Zunic investigated the relationship between nationalism and Serbian literature. He first analysed and evaluated the justifications for a number of critical accusations against Serbian literature. These included mythological and epic foundations (traditionalism), an obsession with national history and with the motif of the Serbian people as an eternal victim, the domination of the collective over the individual (populism), an anachronistic romantic conception of the social function of literature. In order to gain an unbiased judgement of the nationalistic role of contemporary Serbian literature, Zunic prepared a list of those books with the greatest number of copies issued in the decade 1985-1995, and constructed an appropriate hermeneutic procedure of understanding the meaning of the content and form of these works. He concluded that contemporary Serbian literature is in fact occupied with national history, and also with the unmasking of communist totalitarianism. The most influential books express and document either nationally-oriented or civil-oriented world views. The former, although mostly not militant works (with their realistic "closed" form), might have had an ideological influence on the Serbian national consciousness, while the civil-oriented works (with their "open" modern or post-modern form) could not neutralise all these extra-literary effects of the nationally-oriented works, since the predominant way of reading is a communication with the literary content (realised as a testimony of historical facts), and not with the literary form (as a carrier of the artistic value and a "moderator" of any content).

Silent corticotroph adenomas (SCA) are rare pituitary tumors with histologic hallmarks of corticotroph differentiation, including ACTH immunoreactivity, but lacking clinical evidence of Cushing's syndrome. We report on four female patients, aged 19-66 years, each presenting with a nonfunctional macroadenoma. Leading symptoms were headache in two cases and visual field deficits in one. One patient was incidentally diagnosed while undergoing cranial MRI for an unrelated condition. Three patients had marked obesity; none of them presented constitutional signs of Cushing's syndrome. Serum cortisol levels were moderately elevated in the two patients systematically tested in this respect. Marginal to moderate hyperprolactinemia was present in two cases. Two patients also were shown to be deficient in either gonadotroph or thyrotroph axis, while a third had a combined insufficiency of both gonadotroph and thyrotroph axis. MRI scans revealed intratumoral hemorrhage and/or cystic change in three cases, as well as tumor-related occlusive hydrocephalus in one. The latter patient was biopsied only, while the remaining underwent gross total resection. Histologically, all four lesions were diagnosed as SCA subtype I displaying intense immunoreactivity for ACTH. In three tumors, scattered cells coexpressed PRL as well. In addition, Crooke's hyaline change was noted in a significant number of tumor cells and in residual non-neoplastic corticotrophs in one case each. With MIB-1 labeling indices of 1-3%, none of the tumors qualified as atypical adenoma. We conclude that SCAs are more likely to be discovered as expansile tumors, whose advanced local space-occupying character at surgery rather than an inherently aggressive growth potential may negatively influence the clinical outcome. Subtle morphologic evidence of corticotroph suppression in residual pituitary adjacent to tumor lends further support to literature data indicating minimal or intermittent functional activity in this tumor type.

Nasal septal hematoma with abscess (NSHA) is an uncommon complication of trauma and studies on children are especially rare. We discuss the case of a 6-year-old girl, who was initially evaluated independently by three doctors for minor nasal trauma but had to be re-hospitalized 6 days later with NSHA. Although septal hematoma had initially been excluded (5, 7 and 24 hours after trauma), a secondary accumulation of blood seems to have occured. Delayed hematoma formation has been described in the orbit as a result of possible venous injuries after endoscopic sinus surgery. However, such an observation is new for septal hematoma in children. Thus, we recommend re-evaluation for septal hematoma 48h to 72h after paediatric nasal trauma. Such a scheduled re-examination offers a chance to treat delayed subperichondral hematoma on time before almost inevitable superinfection leads to abscess formation and destruction of the nasal infrastructure. We suggest that parents should be vigilant for delayed nasal obstruction as possible herald of hematoma accumulation within the first week.

BACKGROUND: The aim of this study was to report a case of squamous cell carcinoma of the petrous part of the temporal bone associated with a long history of secondary acquired cholesteatoma in a 71-year-old man. PATIENTS AND METHODS: We present the case of a 71-year-old man diagnosed with secondary acquired cholesteatoma in 1950. Treatments consisted of repetitive surgery owing to several relapses. In 2004, he presented with progressive fetid otorrhea. Clinical and computed tomography findings were indicative for relapsing cholesteatoma and a subtotal petrosectomy was performed. RESULTS: Histologic work-up demonstrated a moderately differentiated squamous cell carcinoma. The staging revealed stadium pT3 cN0 cM0. Postoperative treatment consisted of local radiation therapy with intensity-modulated beam geometry with a total of 64.2 Gy in 30 fractions using a simultaneous integrated boost. CONCLUSION: Middle ear carcinoma can arise from acquired cholesteatoma. The pathogenesis of squamous cell carcinoma associated with cholesteatoma has not been elucidated satisfactorily. Due to the complex anatomic features, intensity-modulated radiation therapy is the technique of choice for postoperative radiotherapy.

The purpose of this study was to investigate the generality and temporal endurance of the bivalency effect in task switching. This effect refers to the slowing on univalent stimuli that occurs when bivalent stimuli appear occasionally. We used a paradigm involving predictable switches between 3 simple tasks, with bivalent stimuli occasionally occurring on one of the tasks. The generality of the bivalency effect was investigated by using different tasks and different types of bivalent stimuli, and the endurance of this effect was investigated across different intertrial intervals (ITIs) and across the univalent trials that followed trials with bivalent stimuli. In 3 experiments, the results showed a general, robust, and enduring bivalency effect for all ITI conditions. Although the effect declined across trials, it remained significant for about 4 trials following one with a bivalent stimulus.
