AIMS: Brugada syndrome (BrS) is characterized by arrhythmias leading to sudden cardiac death. BrS is caused, in part, by mutations in the SCN5A gene, which encodes the sodium channel alpha-subunit Na(v)1.5. Here, we aimed to characterize the biophysical properties and consequences of a novel BrS SCN5A mutation. METHODS AND RESULTS: SCN5A was screened for mutations in a male patient with type-1 BrS pattern ECG. Wild-type (WT) and mutant Na(v)1.5 channels were expressed in HEK293 cells. Sodium currents (I(Na)) were analysed using the whole-cell patch-clamp technique at 37 degrees C. The electrophysiological effects of the mutation were simulated using the Luo-Rudy model, into which the transient outward current (I(to)) was incorporated. A new mutation (C1850S) was identified in the Na(v)1.5 C-terminal domain. In HEK293 cells, mutant I(Na) density was decreased by 62% at -20 mV. Inactivation of mutant I(Na) was accelerated in a voltage-dependent manner and the steady-state inactivation curve was shifted by 11.6 mV towards negative potentials. No change was observed regarding activation characteristics. Altogether, these biophysical alterations decreased the availability of I(Na). In the simulations, the I(to) density necessary to precipitate repolarization differed minimally between the two genotypes. In contrast, the mutation greatly affected conduction across a structural heterogeneity and precipitated conduction block. CONCLUSION: Our data confirm that mutations of the C-terminal domain of Na(v)1.5 alter the inactivation of the channel and support the notion that conduction alterations may play a significant role in the pathogenesis of BrS.

OBJECTIVES: Exercise combined with nicotine therapy may help smoking cessation and minimise weight gain after quitting. Low participation in vigorous-intensity physical activity programmes precludes their population-wide applicability. In a randomised controlled trial, we tested whether a population-based moderate-intensity physical activity programme increases quit rates among sedentary smokers receiving nicotine therapy. METHODS: Participants (n=481; 57% male; mean age, 42.2 years (SD 10.1); mean cigarette consumption, 27 (SD 10.2) per day) were offered a nine-week smoking cessation programme consisting of a weekly 15-minute counselling session and the prescription of nicotine replacement therapy. In addition, participants in the physical activity group (n=229) also took part in a programme of moderate-intensity physical activity implemented at the national level, and offering nine weekly 60-minute sessions of physical activity. To ensure equal contact conditions, participants in the control group (n=252) attended weekly 60-minute health behaviour education sessions unrelated to physical activity. The primary outcome was continuous CO-verified smoking abstinence rates at 1-year follow-up. RESULTS: Continuous smoking abstinence rates were high and similar in the physical activity group and the control group at the end of the intervention (47% versus 46%, p=0.81) and at 1-year follow-up (27% versus 29%, p=0.71). The mean weight gain after one year was 4.4 kg and 6.2 kg among sustained quitters of the physical activity and control groups, respectively (p=0.06). CONCLUSION: Participation in a population-based moderate-intensity physical activity programme for 9 weeks in addition to a comprehensive smoking cessation programme did not significantly increase smoking cessation rates. A non-significant reduction in weight gain was observed among participants who quit smoking in the physical activity group. Rapport de synthèseObjectif: le remplacement valvulaire aortique par voie transcathétère est, actuellement, une méthode fiable indiquée pour des patients à haut risque porteurs d'une sténose valvulaire aortique. La voie transfémorale est utilisable seulement en cas d'absence de maladie vasculaire et la voie transapicale est contrindiquée en cas de dysfonction pulmonaire chronique sévère. Une alternative pour ne pas passer a travers l'espace pleural serait par voie sous- xiphoïdienne et trans-septale à travers le ventricule droit.Méthode: une expérience animale a été amené au laboratoire de recherche du CHUV. Cinq cochons (poids : 52.3±10.9 kg) ont été endormi et, sous anesthésie générale, le ventricule droit a été préparé a travers un accès sous- xiphoïdien. Ensuite, sous guide fluoroscopique et avec l'utilisation d'une échocardiographie intracardiaque, un accès trans-septal a été crée entre le ventricule droit et le ventricule gauche en utilisant des dilatateur de diamètre croissant (de 8F à 26F). Par la suite, une valve stentée crée dans le laboratoire en utilisant un stent en nitinol et du péricarde a été chargé dans une cartouche et introduite dans le ventricule gauche à travers un introducteur trans-septal. Enfin, la valve a été amené dans la chambre de chasse du ventricule gauche et ensuite dans la racine aortique et puis déployé au bon endroit. Quand le système a été retiré, le septum ventriculaire a pu être réparé par mise en place d'un système d'occlusion septal Amplazer. Trente minutes après la procédure, les animaux ont été sacrifié et le coeur a été analysé pour étudier le positionnement da la valve stentée, l'efficacité de la fermeture du septum inter-ventriculaire et la fermeture de la paroi du ventricule droit.Résultat : les cinq cochon ont tous eu un parfait positionnement et pose de la prothèse en position aortique au premier essai (efficacité 100%). Les procédures ont duré, moyennement, 49±4 minutes et la progressive dilatation de l'accès trans-septale à donné lieu à une communication inter-ventriculaire mesurable après dilatation avec du 18F et plus. Toutes les valves stentées ont été déployées au bon endroit avec un bon résultat du fonctionnement des valves prothétiques et absence d'insuffisance para prothétique. Pendant les procédures, des battement prématurés ainsi que des épisodes isolées de tachycardie supra ventriculaire ont été détectés. Par contre, il n'y a pas eu de bloc atrio-ventriculaire. Les pertes sanguines pendant les procédures étaient de 280±10mL, et les systèmes d'occlusion Amplatzer étaient tous bien déployés sans shunts inter-ventriculaires résiduels.Conclusion: la technique d'introduction de valve stentées par voie extrapleural (trans-ventriculaire et trans-septale) est techniquement possible et elle jette les bases pour le remplacement valvulaire aortique trans-ventriculaire sous anesthésie locale.

Doxorubicin is an antineoplasic agent active against sarcoma pulmonary metastasis, but its clinical use is hampered by its myelotoxicity and its cumulative cardiotoxicity, when administered systemically. This limitation may be circumvented using the isolated lung perfusion (ILP) approach, wherein a therapeutic agent is infused locoregionally after vascular isolation of the lung. The influence of the mode of infusion (anterograde (AG): through the pulmonary artery (PA); retrograde (RG): through the pulmonary vein (PV)) on doxorubicin pharmacokinetics and lung distribution was unknown. Therefore, a simple, rapid and sensitive high-performance liquid chromatography method has been developed to quantify doxorubicin in four different biological matrices (infusion effluent, serum, tissues with low or high levels of doxorubicin). The related compound daunorubicin was used as internal standard (I.S.). Following a single-step protein precipitation of 500 microl samples with 250 microl acetone and 50 microl zinc sulfate 70% aqueous solution, the obtained supernatant was evaporated to dryness at 60 degrees C for exactly 45 min under a stream of nitrogen and the solid residue was solubilized in 200 microl of purified water. A 100 microl-volume was subjected to HPLC analysis onto a Nucleosil 100-5 microm C18 AB column equipped with a guard column (Nucleosil 100-5 microm C(6)H(5) (phenyl) end-capped) using a gradient elution of acetonitrile and 1-heptanesulfonic acid 0.2% pH 4: 15/85 at 0 min-->50/50 at 20 min-->100/0 at 22 min-->15/85 at 24 min-->15/85 at 26 min, delivered at 1 ml/min. The analytes were detected by fluorescence detection with excitation and emission wavelength set at 480 and 550 nm, respectively. The calibration curves were linear over the range of 2-1000 ng/ml for effluent and plasma matrices, and 0.1 microg/g-750 microg/g for tissues matrices. The method is precise with inter-day and intra-day relative standard deviation within 0.5 and 6.7% and accurate with inter-day and intra-day deviations between -5.4 and +7.7%. The in vitro stability in all matrices and in processed samples has been studied at -80 degrees C for 1 month, and at 4 degrees C for 48 h, respectively. During initial studies, heparin used as anticoagulant was found to profoundly influence the measurements of doxorubicin in effluents collected from animals under ILP. Moreover, the strong matrix effect observed with tissues samples indicate that it is mandatory to prepare doxorubicin calibration standard samples in biological matrices which would reflect at best the composition of samples to be analyzed. This method was successfully applied in animal studies for the analysis of effluent, serum and tissue samples collected from pigs and rats undergoing ILP.

Os sistemas de custeio não são mais do que sistemas de informação e de suporte à tomada de decisão. Porém, a contestação em relação aos sistemas ditos de tradicionais levou à emergência de novas abordagens. Dentre todas, o Custeio Baseado nas Actividades (ABC) tem-se destacado ao nível da literatura e das próprias empresas, sobretudo nas de maiores dimensões. A compreensão e a aplicação do ABC, em especial nas PMEs e a sistematização de metodologias de implementação, assim como a elaboração de estudos que concluam das reais vantagens e desvantagens deste novo sistema são, ainda, um campo por explorar devidamente. Este trabalho procura estudar a validade de um sistema ABC ao nível de uma PME Caboverdiana, caracterizadas na sua maior parte por um grande desconhecimento em relação às técnicas e abordagens de custeio. Para além do trabalho de campo (Parte IV) que aqui é descrito também se realizou um estudo exaustivo da literatura sobre o tema (Parte III). Porém, o ABC não poderia ser entendido, descontextualizado da problemática dos custos e dos sistemas de custeio (Parte I), nem desenquadrado do conjunto de mudanças que se operaram nos últimos anos nos processos produtivos e que proporcionaram o surgimento de outras abordagens não menos interessantes (Parte II). O estudo de caso, consubstanciado na última parte, visou criar um quadro conceptual de referência que permita a realização de outros estudos semelhantes, alargando o conhecimento científico sobre este tema e providenciando ferramentas úteis para as empresas que seguem de perto estas novas abordagens.

Correspondence analysis has found extensive use in ecology, archeology, linguisticsand the social sciences as a method for visualizing the patterns of association in a table offrequencies or nonnegative ratio-scale data. Inherent to the method is the expression of the datain each row or each column relative to their respective totals, and it is these sets of relativevalues (called profiles) that are visualized. This relativization of the data makes perfect sensewhen the margins of the table represent samples from sub-populations of inherently differentsizes. But in some ecological applications sampling is performed on equal areas or equalvolumes so that the absolute levels of the observed occurrences may be of relevance, in whichcase relativization may not be required. In this paper we define the correspondence analysis ofthe raw unrelativized data and discuss its properties, comparing this new method to regularcorrespondence analysis and to a related variant of non-symmetric correspondence analysis.

El estudio se hizo durante el Crucero de Evaluación de la Biomasa Desovante de Anchoveta y Sardina BIC Humboldt 9508-09. Los volúmenes de plancton superficial para agosto-setiembre de 1995 fluctuaron entre 0,1 y 4,6 mL/m3. Las mayores concentraciones en biomasa, fueron localizadas dentro de las 30 mn, con un predominio del fitoplancton en el 50% de las estaciones. Las especies que tipificaron este ambiente, dentro de las 30 mn, correspondieron a diatomeas de alta tasa de reproducción, que constituyen la dieta de los peces fitoplanctófagos, composición que se vio alterada por la presencia de especies oceánicas, hasta muy cerca de la costa, principalmente entre Huarmey y Pucusana. A 10 m de profundidad se identificó un total de 70 especies de fitoplancton: 47 diatomeas, 15 dinoflagelados, 3 cocolitofóridos , 3 fitoflagelados y 2 silicoflagelados. Las diatomeas y el nanoplancton fueron los grupos taxonómicos más importantes, hasta con el 90% de dominancia. Los dinoflagelados y silicoflagelados fueron poco numerosos. Las mayores concentraciones celulares promedio (20.000 cel/50 mL) se encontraron cerca de la costa, con máximos de hasta 60.000 cel/50 mL en la región norte. El dinoflagelado Protoperidinium obtusun indicador de aguas costeras frías (ACF), estuvo restringido a la franja costera, ampliando su distribución hasta por fuera de las 30 mn entre Salaverry y Punta Falsa.

A tuberculose é uma doença infecto-contagiosa, que apresenta 8 milhões de casos novos por ano no mundo. Sua transmissão normalmente ocorre através das vias aéreas, acometendo principalmente os pulmões. De acordo com a localização do foco da doença, ela pode ser classificada em tuberculose pulmonar, tuberculose extrapulmonar e tuberculose miliar (Mota et al., 2010). A pesquisa em estudo fala da tuberculose pulmonar que continua a ser um problema de saúde pública e uma das principais causas de mortalidade em países em desenvolvimento. O diagnóstico e o tratamento precoce são fundamentais para controlar a sua transmissão. Um dos problemas enfrentados por vários países como Brasil, China entre outros é a não adesão ao tratamento da tuberculose pelos doentes. Esta falta de aderência para o tratamento é um dos principais factores que determinam sua eficácia, especialmente em países em desenvolvimento. Na década 90 foi introduzido o tratamento de curta duração contra esta doença, mas a baixa adesão a terapêutica levou a uma caída na taxa de cura, um aumento da transmissão da doença na população e, um risco 5,6 vezes superior de desenvolvimento de resistências aos anti-tuberculosos (Solarte & Barona, 2008). O objectivo principal deste estudo consiste em determinar o grau de adesão dos pacientes com tuberculose ao tratamento com anti-tuberculosos e os factores associados com a não adesão nos Centros de Saúde de Achada Santo António, Tira Chapéu, Ponta D’Água e Delegacia de Saúde de São Felipe - Fogo. Neste estudo foi feito um levantamento dos dados dos casos de tuberculose, registado nos livros existentes em cada estrutura de saúde em estudo, e aplicação de um questionário aos pacientes em tratamento. Com esses dados obtiveram-se 152 casos; 46 a partir dos questionários e 106 dos dados obtidos dos registos nas respectivas estruturas de saúde em estudo. Os resultados obtidos permitem verificar que o sexo masculino é o mais afectado em relação ao feminino. Também constatou-se que os homens abandonam mais o tratamento do que as mulheres. Muitos usam álcool, tabaco e drogas, oque pode ter contribuído para a não adesão ao tratamento. Segundo o estudo os factores para a não adesão ao tratamento foram a falta de emprego, a baixa escolaridade, falta de conhecimento sobre a tuberculose e hábitos de vida dos pacientes tais como o uso de álcool, tabaco e drogas. Segundo os dados obtidos o principal motivo para o abandono do tratamento foi preguiça. Outros motivos de relevância foram: medo que as pessoas saibam da sua doença, falta de recursos financeiros e a duração prolongada do tratamento.

The effects of premature birth on attachment have generally been examined from the infant's perspective. There is a lack of data concerning parental attachment representations toward a premature child. Because of the psychological stress engendered in parents confronted with a premature birth, we hypothesized that their attachment representations would be altered during the first months after the hospital discharge. Fifty families with a premature infant (25-33 gestation weeks) and a control group of 30 families with a full-term infant participated to the study. Perinatal risks were evaluated during hospitalization. To assess mothers' representations of their infant, the Working Model of the Child Interview (WMCI, Zeanah & Benoit, 1995 & Benoit, Zeanah, Parker, Nicholson, & Coolbear, 1997) were administered when their children were 6 and 18 months old. The severity of the perinatal risks was found to have an impact on the mothers' attachment representations. At six months, only 20% of the mothers of a prematurely born infant (30% at 18 months) had secure attachment representations, vs. 53% for the control group (57% at 18 months). Furthermore, mothers of low-risk premature infants more often had disengaged representations, whereas distorted representations were more frequent in the high-risk group of premature children. These findings suggest that the parental response to a premature birth is linked to the severity of postnatal risks. The fact that secure attachment representations are affected in mothers of low-risk infants just as much as they are in mothers of high-risk infants points to the need to conduct further studies aimed at evaluating whether preventive intervention for both low-risk and high-risk premature will be helpful.
