Background: Canine mammary tumors are challenging for clinicians and pathologists because of complex histologic classification, low specificity of cytologic diagnosis, and unpredictable biological behavior. In histologic specimens, expression of tumor proliferation marker Ki-67, a nuclear nonhistone protein, has been shown to have prognostic value for canine mammary tumors and to correlate with malignancy and low survival rates. Objective: The objective of this study was to measure the proliferation index of canine mammary tumors by immunochemical detection of Ki-67 in cytologic specimens and to determine its relationship to clinical and pathologic variables and patient outcome. Methods: Spontaneous mammary tumors from 31 female dogs were surgically excised. Imprint specimens for cytologic evaluation were wet-fixed in ethanol; histologic specimens were prepared routinely. Immunostaining was performed with the PH 177 monoclonal antibody against Ki-67; proliferation index was graded from negative to +++. Dogs were followed for 18 months. Multivariate logistic regression analysis was used to determine correlations between immunocytochemical results, tumor and clinical variables, and patient outcome. Results: Ki-67 proliferation indices in cytologic specimens were significantly lower for nonmalignant tumors than for malignant tumors. High index values of Ki-67 were positively correlated with metastasis, death from neoplasia, low disease-free survival rates, and low overall survival rate. With the exception of 4 specimens for which cellularity was insufficient, positive expression of Ki-67 in cytologic specimens correlated with that of histologic specimens. Conclusions: The prognostic value of the Ki-67 index in canine mammary tumors by using wet-fixed cytology imprint specimens was similar to that observed previously for histologic specimens. Immunocytochemical detection of Ki-67 could improve the accuracy and value of cytology by providing safe and rapid information about malignancy and patient outcome. © 2004 American Society for Veterinary Clinical Pathology.

Endometrial stromal sarcoma is a rare neoplasm of the uterus. Extrauterine locations of this neoplasm, excluding metastases or local extension, are even more unusual and are usually associated with the presence of endometriosis. The authors report a case of endometrial stromal sarcoma presenting as a vaginal wall nodule, without any sign of primary uterine tumor after extensive evaluation or presence of endometriosis. The morphology, immunohistochemical profile, differential diagnoses, and pathogenesis are discussed, as well as a review of the literature on this issue. © 2004 Elsevier Inc. All rights reserved.

Investigation of the efficacy of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in chronic myeloid leukemia patients is essential to predict prognosis and survival. In 20 patients treated at the Bone Marrow Transplantation Unit of São José do Rio Preto (São Paulo, Brazil), we used fluorescence in situ hybridization (FISH) to investigate the frequency of cells with BCR/ABL rearrangement at diagnosis and at distinct intervals after allo-HSCT until complete cytogenetic remission (CCR). We investigated the disease-free survival, overall survival in 3 years and transplant-related mortality rates, too. Bone marrow samples were collected at 1, 2, 3, 4, 6, 12, and 24 months after transplantation and additional intervals as necessary. Success rate of the FISH analyses was 100%. CCR was achieved in 75% of the patients, within on average of 3.9 months; 45% patients showed CCR within 60 days after HSCT. After 3 years of the allo-HSCT, overall survival rate was 60%, disease-free survival was 50% and the transplant-related mortality rate was 40%. The study demonstrated that the BCR-ABL FISH assay is useful for follow-up of chronic myeloid leukemia patients after HSCT and that the clinical outcome parameters in our patient cohort were similar to those described for other bone marrow transplantation units. ©FUNPEC-RP.

Infertility represents one of the main long-term consequences of combination chemotherapy used for the treatment of breast cancer. Approximately 60%-65% of breast cancers express the nuclear hormone receptor in premenopausal women. Adjuvant endocrine therapy is an integral component of care for patients with hormone receptor-positive (HR+) tumours. The GnRH agonist (GnRHa) alone or in combination with tamoxifen produces results at least similar to those obtained with the different chemotherapy protocols in patients with HR+ tumors with respect to recurrence-free survival and overall survival, Presentation of the hypothesis: It is time to indicate adjuvant therapy with GnRHa associated with tamoxifen for patients with breast cancer (HR+ tumours) if they want to preserve their reproductive function. Testing the hypothesis: Assessment of ovarian reserve tests: follicle stimulating hormone (FSH), anti-Mullerian hormone (AMH), inhibin B, antral follicle count (AFC) and ovarian volume 6 months, and 1 year after the end of therapy with GnRHa/tamoxifen. The recurrence-free survival and overall survival should be analysed. Implications of the hypothesis: The major implication will be to avoid adjuvant chemotherapy for patients with breast cancer (HR+ tumours) that request fertility preservation. Urothelial bladder cancer (UBC) is a heterogeneous type of disease. It is urgent to screen biomarkers of tumour aggressiveness in order to clarify the clinical behaviour and to personalize therapy in UBC patients. Raf kinase inhibitory protein (RKIP) is a metastasis suppressor, and its downregulation is associated with metastatic events in an increasing number of solid tumours. We evaluated the clinical and prognostic significance of RKIP expression in patients with high risk of progression UBC. Using immunohistochemistry, we determined RKIP expression levels in a series of 81 patients with high-grade pT1/pTis or muscle-invasive UBC. Staining of CD31 and D2-40 was used to assess blood and lymphatic vessels, in order to distinguish between blood and lymphatic vessel invasion (LVI). We found that 90 % of pT1/pTis tumours, 94 % of non-muscle invasive papillary tumours and 76 % of the cases without LVI occurrence expressed RKIP in >10 % of cells. In this group, we observed a subgroup of tumours (42 %) in which the tumour centre was significantly more intensely stained than the invasion front. This heterogeneous pattern was observed in 63 % of the cases with LVI. Low RKIP expression was associated with poorer 5-year disease-free and overall survival rates, and remained as an independent prognostic factor for disease-free survival. Loss of RKIP expression may be an important prognostic factor for patients with high risk of progression bladder cancer. © 2013 Springer-Verlag Berlin Heidelberg.

O Meduloblastoma é a neoplasia encefálica mais comum na infância, correspondendo a taxas de 16 a 25% de todos os tumores encefálicos em menores de 19 anos e 30-40% de todos os tumores da fossa posterior. Para investigar o papel do polimorfismo de base única (SNP) TP53 Arg72Pro sobre o risco de desenvolvimento, prognóstico e resposta a terapêutica adjuvante em Meduloblastoma, foi realizado um estudo caso-controle com 122 pacientes e 122 controles saudáveis do Brasil. Em comparação com Arg/Arg, que é o genótipo mais comum na população em estudo, tanto o genótipo Arg/Pro e Pro/Pro não influenciaram o risco de desenvolvimento de Meduloblastoma (OR = 1,36 e P = 0,339 para o genótipo Arg/Pro; OR = 1,50 e P = 0,389 para o genótipo Pro/Pro). Com relação ao prognóstico, a sobrevida livre de doença não foi significativamente diferente entre os genótipos SNP TP53 Arg72Pro (P> 0,05), porém o genótipo menos freqüente, Pro/Pro, foi associado a uma menor sobrevida global dos pacientes com Meduloblastoma (P = 0,021 ). Estes dados sugerem que embora não haja nenhuma associação entre o SNP TP53 Arg72Pro e o risco de desenvolvimento de Meduloblastoma, o genótipo Pro/Pro está associado a uma menor sobrevida global dos pacientes submetidos a terapia adjuvante. No entanto, devido à composição interétnica da população brasileira, futuros estudos envolvendo populações maiores e de outras partes do mundo serão essenciais para uma conclusão definitiva da função do SNP TP53 Arg72Pro.

A etiologia do carcinoma epidermóide do pênis (CEP) está relacionada a múltiplos fatores de risco como a presença de prepúcio, pobre higiene, dermatites crônicas e tabagismo. Entretanto, o fator de risco mais extensivamente estudado, éa infecção pelo papiloma vírus humano (HPV). Está bem estabelecido que esteexerce um importante papel etiológico em neoplasias da cérvice uterina; entretanto, sua associação com CEP tem sido foco de debates, demonstrando-se presença viral variando entre 15% a 80% dos casos, sugerindo que apenas um subgrupo desses tumores seja causado pelo HPV. O prognóstico da doença é influenciado negativamente principalmente pela presença de metástases em linfonodos inguinais. Desta forma, a abordagem cirúrgica destes, assume importância decisiva na cura da doença. Poroutro lado, os métodos disponíveis para o estadiamento desses linfonodos são imprecisos e as linfadenectomias são acompanhadas de morbidade significativa. A descrição de outros marcadores histológicos é escassa, devido a raridade desses tumores.O objetivo do presente trabalho é avaliar a prevalência, distribuição e associação do HPV com parâmetros histológicos de pior prognóstico, no sentido de determinar seu possível valor preditivo para metástases inguinais, assim como avaliar os fatores prognósticos já descritos. Espécimes tumorais em parafina de 82 pacientes portadores de carcinoma epidermóide do pênis, foram testados para prevalência e distribuição do genótipo do HPV por PCR. O estado do HPV foi correlacionado com fatores histopatológicos e ocorrência de metástases inguinais. Foi também avaliada a influência de diversascaracterísticas histológicas tumorais, na sobrevida livre de doença inguinal em 5 anos. O seguimento variou entre 1 e 71 meses (média 20 meses). O DNA do HPV foi identificado em 60,9% da amostra, tendo maior prevalência dos tipos virais 11 e 6 (64% e 32%, respectivamente). Não houve correlação independente significativa das variáveis histológicas de pior prognóstico com o estado do HPV. A probalidade de sobrevida livre de doença inguinal em 5 anos, também não foi influenciada pelo HPV (log Rank teste=0,45). Os únicos fatores patológicos independentes para metástases inguinais foram estadiamento T≥T1b-T4 (p=0,02), invasão linfovascular (p=0,04) e fronte de invasão infiltrativa (p-0,03). O estado e a distribuição do HPV não mostrou correlação com fatores histológicos de pior prognóstico, nem mostrou importância na predição de metástases linfonodais em CEP.
