Purpose: High-risk human papillomavirus (HPV) is the main etiologic factor for cervical cancer. The severity of HPV-associated cervical lesions has been correlated to the number of infiltrating macrophages. The objective of this work is to characterize the role of tumor-associated macrophages (TAM) on the immune cellular response against the tumor. Experimental Design: We used the HPV16 E6- and E7-expressing TC-1 mouse tumor model to study the effect of TAM on T-cell function in vitro, and depleted TAM, using clodronate-containing liposomes, to characterize its role in vivo. Results: TAM, characterized by the positive expression of CD45, F4/80, and CD11b, formed the major population of infiltrating tumor cells. TAM displayed high basal Arginase I activity, producing interleukin-10 (IL-10); they were resistant to iNOSll activity induction, therefore reversion to M1 phenotype, when stimulated in vitro with lipopolysaccharide/IFN gamma, indicating an M2 phentoype. In cultures of isolated TAM, TAM induced regulatory phenotype, characterized by IL-10 and Foxp3 expression, and inhibited proliferation of CD8 lymphocytes. In vivo, depletion of TAM inhibited tumor growth and stimulated the infiltration of tumors by HPV16 E7(49-57)-specific CD8 lymphocytes, whereas depletion of Gr1(+) tumor-associated cells had no effect. Conclusions: M2-like macrophages infiltrate HPV16-associated tumors causing suppression of antitumor T-cell response, thus facilitating tumor growth. Depletion or phenotype alteration of this population should be considered in immunotherapy strategies.

Several studies indicate that molecular variants of HPV-16 have different geographic distribution and risk associated with persistent infection and development of high-grade cervical lesions. In the present study, the frequency of HPV-16 variants was determined in 81 biopsies from women with cervical intraepithelial neoplasia grade III or invasive cervical cancer from the city of Belem, Northern Brazil. Host DNAs were also genotyped in order to analyze the ethnicity-related distribution of these variants. Ninie different HPV-16 LCR variants belonging to four phylogenetic branches were identified. Among these, two new isolates were characterized. The most prevalent HPV-16 variant detected was the Asian-American B-2,followed by the European B-12 and the European prototype. Infections by multiple variants were observed in both invasive cervical cancer and cervical intraepithelial neoplasia grade III cases. The analysis of a specific polymorphism within the E6 viral gene was performed in a subset of 76 isolates. The E6-350G polymorphism was significantly more frequent in Asian-American variants. The HPV-16 variability detected followed the same pattern of the genetic ancestry observed in Northern Brazil, with European, Amerindian and African roots. Although African ancestry was higher among women infected by the prototype, no correlation between ethnical origin and HPV-16 variants was found. These results corroborate previous data showing a high frequency of Asian-American variants in cervical neoplasia among women with multiethnic origin.

INTRODUÇÃO: O adenocarcinoma de pâncreas apresenta um mau prognóstico. A utilização de modelos experimentais é necessária para a compreensão do comportamento biológico tumoral, principalmente das lesões precoces (neoplasias intra-epiteliais pancreáticas - NIPan) e para o desenvolvimento de opções terapêuticas. OBJETIVO: Avaliar a carcinogênese pancreática induzida por 7,12-dimetilbenzantraceno (DMBA), em camundongos, aplicando a classificação das neoplasias intra-epiteliais pancreáticas. MÉTODOS: 90 camundongos machos, mus musculus, da cepa CF1, foram submetidos à laparotomia mediana e 1 mg de DMBA foi implantado na porção cefálica do pâncreas. Os animais foram divididos em dois grupos, com eutanásia em 30 e 60 dias. Em seguida, o pâncreas foi retirado, fixado em formalina e foram confeccionadas lâminas coradas com hematoxilina eosina. Os cortes histológicos foram avaliados por dois patologistas de acordo com os seguintes critérios: pâncreas normal, hiperplasia reacional, NIPan 1A, NIPan 1B, NIPan 2, NIPan 3 e carcinoma. As alterações inflamatórias também foram analisadas. RESULTADOS: A avaliação patológica evidenciou, no grupo de 30 dias: 4 (16,7%) animais com hiperplasia reativa, 16 (66,6%) com NIPan e 4 (16,7%) com adenocarcinoma. No grupo de 60 dias: 10 (27,1%) animais com hiperplasia reativa, 13 (35,1%) com NIPan e 14 (37,8%) com adenocarcinoma. A diferença entre os grupos apresentou significância estatistística (P < 0,05 – teste exato de Fisher). A prevalência de alterações inflamatórias em 30 dias foi: pancreatite aguda (n=11), pancreatite crônica (n=5) e inflamação dependente da bolsa (n=8). No grupo de 60 dias 11 espécimes apresentavam pancreatite aguda e 26 pancreatite crônica. CONCLUSÕES: O modelo experimental com DMBA em camundongos, induz neoplasia intra-epitelial pancreática e adenocarcinoma ductal histologicamente semelhantes ao carcinoma pancreático em humanos. Este modelo pode ser utilizado na investigação da carcinogênese com enfoque na progressão molecular das lesões precursoras até o adenocarcinoma.

Prostate differentiation during embryogenesis and its further homeostatic state maintenance during adult life depend on androgens. Abundant biological data suggest that androgens play an important role in the development of the prostate cancer and other prostatic diseases. The objective of this work was to evaluate the effects of the testosterone supplementation in gerbil (a new experimental model) at different ages. Tissues from experimental animals were studied by histological and histochemistry procedures, androgen receptor immunohistochemistry assay, morphometric-stereological analysis, and transmission electron microscopy (TEM). After the treatment were observed increase of prostate weight and epithelium height in all ages studied. In some adult and aged treated animals, hyperplasic and displasic process were observed, including prostatic intraepithelial neoplasias and adenocarcinomas. Increase of the thickness of the smooth muscle cell (SMC) layer was observed in pubescent and adult animals and TEM revealed apparent SMC hypertrophy. An apparent increase in the frequency of blood vessels distributed by the subepithelial stroma in the treated animals was noticed. Reversion of the natural effects of aging on the prostate was observed in the aged treated animals in some acini of the gland. These data demonstrate that the gerbil prostate is susceptible to androgenic action at the studied ages and it can serve, for example, as experimental model to studies of prostate neoplasic process induction and hormonal therapy in aged animals.

Different from the classic view, the prostate is not a gland exclusive to the male, also being an organ of the female genital system presenting morphofunctional similarity between human and rodent. Thus structural, ultrastructural, morphometric-stereological features of the female prostate (Skene's paraurethral gland) and steroid serological levels were evaluated during young, adult, and senile ages in the Mongolian gerbil. The morphofunctional precocity of the female gland in comparison with the male gland occurring in young gland is probably associated with the female circulating steroid levels. The hormonal imbalance in senesce coincides with its susceptibility to histopathological lesions, such as epithelial hypertrophy, metaplasia, and intraepithelial neoplasia. Differently than that of males, the aging degeneration of the female gland involves the accumulation of lipofuscin granules. However, the alterations in senile prostate did not damage its functionality. These analyses reinforce the use of this experimental model for the comprehension of glandular morphofunctional aspects with special attention to senescence. Thus, the appreciation of this organ becomes relevant to avoid future discomfort to women's health.

Purpose: We compared and characterized the effects of intravesical bacillus Calmette-Guerin and/or staphylococcal enterotoxin B for nonmuscle invasive bladder cancer.Materials and Methods: A total of 75 female Fisher 344 rats were anesthetized. of the rats 15 received 0.3 ml saline (control) and 60 received 1.5 mg/kg MNU (N-methyl-n-nitrosourea) intravesically every other week for 6 weeks. The rats were divided into 5 groups. The MNU and control groups received 0.3 ml saline. The bacillus Calmette-Guerin group received 10(6) cfu bacillus Calmette-Guerin. The staphylococcal enterotoxin B group received 10 mu g/ml staphylococcal enterotoxin B. The bacillus Calmette-Guerin plus staphylococcal enterotoxin B group received the 2 treatments simultaneously. Each group was treated intravesically for 6 weeks. At 15 weeks all bladders were collected for histopathological and immunological evaluation, and Western blot.Results: Papillary carcinoma (pTa) and high grade intraepithelial neoplasia (carcinoma in situ) were more common in the MNU group. Papillary hyperplasia was more common in the bacillus Calmette-Guerin and enterotoxin groups. Flat hyperplasia was more common in the bacillus Calmette-Guerin plus enterotoxin group. No significant toxicity was observed. The apoptosis and cellular proliferation indexes decreased in the bacillus Calmette-Guerin, enterotoxin and bacillus Calmette-Guerin plus enterotoxin groups compared to the MNU group. Intensified vascular endothelial growth factor, matrix metalloproteinase-9, Ki-67 and insulin-like growth factor receptor-1 immunoreactivity was verified in the MNU group, moderate in the bacillus Calmette-Guerin and enterotoxin groups, and weak in the bacillus Calmette-Guerin plus enterotoxin and control groups. In contrast, intense endostatin immunoreactivity was verified in the control and bacillus Calmette-Guerin plus enterotoxin groups.Conclusions: Bacillus Calmette-Guerin and staphylococcal enterotoxin B showed similar anti-angiogenic effects. Bacillus Calmette-Guerin plus enterotoxin treatment had additional activity compared to that of monotherapy. It was more effective in restoring apoptosis and balancing cellular proliferation, and it correlated with increased endostatin, and decreased vascular endothelial growth factor, matrix metalloproteinase-9, Ki-67 and insulin-like growth factor receptor-1 reactivity.

Com a finalidade de contribuir para um melhor conhecimento do eventual papel do Papilomavirus humano (HPV) na etiopatogênese do câncer cervical na região norte do Brasil, estudou-se a prevalência do vírus em 228 mulheres portadoras de lesões de cérvix uterina, atendidas no Instituto Ofir Loiola (IOL), em Belém, Pará, no período de março de 1992 a maio de 1996. As pacientes foram submetidas à biópsia de colo uterino, sendo o material coletado encaminhado para histopatologia e pesquisa de HPV por diferentes técnicas laboratoriais. Para fins de análise, as participantes foram distribuídas em 3 grupos, de acordo com o diagnóstico histopatológico. O grupo A foi constituído de 155 mulheres com carcinoma epidermóide invasor ou adenocarcinoma, o grupo B de 54 portadoras de neoplasia intraepitelial cervical grau II ou III (NIC II e NIC 111) e o C, de 19 pacientes com cervicite crônica. Pelas técnicas de PCR e hibridização por dot-blot, registraram-se prevalência de HPV em 70,3%, 63,0% e 36,8% das mulheres reunidas nos grupamentos A, B e C, respectivamente. O tipo de HPV predominante foi o 16, que representou 60,4% das amostras positivas do grupo A e 54,5% daquelas do grupo B. Os HPV tipo 16, 18 e 33 representaram 71,4% dos detectados no grupo C. Em 155 das 228 amostras testadas por PCR, realizou-se também a técnica de hibridização in situ (HIS) com sondas para detectar HPV 6/11, 16/18 e 31/33/35. A prevalência de HPV registrada por essa técnica foi de 17,4%, enquanto que por PCR observou-se, nas mesmas amostras, percentual Positivo de 65,2%. No que diz respeito a outros fatores, também tidos como implicados no desenvolvimento de carcinomas e lesões precursoras em cérvix uterina, verificou-se que cerca de 40% das mulheres dos grupos A e B admitiram iniciação sexual precoce (com 15 anos ou menos). Entretanto, a grande maioria referiu de 1 a 3 parceiros, não caracterizando comportamento sexual promíscuo. Os dados aqui apresentados, a exemplo do que se registrou em outros estudos conduzidos em diversas áreas geográficas, sustentam a hipótese de que o HPV desempenha um importante papel na etiologia dos carcinomas de colo uterino e neoplasias intraepiteliais cervicais. Contudo, investigações adicionais e mais amplas devem ser realizadas, com vistas a uma melhor compreensão das características epidemiológicas da infecção por HPV na Região Amazônica.

Observaram-se prevalências de HPV em 70,3%, 63,0% e 36,8% das mulheres dos grupamentos A, B e C, respectivamente, sendo o HPV 16 registrado em 60,4% das amostras positivas do grupo A e 54,5% daquelas do grupo B. Os tipos 16, 18 e 33 representaram 71,4% dos detectados no grupo C.

O câncer do colo do útero é um dos graves problemas de saúde pública no mundo e no Brasil. O exame colpocitológico e a educação em saúde são estratégias fundamentais para o êxito do programa de prevenção do câncer do colo do útero nos serviços de saúde pública. Foi realizado um estudo transversal e analítico, com o objetivo de identificar e discutir os fatores interferentes nas estratégias de controle do câncer do colo do útero, com ênfase na infecção pelo vírus HPV, sob a perspectiva das clientes do programa. A população do estudo foi de 858 mulheres atendidas nas unidades de saúde pública, em duas estratégias de Saúde da Família e três unidades básicas de saúde, sendo uma um Centro de Saúde Escola, no Município de Belém/PA. Os dados foram coletados através de entrevista. Entre os fatores que interferem negativamente na efetividade das estratégias do respectivo programa estão incluídos: o perfil de clientela quanto a idade, grau de instrução e renda familiar; as características de atendimento e tipo de demanda do programa; o conhecimento de mulheres sobre câncer do colo do útero, neoplasia intraepitelial cervical, vírus HPV e vacina anti-hpv e; o uso das práticas educativas pelas unidades de saúde deste estudo. Conclui-se que intervenções na organização dos serviços de saúde pública podem contribuir na efetividade dessas estratégias, como a efetivação da busca ativa para melhorar o acesso das mulheres nas faixas etárias de maior prevalência de infecção pelo HPV e das mulheres em atraso com o exame colpocitológico anual. Assim como, o estabelecimento das práticas educativas como um recurso organizacional permanente para elevar o nível de conhecimento de mulheres sobre câncer do colo do útero e vírus HPV; além de tornar o monitoramento efetivo de mulheres sob controle anual. Enfim buscar ferramentas organizacionais na perspectiva da prevenção primária e secundária importantes para o controle do câncer do colo do útero e do vírus HPV.

Objective: To compare cervical cytology test results among pregnant and non-pregnant women, and to assess associations with age, screening history, and onset of sexual intercourse. Methods: A retrospective analysis was conducted of cervical smears obtained from women aged 18-34 years in the Campinas region of Brazil between January 2000 and December 2009. Eligible participants had not undergone cytological screening within the previous year and had no history of precursor lesions or cervical cancer. Multinomial logistic regression was performed for different age groups, with high-grade squamous intraepithelial lesions (HSILs) as the endpoint. Results: Overall, 3072 (0.4%) of 861 353 non-pregnant women and 135 (0.4%) of 37 568 pregnant women had HSILs. Odds of HSIL among pregnant and non-pregnant women did not differ in any age group. An increased age at first sexual intercourse among pregnant women reduced odds of HSILs in all age groups (odds ratio 0.9 [95% confidence interval 0.8-0.9] for all). Among women aged 21-24 years, 25-29 years, and 30-34 years, some associations were identified between an interval of less than 5 years since previous screening and reduced odds of HSILs. Conclusion: Mandatory cervical cytology screening does not seem to be necessary for pregnant women; protocols in place for non-pregnant women should be followed. Objectives: Robust evidence now supports human papillomavirus (HPV) testing as a more effective option to screening and as more sensitive than cytology in detecting high-grade cervical intraepithelial neoplasia. Our goal was to analyze the performance of the Hybrid Capture II (HC2) assay for high-risk HPV (hrHPV) in women undergoing gynecological examination at a public health hospital as part of the evaluation of HPV screening as an alternative or complement to cytology. Study Design: This analysis is a subset of a cross-sectional study carried out at a large public hospital serving a predominantly low-resource population. A total of 705 women were enrolled; the sensitivity and specificity of each test were estimated and compared. Results: The analysis identified 272 hrHPV-positive women (mean age 36.3 years) and 433 hrHPV-negative women (mean age 41.2 years). HPV testing showed a significantly increased sensitivity of the HC2 assay versus cytology (84.5 vs. 69.7%; p < 0.0001) but a lower specificity (49.90 vs. 88.78%; p < 0.0001). Background: The objective of this study was to improve the feasibility of applying topic 5-aminolevulinic acid (ALA) in photodiagnosis (PD) and treatment of condyloma caused by human papillomavirus (HPV) using two homemade handheld devices and to discuss the photodynamic therapy (PDT) as a suitable alternative for each of the cases studied. Both, protoporphyrin IX production and photodegradation were analyzed, and the pain experienced during the illumination was correlated with the light intensities. Methods: A total of 40 women with different grades of lesions caused by HPV were chosen from patients of the School of Medicine of Ribeirao Preto (University of Sao Paulo) and of the Unit of Public Health of Araraquara, Sao Paulo. Results: We did not encounter any unexpected difficulties using our devices during the treatment. The existence of an easily observable reddish fluorescence with large intensity concentrated on the Lesions is the clinical indication of the penetration and the selective concentration of protoporphyrin IX in the clinical and subclinical lesions rather than in the healthy tissue. The aesthetic results were much better than those obtained by conventional techniques as surgery or cryogenics, with no recurrence reported after two years of treatment. Conclusions: Our results are proof for the various advantages using ALA cream for the PD and PDT in many different cases of condyloma by HPV. This study will be continued to investigate the PpIX photobleaching and the irradiance and fluence rate to optimize conducting the clinical trials, to improve the devices and therefore increase the treatment response. Background: In addition to the oncogenic human papillomavirus (HPV), several cofactors are needed in cervical carcinogenesis, but whether the HPV covariates associated with incident i) CIN1 are different from those of incident ii) CIN2 and iii) CIN3 needs further assessment. Objectives: To gain further insights into the true biological differences between CIN1, CIN2 and CIN3, we assessed HPV covariates associated with incident CIN1, CIN2, and CIN3. Study Design and Methods: HPV covariates associated with progression to CIN1, CIN2 and CIN3 were analysed in the combined cohort of the NIS (n = 3,187) and LAMS study (n = 12,114), using competing-risks regression models (in panel data) for baseline HR-HPV-positive women (n = 1,105), who represent a sub-cohort of all 1,865 women prospectively followed-up in these two studies. Results: Altogether, 90 (4.8%), 39 (2.1%) and 14 (1.4%) cases progressed to CIN1, CIN2, and CIN3, respectively. Among these baseline HR-HPV-positive women, the risk profiles of incident GIN I, CIN2 and CIN3 were unique in that completely different HPV covariates were associated with progression to CIN1, CIN2 and CIN3, irrespective which categories (non-progression, CIN1, CIN2, CIN3 or all) were used as competing-risks events in univariate and multivariate models. Conclusions: These data confirm our previous analysis based on multinomial regression models implicating that distinct covariates of HR-HPV are associated with progression to CIN1, CIN2 and CIN3. This emphasises true biological differences between the three grades of GIN, which revisits the concept of combining CIN2 with CIN3 or with CIN1 in histological classification or used as a common end-point, e.g., in HPV vaccine trials.

We sought to evaluate the performance of diagnostic tools to establish an affordable setting for early detection of cervical cancer in developing countries. We compared the performance of different screening tests and their feasibility in a cohort of over 12,000 women: conventional Pap smear, liquid-based cytology, visual inspection with acetic acid (VIA), visual inspection with Iodine solution (VILI), cervicography, screening colposcopy, and high-risk human papillomavirus (HPV) testing (HR-HPV) collected by physician and by self-sampling. HR-HPV assay collected by the physician has the highest sensitivity (80 %), but high unnecessary referrals to colposcopy (15.1 %). HR-HPV test in self-sampling had a markedly lower (57.1 %) sensitivity. VIA, VILI, and cervicography had a poor sensitivity (47.4, 55, and 28.6 %, respectively). Colposcopy presented with sensitivity of 100 % in detecting CIN2+, but the lowest specificity (66.9 %). Co-testing with VIA and VILI Pap test increased the sensitivity of stand-alone Pap test from 71.6 to 87.1 % and 71.6 to 95 %, respectively, but with high number of unnecessary colposcopies. Co-testing with HR-HPV importantly increased the sensitivity of Pap test (to 86 %), but with high number of unnecessary colposcopies (17.5 %). Molecular tests adjunct to Pap test seems a realistic option to improve the detection of high-grade lesions in population-based screening programs.
