OBJECTIVES: The goal was to test 2 hypotheses: first, that coronary endothelial function can be measured noninvasively and abnormal function detected using clinical 3.0-T magnetic resonance imaging (MRI); and second, that the extent of local coronary artery disease (CAD), in a given patient, is related to the degree of local abnormal coronary endothelial function. BACKGROUND: Abnormal endothelial function mediates the initiation and progression of atherosclerosis and predicts cardiovascular events. However, direct measures of coronary endothelial function have required invasive assessment. METHODS: The MRI was performed in 20 healthy adults and 17 patients with CAD. Cross-sectional coronary area and blood flow were quantified before and during isometric handgrip exercise, an endothelial-dependent stressor. In 10 severe, single-vessel CAD patients, paired endothelial function was measured in the artery with severe stenosis and the contralateral artery with minimal disease. RESULTS: In healthy adults, coronary arteries dilated and flow increased with stress. In CAD patients, coronary artery area and blood flow decreased with stress (both p </= 0.02). In the paired study, coronary artery area and blood flow failed to increase during exercise in the mildly diseased vessel, but both area (p = 0.01) and blood flow (p = 0.02) decreased significantly in the severely diseased, contralateral artery. CONCLUSIONS: Endothelial-dependent coronary artery dilation and increased blood flow in healthy subjects, and their absence in CAD patients, can now be directly visualized and quantified noninvasively. Local coronary endothelial function differs between severely and mildly diseased arteries in a given CAD patient. This novel, safe method may offer new insights regarding the importance of local coronary endothelial function and improved risk stratification in patients at risk for and with known CAD.

In Quantitative Microbial Risk Assessment, it is vital to understand how lag times of individual cells are distributed over a bacterial population. Such identified distributions can be used to predict the time by which, in a growth-supporting environment, a few pathogenic cells can multiply to a poisoning concentration level. We model the lag time of a single cell, inoculated into a new environment, by the delay of the growth function characterizing the generated subpopulation. We introduce an easy-to-implement procedure, based on the method of moments, to estimate the parameters of the distribution of single cell lag times. The advantage of the method is especially apparent for cases where the initial number of cells is small and random, and the culture is detectable only in the exponential growth phase.

This study aimed at analyzing nipple trauma resulted from breastfeeding based on dermatological approach. Two integrative reviews of literature were conducted, the first related to definitions, classification and evaluation methods of nipple trauma and another about validation studies related to this theme. In the first part were included 20 studies and only one third defined nipple trauma, more than half did not defined the nipple’s injuries reported, and each author showed a particular way to assess the injuries, without consensus. In the second integrative review, no validation study or algorithm related to nipple trauma resulted from breastfeeding was found. This fact demonstrated that the nipple’s injuries mentioned in the first review did not go through validation studies, justifying the lack of consensus identified as far as definition, classification and assessment methods of nipple trauma.

BACKGROUND: Teaching of evidence-based medicine (EBM) has become widespread in medical education. Teaching the teachers (TTT) courses address the increased teaching demand and the need to improve effectiveness of EBM teaching. We conducted a systematic review of assessment tools for EBM TTT courses.To summarise and appraise existing assessment methods for teaching the teachers courses in EBM by a systematic review. METHODS: We searched PubMed, BioMed, EmBase, Cochrane and Eric databases without language restrictions and included articles that assessed its participants. Study selection and data extraction were conducted independently by two reviewers. RESULTS: Of 1230 potentially relevant studies, five papers met the selection criteria. There were no specific assessment tools for evaluating effectiveness of EBM TTT courses. Some of the material available might be useful in initiating the development of such an assessment tool. CONCLUSION: There is a need for the development of educationally sound assessment tools for teaching the teachers courses in EBM, without which it would be impossible to ascertain if such courses have the desired effect.

É de conhecimento de todos, ou pelo menos daqueles que estão mais atentos às questões de natureza económica, que um dos grandes desafios das empresas nos dias que correm, é garantirem a sua sustentabilidade financeira no longo prazo. Nesse sentido, são várias as estratégias elaboradas pelos responsáveis das diversas empresas, e decisores políticos, no sentido colmatar esta dificuldade. Cabo Verde, como país de Desenvolvimento Médio, muito dependente do exterior, apresenta muitas dificuldades em gerar recursos que garantam a sustentabilidade da sua economia. Como forma de reestruturar e dinamizar a economia e o tecido empresarial nacional, foi reaberta em 2005 a Bolsa de Valores de Cabo Verde. E para retratar o impato positivo da sua reabertura, referir que nesse mesmo ano, a Sociedade Caboverdiana de Tabacos, S.A., realizou na Bolsa de Valores de Cabo Verde, uma Oferta Pública de Venda, que segundo os analístas, foi muito superior a oferta, deixando assim, uma ideia clara de que este, com mais incentivo, tem margem de crescimento. Este trabalho visa numa primeira parte caraterizar o mercado de capitais em Cabo Verde, onde está inserida a Bolsa de Valores Local. Num segundo momento, pretendese fazer uma resenha bibliográfica dos principais métodos de avaliação das empresas e de seguida procurar-se, através de um estudo de caso, a Sociedade Cabo-verdiana de Tabacos, identificar algumas possíveis vantagens provenientes da sua entrada na Bolsa de Valores, mediante a avaliação de alguns indicadores de desempenho da empresa. It is known to all, or at least to those who are more attentive to the matters of economic issues, that one of the biggest problems facing companies today is to ensure their financial sustainability. Accordingly, there are several strategies drawn up by leaders of various companies in order to face this difficulty. Cape Verde, as a medium-developed country, is very dependent on other countries‟ help and has great difficulty in generating resources that guarantee a good and healthy economy. As a way to restructure the economy and minimize some financial problems, in 2005 the Stock Exchange of Cape Verde was reopened, to remedy this deficiency. For one to have a clear idea of the importance of this reopening, we can look at the example of Sociedade Caboverdiana de Tabacos – tobacco national company – that, still in this year, has made a bid on Stock Exchange, which, according to specialists, demonstrated an offer a lot greater than the demand, clearly a sign that this company, has a great margin for growth, if it has more financial incentives. This paper seeks to demonstrate and characterize the capital market of Cape Verde, where we will find the National Stock Exchange. Secondly, we intend to make a bibliographical research of the main assessment methods companies use here, to finally, through a case study, at SCT, identify possible advantages that might come from being listed in the Stock Exchange, through the analysis of some performance indicators from this company.

Related to the raise of the awareness of the importance of the Earth heritage, geomorphosites receive increasing attention from the scientific community. Assessment methods, classification and conservation strategies have been developed to safeguard the geomorphological heritage for present and future generations. On the other hand, Earth heritage offers opportunities to develop educational and recreational programs as well as tourism projects. Various interpretive supports and local development projects have been engendered in the past few years to promote geoheritage.¦Be it for the assessment, conservation or promotion of geomorphosites, maps are valuable from many standpoints. They can provide fundamental data for detailed geomorphosite description, serve as visual communication tools helping to guide the selection process in defining protection priority or supporting Earth heritage promotion and interpretàtion.¦This study reviews the main achievements and the objectives yet to be accomplished in the field of geomorphosite mapping and proposes a general framework for the mapping of geomorphosites that takes into account the different aims and publics. The main focus is on mapping geomorphosites for non-specialists in the field of Earth heritage promotion (Geotourism). In this context, maps are often employed to show itineraries or points of interest. Like a scheme or a diagram, a map can also be used as a method for visualising geoscientific information. This function is particularly important since some processes, which contributed to the formation of a geomorphosite or a geomorphological landscape are no longer or not always clearly visible in the landscape. In this case, maps become interpretive media that serve popularisation purposes.¦Mapping for non-specialists holds the challenging task to ensure the information transfer between the cartographer and the user. We therefore focus on both the implementation of the map by the cartographer (which information? which visualisation?) and the interpretation of the map by the user (effectiveness of the knowledge transfer). The research is based on empirical studies carried out in the Maderan valley (Canton of Uri) and in classes of the Cantons of Uri and Tessin that aim to gain knowledge about the familiarity and interests of non- specialists for geoheritage as well as about their map reading skills. The final objective is to formulate methodological proposals for geomorphosite mapping for interpretive purpose.

Fish acute toxicity tests play an important role in environmental risk assessment and hazard classification because they allow for first estimates of the relative toxicity of various chemicals in various species. However, such tests need to be carefully interpreted. Here we shortly summarize the main issues which are linked to the genetics and the condition of the test animals, the standardized test situations, the uncertainty about whether a given test species can be seen as representative to a given fish fauna, the often missing knowledge about possible interaction effects, especially with micropathogens, and statistical problems like small sample sizes and, in some cases, pseudoreplication. We suggest that multi-factorial embryo tests on ecologically relevant species solve many of these issues, and we shortly explain how such tests could be done to avoid the weaker points of fish acute toxicity tests.

OBJECTIVES: Because early etiologic identification is critical to select appropriate specific status epilepticus (SE) management, we aim to validate a clinical tool we developed that uses history and readily available investigations to guide prompt etiologic assessment. METHODS: This prospective multicenter study included all adult patients treated for SE of all but anoxic causes from four academic centers. The proposed tool is designed as a checklist covering frequent precipitating factors for SE. The study team completed the checklist at the time the patient was identified by electroencephalography (EEG) request. Only information available in the emergency department or at the time of in-hospital SE identification was used. Concordance between the etiology indicated by the tool and the determined etiology at hospital discharge was analyzed, together with interrater agreement. RESULTS: Two hundred twelve patients were included. Concordance between the etiology hypothesis generated using the tool and the finally determined etiology was 88.7% (95% confidence interval (CI) 86.4-89.8) (κ = 0.88). Interrater agreement was 83.3% (95% CI 80.4-96) (κ = 0.81). SIGNIFICANCE: This tool is valid and reliable for identification early the etiology of an SE. Physicians managing patients in SE may benefit from using it to identify promptly the underlying etiology, thus facilitating selection of the appropriate treatment.

QUESTIONS UNDER STUDY: The starting point of the interdisciplinary project "Assessing the impact of diagnosis related groups (DRGs) on patient care and professional practice" (IDoC) was the lack of a systematic ethical assessment for the introduction of cost containment measures in healthcare. Our aim was to contribute to the methodological and empirical basis of such an assessment. METHODS: Five sub-groups conducted separate but related research within the fields of biomedical ethics, law, nursing sciences and health services, applying a number of complementary methodological approaches. The individual research projects were framed within an overall ethical matrix. Workshops and bilateral meetings were held to identify and elaborate joint research themes. RESULTS: Four common, ethically relevant themes emerged in the results of the studies across sub-groups: (1.) the quality and safety of patient care, (2.) the state of professional practice of physicians and nurses, (3.) changes in incentives structure, (4.) vulnerable groups and access to healthcare services. Furthermore, much-needed data for future comparative research has been collected and some early insights into the potential impact of DRGs are outlined. CONCLUSIONS: Based on the joint results we developed preliminary recommendations related to conceptual analysis, methodological refinement, monitoring and implementation.

This work aimed at assessing the doses delivered in Switzerland to paediatric patients during computed tomography (CT) examinations of the brain, chest and abdomen, and at establishing diagnostic reference levels (DRLs) for various age groups. Forms were sent to the ten centres performing CT on children, addressing the demographics, the indication and the scanning parameters: number of series, kilovoltage, tube current, rotation time, reconstruction slice thickness and pitch, volume CT dose index (CTDI(vol)) and dose length product (DLP). Per age group, the proposed DRLs for brain, chest and abdomen are, respectively, in terms of CTDI(vol): 20, 30, 40, 60 mGy; 5, 8, 10, 12 mGy; 7, 9, 13, 16 mGy; and in terms of DLP: 270, 420, 560, 1,000 mGy cm; 110, 200, 220, 460 mGy cm; 130, 300, 380, 500 mGy cm. An optimisation process should be initiated to reduce the spread in dose recorded in this study. A major element of this process should be the use of DRLs.

RESUME : Dans de nombreux environnements professionnels, des travailleurs sont exposés à des bioaérosols, que ce soit des bactéries, champignons, virus ou fragments de microorganismes. Ces bioaérosols peuvent être responsables de maladies infectieuses (p.ex. légionellose), ou de maladies non infectieuses (touchant principalement les voies respiratoires). Cependant, pour une majorité des bioaérosols, les relations entre une exposition à une certaine dose et les effets sur la santé humaine sont peu connues. Ce manque de connaissances étant dû principalement à une absence de méthodes adéquates permettant de quantifier cette exposition. La real-time quantitative PCR (Q-PCR) est un outil basé sur la quantification du DNA dont le potentiel de quantification des bioaérosols dans des environnements professionnels n'a pas été exploré. Le but de ce travail est de développer une méthode de Q-PCR permettant de quantifier des bioaérosols - en particulier des bactéries - et d'appliquer ces techniques pour des mesures préventives sur les lieux de travail. Dans ce travail, la Q-PCR a été appliquée à 1a quantification de pathogènes, de groupes taxonomiques spécifiques et de la charge bactérienne totale dans des environnements de travail, stations d'épuration et élevages industriels de volailles. Nous avons montré que la Q-PCR : 1) est capable de quantifier des pathogènes difficilement cultivables si ceux-ci sont présents en concentration importante, 2) a le potentiel pour être un outil performant dans l'étude des communautés bactériennes présentes dans l'air d'environnements professionnels, 3) est aussi performante que le comptage total des bactéries par DAPI pour quantifier 1a charge bactérienne totale et est donc une alternative prometteuse aux techniques culture-dépendantes. La Q-PCR pourrait être utilisée afin d'établir des relations doses-réponses pour la charge bactérienne ; soit dans des populations de travailleurs hautement exposés (p.ex. les éleveurs de volailles), soit en exposant des cellules à des concentrations de bioaérosols mesurées par Q-PCR. ABSTRACT : Many workers are exposed to bioaerosols such as bacteria, fungi, viruses or fragments of microorganisms. These bioaerosols can be responsible of infectious (e.g. legionellosis) or non infectious diseases (mainly respiratory symptoms). However, for a majority of them, the relationship between exposure and effects on human health is not clearly established. This is mainly due to the lack of valid quantitative assessment methods. Real-time quantitative PCR (Q-PCR) is a tool based on the quantification of DNA, of which the potential for the quantification of bioaerosols in work environments has not yet been explored. The aim of this work was to develop a Q-PCR method permitting to quantify bioaerosols -mainly bacteria and to apply those techniques in occupational environments. In this work, Q-PCR was applied to the quantification of pathogens, of specific taxonomic groups and of the total bacterial load in two different occupational settings, namely wastewater treatment plants and poultry houses. We showed that Q-PCR : 1) is capable of quantifying difficult to cultivate pathogens; when they are present at high concentrations, 2) has the potential to be a useful tool for studying bacterial communities in the air of work environments, 3) is as efficient as epifluorescence for the quantification of total bacterial load, and is a promising alternative to the culture-dependent methods. Q-PCR could be used to establish doses-responses relationships for bacterial load, either in populations of highly exposed workers such as poultry farmers, or by exposing cells to concentrations of bioaerosols quantified with Q-PCR.

Exposure to solar ultraviolet (UV) light is the main causative factor for skin cancer. UV exposure depends on environmental and individual factors. Individual exposure data remain scarce and development of alternative assessment methods is greatly needed. We developed a model simulating human exposure to solar UV. The model predicts the dose and distribution of UV exposure received on the basis of ground irradiation and morphological data. Standard 3D computer graphics techniques were adapted to develop a rendering engine that estimates the solar exposure of a virtual manikin depicted as a triangle mesh surface. The amount of solar energy received by each triangle was calculated, taking into account reflected, direct and diffuse radiation, and shading from other body parts. Dosimetric measurements (n = 54) were conducted in field conditions using a foam manikin as surrogate for an exposed individual. Dosimetric results were compared to the model predictions. The model predicted exposure to solar UV adequately. The symmetric mean absolute percentage error was 13%. Half of the predictions were within 17% range of the measurements. This model provides a tool to assess outdoor occupational and recreational UV exposures, without necessitating time-consuming individual dosimetry, with numerous potential uses in skin cancer prevention and research.

Intrusive pain is likely to have a serious impact on older people with limited ability to respond to additional stressors. Frailty is conceptualised as a functional and biological pattern of decline accumulating across multiple physiological systems, resulting in a decreased capacity to respond to additional stressors. We explored the relationship between intrusive pain, frailty and comorbid burden in 1705 community-dwelling men aged 70 or more who participated in the baseline phase of the CHAMP study, a large epidemiological study of healthy ageing based in Sydney, Australia. 9.4% of men in the study were frail (according to the commonly-used Cardiovascular Health Study frailty criteria).Using a combination of self-report and clinical measures, we found an association between frailty and intrusive pain that remained after accounting for demographic characteristics, number of comorbidities, self-reported depressed mood and arthritis (adjusted odds ratio 1.7 (95% confidence interval (CI) 1.1-2.7), p=0.0149). The finding that adjusting for depressed mood, but not a history of arthritis, attenuated the relationship between frailty and intrusive pain points to a key role for central mechanisms. Additionally, men with the highest overall health burden (frail plus high comorbid burden) were most likely to report intrusive pain (adjusted odds ratio 3.0 (95% CI 1.6-5.5), p=0.0004). These findings provide support for the concept that intrusive pain is an important challenge for older men with limited capacity to respond to additional physical stressors. To our knowledge, this is the first study to explore specifically the relationship between pain and frailty.

OBJECTIVE: Transcranial Doppler (TCD) is widely used to monitor the temporal course of vasospasm after subarachnoid hemorrhage (SAH), but its ability to predict clinical deterioration or infarction from delayed cerebral ischemia (DCI) remains controversial. We sought to determine the prognostic utility of serial TCD examination after SAH. METHODS: We analyzed 1877 TCD examinations in 441 aneurysmal SAH patients within 14 days of onset. The highest mean blood flow velocity (mBFV) value in any vessel before DCI onset was recorded. DCI was defined as clinical deterioration or computed tomographic evidence of infarction caused by vasospasm, with adjudication by consensus of the study team. Logistic regression was used to calculate adjusted odds ratios for DCI risk after controlling for other risk factors. RESULTS: DCI occurred in 21% of patients (n = 92). Multivariate predictors of DCI included modified Fisher computed tomographic score (P = 0.001), poor clinical grade (P = 0.04), and female sex (P = 0.008). After controlling for these variables, all TCD mBFV thresholds between 120 and 180 cm/s added a modest degree of incremental predictive value for DCI at nearly all time points, with maximal sensitivity by SAH day 8. However, the sensitivity of any mBFV more than 120 cm/s for subsequent DCI was only 63%, with a positive predictive value of 22% among patients with Hunt and Hess grades I to III and 36% in patients with Hunt and Hess grades IV and V. Positive predictive value was only slightly higher if mBFV exceeded 180 cm/s. CONCLUSION: Increased TCD flow velocities imply only a mild incremental risk of DCI after SAH, with maximal sensitivity by day 8. Nearly 40% of patients with DCI never attained an mBFV more than 120 cm/s during the course of monitoring. Given the poor overall sensitivity of TCD, improved methods for identifying patients at high risk for DCI after SAH are needed.
