Open surgical repair of complex abdominal aortic aneurysms requires more extensive dissection and aortic clamping above the renal or mesenteric arteries. Although results of open surgical series have shown variation, morbidity and mortality is higher compared with infrarenal aortic aneurysm repair. Potential complications include renal insufficiency, mesenteric ischemia, multisystem organ failure, and death. Although endovascular treatment with fenestrated and branched endografts might potentially decrease the risk of complications and mortality, its role is not yet defined and the technology is not widely available. Issues related to durability of the procedure and secondary interventions might limit its application to patients with higher risk or those with hostile anatomy. This article summarizes the clinical results of open surgical repair of pararenal abdominal aortic aneurysms to provide a benchmark for comparison with results of endovascular treatment, using fenestrated and branched techniques. © Annals of Vascular Surgery Inc.

The aim of this study was to evaluate the surgical use of the natural latex biomembrane in diaphragmatic injuries produced experimentally in rabbits. Fifteen healthy adult male and female New Zealand rabbits were employed. The rabbits were assigned to the experimental groups I, II, III, IV and V and analyzed on the 15th, 30th, 45th, 60th and 90th days post surgery, respectively. The surgical procedure consisted in the access to the diaphragm at the eighth right intercostal space, removal of a circle portion of approximately 1.5 cm in diameter following surgical repair with a latex membrane. Macroscopically, it was observed an excellent healing process during the experimental period. The clinical observations, complemented by the histological analysis, indicate that the latex membrane is useful for repair of traumatic inuries of the diaphragm of rabbits.

Objective: Optimal surgical treatment of patients with transposition of the great arteries (TGA), ventricular septal defect (VSD), and pulmonary stenosis (PS) remains a matter of debate. This study evaluated the clinical outcome and right ventricle outflow tract performance in the long-term follow-up of patients subjected to pulmonary root translocation (PRT) as part of their surgical repair. Methods: From April 1994 to December 2010, we operated on 44 consecutive patients (median age, 11 months). All had malposition of the great arteries as follows: TGA with VSD and PS (n = 33); double-outlet right ventricle with subpulmonary VSD (n = 7); double-outlet right ventricle with atrioventricular septal defect (n = 1); and congenitally corrected TGA with VSD and PS (n 3). The surgical technique consisted of PRT from the left ventricle to the right ventricle after construction of an intraventricular tunnel that diverted blood flow from the left ventricle to the aorta. Results: The mean follow-up time was 72 +/- 52.1 months. There were 3 (6.8%) early deaths and 1 (2.3%) late death. Kaplan-Meier survival was 92.8% and reintervention-free survival was 82.9% at 12 years. Repeat echocardiographic data showed nonlinear growth of the pulmonary root and good performance of the valve at 10 years. Only 4 patients required reinterventions owing to right ventricular outflow tract problems. Conclusions: PRT is a good surgical alternative for treatment of patients with TGA complexes, VSD, and PS, with acceptable operative risk, high long-term survivals, and few reinterventions. A child with microdeletion at 22q11.21 was referred to a craniofacial center due to hypernasality, unintelligible speech, and bifid uvula. Velopharyngeal dysfunction remained after surgical repair of submucous cleft palate and speech therapy. A prosthetic-behavioral treatment approach involving total obturation of the velopharynx was successfully implemented for management of velopharyngeal hypodynamism.

Introduction: At the initial consultation, the speech-language pathologist and audiologist may consider possible diagnostic hypotheses based on the child's history and the parents' complaint. Aim: To investigate the association of hearing complaints with the findings obtained in the conventional audiologic assessment in children with cleft lip and palate. Retrospective study. Methods: We analyzed medical charts of 1000 patients with cleft lip and palate who underwent surgical repair between 1988 and 1995 at a mean age of 6 years 8 months. We excluded charts with records of inconsistent audiological responses and charts with missing data for any of the audiologic evaluations considered. Thus, the sample consisted of 393 records. Results: Two hundred thirty-nine patients presented hearing loss in one or both ears, but only 3.8% reported hearing complaints. The most frequent were otorrhea followed by otalgia. There was no statistical significance between the complaint and gender (p = 0.26) nor between the complaint and hearing loss (p = 0.83). Conclusion: This study showed no association between the hearing complaint and the conventional audiologic assessment

Objectives: to use the toy as a therapeutic resource in the preparation of children undergoing surgical repair of cleft lip and palate preoperatively; to describe quali-quantitativelly the behavioral reactions of the child during the two periods, pre-and postoperatively. Method: quali-quantitative study, developed in a specialized hospital, with 40 children aged between 7 and 12 years old who underwent surgery for correction of cleft lip and palate. Data collection was by means of an instrument with 21 behavioral variables preoperatively and postoperatively. Content analysis was used in the speech of mothers and children. This study was the research project approved by the Ethics Committee, CAAE No. 050/2011. Results: the variable is a presented questioner p> 0.0265 and four categories emerged after content analysis. Conclusion: therapeutic toy is a feature that makes children relieve stress, and facilitates the implementation of nursing care.

Angiotensin II induziert intrazellulär die Bildung reaktiver Sauerstoffspezies, welche DNA-Schäden erzeugen können. Um die Hypothese zu prüfen, dass durch Angiotensin II induzierte DNA-Schäden für die erhöhte Krebsinzidenz hypertensiver Menschen verantwortlich sind, wurde eine vierwöchige Behandlung von Mäusen mit Angiotensin II (0,6 μg/kg/min) durchgeführt. Mit der Alkalischen Elution wurden in Zellen aus verschiedenen Organen der Mäuse die Menge an DNA-Einzelstrangbrüchen und oxidativen DNA-Modifikationen bestimmt. In der Niere wurde außerdem mit dem BigBlue® Mutations-Assay die Entstehung von Mutationen analysiert. In keinem der analysierten Organe konnte eine Erhöhung der DNA-Schäden oder eine Erhöhung der Mutationsfrequenzen durch die Angiotensin II-Behandlung nachgewiesen werden. Die durchgeführten Untersuchungen geben somit keinen Hinweis auf eine DNA-schädigende und mutagene Wirkung von Angiotensin II.rnBei der Entstehung und dem Krankheitsverlauf von Arteriosklerose spielen reaktive Sauerstoffspezies ebenfalls eine noch nicht genau geklärte Rolle. Um zu ermitteln, ob oxidative DNA-Schäden die Entstehung der Arteriosklerose begünstigen, wurde die Endothelfunktion von Wildtyp- und reparaturdefizienten Ogg1-/--Mäusen verglichen. Entgegen der Vermutung, dass oxidative DNA-Modifikationen die Endothelfunktion verschlechtern, zeigen die Untersuchungen, dass Ogg1-/--Mäuse, die höhere Spiegel an oxidativen DNA-Modifikationen in ihrem Genom haben, eine signifikant bessere Endothelfunktion besitzen als Wildtyptiere. Dieser Befund weist auf eine neuartige, von der DNA-Reparatur unabhängige Funktion von OGG1 hin.rn
