Developmental stammering (DS, also known as idiopathic stammering or stuttering) is a disorder of speech fluency that affects approximately 0.75% to 1% of the populations of Great Britain, Australia and America,(1-4) although a recent study puts the point prevalence figure at between 1% and 3% in the UK.(5) Prevalence is generally thought to be similar amongst communities worldwide, although there have been occasional suggestions that this figure might be lower in countries where there is less pressure on verbal acuity.(6) DS may be distinguished from neurogenic stammering, which can occur subsequent to neurological damage of various aetiologies (for example, stroke, tumour, degenerative disease) and psychogenic stammering, whose onset can be related to a significant psychological event such as bereavement. While a diagnosis of neurogenic stammering might be made in early childhood and adolescence, both neurogenic and psychogenic types are typically associated with an adult onset. DS is by far the most common form of stammering and usually develops in the pre-school years. The mean age at onset is 4 2, with 75% of cases beginning before the age of 6.(1) However, occasionally, stammering onset may be seen as late as 12 or 13 years of age.

The study examined the effects of psychological response and gender on coping with late life widowhood. Forty-six men and 46 women (55 years +) were interviewed about their experiences of widowhood. Participants were classified as to whether they were coping well or less well. Data were analyzed using grounded theory, content. analysis, and three-way loglinear analyses. Loglinear analyses revealed three-way interactions for Gender, Coping, and Response. Men who report feeling upset or selfish are more likely to be coping, as are women who report being comfortable alone. There were two-way interactions between Coping and Response and Gender and Response. Participants who talk to their dead spouse are more Rely to be coping than those who do not. Those who "keep themselves to themselves" are more likely not to be coping than those who do not. Gender differences, were found in psychological response. Differences were also found between those who coped and those who coped less well. The study has enabled the synthesis of quantitative and qualitative data to present a more complete view of late life widowhood than has previously been possible. In addition, the article draws attention to the importance of distinguishing between the effects of bereavement and those of widowhood.

Despite diversity in family dynamics within and between societies, globally, it is adults who are usually relied upon to care for family members who are sick, disabled or have other care needs. Young people in Zambia and other African countries affected by the HIV epidemic are under increasing pressure to obtain a good education and employment to support their families, whilst some also have to cope with the loss of parents and care for relatives with little external support. This article discusses the findings of qualitative research that explored the experiences of young people (aged 14-30) who had significant family caring responsibilities and those without such responsibilities in Zambia. Interviews and life-mapping methods were conducted with a total of 35 young people living in rural and urban areas, 12 parents and relatives and 12 professionals. We analyse young people's experiences and perceptions of socially expected transitions, such as completing education and earning an income to support themselves and their families, in addition to more unpredictable changes in young people's family lives. ‘Critical moments’ (Thomson et al, 2002), such as bereavement and loss of parents and other family members, disinheritance of assets and property grabbing, migration and mobility between different relatives homes, parental divorce and separation, often had significant impacts on young people’s ability to navigate their pathways to adulthood according to wider social norms and expectations. A more relational conceptualisation of youth transitions is needed that takes account of young people's caring responsibilities and changing family dynamics.

Nowadays, the act of mourning configures as a circumstance of isolation and social anguish due to the impossibility to express the pain by those who suffer the loss of a dear one. At the same time, the symbolic relations established between society and the cemetery contains interpretations about death, pain and fear which confer to the visits a special and individual feature. In this context, this dissertation tries to analyse the formation of a sociability observed among visitors of the Morada da Paz a private cemetery located in Natal/RN as well as the frequency of the visitors to the necropolis. Therefore, the research was accomplished from the observation of the existent relations among these actors whose meetings have procured links based on the experiences´ exchange about pain and bereavement. In this sense, this study tries to analyse, as its principal point, in which way the Morada da Paz space and the relations established there are being appropriate by the visitors in their mourning process, trying to understand what are the role of these bond in the elaboration of the loss for these people

Este estudo tem por objetivo compreender os significados atribuídos por profissionais que atuam em enfermaria pediátrica sobre o cuidar da criança com doença sem possibilidade de cura, hospitalizada e em processo de morte. A estratégia metodológica foi fundamentada na abordagem qualitativa, que corresponde a um método preocupado com as singularidades e particularidades de um objeto, sem a pretensão de generalizações ou de verdades absolutas quanto aos resultados encontrados. A pesquisa foi desenvolvida na Clínica Assistencial Pediátrica do Hospital Universitário João de Barros Barreto, vinculado à Universidade Federal do Pará, em Belém-Pa. Colaboraram com a pesquisa doze (12) profissionais, sendo 3 Médicos, 1 Psicólogo, 1 Terapeuta Ocupacional, 2 Enfermeiros, 1 Fisioterapeuta, 1 Assistente Social e 3 Técnicos em Enfermagem que lidam diariamente com o processo de morrer de crianças internadas nesta instituição. Como instrumento para coleta dos dados foi utilizado a entrevista semi-dirigida, sendo realizada a análise de conteúdo temática, por meio da qual foram identificados três temas centrais: A Negação e Interdição da Morte; Apegos e a Experiência do Luto e Formação para Paliar. Para os colaboradores paliar é uma árdua tarefa envolvendo todo cuidado direcionado à criança sem possibilidades terapêuticas curativas. Contudo, na impossibilidade de evitar a morte da criança os profissionais podem vivenciar intenso sofrimento, o que favorece as ações obstinadas para manutenção da vida, a negação e interdição da morte. Tal como pais apegados e cuidadosos para com seus filhos, na iminência de morte ou óbito da criança, os profissionais vivenciam sentimentos característicos do luto. Em relação à formação do profissional de saúde destaca-se a ausência de disciplinas abordando o tema da morte e do morrer durante os anos da graduação, chamando atenção para a necessidade da inclusão dessas nos currículos da graduação. Os achados sugerem o estranhamento frente à morte da criança e a vivência do luto não autorizado, corroborando em favor das ações de educação para vida e para morte.

Palliative care settings in many countries acknowledge families as their prime focus of care, but in Brazil, to date, researchers have devoted scant attention to that practice setting. In this article, we report the findings of a study that explored how families define and manage their lives when they have a child or adolescent undergoing palliative care at home. Data included individual semistructured interviews with 14 family members of 11 different families. Interviews were transcribed and the coding procedure featured qualitative content analysis methods. The deductive coding was based on the major components of the Family Management Style Framework and the eight dimensions comprising these components. The analysis provides insight into families' daily practices and problems inherent in managing their everyday lives that are encountered when they have a child in palliative care. The article features discussion of implications for the palliative care related development of family nursing practice.

En este estudio se han investigado algunas líneas poéticas de la poesía chilena desde la vanguardia hasta los Ochenta. La perspectiva asumida ha dado relevancia a algunas de las obras que más claramente han instaurado una relación profunda con su propio tiempo y que han incorporado en su enunciación y retórica fenómenos vinculados con los eventos socio-culturales y con los procesos histórico-políticos. Se han analizado algunas obras poéticas y de carácter crítico de Vicente Huidobro, Nicanor Parra, Enrique Lihn, Juan Luis Martínez y Raúl Zurita. En su corpus textual se ha podido verificar la formulación de actos lingüísticos que connotan el sentido de la poesía como vehículo privilegiado para la producción de significantes y contenidos. La concepción de la poesía como testimonio y memoria escritural de una comunidad hablante, así como de desafío a la memoria del lector, son las premisas que articulan el trabajo: la relación entre escritura y realidad, entre escritura e ideología y entre las distintas declinaciones de la cuestión en torno al binomio arte-vida. La componente ética que caracteriza en particular las obras de Lihn y Zurita, surge de la urgencia de elaborar el trauma del golpe de Estado, de la violencia, de la pérdida y la marginación social.

This review deals with an important aspect of organ transplantation, namely the process of psychic organ integration and organ-related fantasies. The body schema and body self are two important concepts in the integration of a transplanted organ. Different models and theories on organ integration are presented and will be discussed. There is evidence that beside the emotional impact and the influence on well-being, organ integration depends closely on psychic processes involving in the incorporation of the transplanted organ and the respective organ-related fantasies. Therefore, these organ fantasies - whether unconscious or conscious - may play an important role in the future development of the instinctive and highly individual relation the patients elaborate with the new organ. Beside the concern with the new organ, a bereavement to the lost old and sick organ may also influence the patients thoughts. Moreover, the good resolving of all these issues evokes the "good practice" patients develop towards the new situation. This will bring up issues as compliance, infections, rejection episodes and - most important - also organ survival.

Bisherige Forschung hat die Verwitwung entweder primär als soziales oder als individuelles Ereignis untersucht, selten jedoch wurden beide Perspektiven verbunden. Zudem ist wenig darüber bekannt, inwiefern bisherige Forschungsergebnisse Perioden- oder Kohorteneffekte wiederspiegeln. In diesem Beitrag wird die persönliche Bilanzierung nach der Verwitwung älterer Schweizer Frauen und Männer im Geschlechterund Zeitvergleich untersucht1. Die Datenbasis beruht auf Befragungen von 1.197 verwitweten Frauen und Männern (Alter: 65-102 Jahre), welche 1979, 1994 und 2011 durchgeführt wurden. Während sich die wirtschaftlichen und sozialen Rahmenbedingungen nach einer Verwitwung – namentlich bei Frauen – im Zeitvergleich verbessert haben, zeigen sich bezüglich psychischer Herausforderungen einer Verwitwung keine periodenspezifischen Veränderungen. Psychisch bleibt der Partnerverlust auch bei günstigen Sozialbedingungen ein kritisches Lebensereignis, das individualisiert bewältigt werden muss.

This working report gives an overview of the Individual Project 12 “Vulnerability and growth. Developmental dynamics and differential effects of the loss of an intimate partner in the second half of life” of the Swiss National Centre of Competence in Research LIVES led by Pasqualina Perrig-Chiello, University of Bern. This longitudinal and interdisciplinary project aims at examining vulnerability and personal growth after a critical life event, namely the break-up of a long-term intimate relationship in the second half of life, be it due to divorce or due to bereavement. In this report we present details about the rationale, the main research questions, the hypotheses and the methods of the study. Special attention is given to the methodological approach. The authors give a first sample description and report on the validity of the data by comparing the sample with Swiss Labour Force Survey and Swiss Health Survey data.

Spousal loss is an inevitable critical life event for most individuals in old age, mostly associated with a negative impact on various well-being measures, ie. lower life satisfaction, higher rates of loneliness and depressive symptoms compared to married peers. While the negative effects on well-being are well documented in literature, the modifying factors accounting for the large variability in adaptation to loss are discussed controversially. The potential relevance of personality in the adaptation process has rarely been examined and findings regarding the role of time since loss are contradictory. Based on a vulnerability-stress-model this contribution aims a) to compare psychological well-being of bereaved individuals with married counterparts and b) to investigate the protective effects of different personality traits (Big Five, resilience), and the role of time since loss for adaptation in terms of life satisfaction, loneliness and depression. Data from a questionnaire study about the loss of a spouse in middle and old age in the German- and French-speaking parts of Switzerland are reported. The study is part of the Swiss National Centre of Competence in Research LIVES (Swiss National Science Foundation). The sample consists of 351 widowed persons (39% men, widowed since 0 - 5 years), and 605 married controls (50% men), aged 60 - 89 years. Group comparisons reveal the detrimental effect of spousal bereavement on all indicators of psychological adaptation. Results from hierarchical regression analyses show furthermore, that the effect of spousal loss on all psychological outcomes is moderated by personality traits. Separate analyses with the group of bereaved individuals suggest, that the protective effect of personality varies by the time passed since loss. Our results contribute to a better understanding of the variability in psychological adaptation to spousal loss in old age and give hints for counselling practice.

Objectives. Despite the large body of literature on bereavement, little is known about the impact of sociohistorical context on individual reactions to spousal loss. This study examines the effect of marital status, time period and gender on physical and mental health, and whether reported difficulties following spousal loss differ at 2 distinctive time periods. Method. Two cohorts of older bereaved persons (n = 753) in Switzerland, surveyed in 1979 and 2011, were compared regarding their reports of difficulties related to marital loss. The bereaved spouses were also compared with a group of married contemporaries (n = 1,517) regarding subjective health and depressive symptoms. Results. Marital status and gender each have independent effects on subjective health and depressive symptoms. The effects of widowhood on subjective health differed significantly at both time points. Widowed individuals in 2011, especially women, reported fewer social and financial difficulties than their counterparts in 1979. However, the effect of widowhood on depressive symptoms and psychological difficulties did not differ significantly across time points. Discussion. Social changes in the late 20th century may be protective for older adults’ physical, social, and financial well-being in the face of spousal loss, yet these changes do not alleviate widow(er)s’ psychological distress.

Marital split-up and spousal loss are among the most stressful critical life events. Numerous studies have documented their detrimental effects on well-being, yet the large individual differences in psychological adaptation are still not well understood. Whereas in old age bereavement is normative and can be anticipated, divorce is an “off-time” transition for this age group. In contrast to bereavement which has been amply studied, research on later life divorce is still missing despite the increasing relevance of the topic due to the significant increase of divorces in older age. Based on a modified and extended view of Amato’s divorce-stress-adjustment model (2000), the aim of this contribution is to explore the differential impact of marital split-up and widowhood in older age on psychological (life satisfaction) and social well-being (social loneliness), and the adaptation to these critical life events. Our analyses are based on data gathered in a questionnaire study, which is part of the Swiss National Centre of Competence in Research LIVES. In a first step we compared three groups of individuals aged 60 to 75 years: a sample of 251 persons with a marital split-up (127 women; 123 men), a sample of 270 widowed persons (170 women; 100 men), and a group of 221 continuously married people (110 women; 111 men), which served as control group. In a second step, we investigated the role of socio-demographic variables, intrapersonal and interpersonal resources and variables of the context of loss as predictors for the psychological adaptation to a marital break-up and loss in old age. First results by ANCOVA indicate significant differences with regard to life satisfaction among the three groups, with divorced persons with the lowest scores, followed by the bereaved ones, and the married controls with the highest. Regarding social loneliness, divorced individuals report higher social loneliness than the bereaved group and the married controls (no significant difference between widowed and the married). In both loss groups, financial and intrapersonal resources, as well as the emotional valence of the loss are the most important predictors for the psychological and social adaptation. However, happiness in the past relationship is an important resource regarding the indicators for adaptation for the widowers, but not for individuals with a marital dissolution.
