Monaghan General Hospital – Proposals for further and future development The Independent Review Panel recommended a review of the existing protocol for emergency obstetric cases presenting at Hospitals such as Monaghan which have no on-site obstetric expertise, and the implementation of a revised protocol. BACKGROUND: Despite the progress over recent decades in developing community mental health services internationally, many people still receive treatment and care in institutional settings. Those most likely to reside longest in these facilities have the most complex mental health problems and are at most risk of potential abuses of care and exploitation. This study aimed to develop an international, standardised toolkit to assess the quality of care in longer term hospital and community based mental health units, including the degree to which human rights, social inclusion and autonomy are promoted. METHOD: The domains of care included in the toolkit were identified from a systematic literature review, international expert Delphi exercise, and review of care standards in ten European countries. The draft toolkit comprised 154 questions for unit managers. Inter-rater reliability was tested in 202 units across ten countries at different stages of deinstitutionalisation and development of community mental health services. Exploratory factor analysis was used to corroborate the allocation of items to domains. Feedback from those using the toolkit was collected about its usefulness and ease of completion. RESULTS: The toolkit had excellent inter-rater reliability and few items with narrow spread of response. Unit managers found the content highly relevant and were able to complete it in around 90 minutes. Minimal refinement was required and the final version comprised 145 questions assessing seven domains of care. CONCLUSIONS: Triangulation of qualitative and quantitative evidence directed the development of a robust and comprehensive international quality assessment toolkit for units in highly variable socioeconomic and political contexts

Objetivamos perceber como se dá a produção de subjetividade dentro dos novos serviços de saúde mental, utilizando como referencial teórico-metodológico uma abordagem inovadora chamada Sociopoética. A produção dos dados deu-se por meio de oficinas com a equipe de trabalhadores de saúde mental de um hospital-dia. Como resultados percebemos aspectos institucionais, sociais, econômicos e pessoais referenciados pela equipe como sendo aqueles mais relevantes na composição do quadro de sua prática dentro dos novos serviços. Concluímos que o processo de pesquisa mostrou-se um importante instrumento de auto-análise, permitindo ao grupo repensar sua prática e multiplicar as possibilidades de produção de sentido para suas experiências.

Relatamos uma investigação para apreender temas que explicam o trabalho de enfermagem, no Hospital-Dia, na perspectiva da reforma psiquiátrica em João Pessoa-PB. Utilizamos o referencial do materialismo histórico e dialético e o trabalho como categoria analítica. O material empírico foi analisado pela técnica de análise do discurso. O estudo revelou um tema coincidente, mudar o modelo de assistência psiquiátrica tradicional, e, nesse sentido, o trabalho de enfermagem incorpora além da qualidade formal, técnica-especializada, uma qualidade política indicando um novo significado ao processo de trabalho de enfermagem.

Este trabalho é recorte de uma investigação que buscou identificar o cotidiano de pessoas com transtornos mentais, que sofrem reinternações psiquiátricas. Nesse percurso foi possível evidenciar que esses sujeitos expressam diversas concepções do processo saúde/doença mental e que caracterizam as formas de tratamento que se utilizam. Utilizou-se como metodologia a abordagem qualitativa fundamentada no conceito de cotidiano da autora Agnes Heller. As entrevistas de pacientes e familiares foram submetidas à análise de discurso que, entre outros resultados, revelou que essa população passou a ter novas ferramentas e possibilidades para compreender a loucura e a assistência ao doente mental.

Este estudo teve como objetivo o processo de construção do significado de desmedicalização para enfermeiras obstétricas. Trata-se de pesquisa qualitativa baseada nos princípios da Grounded Theory. Os dados foram obtidos e analisados entre fevereiro e abril de 2006. Foram entrevistadas oito enfermeiras obstétricas atuantes no ensino, pesquisa e/ou assistência ao parto, que adotam a proposta desmedicalizadora como orientação da sua prática. A análise dos dados levou à construção de quatro categorias: refletindo sobre sua prática, caracterizando a prática obstétrica hospitalar como medicalizada, incomodando-se com a assistência medicalizada e identificando os princípios da desmedicalização, cuja integração permitiu identificar o processo construindo o significado de desmedicalização. O respeito à Fisiologia, bem como não usar desnecessariamente práticas intervencionistas como rotina constituíram princípios da desmedicalização. A reflexão sobre sua vida e a caracterização da prática obstétrica hospitalar, como medicalizada, fizeram parte do processo mental das enfermeiras na desconstrução das habilidades adquiridas no modelo biomédico.

Given the important modifications of the "Loi sur l'Assurance maladie (LAMal)", this article gives a contribution to the hospital planification by identifying the main factors that have determined the current organisation of the psychiatric care network. We notice a gap between the orientations of these networks and the funding scheme forecast in the framework of the LAMal. In order to preserve the progressions of these last years and to avoid the negative effects of a too restrictive funding act for the assignment of the public psychiatry, the planification must result in a consensus between the state, the insurances and the multiple actors of the mental health. Otherwise, this will be done to the detriment of the activities of secondary prevention, of coordination in the network, of support to the natural helpers, and of intervention to the vulnerable populations.

O Serviço de Emergência Psiquiátrica em Hospital Geral (SEPHG) é uma proposta articulada com o movimento da reforma psiquiátrica. Objetivou-se caracterizar os clientes com sofrimento psíquico assistidos no SEPHG Dr. Estevam, em Sobral-CE. Este é um estudo do tipo documental, com abordagem quantitativa, envolvendo 191 clientes atendidos no SEPHG no período de janeiro a dezembro de 2007. Os dados foram coletados a partir de um livro de registro, cujas informações nele contidas foram retiradas dos prontuários dos clientes. Observou-se predomínio de pacientes do sexo masculino (70,15%), com idade entre 30-49 anos (48,71%) e solteiros (74,86%). A maioria era proveniente da cidade de Sobral (69,64 %). Em 42,40% dos casos, o diagnóstico foi transtorno do uso de álcool. Grande parte da clientela (66,50%) deu entrada no serviço por demanda espontânea. Após avaliação do SEPHG, 43,45% desses clientes foram encaminhados ao CAPS-ad. Pelos resultados, depreende-se o quão imprescindíveis são os serviços de saúde mental.

Introduction: Patients who repeatedly attend the Emergency Department (ED) often have a distinct and complex vulnerability profile that includes poor somatic, psychological, and social indicators. This profile has an impact on the patients' well-being as well as on hospital costs. The objective of the study was to specify the characteristics of hyper users (HU) and explore the connection with ED care and hospital costs. Methods: The study sample comprised all adult patients with 12 or more attendances at the ED of the Lausanne University Hospital in 2009. The data were collected by retrospectively searching internal databases to identify the patients concerned and then analysing the profiles of these patients. Information gathered included demographic, somatic, psychological, at-risk behaviour, and social indicators, and health system consumption including costs. Results: In 2009, 23 patients (0.1%) attended 12 times or more (425 attendances, 0.8%). The average age was about 43 years, 60.9% were female, and 47.8% single. Of these 95.7% had basic insurance, 87.0% had a general practitioner, and 30.4% were under legal guardianship. The majority attended in the evening or at night (67.1%), and almost one quarter of these attendances resulted in inpatient treatment (24.0%). Most HU had attended the ED in previous years too (95.7% in 2008). The most prevalent diagnoses concerned 'mental disorders' (87.0%). About 30.4% of patients had attempted suicide (all were female patients). Other frequent diagnoses concerned 'trauma' (65.2%), and the 'digestive' and the 'nervous system' (each 56.5%). At-risk behaviour such as severe alcohol consumption (34.8%), or excessive use of medicines (26.1%) was very frequent, and some patients used illicit drugs (21.7%). There was only a weak association between the number of ED attendances and the resulting costs. However, a reduction of one outpatient visit per patient would have decreased ED outpatient costs by 8.5%. Conclusions: HU often have a particularly vulnerable profile. Mental problems are prevalent among them, as are at-risk behaviour and severe somatic conditions. The complexity of the patients' profiles demands specific care that cannot be guaranteed within an everyday ED routine. The use of an interdisciplinary case management team might be a promising approach in diminishing the number of attendances and the associated costs, although the profiles of HU are such that they probably cannot completely give up ED attendance.

Objective: To identify the opinion of patients with mental disorder about tobacco and its prohibition during psychiatric hospitalization. Method: An exploratory study with 96 patients smokers with mental disorders hospitalized in a psychiatric ward of a general hospital. The interviews were conducted individually, using an instrument designed for this study. The content from the interviews was recorded, transcribed and submitted to a thematic content analysis. Results: The patients with mental disorder were identified as perceiving smoking during the psychiatric hospitalization as a help to support the difficulties in socialization and in the lack of activities. The permission for smoking is seen as a signal of respect to their needs. The subjects mentioned to not accept the total smoking prohibition. Conclusion: Tobacco helps to face difficulties and conflicts in the psychiatric hospitalization. There is resistance regarding the possibility to totally withdraw the smoking permission during hospitalization.

Introduction: Few studies have reported the distribution of all hospital admissions at the entire country level in low and middle-income countries (LMICs). We examined this question in Seychelles, a rapidly developing small island state in the Africa region, in which access to health care is provided free of charge to all inhabitants through a national health system and all hospital admissions are routinely registered. Methods: Based on all admissions to all hospitals in Seychelles in 2005-2008, we calculated the distribution of hospital admissions, age at admission, length of stay and bed occupancy (i.e. cumulated number of patients * number of days spent in all hospitals) according to both hospital departments and broad causes of diseases (using codes of the ICD-10 classification of diseases). Results: Bed occupancy was largest in the surgical wards (36.7% of all days spent in all hospitals), followed by the medical wards (24.3%), gynecology/obstetrics wards (18.4%), pediatric wards (11.2%), and psychiatric wards (7.2%). According to broad causes of diseases/conditions, bed occupancy was highest for obstetrics/gynecology conditions (19.9% of all days spent at hospital), mental diseases (8.6%), cardiovascular diseases (8.1%), upper aerodigestive/pulmonary diseases (8%), infectious/parasitic diseases (8%), gastrointestinal diseases (7.2%), and urogenital diseases (6.7%). Adjusted to 100'000 population, 153 hospital beds are needed every day, including 31 for obstetrics/gynecologic conditions, 13 for mental diseases, 12 for cardiovascular diseases, 12 for upper aerodigestive diseases, 12 for infectious/parasitic diseases, and 11 for gastrointestinal diseases. Conclusion: Our findings give a good indication of the overall distribution of admissions according to both hospital departments and broad causes of diseases in a middle-income country. These findings provide important information for health care planning at the national level

Background: The objective of this study was to determine if mental health and substance use diagnoses were equally detected in frequent users (FUs) compared to infrequent users (IUs) of emergency departments (EDs). Methods: In a sample of 399 adult patients (>= 18 years old) admitted to a teaching hospital ED, we compared the mental health and substance use disorders diagnoses established clinically and consigned in the medical files by the ED physicians to data obtained in face-to-face research interviews using the Primary Care Evaluation of Mental Disorders (PRIME-MD) and the Alcohol, Smoking and Involvement Screening Test (ASSIST). Between November 2009 and June 2010, 226 FUs (>4 visits within a year) who attended the ED were included, and 173 IUs (<= 4 visits within a year) were randomly selected from a pool of identified patients to comprise the comparison group. Results: For mental health disorders identified by the PRIME-MD, FUs were more likely than IUs to have an anxiety (34 vs. 16%, Chi2(1) = 16.74, p <0.001), depressive (47 vs. 25%, Chi2(1) = 19.11, p <0.001) or posttraumatic stress (PTSD) disorder (11 vs. 5%, Chi2(1) = 4.87, p = 0.027). Only 3/76 FUs (4%) with an anxiety disorder, 16/104 FUs (15%) with a depressive disorder and none of the 24 FUs with PTSD were detected by the ED medical staff. None of the 27 IUs with an anxiety disorder, 6/43 IUs (14%) with a depressive disorder and none of the 8 IUs with PTSD were detected. For substance use disorders identified by the ASSIST, FUs were more at risk than IUs for alcohol (24 vs. 7%, Chi2(1) = 21.12, p <0.001) and drug abuse/dependence (36 vs. 25%, Chi2(1) = 5.52, p = 0.019). Of the FUs, 14/54 (26%) using alcohol and 8/81 (10%) using drugs were detected by the ED physicians. Of the IUs, 5/12 (41%) using alcohol and none of the 43 using drugs were detected. Overall, there was no significant difference in the rate of detection of mental health and substance use disorders between FUs and IUs (Fisher's Exact Test: anxiety, p = 0.567; depression, p = 1.000; PTSD, p = 1.000; alcohol, p = 0.517; and drugs, p = 0.053). Conclusions: While the prevalence of mental health and substance use disorders was higher among FUs, the rates of detection were not significantly different for FUs vs. IUs. However, it may be that drug disorders among FUs were more likely to be detected.

Com este estudo pretendeu-se, descrever o papel da Enfermagem no processo de integração do Doente Mental na família. Trata-se de um estudo exploratório descritivo de natureza qualitativa, realizado no Serviço de Saúde Mental do Hospital Baptista de Sousa. Para tal, participaram 5 enfermeiros que prestam os cuidados de enfermagem neste serviço. Os dados foram recolhidos com base numa entrevista semiestruturada e posteriormente submetidos à uma análise de conteúdo, revelando défices a nível dos recursos materiais e humanos, que dificultam o campo de acção do Enfermeiro, diminuindo a sua autonomia. Este estudo tem implicações práticas; incentivar futuras investigações no ramo da Enfermagem, promover e fomentar especialização do Enfermeiro na área de Saúde Mental e Psiquiatria. A nível teórico: servir de suporte bibliográfico, para próximos estudos e investigações. Apelar pela autonomia do Enfermeiro. Em geral, conclui-se que a Enfermagem em Saúde Mental e Psiquiatria, precisa ser mais valorizada, e ganhar maior autonomia profissional, por parte dos Enfermeiros.

Este trabalho de investigação aborda a questão da representação social da doença mental entre familiares de pacientes diagnosticados com esquizofrenia, um aspecto necessário e fundamental para a nossa futura prática clínica. O objectivo era compreender a forma como representam a doença mental e o seu portador, e o trabalho teve como referencial vários autores que retractam a representação social da doença mental nas suas várias dimensões: significado, causas e cura da doença mental, ocupação, tomada da medicação, e futuro do portador de distúrbio psíquico. Os dados foram recolhidos mediante uma entrevista aos familiares, realizada em casa dos pacientes. Para o efeito foi desenvolvido um guião de entrevista. Foram recrutados 6 famílias dos portadores de esquizofrenia entre os utentes actuais do Hospital Agostinho Neto – “Extensão Trindade”. As entrevistas foram posteriormente transcritas e os discursos sobre as várias dimensões da representação social foram categorizados. As famílias se posicionaram caracterizando a doença mental e o seu portador segundo as representações do senso comum, pelo que concluímos que elas têm pouca informação médica e compreensão da real condição psíquica do seu familiar. O significado que a família atribuía à doença mental e à sua causa não foi influenciado pelo nível de escolaridade das famílias, mas sim corresponde aos valores culturais da sociedade cabo-verdiana. A partir desta investigação, percebemos a importância de assistir e informar os familiares que acompanham o doente mental, durante o seu tratamento.
