A cross observational study was achieved about hypertensive patients from the Basic Health Unit in Betel, Paulinia - S.P. assessing blood pressure levels, anthropometric data, biosocial variables, responses to questionaires about physical activity (Ipaq), and quality of life (Whoqol), as well as estimates of cardiovascular risk by Framingham score. With descriptive purposes, absolute and relative frequencies distributions are presented. From originally 95 people in the study, 18 were eliminated, and the remaining 77 were predominately composed by women, averaged 55.87 years old (standard deviation 11.88); white ethnic; basic schooling education, and income of 3 minimum salaries. Although clinical criteria verified altered values, it was the physical evaluation that indicated more clearly risks of contracting cardiovascular diseases, thus showing how different complementary evaluations can better predict risk. These results demonstrated that other professionals should be an asset as an integral part of the health team. © Copyright Moreira Jr. Editora. Todos os direitos reservados.

Physical inactivity is associated with chronic diseases in older adults. There are different domains of physical activity, such as leisure and locomotion. This study investigate the associations between different domains of physical activity, comorbidities and aging in hypertensive adults. Participants included 192 hypertensive respondents classified in groups of active and insufficiently active for leisure-time, locomotion, occupation and total time. They were stratified in age groups. Waist circumference was measured and comorbidities were self-reported. The chi-square test analyzed these associations; a one-way ANOVA compared the age groups (significance at p <0.05). The mean total time and occupational physical activity were lower for groups ages over 60 and 70 years, respectively. Occupational activity was associated with age in the female group, and with cholesterol and diabetes in males, who also had physical activity of locomotion associated with waist circumference. Physical activity of hypertensive patients is associated with comorbidities in different age groups and sex.

OBJETIVO: Avaliar o comportamento do potencial oscilatório escotópico do eletrorretinograma de campo total (ERG) na retinopatia hipertensiva. MÉTODOS: Quarenta e quatro pacientes foram submetidos à avaliação clínica e subdivididos em dois grupos: 26 hipertensos (HT) com média de idade de 52,23 ± 5,79 anos divididos em 10 homens (38,46%) e 16 mulheres (61,54%) e 18 normotensos (NT) com média de idade de 51,79 ± 10,23 anos divididos em 5 homens (27,78%) e 13 mulheres (72,22%). Foram incluídos no estudo apenas hipertensos leves a moderados (estágio 1 e 2 respectivamente) sem lesões em outro órgão-alvo.Os pacientes hipertensos foram mantidos sob placebo durante o período do estudo. Em seguida, foram submetidos à avaliação oftalmológica e realização do ERG. O eletrorretinograma de campo total (ERG), com registro das respostas: escotópica, escotópica máxima, PO escotópico, fotópica e "flicker". Para análise da resposta do PO foi considerada a latência dos dois primeiros picos e o valor médio da amplitude dos três picos do complexo de três respostas consecutivas, denominado índice oscilatório (IO). RESULTADOS: A hipertensão arterial acometia 26 (59,1%) dos pacientes, ao passo que 18 (40,9%) eram normotensos. A média do IO obtido foi de 257,41µV no grupo de NT e de 217,81 µV no HT (p=0,006). As médias de latências obtidas para os picos 1 (NT-18,42 ms e HT-17,91 ms) e 2 (NT-24,54 ms e HT- 24,29 ms) não foram diferentes entre os grupos (p>0,05). CONCLUSÃO: Os hipertensos apresentam índice oscilatório significativamente menor que os normotensos, sugerindo que a hipertensão arterial pode ocasionar disfunção da retina interna.

A Doença Renal Crônica é um problema de saúde pública crescente no mundo. A detecção e o tratamento precoces reduziriam as altas taxas de morbimortalidade e os custos associados. Este trabalho buscou identificar o panorama do acesso ao cuidado a partir da conduta dos médicos da Atenção Primária à Saúde na linha de cuidado da doença. Aplicaram-se questionários para 62 médicos de família dos Centros de Saúde da Família do município de Fortaleza. Os achados apontam que a Taxa de Filtração Glomerular foi mensurada por apenas 8.1% dos médicos para pacientes diabéticos e 4.8% para pacientes hipertensos. Mais da metade dos médicos (51.2%) referenciariam o paciente apresentando redução leve/moderada da Taxa de Filtração Glomerular ao nível secundário. Por outro lado, 25.8% dos médicos não referenciariam o paciente com Doença Renal Crônica avançada ao especialista. A lacuna entre esses dois níveis da atenção implica em barreira de acesso ao usuário, podendo comprometer avanços no plano da integralidade. A criação de novos dispositivos no processo de trabalho torna-se urgente e o apoio matricial apresenta-se como proposta viável para a articulação das ações entre os níveis da atenção no cuidado do portador da Doença Renal Crônica ou seus fatores de risco.

Aerobic exercise has been suggested as a non-pharmacological treatment for hypertension, and the previous paper of this set demonstrated some of the physiological responses induced by exercise. It has been shown an increment on expenditures for appropriate hypertension management in both, public and private services, which reinforces the inclusion of preventive programs to reduce healthcare costs. However, little is known about physical exercise cost-effectiveness for hypertensive patients. There are several interventions like a simple doctor/dietitian counselling in order to change life style, wed-based nutrition program, pharmacological treatment and assisted or non-assisted physical exercise program that evaluate the costs savings. We have shown that regular exercise (combined or not with another diet counselling and antihypertensive treatment) may effectively contribute to reduce the health care costs (up to -38%). Also, we have shown that exercise improves body composition and lipid profile which are important risk factors to development of cardiovascular disease. So, exercise can lead to significant reduction in blood pressure medication use and, therefore, it causes cost savings, justifying the implementation of exercise programs in all healthcare units.

Hypertension is characterized by peripheral vascular resistancethat leads to blood pressure increase and severalsystemic changes that may negatively influence one s oralhealth. Thus, the aim of this study was to conduct a literaturereview on the influence of hypertension over oral conditionsand endodontic treatment. Hypertension mainly affects theblood vessels, brain and kidneys. A hypertensive conditioncan lead to increased levels of parathyroid hormones, abnormalvitamin D metabolism, reduction in the concentrationof ionized calcium and decreased calcium absorption.Therefore, hypertension can be closely associated with oralproblems such as periodontal diseases, implant loss, difficultyin bone healing, reduced salivary flow and protein concentrationin saliva, increased number of neutrophils and, as a consequence,favoring of inflammatory processes. It has alsobeen suggested that the success rate of endodontic treatmentin hypertensive patients is lower than in normotensiveones. The response of hypertensive patients to root canaltreatment, intracanal medications and sealers should be furtherstudied in order to provide knowledge on the changes,failures and success of endodontic treatment.

The aim of this study is to investigate the spontaneous use of medicinal plants by volunteer patients in the treatment of hypertension and to determine the most used plants in this situation. Data were collected through a questionnaire and a semi-structured guided interview applied to patients from a health center in the Midwest region of the state of Sao Paulo, Brazil. Quantitative analysis identified a high number of hypertensive patients spontaneously using herbal treatment. The referred plants were identified by the Herbarium Botu and researched in the literature as to their therapeutic actions. The biggest mistake was observed in the misuse of the Bilberry for the treatment of hypertension, which reveals that health professionals and the community in general should be more careful. As a result we have published an informative booklet, with emphasis on hypertension concepts and on the use of medicinal plants as an alternative therapy method for this disease in order to provide scientific knowledge and scientific research evidence to this practice.

This study aimed to investigate the relation between physical activity levels (PAL) of hypertensive patients, seeing different do- mains, and, to compare anthropometric variables and blood pressure (BP). The survey was conducted with 192 patients, mean age 63 ± 11 years. The anthropometric variables were body weight, body mass index (BMI) and waist circumference (WC). PAL was estimated using Baecke ́s questionnaire and categorized in four domains (OPA = occupational; leisure time = LTPA; locomotion = LPA and Total PA). Student t test unpaired was applied for comparisons between active and insufficiently active. Differences between active and sedentary in relation to anthropometry and resting blood pressure were observed for the LTPA, LPA and Total PA and significant negative correlations were observed between LPA and Total PA with systolic BP (p<0,05). Conclusion: the LPA and LTPA were associated with resting BP in hypertensive. No differences in BMI and WC among physically active and insuffi- ciently active were found.

Due to the high incidence and prevalence of hypertension, especially in the elderly population, several studies have been developed to understand the relationship between etiological factors and blood pressure control. It has been demonstrated that hypertensive patients tend to present a status of hyperuricemia. This result suggested that there is a relationship between blood pressure and uric acid concentrations. However there is still a lack of studies that focus on this relationship, and especially how physical exercise could affect the relationship between both of them. Thus, the purpose of this study is to review and discuss the relationship between hypertension and uric acid concentration pointing the oxidative stress as the main factor of this relationship and discuss the physical exercise as the main preventive factor of high uric acid concentrations and oxidative stress. It has been described an increase in oxidative stress during the uric acid pathway because the high production of anions superoxide. This in turn, increases the activation of renin-angiotensin system and decreases nitric oxide bioavailability which will compromise the vasodilatation mechanism. However physical exercises have been associated with improvements in antioxidant capacity and nitric oxide production and bioavailability which will improve the blood pressure control.

People with hypertension constantly deal with issues related to mental health due to the psychosocial determinants of this illness, and leisure is an important control strategy. The objective of this study was to promote health education to a group of hypertensive patients, through research-action, based on critical-social pedagogy, and taking into consideration the participants' perception of leisure. Educational activities were conducted and, following, an evaluation was performed regarding the subjects' opinion about the impact of leisure on their lives and mental health. The group perceived leisure as a coping strategy for loneliness and also as a late development of independence; it was also regarded as a means of socialization and as a promotion of mental health. These perceptions revealed two themes: aging, leisure, and chronic disease; and knowledge and leisure experiences. Educational actions, such as group dynamics and discussions, were planned considering these themes with the purpose of providing the necessary conditions for socializing and exchanging experiences.

Objective: To determine the prevalence of patients with type 1 diabetes mellitus who meet the glycemic and cardiovascular (CV) risk factors goals and the frequency of screening for diabetic complications in Brazil according to the American Diabetes Association guidelines. Research design and methods: This was a cross-sectional, multicenter study conducted between December 2008 and December 2010 in 28 public clinics in 20 Brazilian cities. Data were obtained from 1774 adult patients (56.8% females, 57.2% Caucasians) aged 30.3 +/- 9.8 years with diabetes duration of 14.3 +/- 8.8 years. Results: Systolic blood pressure was at goal in 40.3% and diastolic blood pressure was at goal in 26.6% of hypertensive patients. LDL cholesterol and HbA1c were at the goal in 45.2% and 13.2% of the patients, respectively. Overweight was presented in 25.6% and obesity in 6.9%. Among those with more than 5 years of disease, screening for retinopathy was performed in the preceding year in 70.1%. Nephropathy and feet complications were screened in 63.1% and 65.1%, respectively. Conclusions: The majority of patients did not meet metabolic control goals and a substantial proportion was not screened for diabetic complications.
