portuguese colonizers and the native population, resulting in a different facial pattern from other populations. It is important that this pattern be thoroughly understood, along with its minimum and maximum values so that they can be assessed and respected, allowing professionals who deal with the craniofacial complex to work more efficiently and scientifically. Aim: To characterize the morphological pattern of individuals from Ceará state, whose father and grandfather are also native from Ceará, in the 10-12 year age group, not submitted to previous orthodontic treatment, in order to determine: 1) the prevalence of occlusal pattern; 2) the prevalence of dental anomalies (DA) and, 3) the skeletal and dental cephalometric characteristics of individuals that present with normal occlusion and harmonious facial pattern. Methodology: A list of 10-12 year-olds was obtained from 515 schools containing 162,713 students (Education Secretariat of Ceará State), from which 234 individuals were examined (107 boys and 157 girls). The assessment criteria adopted were: 1) Angle s Classification System to determine occlusal pattern. The occlusal characteristics were measured through overbite, overjet, crowding and interincisal diastema. 2) DA are anomalies of number, shape, size, eruption and structure and, 3) in the group that presented with normal occlusion, we used cephalometric analysis measures proposed by Downs, Steiner, Tweed, Holdaway, Jacobson and McNamara. Results: 1) 25.8% of the schoolchildren had normal occlusion, 47.5% class I malocclusion, 22.3% class II malocclusion and 4.2% class III malocclusion. No statistically significant difference was found between the age group studied and sex. Thirty percent of the individuals had normal overbite, while 36.7% and 19.7% had increased and reduced overbite, respectively. Normal overjet was found in 33.7% of the individuals, increased overjet in 50% and reduced in 16.3%. Dental crowding was observed in 62.5% of the individuals and the presence of interincisal diastema in 14.8%. 2) The prevalence of DA was 56.1%, 6.8% in the number, 10.8% in shape, 4.1% in size, 34.5% in eruption, 26.4% in structure and 17.4% had more than one DA. No association was found between DA and sex, but DA was significantly associated to malocclusion (p<0.05); 3) there was no association between sex or facial type between the measures of nasal-labial angle, position and effective maxillary length, effective mandibular length and the sagittal relationship between the molars, overjet and overbite, position of upper incisors, lower incisors and between the incisors themselves. There was a difference between sex, on the VERT index and in lower anterior facial height, upper incisor inclination and line-H, between facial types for the occlusal plane angles, mandibular plane, facial axis, lower incisor inclination, mandibular position, upper incisor position, lower anterior facial height, ANB and line-H. It was concluded that: 1) the most prevalent occlusal type was class I malocclusion, with no distinction for sex or age group, and the assessment of occlusal characteristics showed that excessive overbite and overjet were the most predominant findings, along with a high occurrence of tooth crowding; 2) a high prevalence of DA was found, particularly eruption anomalies, not influenced by sex but significantly associated to malocclusion and 3) individuals from Ceará are predominantly brachyfacial, exhibiting a number of similarities inherent to their facial pattern, such as a convex profile, retracted jaw, reduced lower third and protruded lower incisors. This study was multidisciplinary, involving researchers from the areas of epidemiology, radiology and dentistry, thereby meeting the multidisciplinarity requirements of the Postgraduate Program in Health Sciences

Ideally the smile should expose minimal gingival, therefore patients with gummy smile and passive eruption altered or excessive marginal gingivae, usually excessive gingival display because incomplete anatomical crown exposure is present. If the maxillary incisor show at rest is optimal, active upper incisor intrusion should not be iniciated. To achieve a smile with minimal gingival exposure, the anatomic crown should be fully exposed by surgical crown lengthening. Precise determination of the location of cementoenamel junction prior to surgery, precise placement of incisions and correct establish of biological width are necessary in order to achive this goal. The aim of this study was to comparatively evaluate the mechanical strength of squared and rectangular 2.0 mm system miniplates comparing them to the standard configuration with 2 straight miniplates in stabilizing fractures in the anterior mandible. Ninety synthetic polyurethane mandible replicas were used in mechanical test. The samples were divided into six groups of three different methods for fixation. Groups 1, 2 and 3 showed complete fractures in symphysis, characterized by a linear separation between the medial incisor, and groups 4, 5 and 6 showed complete fractures in parasymphysis with oblique design. Groups 1 and 4 were represented by the standard technique with two straight miniplates parallel to each other. Groups 2 and 5 were stabilized by squared miniplates and groups 3 and 6 were fixed by rectangular design. Each group was subjected to a mechanical test at a displacement speed of 10 mm/min on a universal testing machine, receiving linear vertical load on the region of the left first molar. The values of the maximum load and when displacements reached 5 mm were obtained and statistically analyzed by calculating the confidence interval of 95%. Fixation systems using squared (G2) and rectangular (G3) miniplates obtained similar results. No statistically significant differences with respect to the maximum load and the load at 5 mm displacement were found when compared to standard method in symphyseal fractures (G1). In parasymphysis the fixation method using squared miniplates (G5) obtained results without significant differences regarding the maximum load and the load at 5 mm when compared to the standard configuration (G4). The fixation method using rectangular miniplates (G6) showed inferior results which were statistically significant when compared to the standard configuration (G4) for parasymphysis fractures. The mechanical behavior of the fixation methods was similar, except when rectangular miniplates were used. The fixation methods showed better results with statistical significance in symphyseal fractures

Foram avaliadas as técnicas radiográficas dentárias intra (TIB) e extrabucal (TEB) em 50 cães com doença periodontal, no intuito de padronizar os procedimentos de diagnóstico dessa síndrome. A TIB revelou que 16 animais não apresentaram lesões ósseas visíveis, enquanto a TEB apontou que 39 pacientes foram negativos para as mesmas lesões. em resumo, a TIB foi mais eficaz na detecção de sinais radiográficos, especialmente as chamadas lesões finas, que a TEB, sendo a técnica de escolha na síndrome periodontal.

O desempenho produtivo e a possível interferência do flúor sobre a saúde dos animais foram investigados em bovinos Nelore suplementados, por 866 dias, com distintas fontes alternativas de fósforo com diferentes relações fósforo:fluor (P:F). Os tratamentos experimentais foram: Controle negativo (CONTNEG, sem qualquer suplementação com P), fosfato bicálcico (FB 120:1, FB 30:1 e FB 10:1), fosfato monobicálcico (FMBC 60:1), superfosfato triplo (SFT 30:1) e fosfato de rocha de Cajati (FR 10:1). Foram utilizados 49 novilhos, desmamados aos oito meses de idade, castrados e com 230 kg de peso médio, distribuídos em sete piquetes com água e mistura mineral formulada sem P. A dieta padrão foi feita com bagaço de cana (0,03% de P) como volumoso e um concentrado contendo 0,239 % de P oferecido na base de 1% do peso dos animais para permitir um ganho de peso aproximado de 0,50 kg/dia. Até o dia 134, não houve diferença estatística entre os diversos lotes, inclusive para o tratamento CONTNEG, que não recebeu fósforo suplementar na dieta e ganhou 71,6 kg de peso ou 0,633 kg/dia. Após 866 dias de confinamento (2,37 anos), os animais suplementados com o fosfato bicálcico padrão (120:1) ganharam menos peso que os suplementados com as fontes FMCB 60:1, FB 30:1 e SFT 30:1. Até um ano de suplementação fosfórica com fosfato bicálcico padrão (120:1) artificialmente fluoretado com NaF ou com o fosfato de rocha não se detectou danos à saúde ou ao ganho de peso dos animais. As análises de fósforo nos ossos mostraram diferença estatística apenas entre o tratamento CONTNEG e os que tinham fosfato bicálcico. As concentrações de flúor nos ossos se mostraram intimamente associadas à quantidade de flúor disponível nas fontes utilizadas. Conforme a proporção P:F na dieta foi diminuindo, características relacionadas à fluorose dentária ficaram mais evidentes, sendo que os animais que receberam fontes com relação 10:1, apresentaram, ao final do experimento, dentes incisivos permanentes mal formados, quebradiços e com manchas esbranquiçadas.
