Fallvignette : Herr Blanc ist ein 78-jähriger Patient mit Hypertonie, Prostatabeschwerden (ohne Inkontinenz), chronischen Lumbalgien und Arthrose im rechten Knie. Er wird zu seinem halbjährlichen Kontrolltermin vorstellig. Der Patient führt von sich aus an Beschwerden lediglich eine leichte Verstärkung der chronischen Lumbalgien an und dass er sich in den letzten Monaten ein wenig erschöpfter und lustloser fühlt. Er erklärt dies mit dem trüben, kalten Wetter («was für ein verregneter Sommer!»), wodurch er nicht so oft wie gewünscht aus dem Haus kam. Bei diesem Patienten von eher guter Gesundheit könnte die Sprechstunde hier bereits enden, nachdem man ihm vielleicht noch einige Ratschläge gegeben hat, wie er (angesichts seiner Erschöpfung) ein wenig ausspannen kann, und sein Paracetamol-Rezept erneuert wurde. Die Sprechstunde könnte (sollte!) jedoch auch eine Gelegenheit sein, um bei diesem älteren Patienten eine gesundheitliche Situation mit hohem Risiko für funktionellen Abbau zu erkennen und ihn zu motivieren, sich wieder mehr zu bewegen. Dieser Artikel mit praktischem Ansatz soll dazu anregen, wenig aktive ältere Patienten, die sich in unserer Praxis vorstellen, nach einer initialen Erfassung ihres Bewegungsverhaltens zu informieren, zu beraten und sie von den Vorteilen zu überzeugen, die angemessene Bewegung zur Aufrechterhaltung von Selbständigkeit, Autonomie und Lebensqualität leisten kann.

BACKGROUND: The writing of prescriptions is an important aspect of medical practice. This activity presents some specific problems given a danger of misinterpretation and dispensing errors in community pharmacies. The objective of this study was to determine the evolution of the prescription practice and writing quality in the outpatient clinics of our paediatric university hospital.¦METHODS: Copies of prescriptions written by physicians were collected from community pharmacies in the region of our hospital for a two-month period in 2005 and 2010. They were analysed according to standard criteria, including both formal and pharmaceutical aspects.¦RESULTS: A total of 597 handwritten prescriptions were reviewed in 2005 and 633 in 2010. They contained 1,456 drug prescriptions in 2005 and 1,348 in 2010. Fifteen drugs accounted for 80% of all prescriptions and the most common drugs were paracetamol and ibuprofen. A higher proportion of drugs were prescribed as International Nonproprietary Names (INN) or generics in 2010 (24.7%) compared with 2005 (20.9%). Of the drug prescriptions examined, 55.5% were incomplete in 2005 and 69.2% in 2010. Moreover in 2005, 3.2% were legible only with difficulty, 22.9% were ambiguous, and 3.0% contained an error. These proportions rose respectively to 5.2%, 27.8%, and 6.8% in 2010.¦CONCLUSION: This study showed that fifteen different drugs represented the majority of prescriptions, and a quarter of them were prescribed as INN or generics in 2010; and that handwritten prescriptions contained numerous omissions and preventable errors. In our hospital computerised prescribing coupled with advanced decision support is eagerly awaited.

Sumário O presente estudo envolveu utentes que usufruíram dos serviços prestados pelas farmácias comunitárias (FX e FY) na cidade da Praia, baseado num levantamento de dados feitos a partir de observações e questionários. Na população estudada foi analisado características relevantes e constatou-se o seguinte nas respectivas farmácias (FX / FY): prescrição de anti-bacterianos (63% FY FX / 60% FY ), analgésicos e antipiréticos (24% ) e anti-histamínicos (19% FX / 18% FY FX / 26% FY ), anti-hipertensores (19% ); com denominação genérica (100% ); prescreve-se uma (1) embalagem (83% FY ), suspensões (43% FX / 36% FY FX / 72% ), cápsulas (29% FY ); em forma de comprimidos (76% FX / 36% FY ), e pomadas (27% ); administradas via oral (91% FX /96% FY ), dérmica (26% FX / 3% ); a dosagem em miligramas (86% FX FX / 54% / 94% FY FY ) e em mililitros (54% ), pelo médico ORL (11% FX FX / 44% / 6% FY FY FY FX FX / 16% / 88% FX FX FY / 82% / 38% ); atendimento pelo médico clínico geral (53% ), e restantes pelos médicos ginecologista (7% / 6% FY ), cirurgião (7% FX / 16% FY ) e pediatra (3% FX / 2% ); atendidas em clínicas área adulta (29% FX / 48% FY ), e em hospital área pediatria (18% FX FY / 8% ). Da avaliação feita ao receituário quanto a presença de elementos essenciais, destaca-se a ausência de posologia (10% FY ) e data (1% FX / 6% FY FY ). Quanto a legibilidade, em parte ausente, o medicamento prescrito (4% FX / 14% FY ) e o carimbo médico (28% FX / 28% ). Os utentes automedicaram de forma significativa nas farmácias comunitárias (FX e FY), e os medicamentos mais usados foram: Paracetamol (23%), Ibuprofeno (18%), Hidroclorotiazida (16%), Ácido ascórbico (10%), e Diclofenac de sódio (9%), justificando a prática segundo sintomas como, febre e dores (42%), inflamações cutâneas e dores (34%), HTA (30%), e dores nas articulações e músculos (17%). Os resultados demostram que o uso racional de medicamentos depende de uma prescrição médica racional, automedicação responsável, e intervenções específicas para melhorias de qualidade de serviços de saúde.

Tolperisone (Mydocalm) is a centrally acting muscle relaxant with few sedative side effects that is used for the treatment of chronic pain conditions. We describe three cases of suicidal tolperisone poisoning in three healthy young subjects in the years 2006, 2008 and 2009. In all cases, macroscopic and microscopic autopsy findings did not reveal the cause of death. Systematic toxicological analysis (STA) including immunological tests, screening for volatile substances and blood, urine and gastric content screening by GC-MS and HPLC-DAD demonstrated the presence of tolperisone in all cases. In addition to tolperisone, only the analgesics paracetamol (acetaminophen), ibuprofen and naproxen could be detected. The blood ethanol concentrations were all lower than 0.10 g/kg. Tolperisone was extracted by liquid-liquid extraction using n-chlorobutane as the extraction solvent. The quantification was performed by GC-NPD analysis of blood, urine and gastric content. Tolperisone concentrations of 7.0 mg/l, 14 mg/l and 19 mg/l were found in the blood of the deceased. In the absence of other autopsy findings, the deaths in these three cases were finally explained as a result of lethal tolperisone ingestion. To the best of our knowledge, these three cases are the first reported cases of suicidal tolperisone poisonings.

Introduction Health care professionals' perception of risk mayimpact on therapeutic management of women during pregnancy.Since the thalidomide tragedy, the use of drugs during pregnancygenerates fear. This concern might affect the estimation of the riskassociated with drug intake during pregnancy, leading to prematurediscontinuation of a required treatment, superfluous anxiety orpointless termination of a desired pregnancy. Although data regardingthe security of drugs during pregnancy are still scarce, a few specializedinformation sources exist providing reliable recommendationsfor daily practice. This study aimed at characterizing therisk perception associated with drugs during pregnancy in a sample ofSwiss health care professionals.Materials & Methods An online French and German survey was sentby email to the Swiss professional societies of Pharmacists, Gynecologists,Mid-wives and Pediatricians. The questionnaire wasconstructed to assess (a) the characteristics of the population and theopinion of the professionals regarding the medication use pattern intheir pregnant patients, (b) to evaluate the sources of information usedduring their practice and finally (c) to assess their risk perceptionassociated with drugs during pregnancy. Results were analyzed bydescriptive statistics.Results A total of 1,310 questionnaires were collected (18% responserate). Most health care professionals believe that 30-60% of theirpregnant patients are taking at least one treatment during their pregnancyand that 80% are adherent to it. A large majority think,however, that women are anxious when they must take their medication.More than 80% of health professionals commonly use theSwiss Drug Reference Book (Compendium) to assess the risk associatedwith drugs during pregnancy, despite the uniformly low levelof credibility and utility they express about this reference. Except forsome gynecologists, the majority of professionals are not aware of ordo not use specialized books. The majority of participants thinkwrongly that more than 30% of drugs are teratogenic. About 20% ofthem are not aware of the risk associated with paracetamol intakeduring pregnancy. More than 70% agree that phytotherapeutic mixturesare not safer than conventional drugs, with the exception of midwiveswho tend to overestimate the safety of such drugs. With thenotable exception of gynecologists, the risk related to drug intake wasoverall overestimated.Discussion & Conclusion Swiss professionals differ in their perceptionof the risk associated with drugs during pregnancy and tend tooverestimate it. The differences might be attributed to the level oftraining and awareness of specialized sources offering a realisticestimation of the risk. Further efforts are needed to expand thetraining and the tools for health care professionals to optimize druguse during pregnancy.

Treatment of pediatric fever is based on two main molecules, paracetamol and ibuprofen. Fever should be treated when associated with discomfort. The two molecules have almost similar efficacy and safety. Monotherapy should be preferred to a combined or alternating treatment. Antipyretics do not seem to prevent febrile seizures.

Treatment options for eosinophilic esophagitis (EoE) include drugs, diets and esophageal dilation. Esophageal dilation can be performed using either through-the-scope balloons or wire-guided bougies. Dilation can lead to long-lasting symptom improvement in EoE patients presenting with esophageal strictures. Esophageal strictures are most often diagnosed when the 8- to 9-mm outer diameter adult gastroscope cannot be passed any further or only against resistance. A defined esophageal diameter to be targeted by dilation is missing, but the majority of patients have considerable symptomatic improvement when a diameter of 16-18 mm has been reached. A high complication rate, especially regarding esophageal perforations, has been reported in small case series until 2006. Several large series were published in 2007 and later that demonstrated that the complication risk (especially esophageal perforation) was much lower than what was reported in earlier series. The procedure can therefore be regarded as safe when some simple precautions are followed. It is noteworthy that esophageal dilation does not influence the underlying eosinophil-predominant inflammation. Patients should be informed before the procedure that postprocedural retrosternal pain may occur for some days, but that it usually responds well to over-the-counter analgesics such as paracetamol. Dilation-related superficial lacerations of the mucosa should not be regarded and reported as complications, but instead represent a desired effect of the therapy. Patient tolerance and acceptance for esophageal dilation have been reported to be good.

PRINCIPLE: Healthcare professionals' (HCPs') perception of risk associated with drug use in pregnancy may have an impact on the pharmacological treatment of some women. The aim of this study was to examine this risk perception in a sample of Swiss HCPs with a special focus on their knowledge and use of available specialised information sources. METHOD: An online, French and German, questionnaire was e-mailed to 7,136 members of four Swiss professional societies (gynaecologists, paediatricians, midwives and pharmacists). The questionnaire was designed (a) to collect demographic characteristics, (b) to evaluate the frequency of use of several specialised sources of information on drugs in pregnancy in their daily practice, and (c) to examine the perception of risk associated with drug use during pregnancy. RESULTS: A total of 1,310 questionnaires were collected (response rate of 18.4%). More than 80% of the respondent HCPs use the Swiss Drug Reference Book (Compendium) to assess the risk associated with drugs during pregnancy and are not aware of available specialised information sources (books, websites or information centres). Despite some disparities between HPCs, the risk related to drug intake was overall highly misperceived. Blinded reading of three product monographs in the Compendium was associated with an overestimated perception of risk (e.g., after reading the "paracetamol" monograph, 38% of the participants stated they would probably not advise the use of this drug to a pregnant patient). CONCLUSION: Overall, an overestimation of the risk associated with drug use during pregnancy has been observed in our sample of HCPs, which might be related to the underuse of specialised information source among other factors. These findings evidenced the need for increased training for HCPs in order to optimise medication use during pregnancy. Further studies are needed to confirm these results and identify causes.

The objectives of this study were 1) to determine the evolution of the outpatient prescription practice in our paediatric university hospital during 2 periods separated by 5 years; 2) to assess the writing quality of outpatient prescriptions during the same period.Materials & Methods Design: Copies of prescriptions written by physicians were collected twice from community pharmacies in the region of our hospital for a 2-month period in 2005 and 2010. They were analysed according to standard criteria regarding both formal and pharmaceutical aspects. Drug prescriptions were classified as a) complete when all criteria for safety were fulfilled, b) ambiguous when there was a danger of a dispensing error because of one or more missing criteria, or c) containing an error.Setting: Paediatric university hospital.Main outcome measures: Proportion of generic drugs; outpatient prescription writing quality.Results: A total of 651 handwritten prescriptions were reviewed in 2005 and 693 in 2010. They contained 1570 drug prescriptions in 2005 (2.4 ± 1.2 drugs per patient) and 1462 in 2010 (2.1 ± 1.1). The most common drugs were paracetamol, ibuprofen, and sodium chloride. A higher proportion of drugs were prescribed as generic names or generics in 2010. Formal data regarding the physicians and the patients were almost complete, except for the patients' weight. Of the drug prescriptions, 48.5% were incomplete, 11.3% were ambiguous, and 3.0% contained an error in 2005. These proportions rose to 64.2%, 15.5% and 7.4% in 2010, respectively.Discussions, Conclusion This study showed that physicians' prescriptions comprised numerous omissions and errors with minimal potential for harm. El embarazo conlleva una serie de cambios orgánicos, fisiológicos (del sistema cardiovascular y gastrointestinal, de la función pulmonar, hematológicos...) y de conducta que pueden repercutir en la cavidad bucal. El granuloma gravídico se considera una entidad patológica propia del embarazo. Podremos observar también en las pacientes gestantes un aumento en la incidencia de caries, asociado fundamentalmente a un incremento de Jos factores locales cariogénicos (descuido de la higiene bucal, cambio de Jos hábitos dietéticos y horarios...), y un posible empeoramiento de la patología gingival y periodontal asociado a las variaciones hormonales que acompañan al embarazo y a factores locales irritativos. El segundo trimestre de gestación será el momento ideal para llevar a cabo cualquier tratamiento dental rutinario. Deberemos ir con sumo cuidado en lo que se refiere a la administración de fármacos (penicilina, eritromicina, cefalosporinas de primera generación, paracetamol y lidocaína con vasoconstrictor se consideran fármacos de prescripción segura durante el embarazo) y la realización de radiografías dentales (es imprescindible colocar un delantal plomado a la paciente).

Objectives: To compare upper lip frenulum reinsertion, bleeding, surgical time and surgical wound healing in frenectomies performed with the CO2 laser versus the Er, Cr:YSGG laser. Study design: A prospective study was carried out on 50 randomized pediatric patients who underwent rhomboidal resection of the upper lip frenulum with either the CO2 laser or the Er,Cr:YSGG laser. Twenty-five patients were assigned to each laser system. All patients were examined at 7, 14, 21 days and 4 months after the operation in order to assess the surgical wound healing. Results: Insertion of the frenulum, which was preoperatively located between the upper central incisors, migrated to the mucogingival junction as a result of using both laser systems in all patients. Only two patients required a single dose of 650 mg of paracetamol, one of either study group. CO2 laser registered improved intraoperative bleeding control results and shorter surgical times. On the other hand, the Er,Cr:YSGG laser achieved faster healing. Conclusions: Upper lip laser frenectomy is a simple technique that results in minimum or no postoperative swelling or pain, and which involves upper lip frenulum reinsertion at the mucogingival junction. The CO2 laser offers a bloodless field and shorter surgical times compared with the Er,Cr:YSGG laser. On the other hand, the Er,Cr:YSGG laser achieved faster wound healing.

Which treatments are used for dysmenorrhea and with what reported outcome? A questionnaire was sent to 2400 students and apprentices, following the "retrospective treatment-outcome" method. The response rate was 22%. Most frequent treatments used are ibuprofene (53%), paracetamol (51%), hormonal contraception (40%), hot-water bottle (or hot pad) (35%), food supplements or medicinal plants (23%). Physicians only discuss a tiny proportion of dysmenorrhea treatment in their consultation, because it is mostly a matter of self-treatment, with the family as the source of information in 80% of the cases. Rather surprising because not mentioned in most official guidelines, hot-water bottle (or hot pad) appears as the treatment followed by the best reported outcome (satisfactory in 92% of users).

The illicit drug cutting represents a complex problem that requires the sharing of knowledge from addiction studies, toxicology, criminology and criminalistics. Therefore, cutting is not well known by the forensic community. Thus, this review aims at deciphering the different aspects of cutting, by gathering information mainly from criminology and criminalistics. It tackles essentially specificities of cocaine and heroin cutting. The article presents the detected cutting agents (adulterants and diluents), their evolution in time and space and the analytical methodology implemented by forensic laboratories. Furthermore, it discusses when, in the history of the illicit drug, cutting may take place. Moreover, researches studying how much cutting occurs in the country of destination are analysed. Lastly, the reasons for cutting are addressed. According to the literature, adulterants are added during production of the illicit drug or at a relatively high level of its distribution chain (e.g. before the product arrives in the country of destination or just after its importation in the latter). Their addition seems hardly justified by the only desire to increase profits or to harm consumers' health. Instead, adulteration would be performed to enhance or to mimic the illicit drug effects or to facilitate administration of the drug. Nowadays, caffeine, diltiazem, hydroxyzine, levamisole, lidocaïne and phenacetin are frequently detected in cocaine specimens, while paracetamol and caffeine are almost exclusively identified in heroin specimens. This may reveal differences in the respective structures of production and/or distribution of cocaine and heroin. As the relevant information about cutting is spread across different scientific fields, a close collaboration should be set up to collect essential and unified data to improve knowledge and provide information for monitoring, control and harm reduction purposes. More research, on several areas of investigation, should be carried out to gather relevant information.

A partir de una primera experiencia en el desarrollo de una aplicación interactiva multimedia, como fue la elaboración de comprimidos infantiles de paracetamol y aplicación de las Normas de Correcta Fabricación , y visto que ha tenido una gran aceptación por parte de los alumnos, se decidió el realizar una nueva aplicación interactiva multimedia que considerara la otra práctica que se realizaba en la planta piloto del SDM (Servei de Desenvolupament del Medicament), la cual consiste en la elaboración de una emulsión silicónica. Ambas prácticas se llevan a cabo presencialmente en la asignatura de Tecnología Farmacéutica II. En esta segunda aplicación multimedia, si bien se ha mantenido una estructura similar a la primera editada, se ha intentado mejorar todos aquellos aspectos que surgieron durante la programación y la fase de pruebas de dicha multimedia, como por ejemplo el más importante que ha sido el utilizar un tipo de programación que permita visualizar la actividad a través de Internet entre otras muchas mejoras. Además en esta nueva aplicación se ha introducido una actividad que consiste en un simulador de la elaboración de una emulsión en donde los alumnos podrán probar diferentes parámetros de fabricación (velocidad y tiempo de agitación) para encontrar aquellos que den unas características de viscosidad y extensión lo más óptimas posibles. A día de hoy faltan todavía algunos ajustes a dicha aplicación que se pretende ensayar en el curso académico 2004-2005 donde se valorará por parte de los alumnos y profesores su aplicabilidad y su aceptación real. En esta comunicación se presenta las áreas desarrolladas.
