Background/Aims: Patients with chronic liver disease undergoing liver transplantation have reduced body fat and muscle mass. The extent to which nutritional indicators and Child-Pugh class are predictive of postoperative outcome in adults is unclear. The aims of this study were to determine in adult patients undergoing transplant 1) the influence of preoperative Child-Pugh class and nutritional indicators on early transplant outcomes and one-year survival, 2) the relationship between nutritional indicators and Child-Pugh class and disease type. This study included 80 patients (1990-1994). Methodology: The nutritional indicators utilized were grip strength, triceps skinfold thickness and uncorrected mid-arm muscle area. Measured outcomes were ventilator time, intensive care stay, postoperative hospital stay and one-year survival. Results: Early morbidity was determined in survivors. Child-Pugh class C patients required longer ventilation and spent more time in the intensive care unit than Child-Pugh classes A and B. No significant relationships were found length of hospital stay. Relationships between the nutritional indicators (when controlled for Child-Pugh class) and early morbidity could not be determined due;to insufficient data. No relationship was established between one-year survival and Child-Pugh class or the nutritional indicators. Grip strength and mid-arm muscle area were lower in the patients in Child-Pugh:classes B and C. Parenchymal liver disease was associated with lower grip strength and mid-arm muscle area when compared to cholestatic disease. Conclusions: Child-Pugh class C is associated with greater early postoperative morbidity. Advanced Child-Pugh class is also associated with diminished muscle status and parenchymal disease.

Patients with chronic liver disease (CLD) are catabolic and GH-resistant. The effects of supraphysiological recombinant human GH (rhGH; 0.2 IU.kg(-1).d(-1)) treatment in adults with CLD were assessed in a randomized, double-blind, placebo-controlled cross-over trial (4-wk dietary run-in, 4-wk treatment, and 2-wk wash-out phases). Nine adults with mild- to moderate-severity CLD participated (median age, 49 yr; three males and six females; Child's classification A in six and B in three). Biopsy-proven etiologies were: alcohol (four patients), primary biliary cirrhosis (three patients), non-A, non-B, non-C hepatitis (one patient), and cryptogenic (one patient). Treatment with rhGH increased serum IGF-I (median increase over placebo, +93 mug.liter(-1); P = 0.004), IGF-binding protein-3 (+0.9 mg.liter(-1): P = 0.004), and acid labile subunit (+10.7 nM; P = 0.004). Total body potassium (+8.0 g; P = 0.023), body weight (+1.6 kg; P = 0.008), and total body water (by bioelectrical impedance; +4.9 kg; P = 0.004) increased. Resting metabolic rate (+313 ml.kg(-1).min(-1); P = 0.004) and lipid oxidation (+1072.0 kcal.d(-1); P = 0.032) increased. Metabolic changes included increased fasting plasma glucose (+1.2 mm; P = 0.008), insulin (+33.8 mU.liter(-1); P = 0.004), C-peptide (+0.7 nM; P = 0.004), and free-fatty acids (+0.1 mEq.liter(-1); P = 0.04). Clinical side effects included worsening edema and ascites. Hepatocellular function did not change. Therefore, rbGH treatment in CLD: 1) overcame hepatic GH resistance; 2) may have improved whole-body protein catabolism; 3) increased lipolysis and lipid oxidation; 4) increased insulin resistance; and 5) had potent antinatriuretic effects. Long-term safety and efficacy require further assessment.

Parrondo's paradox arises when two losing games are combined to produce a winning one. A history-dependent quantum Parrondo game is studied where the rotation operators that represent the toss of a classical biased coin are replaced by general SU(2) operators to transform the game into the quantum domain. In the initial state, a superposition of qubits can be used to couple the games and produce interference leading to quite different payoffs to those in the classical case. Background and Aims: Zomepirac (ZP), a non-steroidal anti-inflammatory drug (NSAID), has been reported to cause immune-mediated liver injury. In vivo, ZP is metabolized to a chemically reactive acyl glucuronide conjugate (ZAG) which can undergo covalent adduct formation with proteins. Such acyl glucuronide-derived drug-protein adducts may be important in the development of immune and toxic responses caused by NSAID. We have shown using immunoabsorptions that the 110 kDa CD26 (dipeptidyl peptidase IV) is one of the hepatic target proteins for covalent modification by ZAG. In the present study, a CD26-deficient mouse strain was used to examine protein targets for covalent modification by ZP/metabolites in the liver. Methods and Results: The CD26-deficient phenotype was confirmed by immunohistochemistry, flow cytometry analysis, RT-PCR, enzyme assay and immunoblotting. Moreover, by using monoclonal antibody immunoblots, CD26 was not detected in the livers of ZP-treated CD26-deficient mice. Immunoblots using a polyclonal antiserum to ZP on liver from ZP-treated mice showed three major sizes of protein bands, in the 70, 110 and 140 kDa regions. Most, but not all, of the anti-ZP immunoreactivity in the 110 kDa region was absent from ZP-treated CD26-deficient mice. Conclusion: These data definitively showed that CD26 was a component of ZP-modified proteins in vivo. In addition, the data suggested that at least one other protein of approximately 110 kDa was modified by covalent adduct formation with ZAG. (C) 2002 Blackwell Science Asia Pty Ltd.

O presente trabalho reúne os elementos que compõem a atual concepção de assistência social no Brasil, a partir da promulgação da constituição de 1988, quando a assistência social foi reconhecida pela primeira vez como direito de cidadania e dever legal do Estado, garantido pela Lei Suprema. Nesta lei, a assistência social pressupunha uma lógica de pleno emprego, destinada, portanto, prioritariamente aos incapazes para o trabalho. No entanto, em um contexto de desemprego estrutural esta passa a ser compreendida em termos de garantias de seguranças, buscando assumir a proteção social daqueles capazes para o trabalho, tendo em vista a deterioração do mercado de trabalho, restrição de oportunidades e de renda e o crescimento progressivo do desemprego e da informalidade. A ideia central é a de que se trata de uma descrição crítica da concepção de assistência social no Brasil, problematizando cada um de seus argumentos mais explícitos com o intuito de revelar uma intencionalidade vinculada à uma perspectiva de Estado. Utilizamos o termo concepção no sentido de conceber, pensar, sentir, entender ou interpretar algo. A assistência social, na atualidade, responde a um único processo que reúne aspectos históricos, econômicos, políticos, sociais e ideológicos e neste sentido, representa uma concepção de mundo e um projeto de sociedade, defendido pela classe dominante, pautado pela exploração do trabalho. A atual concepção de assistência social segue, portanto, uma nova forma de política social a partir da perspectiva de desenvolvimento humano e combate à pobreza em que a grande ênfase tem sido a de retirar as discussões e a intervenção na pobreza do âmbito da questão social, alocando-a nos indivíduos e em suas “incapacidades”. A assistência social ao assumir a responsabilidade ou coresponsabilidade no desenvolvimento de capacidades dos indivíduos sinaliza a tendência de uma nova concepção de bem-estar social.

Seguindo-se uma sistemática epidemiológica, foram estudados os trabalhadores de 3 fiações de algodão, com diferentes quantidades de poeira de algodão em suspensão na atmosfera dos locais de trabalho, comparando-se com 135 pessoas do grupo controle não exposto a essas poeiras. Pelas respostas ao questionário padrão internacional sobre doenças respiratórias, verificou-se que os trabalhadores das fábricas, onde era mais elevada a concentração de poeira de algodão, apresentavam um ou mais sintomas característicos da bissinose; por outro lado, as provas de função pulmonar, realizadas imediatamente antes e após o início do trabalho às segundas-feiras, mostravam-se alteradas. Como no grupo controle, verificou-se melhoria significativa da função pulmonar no término do dia de trabalho. E como tal fato não foi observado mesmo na fábrica onde a concentração de poeira no ar era muito baixa e onde não se notou piora das provas de função pulmonar entre seus trabalhadores, levantou-se a hipótose de que essa ausência de melhoria possa constituir um sintoma precoce da bissinose.

Estudaram-se 450 escolares da periferia de Londrina, PR, Brasil, com o objetivo de demonstrar uma possível associação entre desnutrição e aproveitamento escolar. Destes 450 escolares, 227 eram do sexo masculino e 223 do feminino. A incidência de desnutrição foi de 34,89%. Das crianças desnutridas, apenas 56,69% obtiveram rendimento satisfatório, em comparação aos 73,88% das crianças eutróficas, demonstrando uma relação significativa entre desnuirição e aproveitamento escolar. As crianças desnutridas provinham de famílias com baixa renda mensal bruta per capita em maior proporção que as eutróficas. O aproveitamento escolar mostrou estar mais relacionado ao gasto mensal per capita em alimentação do que a renda mensal bruta per capita.

São apresentados resultados sobre a infecção experimental de Calomys callosus (Rodentia) e duas cepas (Y e Berenice) de Trypanosoma cruzi, isoladas de casos humanos. O estudo da evolução foi feito comparado com Mus musculus albino cepa "Swiss", quanto a prepatência, parasitemia, patência e letalidade. Análise histopatológica foi também conduzida em C. callosus, com o objetivo de verificar o tropismo tissular e agressividade das cepas neste roedor. Os experimentos mostraram que a evolução da infecção em C. Callosus foi diferente para as duas cepas de T. cruzi. A cepa Y apresentou maior parasitemia do que a cepa Berenice. O período prepatente variou com as doses utilizadas tendo sido mais curto nos animais inoculados com a cepa Y (2, 2-5, 2 dias) do que naquelas com a cepa Berenice (3, 2-7 dias). Embora as duas cepas inoculadas nos C. callosus tenham-se mostrado miotrópicas, as alterações tissulares foram mais acentuadas com a Y. Os resultados obtidos abrem perspectivas quanto à possibilidade do uso de C. callosus como animal experimental para T. cruzi.

São apresentados resultados em relação a infecção expertmental de Zygodontomys lasiurus (Rodentia-Cricetidae) com duas cepas de T. cruzi isoladas de casos humanos, Y e Berenice, e uma isolada de um triatomíneo silvestre, chamada costalimai. Foram realizados estudos em relação a evolução da parasitemia, duração da patência e prepatência da infecção. Com o objetivo de verificar a agressividade e tropismo tissular das cepas de T. cruzi nesta espécie de roedor, foram também realizados estudos histopatológicos. Os resultados obtidos mostraram que os níveis de parasitemias foram baixos para as três cepas estudadas. A patência da infecção variou de 14 a 16 dias nos animais inoculados com a cepa Y, 26 a 29 dias com a Berenice e 9 a 13 dias com a costalimai. O período prepatente variou de 3 a 5 dias nos animais inoculados com a cepa Y, de 2 a 6 dias com a cepa Berenice e de 6 a 8 dias com a costalimai. As três cepas apresentaram em Z. lasiurus, comprometimento nitidamente muscular, provocando reações leves, moderadas e intensas.

The reaction of nine vector species of Chagas' disease to infection by seven different Trypanosoma cruzi strains; Berenice, Y, FL, CL, S. Felipe, Colombiana and Gávea, are examined and compared. On the basis of the insects' ability to establish and maintain the infection, vector species could be divided into two distinct groups which differ in their reaction to an acute infection by T. cruzi. While the proportion of positive bugs was found to be low in Triatoma infestans and Triatoma dimidiata it was high, ranging from 96.9% to 100% in the group of wild (Rhodnius neglectus, Triatoma rubrovaria)and essentially sylvatic vectors in process of adaptation to human dwellings, maintained under control following successful insecticidal elimination of Triatoma infestans (Panstrongylus megistus, Triatoma sordida and Triatoma pseudomaculata). An intermediate position is held by Triatoma brasiliensis and Rhodnius prolixus. This latter has been found to interchange between domestic and sylvatic environments. The most important finding is the strikingly good reaction between each species of the sylvatic bugs and practically all T. cruzi strains herein studied, thus indicating that the factors responsible for the excellent reaction of P.megistus to infection by Y strain, as previously reported also come into operation in the reaction of the same vector species to acute infections by five of the remaining T.cruzi strains. Comparison or data reported by other investigators with those herein described form the basis of the discussion of Dipetalogaster maximus as regards its superiority as a xenodiagnostic agent.

Previous studies (1982,1987) have emphasized the superiority of sylvatic vector species over domestic species as xenodiagnostic agents in testing hosts with acute or chronic infections by T. cruzi "Y" stock. The present study, which is unique in that it contains data on both infectivity rates produced by the same stock in 11 different vector species and also the reaction of the same vector species to seven different parasite stocks, establishes the general validity of linking efficiency of xenodiagnosis to the biotope of its agent. For example, infectivity rates produced by "São Felipe" stock varied from 82.5% to 98.3% in sylvatic vectors but decreased to 42.5% to 71.3% in domestic species. "Colombiana" stock produced in the same sylvatic vectors infectivity rates ranging from 12.5% to 45%. These shrank to 5%-22.5% in domestic bugs. The functional role of the biotope in the vector-parasite interaction has not been eluddated. But since this phenomenon has been observed to be stable and easy to reproduce, it leads us to believe that the results obtained are valid. Data presented also provide increasing evidence that the infectivity rates exhibited by bugs from xenodiagnosis in chronic hosts, are parasite stock specific. For example, infectivity rates produced by "Berenice", "Y", "FL" and "CL" varied in R. neglectus from 26.3% to 75%; in P. megistus from 56.3% to 83.8%; in T. sordida from 28.8% to 58.8% in T. pseudomaculata from 41.3% to 66.3% and in T. rubrovaria from 48.8% to 85%. Data from xenodiagnosis in the same hosts, carrying acute infections by the same parasite stocks, gave the five sylvatic vectors a positive rating of approximately 100%, thus suggesting that the heavy loads of parasites circulating in the acute hosts obscured the characteristic interspecific differences for the parasite stock. Nonetheless these latter were revealed in the same hosts with chronic infections stimulated by very low numbers of the same parasite stocks. Certain observations here described lead us to speculate as to the possibility of further results from other parasite stocks, allowing the association of the infectivity rates produced in bugs by different parasite stocks with the isoenzymic patterns revealed by these stocks.
