Candida albicans and other yeasts from recreational water or clinical materials were isolated. Sixty-six water samples, originating from eight swimming pools and five lakes with beaches were examined for the presence of these yeasts, by a membrane filter method and 'pour plate' technique. Sixty-two clinical materials from suspected cases of candidiasis were studied in the same period. Rhodotorula sp and C. albicans were more frequently isolated from lakes and swimming pools, respectively; C. albicans and C. parapsilosis from clinical materials. From 44 samples of C. albicans, 90,9% were serotype A, and 9,1%, serotype B; C. albicans from recreational waters belong only serotype A. No difference was observed in the M.I.C. of C. albicans strains from waters and clinical materials. All strains were susceptible to the antifungal drugs tested.

A pneumonia associada a ventilação mecânica é a infecção hospitalar mais comum em pacientes de unidade de terapia intensiva. Estratégias de prevenção podem ser mais bem elaboradas com o conhecimento dos fatores de risco para esta infecção. Com o objetivo principal de identificar fatores associados com maior risco de desenvolvimento de pneumonia em pacientes que recebem ventilação mecânica, foi realizado estudo retrospectivo, caso controle, não pareado, em uma unidade de terapia intensiva, clínico-cirúrgica, de um hospital universitário na cidade de Belém do Pará, Brasil. O período de estudo foi de 19 meses (janeiro de 2003 a julho de 2004). Os critérios de definição foram adaptados a partir dos critérios dos Centers for Diseases Control and Prevention. Foram avaliadas características demográficas, procedimentos invasivos, morbidades associadas e variáveis dependentes de tempo (ventilação mecânica, nutrição, exposição a drogas), entre outros fatores, em 27 casos e 27 controles. A pneumonia associada à ventilação mecânica teve uma taxa bruta e incidência por 1000 ventiladores/dia de 10,6% e 12,3 episódios, respectivamente. O tempo médio de permanência na unidade de terapia intensiva dos pacientes foi de 34,2 ± 27,7 dias, enquanto dos controles foi de 15,4 ± 13,6 dias (p=0,003). O tempo médio para início da pneumonia foi de 14,29 ± 9,16 dias. A taxa global de mortalidade foi similar nos dois grupos (OR=1,60; p=0,576). A análise univariada demonstrou que medicamentos administrados em aerossóis (OR=4,75; p=0,01) e uso de curares (OR=8,61; p=0,003) estiveram associados a um maior risco de desenvolvimento da infecção. Conclui-se que a pneumonia associada a ventilação mecânica esteve associada ao uso de curares e aerossóis, sendo que medidas preventivas poderiam ser direcionadas a estes fatores por serem eles, potencialmente modificáveis.

A infecção hospitalar (IH) é um grave problema de saúde pública, principalmente em pacientes internados em Unidade de Terapia Intensiva (UTI), devido à gravidade do quadro clínico, uso constante de antimicrobianos e frequência do emprego de procedimentos invasivos. O Staphylococcus aureus (S. aureus) é um dos principais patógenos que coloniza indivíduos saudáveis e responde também, por infecções em pacientes hospitalizados. O presente estudo objetivou a identificação do perfil de suscetibilidade, principais sítios acometidos por infecção e possíveis fatores de risco associados à infecção ou colonização por S. aureus isolados de pacientes e profissionais de saúde da UTI de Hospital de Urgência e Emergência de Rio Branco (HUERB) – Acre. Foi desenvolvido um estudo transversal no período de janeiro a agosto de 2009. Para pesquisa de portadores, foram coletadas amostras biológicas da microbiota dos pacientes e profissionais de saúde. Para o levantamento de casos de pacientes com IH, foram coletadas amostras biológicas dos sítios suspeitos de estarem acometidos, a partir de 72 horas da data de sua admissão, até alta, transferência ou óbito. Dos 62 pacientes inseridos nos estudo, 19,3% foram portadores e 6,4% desenvolveram IH por S. aureus; e dos 35 profissionais, 28,6% foram portadores de S. aureus. Foi a segunda espécie bacteriana mais isolada de pacientes portadores e a quinta mais isolada de casos de IH. Não houve comprovação estatística para as variáveis abordadas no estudo serem consideradas fatores de risco para aquisição de IH por S. aureus. Os sítios anatômicos acometidos por IH por S. aureus foram o trato respiratório (n=2), seguido de corrente sanguínea (n=1). A amostra ponta de cateter foi responsável por 1 isolado. Um (1,6%) paciente desenvolveu IH por MRSA; e 5 (8,1%) pacientes e 2 (5,7%) profissionais foram portadores de MRSA, ocorrência baixa quando se relaciona com os resultados do restante do Brasil e do mundo. Destaca-se ainda, a incidência do MSSA sobre o MRSA e a baixa resistência dos MRSA aos antimicrobianos, demonstrando que na UTI do HUERB, as IH por S. aureus ainda não se constituem um problema de saúde pública. Não houve isolados de S. aureus resistentes à vancomicina, podendo ser considerada uma opção terapêutica para os casos de IH por MRSA. Vale ressaltar a importância desse estudo no Estado do Acre, por constituir o primeiro desta natureza em UTI, envolvendo S. aureus e MRSA.

A espécie Pyrostegia venusta é uma liana trepadeira popularmente conhecida como flor-de-São-João. É utilizada na medicina popular para o tratamento de diversas doenças. Na literatura científica, vários compostos bioativos já foram isolados e algumas atividades biológicas importantes, como a atividade antimicrobiana e imunomoduladora, já foram atribuídas a esta espécie. A cárie e a doença periodontalsão as doenças bucais mais prevalentes na população brasileira, sendo o biofilme dental o seu fator etiológico primário. A candidíase oral ocorre devido à infecção por espécies do gênero Candida que, em determinadas situações, podem se comportar como patógenos oportunistas. O objetivo deste trabalho foi realizar um estudo físico, químico e biológico da espécie vegetal Pyrostegia venusta e investigar seu potencial na prevenção das principais doenças bucais. Foram realizados ensaios de citotoxicidade do extrato bruto e frações em células mononucleares do sangue periférico e em macrófagos murinos. Para avaliar a atividade imunomoduladora do extrato bruto e frações, foi realizada dosagem de óxido nítrico (NO) produzido por macrófagos estimulados com lipopolissacarídeo (LPS). Foi realizado ensaio de microdiluição em caldo para determinar a concentração inibitória mínima (CIM), concentração bactericida mínima (CBM) e concentração fungicida mínima (CFM) do extrato bruto, frações e marcadores. Também foram avaliados o efeito na inibição da aderência de Streptococcus mutans em lamínula de vidro, o efeito na redução do pH e a capacidade de inibição do brotamento de Candida albicans do extrato bruto e frações. A espécie Pyrostegia venusta não foi citotóxica em baixas concentrações. A produção de NO foi inibida pelas frações acetato de etila e n-butanólica. O extrato bruto, frações e marcadores apresentaram atividade antimicrobiana contra Streptococcus mutans, Streptococcus mitis, Streptococcus oralis e Candida albicans, capacidade de inibição da aderência e redução do pH. O extrato bruto e frações hexânica, acetato de etila e n-butanólica apresentaram capacidade de inibição do brotamento. A espécie Pyrostegia venusta pode ser uma alternativa na prevenção e tratamento das principais doenças bucais. No entanto, mais estudos são necessários antes de considerá-la realmente promissora.

There are many infectious complications related to vascular access in patients undergoing maintenance hemodialysis. We report two cases of endophthalmitis as a metastatic infection associated with a tunneled catheter and a temporary dual lumen catheter. Both patients were diabetic. A 61-year-old female on maintenance hemodialysis by a jugular tunnelized catheter during the past year was receiving parenteral antibiotics for catheter salvage due to fever episodes in the last 3 months. She was admitted to the hospital presenting pain, proptosis, conjunctival hyperemia, corneal infiltrate, and visual acuity of no light perception (NLP). A 51-year-old male recently undergoing hemodialysis by a temporary dual lumen catheter presented fever. His catheter was removed, but he was admitted to the hospital presenting fever, decreased vision, edema, and pain in his left eye. On examination, eyelid edema, conjunctival hyperemia, purulent secretion, hypopyon in the pupils, and visual acuity of NLP were verified. A diagnosis of endogenous endophthalmitis was made in both patients on clinical grounds and computed tomography. Evisceration of the left eye was the first option of treatment for both patients due to poor vision. Cultures of the eviscerated ocular globes showed Staphylococcus hemolyticus and Staphylococcus aureus, respectively. After evisceration, both patients received treatment, had a good outcome, and were discharged to continue their hemodialysis program. Metastatic bacterial endophthalmitis is a rare complication of dialysis catheter-related bacteremia. When suspected, urgent ophthalmologic evaluation and treatment are needed to reduce the risk of losing vision in the affected eye.

Abstract Background Hepatitis C chronic liver disease is a major cause of liver transplant in developed countries. This article reports the first nationwide population-based survey conducted to estimate the seroprevalence of HCV antibodies and associated risk factors in the urban population of Brazil. Methods The cross sectional study was conducted in all Brazilian macro-regions from 2005 to 2009, as a stratified multistage cluster sample of 19,503 inhabitants aged between 10 and 69 years, representing individuals living in all 26 State capitals and the Federal District. Hepatitis C antibodies were detected by a third-generation enzyme immunoassay. Seropositive individuals were retested by Polymerase Chain Reaction and genotyped. Adjusted prevalence was estimated by macro-regions. Potential risk factors associated with HCV infection were assessed by calculating the crude and adjusted odds ratios, 95% confidence intervals (95% CI) and p values. Population attributable risk was estimated for multiple factors using a case–control approach. Results The overall weighted prevalence of hepatitis C antibodies was 1.38% (95% CI: 1.12%–1.64%). Prevalence of infection increased in older groups but was similar for both sexes. The multivariate model showed the following to be predictors of HCV infection: age, injected drug use (OR = 6.65), sniffed drug use (OR = 2.59), hospitalization (OR = 1.90), groups socially deprived by the lack of sewage disposal (OR = 2.53), and injection with glass syringe (OR = 1.52, with a borderline p value). The genotypes 1 (subtypes 1a, 1b), 2b and 3a were identified. The estimated population attributable risk for the ensemble of risk factors was 40%. Approximately 1.3 million individuals would be expected to be anti-HCV-positive in the country. Conclusions The large estimated absolute numbers of infected individuals reveals the burden of the disease in the near future, giving rise to costs for the health care system and society at large. The known risk factors explain less than 50% of the infected cases, limiting the prevention strategies. Our findings regarding risk behaviors associated with HCV infection showed that there is still room for improving strategies for reducing transmission among drug users and nosocomial infection, as well as a need for specific prevention and control strategies targeting individuals living in poverty.

Premature birth, chronic lung disease of prematurity (CLD), congenital heart disease and immunodeficiency predispose to a higher morbidity and mortality in respiratory syncytial virus (RSV) infection. This study describes the preterms hospitalised with RSV infection from the prospective German DSM RSV Paed database. The DMS RSV Paed database was designed for the prospective multicentre documentation and analysis of clinically relevant aspects of the management of inpatients with RSV infection. This study covers six consecutive RSV seasons (1999-2005); the surveillance took place in 14 paediatric hospitals in Germany. Of the 1,568 prospectively documented RSV infections, 26% (n=406) were observed in preterms [vs. 1,162 children born at term (74%)] and 3% (n=50) had CLD, of which 49 had received treatment in the last 6 months ('CLDplus'). A significantly higher proportion in the preterm group had congenital heart disease, nosocomial infection, and neuromuscular impairment. There were significantly more children older than 24 months in the preterm group. The attributable mortality was 0.2% (n=2) in children born at term vs. 1.2% (n=5) in the preterm group (p=0.015) [preterm plus CLD 8.0% (n=4 of 50); McIntosh grade 1, 8.6% (n=3 of 35) and McIntosh Grade 4, 15% (n=3 of 20)]. Eight patients were categorized as 'palivizumab failures'. In the multivariate analysis, premature birth, CLD(plus), and nosocomial infection were significantly and independently associated with the combined outcome 'complicated course of disease'. In conclusion, this is the first prospective multicentre study from Germany that confirms the increased risk for severe RSV disease in preterms, in particular in those with CLD treated in the last 6 months before the onset of the infection. From the perspective of our results, the statements of the German Society of Paediatric Infectious Diseases considering the use of passive immunisation (2003) seem reasonable.

Group B Streptococcus (GBS) is a leading cause of life-threatening infection in neonates and young infants, pregnant women, and non-pregnant adults with underlying medical conditions. Immunization has theoretical potential to prevent significant morbidity and mortality from GBS disease. Alpha C protein (α C), found in 70% of non-type III capsule polysaccharide group B Streptococcus, elicits antibodies protective against α C-expressing strains in experimental animals and is an appealing carrier for a GBS conjugate vaccine. We determined whether natural exposure to α C elicits antibodies in women and if high maternal α C-specific serum antibody at delivery is associated with protection against neonatal disease. An ELISA was designed to measure α C-specific IgM and IgG in human sera. A case-control design (1:3 ratio) was used to match α C-expressing GBS colonized and non-colonized women by age and compare quantified serum α C-specific IgM and IgG. Sera also were analyzed from bacteremic neonates and their mothers and from women with invasive GBS disease. Antibody concentrations were compared using t-tests on log-transformed data. Geometric mean concentrations of α C-specific IgM and IgG were similar in sera from 58 α C strain colonized and 174 age-matched non-colonized women (IgG 245 and 313 ng/ml; IgM 257 and 229 ng/ml, respectively). Delivery sera from mothers of 42 neonates with GBS α C sepsis had similar concentrations of α C-specific IgM (245 ng/ml) and IgG (371 ng/ml), but acute sera from 13 women with invasive α C-expressing GBS infection had significantly higher concentrations (IgM 383 and IgG 476 ng/ml [p=0.036 and 0.038, respectively]). Convalescent sera from 5 of these women 16-49 days later had high α C-specific IgM and IgG concentrations (1355 and 4173 ng/ml, respectively). In vitro killing of α C-expressing GBS correlated with total α C-specific antibody concentration. Invasive disease but not colonization elicits α C-specific IgM and IgG in adults. Whether α C-specific IgG induced by vaccine would protect against disease in neonates merits further investigation. ^

Non routine hospital settings are those that are infrequently used in hospitals and that often do not come to mind when sanitation and disinfection practices are used. These settings are a major source of nosocomial, or hospital acquired, infections, and are often overlooked. Data on these sources are often scattered and scarce, but these sources are significant such that they warrant equal attention of commonly recognized nosocomial infection sources in order to help reduce incidence of nosocomial infections. ^

This investigation was designed as a hospital-based, historical cohort study. The objective of the study was to determine the association between premature rupture of the membranes (PROM) and its duration on neonatal sepsis, infection, and mortality. Neonates born alive with gestational ages between 25 and 35 weeks from singleton pregnancies complicated by PROM were selected. Each of the 507 neonates was matched on gestational age, gender, ethnicity, and month of birth with a neonate without the complication of PROM.^ Data were abstracted from deliveries between January 1979 and December 1985 describing the mother's demographics, labor and delivery treatments and complications, the neonate's demographics, infection status, and medical care. The matched pairs analysis reveals a significant increase in risk of neonatal sepsis (RR = 3.5) and neonatal infection (RR = 2.4) among preterm births complicated by PROM, with a PROM exposure contributing an excess 4 to 5 cases of sepsis per 100 infants (RD = 0.04 for infection and RD = 0.05 for sepsis). Generally PROM remains an important risk factor for sepsis and infection when controlling for various other characteristics, and the risk difference remains constant.^ PROM was not significantly associated with neonatal mortality (RR = 1.02). There is an increase in risk difference for mortality associated with PROM among septic and infected infants, but it is not significant.^ A clear increase in risk of sepsis and infection from PROM occurs when durations of PROM are long (more than 48 hours), e.g., for sepsis the RR is 2.42 for short durations and RR is 6.0 for long durations. No such risk with long duration appears for neonatal mortality.^ This study indicates the importance of close observation of neonates with PROM for sepsis and infection so treatment can be initiated early. However, prematurity is the major risk for sepsis and the practice of early delivery to avoid prolonged durations of PROM does not alter the magnitude of risk. The greatest protection against these infection complications was provided when the neonate weighed over 1500 grams or had more than 33 weeks gestation. ^

Introduction: The production of KPC (Klebsiella pneumoniae carbapenemase) has become an important mechanism of carbapenem-resistance among Enterobacteriaceae strains. In Brazil, KPC is already widespread and its incidence has increased significantly, reducing treatment options. The “perfect storm” combination of the absence of new drug developmentand the emergence of multidrug-resistant strains resulted in the need for the use of older drugs, with greater toxicity, such as polymyxins. Aims: To determine the occurrence of carbapenemase-producing strains in carbapenem-resistant Enterobacteriaceae isolated from patients with nosocomial infection/colonization during September/2014 to August/2015, to determine the risk factors associated with 30-day- mortality and the impact of inappropriate therapy. Materials and Methods: We performed a case control study to assess the risk factors (comorbidities, invasive procedures and inappropriate antimicrobial therapy) associated with 30-day-mortality, considering the first episode of infection in 111 patients. The resistance genes blaKPC, blaIMP, blaVIM and blaNDM-1 were detected by polymerase chain reaction technique. Molecular typing of the strains involved in the outbreak was performed by pulsed field gel electrophoresis technique. The polymyxin resistance was confirmed by the microdilution broth method. Results: 188 episodes of carbapenem-resistant Enterobacteriaceae infections/colonizations were detected; of these, 122 strains were recovered from the hospital laboratory. The presence of blaKPC gene were confirmed in the majority (74.59%) of these isolates. It was not found the presence of blaIMP , blaVIM and blaNDM-1 genes. K. pneumoniae was the most frequent microorganism (77,13%), primarily responsible for urinary tract infections (21,38%) and infections from patients of the Intensive Care Unit (ICU) (61,38%). Multivariate statistical analysis showed as predictors independently associated with mortality: dialysis and bloodstream infection. The Kaplan-Meier curve showed a lower probability of survival in the group of patients receiving antibiotic therapy inappropriately. Antimicrobial use in adult ICU varied during the study period, but positive correlation between increased incidence of strains and the consumption was not observed. In May and July 2015, the occurrence rates of carbapenem-resistant Enterobacteriaceae KPC-producing per 1000 patient-days were higher than the control limit established, confirming two outbreaks, the first caused by colistin-susceptible KPC-producing K. pneumoniae isolates, with a polyclonal profile and the second by a dominant clone of colistin-resistant (≥ 32 μg/mL) KPC-producing K. pneumoniae. The cross transmission between patients became clear by the temporal and spatial relationships observed in the second outbreak, since some patients occupied the same bed, showing problems in hand hygiene adherence among healthcare workers and inadequate terminal disinfection of environment. The outbreak was contained when the ICU was closed to new admissions. Conclusions: The study showed an endemicity of K. pneumoniae KPC-producing in adult ICU, progressing to an epidemic monoclonal expansion, resulted by a very high antibiotic consumption of carbapenems and polymyxins and facilitated by failures in control measures the unit.
