FUNDAMENTO: O controle das respostas cardiovasculares durante exercício resistido (ER) é importante para a segurança do paciente. OBJETIVO: Investigar a influência do número de repetições máximas (RM) e dos intervalos de recuperação entre séries (IR) sobre a frequência cardíaca (FC), pressão arterial sistólica (PAS) e duplo produto (DP) durante ER. MÉTODOS: Vinte sujeitos saudáveis (26 ± 5 anos) realizaram protocolos de ER envolvendo três séries do leg press (6 e 12 RM) e IR proporcional ao tempo de contração (1:3 e 1:5). Aferiu-se a FC continuamente com cardiofrequencímetro e a PAS foi verificada ao final das séries, por meio de protocolo validado com método auscultatório. RESULTADOS: A FC sofreu influência da carga (p = 0,008) e das séries (p < 0,001), mas não do IR (p = 0,087). A PAS sofreu efeito isolado do número de séries (p < 0,001) e do IR (p = 0,017), mas não da carga (p = 0,95). O DP elevou-se em relação direta com a carga (p = 0,036) e com as séries (p < 0,001), mas inversamente ao IR (p = 0,006). Nos protocolos de 6 RM, a variação da FC foi maior para IR = 1:3 (Δ = 11,2 ± 1,1 bpm) do que para IR = 1:5 (Δ = 4,5 ± 0,2 bpm; p = 0,002), mas não houve diferença para 12 RM (Δ 1:3 = 21,1 ± 2,2 bpm; Δ 1:5 = 18,9 ± 2,0 bpm, p = 0,83). O IR influenciou a variação da PAS em todas as cargas (6 RM - Δ 1:3 = 10,6 ± 0,9 mmHg, Δ 1:5 = 6,6 ± 0,7 mmHg; p = 0,02 e 12 RM - Δ 1:3 = 15,2 ± 1,1 mmHg, Δ 1:5 = 8,4 ± 0,7 mmHg; p = 0,04). O DP elevou-se proporcionalmente à carga (p = 0,036) e para séries (p < 0,001), mas inversamente ao IR (p = 0,006). Com IR = 1:3, houve diferença de DP para 6 RM (Δ = 2.892 ± 189 mmHg.bpm) e 12 RM (Δ = 4.587 ± 300 mmHg.bpm; p = 0,018), mas não com IR = 1:5 (6 RM: Δ = 1.224 ± 141 mmHg.bpm, 12 RM: Δ = 2.332 ± 194 mmHg.bpm; p = 0,58). CONCLUSÃO: Independentemente da carga, um maior IR associou-se a menores respostas cardiovasculares durante ER, especialmente de PAS.

FUNDAMENTO: A associação entre parâmetros antropométricos e carga aterosclerótica ainda não está bem estabelecida e poucos estudos na literatura abordam esse tema. OBJETIVO: Avaliar a associação de diferentes parâmetros antropométricos com a carga aterosclerótica coronariana. MÉTODOS: Participaram do estudo pacientes adultos submetidos à cineangiocoronariografia. Dados sociodemográficos e fatores de risco cardiovasculares foram coletados em um questionário padronizado. Foram medidos peso, altura, circunferência da cintura (CC), circunferência abdominal (CABD), circunferência do quadril (CQ) e circunferência do pescoço (CP), com o cálculo do índice de massa corporal (IMC) e das relações cintura-quadril (RCQ), circunferência abdominal-quadril (RCABDQ) e cintura-altura (RCALT). A carga aterosclerótica coronariana na cineangiocoronariografia foi medida pelo Escore de Friesinger (EF). Aterosclerose significativa foi considerada quando o EF > 5. RESULTADOS: A amostra foi constituída por 337 pacientes, dos quais 213 eram homens (63,2%). A idade média foi de 60,1 ± 10 anos. Somente a RCQ (r = 0,159 e p = 0,003) apresentou correlação linear significativa com a carga aterosclerótica coronariana medida pelo EF. Quando separamos a amostra por gênero, nas mulheres encontramos correlação significativa da RCABDQ (r = 0,238 e p = 0,008) e da RCQ (r = 0,198 e p = 0,028) com o EF. Não se encontrou nos homens correlação entre parâmetros antropométricos e EF. Após os ajustes para as variáveis sexo, idade, HAS, tabagismo e DM, nenhum parâmetro antropométrico foi associado com a carga aterosclerótica coronariana medida pelo EF na amostra total ou separada por gênero. CONCLUSÃO: Nenhum parâmetro antropométrico foi fator de risco independente para a carga aterosclerótica coronariana.

O presente trabalho se propõe a verificar de que modo a orientação de uma edificação rural influe na carga térmica radiante (radiação solar) interceptada pela mesma. Duas situações freqüentemente encontradas na prática são examinadas: a orientação Norte-Sul e a Leste-Oeste. Para determinar a carga térmica radiante, de importância nos problemas deventilação e refrigeração, é adotado um modelo de construção a título de exemplo, sendo que as relações aqui propostas são aplicáveis a qualquer modelo de construção desejado. Os resultados mostraram quea carga térmica radiante da exposição Leste-Oeste chega a ser 74% da carga na exposição Norte-Sul.

To compare the impact of meeting specific classification criteria [modified New York (mNY), European Spondyloarthropathy Study Group (ESSG), and Assessment of SpondyloArthritis international Society (ASAS) criteria] on anti-tumor necrosis factor (anti-TNF) drug retention, and to determine predictive factors of better drug survival. All patients fulfilling the ESSG criteria for axial spondyloarthritis (SpA) with available data on the axial ASAS and mNY criteria, and who had received at least one anti-TNF treatment were retrospectively retrieved in a single academic institution in Switzerland. Drug retention was computed using survival analysis (Kaplan-Meier), adjusted for potential confounders. Of the 137 patients classified as having axial SpA using the ESSG criteria, 112 also met the ASAS axial SpA criteria, and 77 fulfilled the mNY criteria. Drug retention rates at 12 and 24 months for the first biologic therapy were not significantly different between the diagnostic groups. Only the small ASAS non-classified axial SpA group (25 patients) showed a nonsignificant trend toward shorter drug survival. Elevated CRP level, but not the presence of bone marrow edema on magnetic resonance imaging (MRI) scans, was associated with significantly better drug retention (OR 7.9, ICR 4-14). In this cohort, anti-TNF drug survival was independent of the classification criteria. Elevated CRP level, but not positive MRI, was associated with better drug retention.

PURPOSE: To assess the impact of axial traction during acquisition of direct magnetic resonance (MR) arthrography of the wrist with regard to joint space width and amount of contrast material between the opposing cartilage surfaces. MATERIALS AND METHODS: Fifteen consecutive patients (12 male, mean age 38.1 years) were included in this Institutional Review Board-approved prospective study. Three-compartment wrist MR arthrographies were performed between October and December 2009 on a 3 T unit using a fat-suppressed T1-weighted isotropic high-resolution volumetric interpolated breathhold examination (VIBE) sequence in the coronal plane, with and without axial traction (3 kg). Two radiologists measured radiocarpal (radioscaphoid, radiolunate) and midcarpal (lunocapitate, hamatolunate) joint space widths, with and without traction, and assessed the amount of contrast material between the opposing cartilage surfaces using a three-point scale: 0 = absence, 1 = partial, 2 = complete. RESULTS: With traction, joint space width increased significantly at the radioscaphoid (Delta = 0.78 mm, P < 0.01), radiolunate (Delta = 0.18 mm, P < 0.01), and lunocapitate (Delta = 0.45 mm, P < 0.01) spaces, and both observers detected significantly more contrast material between the cartilage surfaces. At the hamatolunate space, the differences in joint space width (Delta = 0.14 mm, P = 0.54) and amount of contrast material were not significant. CONCLUSION: Direct wrist MR arthrography with axial traction of 3 kg increases joint space width at the radiocarpal and lunocapitate spaces, and prompts better coverage of the articular cartilage by the contrast material. Objectives:¦The aim of this study was to estimate the prevalence of subclinical small joint synovitis detected by ultrasonography in patients with axial SpA, and to evaluate their relevance in terms of function or and disease activity.¦Methods:¦Forty axial SpA patients, 40 RA and 20 healthy subjects were evaluated by ultrasonography, using a reproducible semi-quantitative score by B-mode and Doppler, for synovitis, while disease activity and function were assessed using validated instruments (DAS28, BASDAI, BASFI, m-SACRAH and HAQ).¦Results:¦Median B-mode score were respectively 8.2 for axial SpA, 11.5 for RA and 6.0 for healthy subjects, corresponding to a prevalence of clinical significant synovitis of respectively 37.5%, 60% and 11% for a level of significance at > 8 chosen to¦classify as active > 75% of RA patient with DAS28 >2.6 and < 10% of controls. Addtionally, Doppler was positive in 8% of SpA, 30 % of RA and none of the healthy subjects. Echographic synovitis correlated with disease activity (DAS28) and function¦(HAQ, mSACRAH) in RA patients, but no correlation were found for SpA patients with disease activity (BASDAI) or function (BASFI, HAQ, mSACRAH). Cases of synovitis using classification by Doppler positivity were insufficient to allow any¦statistical analysis.¦Conclusions:¦B-mode ultrasonographic evaluation can demonstrate subclinical synovitis in almost 40% of SpA patients, but they do not appear to correlate with disease activity or function on the contrary to what is observed in RA patients, representing potentially different processes

L'alineació rotacional a l'astroplàstia total de genoll es refereix a la posició dels implants en el pla axial. Un mínim desplaçament afectarà la distribució de la càrrega y variarà l'estabilitat, l'alineació i la cinemàtica de l'articulació. Tot i que hi ha un consens pel que fa als efectes adversos d'una alineació rotacional incorrecta, encara no està clar quines són les referències anatòmiques que cal utilitzar. Les mesures de l'alineació rotacional requereixen un estudi d'alta qualitat. La TC és la modalitat d'imatge d'elecció per a l'estudi postoperatori de l'artroplàstia total de genoll. L'objectiu d'aquest treball es avaluar la capacitat diagnòstica de la TC per a l'estudi de l'alineació rotacional postoperatòria en l'artroplàstia total de genoll i presentar les variacions en el protocol de TC que minimitzen l'artefacte metàl•lic. Aquest estudi avalua la reproductibilitat de l'angle condili posterior, format per l'eix epicondili quirúrgic i la línia bicondilia posterior, mesurant la correlació entre dos observadors. El coeficient de correlació de Pearson mostra una forta correlació positiva (R=0.84) entre els dos observadors. La mitja d'error de les mesures adquirides és de -0.14º amb una desviació estàndar de 2.06º (-0.14 ± 2.06), per la qual cosa la modificació dels paràmetres tècnics de la TC per a la minimització dels artefactes metàl•lics és eficaç per l'avaluació rotacional. No obstant, l'alta magnitud de l'error indica que el radiòleg ha de ser prudent en el diagnòstic de mínimes variacions angulars que impliquin canvis en l'actitud mèdica i terapèutica.

Background: patients with axial Spondyloarthritis (SD), even withoutany obvious peripheral joint synovitis, often complain of pain in thejoints of arms and legs. Several musculoskeletal ultrasound (US)scores developed in rheumatoid arthritis have demonstrated theircapacity of discovering subclinical synovitis which were relevant interm of disease activity and for treatment strategies. None of thesescores however have been, to our knowledge, applied tospondyloarthritis patients.Objectives: to determine if subclinical synovitis can be detected byechography in patients with SD and if these synovitis are relevantcompared with RA and controls.Methods: the Swiss Sonography in Arthritis and Rheumatism(SONAR) group has developed a reproducible semi-quantitative scorefor RA using OMERACT criteria for synovitis. The score includes Bmode and Doppler mode. 35 out of 40 enrolled SD patients fulfillingthe 2010 diagnostic criteria were evaluated according to the SONARscore. In none of them, peripheral synovitis was clearly demonstrated,although some have or reported recurrent peripheral joint pain. Thescore was also applied to 20 matched controls and 40 consecutive RApatients (RA). 19 of them were in remission (DAS: <2.6), 10 with alow activity (DAS: 2.6 <>3.4) and 11 with a moderate activity disease(DAS: 3.5 <>5.1). All the patients and the controls had a completeclinical, biological and auto-evaluation assessment (joint pain andswelling counts, DAS28, HAQ, BASDAI BASMI, BASFI, m-SACRAH).The ultra-sonographer was blind to all these parameters.Results: a B mode score >8, was set up as a cut-off value forsignificant synovitis as only 10% of the controls (median: 5.9 ± 2.2)and 90% of active RA had a higher score .34% of SD had significantsynovitis which remained mostly mild. Their median B mode score(12 ± 1.6) was higher but not significantly than in remission Ra (7.1 ±3.4). Only active RA (DAS >3.5) had significant higher echographicscores: B mode (17 ± 11), Doppler score and cumulative score forsynovitis grade >1. BASDAI, BASFI, BASMI, m-SACRAH, DAS28 andCRP were not significantly different in SD patients with or withoutsynovitis.Conclusions: some patients with axial Spondyloarthritis havesubclinical but significant peripheral synovitis detected by echography.The impact of these synovitis remains uncertain as their presencedoes not seem to significantly influence disease activity and functionevaluation tools.

We report on advanced dual-wavelength digital holographic microscopy (DHM) methods, enabling single-acquisition real-time micron-range measurements while maintaining single-wavelength interferometric resolution in the nanometer regime. In top of the unique real-time capability of our technique, it is shown that axial resolution can be further increased compared to single-wavelength operation thanks to the uncorrelated nature of both recorded wavefronts. It is experimentally demonstrated that DHM topographic investigation within 3 decades measurement range can be achieved with our arrangement, opening new applications possibilities for this interferometric technique. ©2008 COPYRIGHT SPIE

OBJECTIVE: To evaluate the initiation of and response to tumor necrosis factor (TNF) inhibitors for axial spondyloarthritis (axSpA) in private rheumatology practices versus academic centers. METHODS: We compared newly initiated TNF inhibition for axSpA in 363 patients enrolled in private practices with 100 patients recruited in 6 university hospitals within the Swiss Clinical Quality Management (SCQM) cohort. RESULTS: All patients had been treated with ≥ 1 nonsteroidal antiinflammatory drug and > 70% of patients had a baseline Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) ≥ 4 before anti-TNF agent initiation. The proportion of patients with nonradiographic axSpA (nr-axSpA) treated with TNF inhibitors was higher in hospitals versus private practices (30.4% vs 18.7%, p = 0.02). The burden of disease as assessed by patient-reported outcomes at baseline was slightly higher in the hospital setting. Mean levels (± SD) of the Ankylosing Spondylitis Disease Activity Score were, however, virtually identical in private practices and academic centers (3.4 ± 1.0 vs 3.4 ± 0.9, p = 0.68). An Assessment of SpondyloArthritis international Society (ASAS40) response at 1 year was reached for ankylosing spondylitis in 51.7% in private practices and 52.9% in university hospitals (p = 1.0) and for nr-axSpA in 27.5% versus 25.0%, respectively (p = 1.0). CONCLUSION: With the exception of a lower proportion of patients with nr-axSpA newly treated with anti-TNF agents in private practices in comparison to academic centers, adherence to ASAS treatment recommendations for TNF inhibition was equally high, and similar response rates to TNF blockers were achieved in both clinical settings.

D'aquesta manera intentem oferir a les persones involucrades en la promoció unes pautes de comportament que poden guiar les seves decisions finals. El principal objectiu és analitzar estadísticament la quantitat de documents que es carreguen en els dipòsits centrals (CR) i institucionals (IR).

Since 1986, several near-vertical seismic reflection profiles have been recorded in Switzerland in order to map the deep geologic structure of the Alps. One objective of this endeavour has been to determine the geometries of the autochthonous basement and of the external crystalline massifs, important elements for understanding the geodynamics of the Alpine orogeny. The PNR-20 seismic line W1, located in the Rawil depression of the western Swiss Alps, provides important information on this subject. It extends northward from the `'Penninic front'' across the Helvetic nappes to the Prealps. The crystalline massifs do not outcrop along this profile. Thus, the interpretation of `'near-basement'' reflections has to be constrained by down-dip projections of surface geology, `'true amplitude'' processing, rock physical property studies and modelling. 3-D seismic modelling has been used to evaluate the seismic response of two alternative down-dip projection models. To constrain the interpretation in the southern part of the profile, `'true amplitude'' processing has provided information on the strength of the reflections. Density and velocity measurements on core samples collected up-dip from the region of the seismic line have been used to evaluate reflection coefficients of typical lithologic boundaries in the region. The cover-basement contact itself is not a source of strong reflections, but strong reflections arise from within the overlaying metasedimentary cover sequence, allowing the geometry of the top of the basement to be determined on the basis of `'near-basement'' reflections. The front of the external crystalline massifs is shown to extend beneath the Prealps, about 6 km north of the expected position. A 2-D model whose seismic response shows reflection patterns very similar to the observed is proposed.

BACKGROUND AND OBJECTIVE: Off-pump coronary artery bypass grafting has stimulated the development of micro-pumps designed to prevent the hemodynamic instability induced by heart luxation for the exposure of target vessels of the posterior wall. Impella (Aachen, Germany) developed micro-pumps with a miniaturized propeller system for both sides of the heart. The aim of this study was to analyze the impact of both pumps working together on blood cell integrity. MATERIALS AND METHODS: Both right and left-sided micro-pumps were implanted in 5 calves (body weight, 72_4 Kg) during 3 h. Blood samples for hematology and hemolysis parameters were drawn hourly. RESULTS: Both pumps performed well with a flow of 3.6 L +/- 0.3 L during the 3 h of the experiment with stable hemodynamic conditions. Mixed venous oxygen saturation was 63.4 +/- 15.2% at baseline and 63.8 +/- 16.3% at the end of the experiment (P = ns). Red cell count, LDH and free plasma hemoglobin were 6.7 +/- 2.1 x 10(12)/L, 1807 +/- 437 IU/L, and 32 +/- 9 mg/L at baseline vs. 6.1 +/- 2.1 x 10(12)/L, 1871 +/- 410 IU/L, and 52 +/- 9 mg/L at the end of the experiment (P = ns for all comparisons). Platelet count exhibited a non-significant drop (872 +/- 126 vs. 715 +/- 22 x 10(9)/L). CONCLUSIONS: This double pump system based on the Archimed screw principle is hematologically well tolerated under conditions of prolonged cardiac assist.
