OBJETIVO: Quantificar a gordura presente no músculo levantador da pálpebra de portadores de ptose congênita, correlacionando este achado com fatores clínico-epidemiológicos desta afecção. MÉTODOS: Vinte e duas amostras de músculo levantador da pálpebra superior, provenientes de portadores de ptose congênita, foram avaliadas morfometricamente, com o intuito de quantificar a gordura presente nos espécimes e correlacionar este achado com características como idade, sexo, grau de ptose e função do músculo levantador. RESULTADOS: Não houve correlação entre a quantidade de gordura encontrada no músculo levantador de portadores de ptose congênita e os dados clínicos dos pacientes estudados. CONCLUSÃO: A quantidade de gordura presente no músculo levantador da pálpebra superior, nas condições do presente estudo, não está associada com idade, sexo, grau de ptose ou função do músculo levantador. Novos estudos serão necessários para avaliar a real alteração que ocorre no músculo levantador da pálpebra de indivíduos com ptose palpebral.

A retrospective study of etiology, antimicrobial susceptibility profile and multiple drug resistance, and major epidemiological aspects were investigated in 616 cases of canine otitis. Staphylococcus beta hemolitic (26.27%), Malassezia pachydermatis (12.35%), and Pseudomonas aeruginosa (8.8%) were the most common microorganisms identified. The isolates were susceptible mainly to norfloxacin (89.62%), gentamicin (83.25%), and ofloxacin (80.16%). High occurrence of resistance of isolates was observed to neomicin (30.84%) and cephalexin (27.63%). Multiple drug resistance to three or more and five or more of antimicrobials tested was observed in 34.9% and 15.5% of isolates, respectively. The cases of canine otitis occurred predominantly in first years of age, in mixed breeds animals, at autumn season. The presence of itch, bad smell, and secretion in ear conduct were the major signs observed at clinical examination.

Brazil is a wide country with huge contrasts. Its peculiarities can highlight environmental factors that could influence the frequencies of different cancers. The standard treatment and results achieved from several different areas of the country may not be found in others. The establishment of a national cooperative group has the potential to improve outcomes. The The Brazilian Cooperative Group on Pediatric Patients with Myelodysplastic Syndrome (BCG-MDS-PED) was first organized in January 1997 as a working group of hematologists, pediatric oncologists, pediatric-hematologists, molecular biologists and other professionals in order to study pediatric (age < 18 years) MDS. Six distinct subcommittees constituted with members from several universities: cytology, histopathology, clinical, cytogenetics, molecular biology and epidemiology. The goals of the BCG-MDS-PED were: (i) to offer support for diagnosis and orientation for treatment; (ii) educational Support for the colleagues all over the country and (iii) research on pathogenesis and new approaches for pediatric MDS patients. There are socio-economical differences among the five regions of the country. The BCG-MDS-PED believes that it is absolutely necessary to Study the clinical, cellular, molecular and epidemiological aspects of MDS, taking in account these peculiar differences among populations and regions. Since 1997, 114 pediatric cases were referred to the BCG-MDS-PED from 21 centres. Seven Brazilian states have sent cases to the group, 31 patients were referred from universities, 73 patients from pediatric oncology units (foundations) and 10 patients came from private clinics. Some of these patients have been followed up and/or treated by the physician who referred them to the BCG-MDS-PED for confirmation of the initial diagnosis. The majority of these physicians have required orientation on diagnostic and treatment issues, as well as to complete cytogenetic and molecular studies. From these 114 patients, 64 patients were confirmed as MDS. We believe that, the more numerous the MDS-studied cases, the more experienced will be the referee group on clinical and laboratory features on childhood MDS in Brazil. Rabies is considered a fatal disease once clinical symptoms have developed. The aim of this study was to evaluate epidemiological aspects and immune response in patients attacked by domestic and wild animals and subjected to post-exposure rabies treatment with equine serum and associated vaccine. Thirty-three patients were evaluated; they were between 13 and 65 years old, 75.8% were male and 24.2% female, and from the Botucatu neighborhood. Twenty healthy control individuals with the same age range were also studied. Specific antibodies to equine immunoglobulins and IFN-γ, IL-2, IL-4, and IL-10 production were evaluated by ELISA. IgM, IgE, IgG and subclasses, and rabies virus antibodies serum levels were determined by nephelometry and seroneutralization methods, respectively. No anaphylactic or serum sickness allergic reactions were observed in patients after treatment. Anti-equine IgG levels were significantly higher than those of IgM after 14 and 28 days of treatment. Protective antibodies to rabies virus > 0.5 UI/ml were detected in 84.6% and 75% of patients at days 14 and 28, respectively. IFN-γ, IL-2 and IL-10 levels in patients before and 48h after treatment were significantly higher than in controls suggesting that both Th1 and Th2 cells were activated in the patients. Serum IgM levels were higher at day 14, and IgG 2 and IgE levels were higher at day 28 of treatment. These results suggest that post-exposure rabies treatment in humans induces significant alterations in patient immune response characterized by increased levels of cytokines, serum levels of specific rabies virus antibodies, and the equine serum components employed in the treatment.

In Brazil, the rates of mother-to-child-transmission (MTCT) of human immunodeficiency virus (HIV) decreased from 20% to 1-2% in some regions. However, the country contains 90% of individuals infected with visceral leishmaniasis (VL) in Latin America, and the west region of São Paulo state faces an alarming expansion of the disease. We describe the epidemiological aspects of the expanding infection of VL and a case report of an HIV-VL-co-infected child from the west region of São Paulo state. The patient was an AIDS-C3 with low levels of CD4, high viral load, severe diarrhea, oral and perineal candidiasis, severe thrombocytopenia, and protein-caloric malnourishment. She evolved with sepsis, renal and cardiac failure. An rK rapid diagnosis test, indirect fluorescent antibody test (IFAT), and bone marrow aspirate were performed for VL. Her symptoms improved significantly after liposomal amphotericin B administration. From the 45 municipalities that compose the Regional Health Department of Presidente Prudente, Lutzomyia longipalpis vectors were found in 58% of them. VL infected dogs were found in 33% of those municipalities, infected dogs and humans were found in 29%, 20% are starting and 33% of the municipalities are preparing VL investigation. It is likely, in this patient, that VL advanced the clinical progression of the HIV disease and the development of AIDS severity. Supported by favorable conditions, the region becomes a new frontier of VL in Brazil. © 2013.

Inquiries about cryptossporidiosis are essential because its zoonotic potential and pathogenicity in pets and production animals. The horse can be involved in the transmission of the infection by Cryptosporidium and can have an epidemiological role in this disease. The great possibility of adquiring this protozoan, justify the importance of attempting against for the occurrence of this illness. The aim of this article was to demonstrated through literature revision, some epidemiological aspects of the cryptosporidiosis in the equine specie.

The cytological exam is a practical diagnostic aplication which allows the detection of inflammatory, proliferative and infectious disturbances. In the present study, the results of cytological exams during six years were evaluated, determining the most common type of disorder (inflammatory, infectious, proliferative). Epidemiological aspects such as species, breed, sex and age were taken into account. 270 reviews were evaluated and the prevalence was neoplasms (53%). The canine specie was the most observed, with 92% of cases. Females totaled 53%. The age ranged from one to 17 years and the interval of six to 10 years was most observed (40% of cases). The round cell neoplasms were the most prevalent, followed by breast neoformations.

Descrevem-se os aspectos epidemiológicos e os sinais clínicos dos primeiros casos de intoxicação natural por Ipomoea asarifolia em búfalos. A doença foi diagnosticada em quatro bubalinos de três diferentes propriedades, no município de Cachoeira do Arari, Ilha de Marajó, PA, e ocorreu nos meses de novembro e dezembro, o período mais seco do ano nesta região e de escassez de alimento. Os sinais clínicos observados foram relacionados ao sistema nervoso central, como andar trôpego, hipermetria, acentuados tremores musculares, queda ao solo em posições incomuns, nistagmo e marcada excitação, sinais que se agravavam após movimentação. Baseado nos aspectos epidemiológicos, sinais clínicos e na ausência de leões histopatológicas, concluiu se tratar de intoxicação por Ipomoea asarifolia.

As águas da Baía de Marajó tornam-se doce nos meses de maior precipitação pluviométrica e saloba no outro período, o que torna possível o encontro de arraias da família Potamotrygonidae e exemplares de águas marítimas, as que são providas de ferrões pontiagudos e retrosserrilhados que, ao penetrarem nas vítimas, danificam o tegumento que as células glandulares de veneno, expondo os tecidos a toxina, levando ao aparecimento de manifestações clínicas. Empregou-se um modelo observacional e descritivo da prevalência, para avaliar os aspectos clínicos e epidemiológicos de 116 acidentes por arraias, ocorridos nos Distritos de Mosqueiro e Outeiro, Belém-Pará, no período de julho de 1999 a agosto de 2001. Estes ocorreram, principalmente, em Mosqueiro (80,2%) com maior freqüência no sexo masculino (63,8%), no grupo etário dos adolescentes e adultos (38,8% e 50,0%), respectivamente, no mês de julho (37,9%). Em todas as marés houve vítimas, porém, com menor freqüência na preamar (4,3%). A maioria dos acidentes ocorreu no fundo do rio com areia (49,1%) e no período vespertino (62,1%). As arraias mais capturadas nas praias de Mosqueiro foram da família Potamotrygonidae. As vítimas socorridas na primeira hora em 79,3% dos casos, sendo o membro inferior a região mais atingida em 84,5%, particularmente o pé (75,0%). Todos os pacientes apresentaram manifestações clínicas locais, com dor em 99,1% e cianose em 42,2% dos casos. Os sintomas sistêmicos estiveram presentes em 14,6%, sendo a tontura (6,0%) e a sudorese (5,2%) os mais encontrados. A pressão arterial acima do normal presente em 6,0%, e a taquicardia em 9,5%. O debridamento foi o manejo cirúrgico mais utilizado (94,5%). Retornaram para reavaliação somente 18,1% dos pacientes, dos quais, 33,4% evoluíram com necrose e 28,6% com infecção local. Não houve significância estatística na correlação entre evolução clínica, os tratamentos instituídos e o tempo entre o acidente e o início do atendimento médico. Conclui-se que acidentes por arraias são um importante agravo de saúde para a população que apesar das complicações locais, as vítimas não apresentam quadro grave. O período das águas salobras parece que influencia no aumento dos casos.

A tinea capitis é uma doênça infecto-contagiosa que atinge preferencialmente crianças e tem, com principais características clínicas, a presença de prurido, descamação, tonsura e alopecia. O tratamento convencional é baseado no uso da griseofulvina, droga que necessita ser utilizada por, em média, 45 dias e possui efeitos colaterais importantes. Neste trabalho prospectivo, foram avaliados os aspectos clínico-epidemiológicos da tinea capitis em crianças atendidas na região de Marituba, Pará, bem como a eficácia do tratamento desta afecção com a droga antifúngica terbinafina administrada por um período de duas semanas. Dentre os principais achados estão: 1) a presença do gênero Trichophyton rubrum; 2) a caracterização do quadro clínico com prurido, descamação, tonsura e alopecia, o que é consistente com a literatura; 3) a ausência de uma relação específica entre o quadro clínico e o agente etiológico encontrado, o que não permite o diagnóstico da espécie envolvida somente pelo contato com os animais, apesar dos altos índices de cães e gatos convivendo com as crianças estudadas; e 5) a segurança da terbinafina, que é bem tolerada e eficaz, com índice de cura elevado (94,11%), especialmente considerando o fato de que o gênero predominante na região do estudo foi o Trichophyton. Desta maneira, a utilização da terbinafina nos pacientes de tinea capitis nesta região é uma alternativa viável ao uso da griseofulvina.
