A Leishmaniose Tegumentar Americana no estado do Amapá ainda está restrita às áreas rurais. Para estratificar o Amapá em áreas de risco para LTA, e conhecer seu perfil epidemiológico, foram coletados casos registrados no Sistema de Informação Nacional de Agravos de Notificação e classificados os municípios quanto ao risco, de acordo com as médias dos coeficientes de prevalência de 2002 a 2006 de cada município. Foram registrados 2.664 casos autóctones de LTA: 2002 (353 casos); 2003 (481); 2004 (968), 2005 (422) e 2006 (440). Desses, 653 (24,5%) com origem da infecção no município de Porto Grande. A média do coeficiente de prevalência do estado nos 5 anos estudados foi de 95,51/100.000hab. Os municípios foram agrupados em áreas quanto ao grau de risco: de baixo risco para LTA (Macapá, Cutias, Santana e Amapá); médio risco (Itaubal, Pracuúba, V. do Jari e Ferreira Gomes); alto risco (L. do Jari, Mazagão, Tartarugalzinho e Oiapoque) e muito alto risco (Calçoene, P. Grande, P. B. do Amapari e S. do Navio). O Amapá apresentou 72% dos casos no 1ºsemestre, com 16,6% dos casos no mês de abril; 38,8% em pacientes com idade entre 20 a 34 anos; 79,7% de ocorrência no sexo masculino, variando na área de médio risco 83% e baixo risco 72%; 29% em pacientes com escolaridade de 4 a 7 anos de estudo. Com 96,2% com confirmação laboratorial na área de muito alto risco; 98,9% casos de leishmaniose cutânea e apenas 42,9% com cura confirmada, na área de alto risco ocorreram 65,3% de cura. A LTA apresenta 100% de autoctonia no estado do Amapá, com um importante surto em 2004 e o município de P. Grande é o que mais contribui para a manutenção desta endemia.

A cidade de Alenquer, localizada na região do Baixo Amazonas do Estado do Pará, sofre historicamente com a ameaça de inundação. Todos os anos, na época da enchente, o furo Surubiú aumenta seu nível, inundando as partes mais baixas da cidade. A presente dissertação analisa o risco a inundação existente na cidade de Alenquer proporcionando instrumentos para a gestão e melhorias das ações do poder público. A análise de ameaça se baseou em uma metodologia que agrega dados históricos, hidrológicos, mapa de construção participativo e trabalho de campo com GPS, identificado três áreas distintas na cidade: (i) área de alta suscetibilidade, anualmente afetada pelas inundações, abrange 8,4% da área da cidade (ii) área de moderada suscetibilidade, apenas atingida quando observado os maiores índices fluviométricos e (iii) área que mesmo na maiores enchentes não é atingida. O cálculo da vulnerabilidade ocorreu a partir da construção de um índice que inclui dados do censo de demográfico, trabalho de campo para a identificação dos elementos essenciais e unidades de respostas. Após a aplicação do índice verificou-se que dos 17 setores da cidade, cinco apresentam alta vulnerabilidade, e os demais moderada vulnerabilidade. Na análise de risco os dados de vulnerabilidade e ameaças foram integrados e novamente três zonas na cidade foram identificadas (i) a zona de alto risco corresponde a 9,45 % da cidade, onde há grande concentração de elementos essenciais e algumas unidades de respostas, o que provoca danos sociais e econômicos quando da ocorrência de inundação (ii) zona de moderado risco, 30% da cidade e (iii) zona de baixo risco que corresponde a 60,55% da cidade. O Plano Diretor do município foi analisado com alguns questionamentos para verificar a inclusão da temática de ameaça, vulnerabilidade e risco nesse documento, após a análise pode se concluir a falta de abordagem dessa temática no plano diretor. Os resultados obtidos com a pesquisa são importantes para subsidiar políticas publicas e os documentos exigidos pela Defesa Civil quando da ocorrência de desastres.

This study presents the results of a cost-effectiveness analysis in a controlled clinical trial on the effectiveness of a modified glass ionomer resin sealant ( Vitremer, 3M ESPE) and the application of fluoride varnish (Duraphat, Colgate) on occlusal surfaces of first permanent molars in children 6-8 years of age (N = 268), according to caries risk (high versus low). Children were examined semiannually by the same calibrated dentist for 24 months after allocation in six groups: high and low risk controls (oral health education every three months); high and low risk with varnish (oral health education every three months + varnish biannually); and high and low risk with sealant (oral health education every three months + a single application of sealant). Economic analysis showed that sealing permanent first molars of high-risk schoolchildren showed a C/E ratio of US$ 119.80 per saved occlusal surface and an incremental C/E ratio of US$ 108.36 per additional saved occlusal surface. The study concluded that sealing permanent first molars of high-risk schoolchildren was the most cost-effective intervention.

The pipelines are characterized by the great length and linearity, these enterprises are usually associated with the horizon of soil and decomposed rock and accidents in this system usually have high complexity. So this work proposes a study of geological and geotechnical characterization of approximately 40 km ² and a study of risk analysis along 18.3 km of gas pipeline GASBOL, in the municipalities of Ipeúna-SP and Itirapina-SP, in scale 1:25.000, besides correlation between studies. The method for geological and geotechnical characterization is based on physiographic integrated analysis technique, which is supported by literature research, observation and interpretation of satellite imagery (photointerpretation), correlation of products numerical model of ground, fieldwork. The method adopted for risk classification is based on preliminary analysis of hazards (APP) technique, which is identified the geological process, this feature is classified as the susceptibility of occurrence, and then classified as the possible consequence of the process, if this evolves. As a product of the research, we designed a map in 1:25,000 scale, containing a table of geological and geotechnical characteristics of the mapped drives, as well as sectorization risk in each section of the pipeline. Were mapped geological and geotechnical seven units in the study area, delimited in excerpts of which five are classified as high risk, totaling 1.1 km in length of 18.3 km total

In the last few decades, Brazil has experienced an accelerated urbanization process in which many cities have grown in a disorderly way occupying environmentally fragile areas unsuitable for habitation. Anthropogenic actions such as high levels of impermeable soil, structural changes in watercourses, lack of riparian vegetation, illegal presence of trash and rubbish along the river banks added to irregular settlements in floodplains result in the rise of high risk areas. When accompanied by intense and prolonged rainfall phenomena, those areas have been the scenery of serious accidents such as floods. This study aims to classify the level of the risk of floods in the neighborhood of Jardim Inocoop, in the town of Rio Claro, São Paulo countryside, Brazil. One of the main technical support to tackle this issue is the identification and classification of the risks. In order to classify the risk level of flood in this case study, the methodology adopted was developed by the Ministry of Cities and Technology Research Institute, and take into account the arrangement of the hydrological scenario, vulnerability of households and dangerous process according to the distance of the houses from the axis of drainage. Therefore, the risk levels adopted to classify are listed below: very high (MA), high risk (A), moderate (M) and low risk (B). In conclusion, it is imperative to develop prevention plans in order to avoid or to minimize the damages caused by natural disasters. Therefore, the zoning of the risk sceneries remains as an important issue once it helps to identify the areas with high level risk of flood. Consequently, the occupation must be regulated where there is low or absent risk and it must be often forbidden where the high risk of flood is detected. The aims of this research were to evaluate the acute toxicity (LC/EC50) and the environmental risk of toltrazuril (TOL) and florfenicol (FFC) for plant Lemna minor, snail Pomacea canalicuta, fish Piaractus mesopotamicus and Hyphessobrycon eques and the microcrustacean Daphnia magna. The organisms were acclimated in room bioassay with controlled temperature according to standard to each one. They were exposed at nominal concentrations in static system. For environmental risk (RQ) was used the estimated environmental concentration (EEC) that is the dosage indicate to treatment and the lethal or effective concentration (LC/CE50) from each drug in acute exposure. FFC showed LC50;7d of 97.03 mg L-1 for L. minor; >100.0 mg L-1 for P. mesopotamicus and H. eques and EC50;48h > 100.0 mg L-1 for P. canaliculata and D. magna, and it was classified low risk (RQ = 0.01) for all bioindicators. TOL howed LC50;7d >100.0 mg L-1 for L. minor, 3.72 mg L-1 for P. mesopotamicus; 6.22 mg L-1 for H. eques and CE50;48h of 7.59 mg L-1 for P. canaliculata and 18.57 mg L-1 for D. magna, and it was classified low risk (RQ = 0.01) for L. minor and high risk for P. mesopotamicus (RQ = 2.68), H. eques (RQ = 6.22), P. canaliculata (RQ = 1.31) and D. magna (RQ = 0.53). Lemna minor was the bioindicator indicating of FFC toxicity and H. eques, bioindicator of the TOL. FFC is safety however and the use of TOL necessaries cautions to treat the wastewater before discard on the environment.

The most common sleep disorder is obstructive sleep apnea syndrome (OSAS), that is characterized by repeated pauses in breathing during sleep, resulting in partial or complete obstruction of the upper airway. It is associated with systemic arterial hypertension and obesity. Objective: To assess the frequency of OSAS in the adult population of Botucatu by the Berlin questionnaire. Method: Transveral study was performed in which the sample size was estimated at 385 individuals. The Berlin questionnaire was administered to adults randomly on the streets, supermarkets and local shops. Data were analyzed taking into account age, gender, "high risk for OSAS", hypertension, obesity and correlations for OSAS. Results: 468 people interviewed were aged between 18 and 95 years, average of 59 years. 166 (35.25%) individuals were identified as "high risk for OSAS", being more frequent in the population above 40 years. Obesity was present in 23.3%, more common in women (60.2%). 83.5% of the population obese also had a "high risk for OSAS". Hypertension was observed in 27.3%, of these 71.9% were at "high risk for OSAS". Conclusion: The study showed a high frequency of "high risk for OSAS" in the adult population. Despite the dissemination by the media about sleep apnea, patients are not investigated, even under medical care for hypertension.
