BACKGROUND A recombinant, replication-competent vesicular stomatitis virus-based vaccine expressing a surface glycoprotein of Zaire Ebolavirus (rVSV-ZEBOV) is a promising Ebola vaccine candidate. We report the results of an interim analysis of a trial of rVSV-ZEBOV in Guinea, west Africa. METHODS For this open-label, cluster-randomised ring vaccination trial, suspected cases of Ebola virus disease in Basse-Guinée (Guinea, west Africa) were independently ascertained by Ebola response teams as part of a national surveillance system. After laboratory confirmation of a new case, clusters of all contacts and contacts of contacts were defined and randomly allocated 1:1 to immediate vaccination or delayed (21 days later) vaccination with rVSV-ZEBOV (one dose of 2 × 10(7) plaque-forming units, administered intramuscularly in the deltoid muscle). Adults (age ≥18 years) who were not pregnant or breastfeeding were eligible for vaccination. Block randomisation was used, with randomly varying blocks, stratified by location (urban vs rural) and size of rings (≤20 vs >20 individuals). The study is open label and masking of participants and field teams to the time of vaccination is not possible, but Ebola response teams and laboratory workers were unaware of allocation to immediate or delayed vaccination. Taking into account the incubation period of the virus of about 10 days, the prespecified primary outcome was laboratory-confirmed Ebola virus disease with onset of symptoms at least 10 days after randomisation. The primary analysis was per protocol and compared the incidence of Ebola virus disease in eligible and vaccinated individuals in immediate vaccination clusters with the incidence in eligible individuals in delayed vaccination clusters. This trial is registered with the Pan African Clinical Trials Registry, number PACTR201503001057193. FINDINGS Between April 1, 2015, and July 20, 2015, 90 clusters, with a total population of 7651 people were included in the planned interim analysis. 48 of these clusters (4123 people) were randomly assigned to immediate vaccination with rVSV-ZEBOV, and 42 clusters (3528 people) were randomly assigned to delayed vaccination with rVSV-ZEBOV. In the immediate vaccination group, there were no cases of Ebola virus disease with symptom onset at least 10 days after randomisation, whereas in the delayed vaccination group there were 16 cases of Ebola virus disease from seven clusters, showing a vaccine efficacy of 100% (95% CI 74·7-100·0; p=0·0036). No new cases of Ebola virus disease were diagnosed in vaccinees from the immediate or delayed groups from 6 days post-vaccination. At the cluster level, with the inclusion of all eligible adults, vaccine effectiveness was 75·1% (95% CI -7·1 to 94·2; p=0·1791), and 76·3% (95% CI -15·5 to 95·1; p=0·3351) with the inclusion of everyone (eligible or not eligible for vaccination). 43 serious adverse events were reported; one serious adverse event was judged to be causally related to vaccination (a febrile episode in a vaccinated participant, which resolved without sequelae). Assessment of serious adverse events is ongoing. INTERPRETATION The results of this interim analysis indicate that rVSV-ZEBOV might be highly efficacious and safe in preventing Ebola virus disease, and is most likely effective at the population level when delivered during an Ebola virus disease outbreak via a ring vaccination strategy. This research examines the site and situation characteristics of community trails as landscapes promoting physical activity. Trail segment and neighborhood characteristics for six trails in urban, suburban, and exurban towns in northeastern Massachusetts were assessed from primary Global Positioning System (GPS) data and from secondary Census and land use data integrated in a geographic information system (GIS). Correlations between neighborhood street and housing density, land use mix, and sociodemographic characteristics and trail segment characteristics and amenities measure the degree to which trail segment attributes are associated with the surrounding neighborhood characteristics.

The persistence of low birth weight and intrauterine growth retardation (IUGR) in the United States has puzzled researchers for decades. Much of the work that has been conducted on adverse birth outcomes has focused on low birth weight in general and not on IUGR. Studies that have examined IUGR specifically thus far have focused primarily on individual-level maternal risk factors. These risk factors have only been able to explain a small portion of the variance in IUGR. Therefore, recent work has begun to focus on community-level risk factors in addition to the individual-level maternal characteristics. This study uses Social Ecology to examine the relationship of individual and community-level risk factors and IUGR. Logistic regression was used to establish an individual-level model based on 155, 856 births recorded in Harris County, TX during 1999-2001. IUGR was characterized using a fetal growth ratio method with race/ethnic and sex specific mean birth weights calculated from national vital records. The spatial distributions of 114,460 birth records spatially located within the City of Houston were examined using choropleth, probability and density maps. Census tracts with higher than expected rates of IUGR and high levels of neighborhood disadvantage were highlighted. Neighborhood disadvantage was constructed using socioeconomic variables from the 2000 U.S. Census. Factor analysis was used to create a unified single measure. Lastly, a random coefficients model was used to examine the relationship between varying levels of community disadvantage, given the set of individual-level risk factors for 152,997 birth records spatially located within Harris County, TX. Neighborhood disadvantage was measured using three different indices adapted from previous work. The findings show that pregnancy-induced hypertension, previous preterm infant, tobacco use and insufficient weight gain have the highest association with IUGR. Neighborhood disadvantage only slightly further increases the risk of IUGR (OR 1.12 to 1.23). Although community level disadvantage only helped to explain a small proportion of the variance of IUGR, it did have a significant impact. This finding suggests that community level risk factors should be included in future work with IUGR and that more work needs to be conducted. ^

This cross-sectional study is based on the qualitative and quantitative research design to review health policy decisions, their practice and implications during 2009 H1N1 influenza pandemic in the United States and globally. The “Future Pandemic Influenza Control (FPIC) related Strategic Management Plan” was developed based on the incorporation of the “National Strategy for Pandemic Influenza (2005)” for the United States from the U.S. Homeland Security Council and “The Canadian Pandemic Influenza Plan for the Health Sector (2006)” from the Canadian Pandemic Influenza Committee for use by the public health agencies in the United States as well as globally. The “global influenza experts’ survey” was primarily designed and administered via email through the “Survey Monkey” system to the 2009 H1N1 influenza pandemic experts as the study respondents. The effectiveness of this plan was confirmed and the approach of the study questionnaire was validated to be convenient and the excellent quality of the questions provided an efficient opportunity to the study respondents to evaluate the effectiveness of predefined strategies/interventions for future pandemic influenza control.^ The quantitative analysis of the responses to the Likert-scale based questions in the survey about predefined strategies/interventions, addressing five strategic issues to control future pandemic influenza. The effectiveness of strategies defined as pertinent interventions in this plan was evaluated by targeting five strategic issues regarding pandemic influenza control. For the first strategic issue pertaining influenza prevention and pre pandemic planning; the confirmed effectiveness (agreement) for strategy (1a) 87.5%, strategy (1b) 91.7% and strategy (1c) 83.3%. The assessment of the priority level for strategies to address the strategic issue no. (1); (1b (High Priority) > 1a (Medium Priority) > 1c (Low Priority) based on the available resources of the developing and developed countries. For the second Strategic Issue encompassing the preparedness and communication regarding pandemic influenza control; the confirmed effectiveness (agreement) for the strategy (2a) 95.6%, strategy (2b) 82.6%, strategy (2c) 91.3% and Strategy (2d) 87.0%. The assessment of the priority level for these strategies to address the strategic issue no. (2); (2a (highest priority) > 2c (high priority) >2d (medium priority) > 2b (low priority). For the third strategic issue encompassing the surveillance and detection of pandemic influenza; the confirmed effectiveness (agreement) for the strategy (3a) 90.9% and strategy (3b) 77.3%. The assessment of the priority level for theses strategies to address the strategic Issue No. (3) (3a (high priority) > 3b (medium/low priority). For the fourth strategic issue pertaining the response and containment of pandemic influenza; the confirmed effectiveness (agreement) for the strategy (4a) 63.6%, strategy (4b) 81.8%, strategy (4c) 86.3%, and strategy (4d) 86.4%. The assessment of the priority level for these strategies to address the strategic issue no. (4); (4d (highest priority) > 4c (high priority) > 4b (medium priority) > 4a (low priority). The fifth strategic issue about recovery from influenza and post pandemic planning; the confirmed effectiveness (agreement) for the strategy (5a) 68.2%, strategy (5b) 36.3% and strategy (5c) 40.9%. The assessment of the priority level for strategies to address the strategic issue no. (5); (5a (high priority) > 5c (medium priority) > 5b (low priority).^ The qualitative analysis of responses to the open-ended questions in the study questionnaire was performed by means of thematic content analysis. The following recurrent or common “themes” were determined for the future implementation of various predefined strategies to address five strategic issues from the “FPIC related Strategic Management Plan” to control future influenza pandemics. (1) Pre Pandemic Influenza Prevention, (2) Seasonal Influenza Control, (3) Cost Effectiveness of Non Pharmaceutical Interventions (NPI), (4) Raising Global Public Awareness, (5) Global Influenza Vaccination Campaigns, (6)Priority for High Risk Population, (7) Prompt Accessibility and Distribution of Influenza Vaccines and Antiviral Drugs, (8) The Vital Role of Private Sector, (9) School Based Influenza Containment, (10) Efficient Global Risk Communication, (11) Global Research Collaboration, (12) The Critical Role of Global Public Health Organizations, (13) Global Syndromic Surveillance and Surge Capacity and (14) Post Pandemic Recovery and Lessons Learned. The future implementation of these strategies with confirmed effectiveness to primarily “reduce the overall response time’ in the process of ‘early detection’, ‘strategies (interventions) formulation’ and their ‘implementation’ to eventually ensure the following health outcomes: (a) reduced influenza transmission, (b) prompt and effective influenza treatment and control, (c) reduced influenza related morbidity and mortality.^

Li- Fraumeni Syndrome (LFS) is a rare autosomal dominant hereditary cancer syndrome caused by mutations in the TP53 gene that predisposes individuals to a wide variety of cancers, including breast cancer, soft tissue sarcomas, osteosarcomas, brain tumors, and adrenocortical carcinomas. Individuals found to carry germline mutations in TP53 have a 90% lifetime cancer risk, with a 20% chance to develop cancer under the age of 20. Despite the significant risk of childhood cancer, predictive testing for unaffected minors at risk for LFS historically has not been recommended, largely due to the lack of available and effective screening for the types of cancers involved. A recently developed screening protocol suggests an advantage to identifying and screening children at risk for LFS and we therefore hypothesized that this alongside with the availability of new screening modalities may substantiate a shift in recommendations for predictive genetic testing in minors at risk for LFS. We aimed to describe current screening recommendations that genetic counselors provide to this population as well as explore factors that may have influenced genetic counselors attitude and practice in regards to this issue. An online survey was emailed to members of the National Society of Genetic Counselors (NSGC) and the Canadian Association of Genetic Counsellors (CAGC). Of an estimated 1000 eligible participants, 172 completed surveys that were analyzed. Though much attention has been focused in recent years on the melting of ice from Greenland and Antarctica, nearly half of the ice volume currently being lost to the ocean is actually coming from other mountain glaciers and ice caps. Ice loss from a group of islands in northern Canada accounts for much of that volume. In a study published in April 2011 in the journal Nature, a team of researchers led by Alex Gardner of the University of Michigan found that land ice in both the northern and southern Canadian Arctic Archipelago has declined sharply. The maps above show ice loss from surface melting for the northern portion of the archipelago from 2004-2006 (left) and 2007-2009 (right). Blue indicates ice gain, and red indicates ice loss. In the six years studied, the Canadian Arctic Archipelago lost an average of approximately 61 gigatons of ice per year. (A gigaton is a billion tons of ice.) The research team also found the rate of ice loss was accelerating. From 2004 to 2006, the average mass loss was roughly 31 gigatons per year; from 2007 to 2009, the loss increased to 92 gigatons per year. Gardner and colleagues used three independent methods to assess ice mass, all of which showed the same trends. The team used a model to estimate the surface mass balance of ice and the amount of ice discharged. They also compiled and analyzed measurements from NASA's Ice, Cloud and Land Elevation Satellite (ICESat) to assess changes in the surface height of ice. Finally, they gathered observations from NASA's Gravity Recovery and Climate Experiment (GRACE) to determine changes in the gravity field in the region, an indicator of the amount of ice gained or lost. The Canadian Arctic Archipelago generally receives little precipitation, and the amount of snowfall changes little from year to year. But the rate of snow and ice melting varies considerably, so changes in ice mass come largely from changes in summertime melt. During the 2004 to 2009 study period, the Canadian Arctic Archipelago experienced four of its five warmest years since 1960, likely fueling the melting. Gardner notes that from 2001 to 2004, the sum of melting from all mountain glaciers and ice caps around the world (but not the Greenland and Antarctic ice sheets) contributed an estimated 1 millimeter per year to global sea level rise. Recent estimates suggest the Greenland and Antarctic ice sheets add another 1.3 millimeters per year to sea level. "This means 1 percent of the land ice volume-mountain glaciers and ice caps-account for about half of all ice loss to the world's oceans," Gardner said. "Most of the ice loss is coming from the Canadian Arctic Archipelago, Alaska, Patagonia, the Himalayas, and the smaller ice masses surrounding the main Greenland and Antarctic ice sheets."

Esta ponencia es la presentación del proyecto de tesis, que tiene por objetivo estudiar el proceso de constitución y consolidación del poder judicial, responsable de la administración de justicia, como integrante del Estado provincial, legitimador de su accionar político y mediador con la sociedad civil. La investigación se ubica en la provincia de Buenos Aires, entre 1853 y 1881, que sancionó su propia Constitución en 1854, en la que declaraba en su artículo 118 que el poder judicial sería independiente de todo otro en el ejercicio de sus funciones. Es decir que en esta provincia la consagración de la teoría de los poderes del Estado y la ley como definidora de lo que era justo, requirieron de la organización del poder judicial. El sistema judicial de la provincia de Buenos Aires comenzó su formación como poder del Estado a partir de la reforma rivadaviana y el proceso alcanzó su madurez entre 1853 y 1881, como parte fundamental del proyecto liberal, asentado sobre la autoridad de la ley. El poder judicial garantizaba la legitimidad del sistema político republicano de matriz liberal, pero la dependencia con el poder ejecutivo se mantuvo en parte, representada en el presupuesto, los nombramientos y los jurys. La organización judicial en formación incluyó al ámbito rural con una fisonomía institucional que tendía a consolidar la relación entre Estado y sociedad civil; ésta manifestaba sus necesidades a través de la opinión pública y el poder las interpretaba con el fin de legitimar su acción política. En este proceso de legitimación, el poder judicial era funcional al Estado, lo que se consolidó con el tiempo. La Constitución de 1873, a pesar de ser ideal en algunas de sus propuestas, dio protagonismo y efectividad a las instituciones judiciales que se formaron y pusieron en funcionamiento con una nueva concepción de justicia, que sin dejar de lado el derecho natural se consolidaba en una función más positiva, atenta a los derechos individuales y de propiedad. Para comprender el rol que jugó el sistema de justicia en la formación del Estado es necesario desenmarañar la compleja trama de relaciones entre el poder político y los magistrados, y a su vez entre ellos y la sociedad. Pero esto no se puede realizar sin conocer y comprender que entendían por justicia, y definir de qué forma organizaron su administración. En la realización de esta investigación atenderé a las soluciones que el ordenamiento jurídico debía brindar a una determinada sociedad, las instituciones que lo formaron y las ideas de quienes lo gestaron. El uso de la fuente judicial será fundamental para observar si la letra de la ley fue cumplida en la administración de justicia y de qué manera. Pero esa fuente judicial confirmará o no la aplicación de las leyes dictadas por el poder legislativo y los decretos emanados del ejecutivo, por ello será necesario reconstruir los organigramas del Poder Judicial a partir del estudio de los Registros Oficiales y los debates legislativos de la provincia.

Data from sections across the Eurasian Basin of the Arctic Ocean occupied by the German Research Vessel Polarstern in 1987 and by the Swedish icebreaker Oden in 1991 are used to derive information on the freshwater balance of the Arctic Ocean halocline and on the sources of the deep waters of the Nansen, Amundsen and Makarov basins. Salinity, d18O and mass balances allow separation of the river-runoff and the sea-ice meltwater fractions contained in the Arctic halocline. This provides the basis for tracking the river-runoff signal from the shelf seas across the central Arctic Ocean to Fram Strait. The halocline has to be divided into at least three lateral regimes: the southern Nansen Basin with net sea-ice melting, the northern Nansen Basin and Amundsen Basin with net sea-ice formation and increasing river-runoff fractions, and the Canadian Basin with minimum sea-ice meltwater and maximum river-runoff fractions and water of Pacific origin. In the Canadian Basin, silicate is used as a tracer to identify Pacific water entering through Bering Strait and an attempt is made to quantify its influence on the halocline waters of the Canadian Basin. For this purpose literature data from the CESAR and LOREX ice camps are used. Based on mass balances and depending on the value of precipitation over the area of the Arctic Ocean the average mean residence time of the river-runoff fraction contained in the Arctic Ocean halocline is determined to be about 14 or 11 years. Water column inventories of river-runoff and sea-ice meltwater are calculated for a section just north of Fram Strait and implications for the ice export rate through Fram Strait are discussed. Salinity, tritium, 3He and the d18O ratio of halocline waters sampled during the 1987 Polarstern cruise to the Nansen Basin are used to estimate the mean residence time of the river-runoff component in the halocline and on the shelves of the Arctic Ocean. These estimates are done by comparing ages of the halocline waters based on a combination of tracers yielding different time information: the tritium 'vintage' age which records the time that has passed since the river-runoff entered the shelf and the tritium/3He age which reflects the time since the shelf waters left the shelf. The difference between the ages determined by these two methods is about 3 to 6 years. Correction for the initial tritium/3He age of the shelf waters (about 0.5 to 1.5 years) yields a mean residence time of the river-runoff on the shelves of about 3.5 ± 2 years. Comparison of the 18O/16O ratios of shelf water, Atlantic water and the deep waters of the Arctic Ocean indicate that the sources of the deep and bottom waters of the Eurasian Basin are located in the Barents and Kara seas.

El sistema judicial de la provincia de Buenos Aires comenzó su formación como poder del Estado a partir de la reforma rivadaviana y el proceso alcanzó su madurez entre 1853 y 1881, como parte fundamental del proyecto liberal, asentado sobre la autoridad de la ley. En la realización de esta investigación atenderé a las soluciones que el ordenamiento jurídico debía brindar a una determinada sociedad, las instituciones que lo formaron y las ideas de quienes lo gestaron. El uso de la fuente judicial será fundamental para observar si la letra de la ley fue cumplida en la administración de justicia y de qué manera. Pero esa fuente judicial confirmará o no la aplicación de las leyes dictadas por el poder legislativo y los decretos emanados del ejecutivo, por ello será necesario reconstruir los organigramas del Poder Judicial a partir del estudio de los Registros Oficiales y los debates legislativos de la provincia.

La Constitución de 1873, a pesar de ser ideal en algunas de sus propuestas, dio protagonismo y efectividad a las instituciones judiciales que se formaron y pusieron en funcionamiento con una nueva concepción de justicia, que sin dejar de lado el derecho natural se consolidaba en una función más positiva, atenta a los derechos individuales y de propiedad. Para comprender el rol que jugó el sistema de justicia en la formación del Estado es necesario desenmarañar la compleja trama de relaciones entre el poder político y los magistrados, y a su vez entre ellos y la sociedad. Pero esto no se puede realizar sin conocer y comprender que entendían por justicia, y definir de qué forma organizaron su administración. En la realización de esta investigación atenderé a las soluciones que el ordenamiento jurídico debía brindar a una determinada sociedad, las instituciones que lo formaron y las ideas de quienes lo gestaron. El uso de la fuente judicial será fundamental para observar si la letra de la ley fue cumplida en la administración de justicia y de qué manera. Pero esa fuente judicial confirmará o no la aplicación de las leyes dictadas por el poder legislativo y los decretos emanados del ejecutivo, por ello será necesario reconstruir los organigramas del Poder Judicial a partir del estudio de los Registros Oficiales y los debates legislativos de la provincia.

En este artículo se abordará la problemática transición política producida en la provincia de Buenos Aires luego de la batalla de Caseros en febrero de 1852 y el impacto que generó en la campaña. Partimos del siguiente argumento: la intervención política de los habitantes rurales y el grado de dependencia/independencia de sus reclamos se explica en gran medida a través del entramado de relaciones de poder (complejo y cambiante) que se construyó en cada pueblo. En primera instancia, la atención estará puesta en las diferentes expresiones políticas que se manifestaron en el vecindario de la Guardia de Luján durante el rosismo y las repercusiones del día después. Luego, se estudiará el pronunciamiento del General Hilario Lagos del 1º de diciembre de 1852 y la participación del vecindario. Por último, se reseñaran las repercusiones locales producto del derrocamiento de Juan Manuel de Rosas

Cryosols are permafrost-affected soils whose genesis is dominated by cryogenic processes, resulting in unique macromorphologies, micromorphologies, thermal characteristics, and physical and chemical properties. In addition, these soils are carbon sinks, storing high amounts of organic carbon collected for thousands of years. In the Canadian soil classification, the Cryosolic Order includes mineral and organic soils that have both cryogenic properties and permafrost within 1 or 2 m of the soil surface. This soil order is divided into Turbic, Static and Organic great groups on the basis of the soil materials (mineral or organic), cryogenic properties and depth to permafrost. The great groups are subdivided into subgroups on the basis of soil development and the resulting diagnostic soil horizons. Cryosols are commonly associated with the presence of ground ice in the subsoil. This causes serious problems when areas containing these soils are used for agriculture and construction projects (such as roads, town sites and airstrips). Therefore, where Cryosols have high ice content, it is especially important either to avoid these activities or to use farming and construction methods that maintain the negative thermal balance.
