O presente estudo objetivou identificar as principais causas da mortalidade das micro e pequenas empresas brasileiras e como os instrumentos de planejamento e gerenciamento econômico-financeiros podem reduzir esta mortalidade. Foi realizada pesquisa bibliográfica sobre as micro e pequenas empresas (MPE) em que se identificaram: as definições, as principais características, a importância, as principais necessidades e dificuldades, a taxa de mortalidade e os fatores de mortalidade desse segmento de empresas. Dessa forma, verificou-se que as MPE têm grande importância na economia e sociedade brasileiras. A maioria expressiva (98%) das empresas brasileiras é classificada como MPE, sendo elas responsáveis por 43% da oferta de empregos e por quase a metade de toda a receita e valor da produção do país. O trabalho abordou pesquisas mostrando que as taxas médias de mortalidade das empresas brasileiras são significativas e preocupantes, podendo atingir índices próximos a 60% somente no primeiro ano de sua constituição, em alguns Estados do Brasil. Este fenômeno não pode ser atribuído a um fator isoladamente, mas a um conjunto de fatores ambientais, técnicos, administrativos e outros mais que, combinados, podem ampliar os riscos de fechamento das x empresas. No entanto, grande parte dos estudos aponta problemas de origem financeira como um dos principais fatores condicionantes da mortalidade das MPE. As necessidades mais prementes nas MPE, identificadas na pesquisa bibliográfica, foram: instrumentos de planejamento e gestão, treinamento especializado, capital de giro, crédito a taxas compatíveis com a rentabilidade e formalização dos processos. A pesquisa principal do trabalho é o estudo de caso abordando a experiência vivencial de implantação, planejamento, gerenciamento e controle do Restaurante Buongustaio em Brasília, nos seus 10 meses iniciais de funcionamento. Foram abordadas as principais dificuldades, problemas enfrentados e soluções envolvendo planejamento, gestão, recursos humanos, capital de giro e atendimento a clientes, dentre outros. o estudo de caso possibilitou a aferição de vários conceitos e conteúdos abordados na pesquisa bibliográfica e conclui-se que a utilização dos instrumentos de planejamento e gerenciamento econômico-financeiros, como: plano de negócios, análise econômico-financeira, gerenciamento do fluxo de caixa e de capital de giro, fornece importante contribuição para a sobrevivência e crescimento da micro e pequena empresa, reduzindo o seu risco de falência ou mortalidade prematura.

A bronquiolite viral aguda (BVA) é uma doença respiratória que acomete crianças principalmente no primeiro ano de vida. O Vírus Sincicial Respiratório é responsável por aproximadamente 75% dos casos de bronquiolite viral aguda; entretanto, outros agentes também podem desencadear doença semelhante, como Adenovirus1, 7,3 e 21, Rinovírus, Parainfluenza, Influenza, Metapneumovirus e, menos freqüentemente, o Mycoplasma pneumoniae. A BVA é uma doença com padrão sazonal, de evolução benigna na maioria dos lactentes hígidos, entretanto 0,5% a 2% necessitam hospitalização, dos quais 15% necessitam cuidados intensivos, e destes apenas 3 a 8% desenvolvem falência ventilatória necessitando de ventilação mecânica. A mortalidade entre crianças previamente hígidas está em torno de 1% dos pacientes internados. O objetivo deste trabalho é identificar fatores de prognóstico na BVA e correlacionar com tempo de internação em lactentes previamente hígidos. Durante o inverno de 2002, foram acompanhados em estudo de coorte 219 pacientes menores de um ano de idade com diagnóstico clínico de bronquiolite viral aguda. Estes pacientes foram avaliados e classificados conforme escore clinico modificado (DE BOECK et al., 1997) na internação, no terceiro dia e no momento da alta hospitalar. O tempo de internação real foi registrado e foi estimado o tempo de internação ideal, conforme critérios de alta clínica definidos por Wainwright e cols. , em 2003, como não uso de oxigênio por mais de 10 horas, tiragem intercostal mínima ou ausente, sem uso de medicação parenteral e com capacidade de alimentação via oral. O escore clinico na internação foi 3,88±1, 81, o tempo médio de uso de oxigênio 5,3±3,83 dias. Estes pacientes apresentaram tempo de internação real de 7,02±3,89 dias e tempo de internação ideal de 5,92±3,83 dias (p<0,001). Considerando tempo de internação ideal como variável dependente em um modelo de regressão logística, observa-se que para cada ponto de aumento no escore clinico aumenta em 1,9 a chance de o paciente permanecer internado por mais de três dias. Conclui-se, então, que se pode predizer o tempo de internação de lactentes hígidos com BVA através do escore clínico, indicando seu uso na avaliação inicial destes pacientes.

Objetivou-se avaliar a atenção à saúde de recém-nascidos de risco, acompanhados no primeiro ano de vida pelo Programa Crescer Feliz, desenvolvido em município do interior paulista. Trata-se de estudo epidemiológico populacional, do tipo avaliação de programa de saúde, que se baseou, para análise dos dados, nas diretrizes nacionais da Agenda de Compromissos da Criança. Os resultados evidenciaram a vulnerabilidade institucional do programa, decorrente de problemas relacionados à estrutura e processo, com implicações nos resultados. Considerando a adequação dos critérios adotados pelo programa, para definição dos recém-nascidos de risco e das intervenções e estratégias propostas, que se mostram em consonância com a Agenda de Compromissos, aponta-se a necessidade dos gestores priorizá-lo, inserindo-o, efetivamente, na política pública de saúde a ser desenvolvida no âmbito do município, para reversão da vulnerabilidade institucional identificada.

Este trabalho baseia-se na análise de dados obtidos de 1891 exemplares de Cynoscion jamaicensis coletados durante quatro cruzeiros oceanográficos realizados por intermédio do N/Oc. Prof. W. Besnard, na área entre Cabo de Sao Tomé (22º04'S) e Torres (29º30'S), até a isóbata de 200 m, dentro do programa FAUNEC. A espécie distribui-se ao longo da plataforma continental, preferencialmente na região banhada pela agua costeira, com temperaturas entre 27ºC e 18ºC. Ocorrem concentrações de jovens e adultos, indiscriminadamente da distância da costa; os jovens ocorreram, em fevereiro-março, entre 22ºS e 27ºS e, nos demais períodos, entre 23º20'S e 27º30'S, enquanto os adultos distribulram-se, em fevereiro-março e maio, entre 26ºS e 27º30'S e, em setembro e novembro, entre 23ºS e 29º30'S. A desova ocorre entre as latitudes 24º30'S e 26º30'S, durante o fim do inverno-primavera (setembro-novembro). Concordando com o ciclo reprodutivo, o fator de condição mostrou variações cíclicas, com valor mais baixo durante setembro, período de desova. O pico de recrutamento verificou-se em maio, quando ocorreram indivíduos com comprimento total entre 70 e 90 mm, provenientes da desova do ano anterior, que ainda nao completaram um ano de idade. 0 comprimento médio de início da primeira maturação sexual é de 154 mm, sendo que aos 200 mm todos os indivíduos estão aptos para a reprodução. A relação peso total/comprimento total apresentou diferenças estatisticamente significativas entre sexos, com α = 3,25 para machos e 3,10 para fêmeas.

Aim: To investigate: a) caries experience in 5 year-old children and its relationship to the caries risk evaluation made before 1 year of age; b) compliance of parents to an infant oral health program as well as the abandonment reasons. Methods: Group A (GA) - 242 children (60-71 months old) who stayed in the program and had been enrolled since their first year of life underwent a clinical examination according to the WHO's criteria; their parents/caregivers were interviewed to ascertain their compliance to the program. Group B (GB) - parents of 60 children, who had dropped out of the program, were interviewed to check the reasons of withdrawing. The caries risk classification was based on the file at the first appointment. Chi-square test was used (α=0.05) for statistical analyses. Results: Most of the children were free of caries (71.1%). Caries risk evaluation showed low sensibility (34.3%, 95% CI = 22.4-46.1) and high specificity (74.4%, 95% CI = 67.6-81.2) to caries experience. Sixty-two mothers (25.6%) reported difficulty to follow the guidelines. The main reason for dropping out was changing to similar health services (40.0%). Conclusions: Caries risk in the first year of life was not efficient to predict dental caries experience at 5 years of age. Educational practice should be improved to increase the compliance of the parents to the program.

Life-history information constitutes the raw data for building population models used in species conservation. We provide life-history data for the endangered Santa Catalina Island Rattlesnake, Crotalus catalinensis. We use data from 277 observations of C. catalinensis made between 2002 and 2011 on the island. Mean snout-vent length (SVL) of adult C. catalinensis was 643 mm for males and 631 mm for females; the difference was not significant. The degree of sexual size dimorphism (SSD; using SVL) was -0.02. However, sexes were dimorphic in total length ( SVL + tail length), relative tail length, and stoutness. Juvenile recruitment occurs during late-summer. In their first year of life, juveniles seem to grow at a rate of about 1.7 cm/mo. Females seem to become mature around 570 mm SVL, probably in the year when they become 2 y old. Scattered literature data corroborates the time of juvenile recruitment described herein. Growth in C. catalinensis seems to be slower than that of C. ruber, its sister taxa, but similar to other rattlesnakes.

Social stressors at work (such as conflict or animosities) imply disrespect or a lack of appreciation and thus a threat to self. Stress induced by this offence to self might result, over time, in a change in body weight. The current study investigated the impact of changing working conditions--specifically social stressors, demands, and control at work--on women's change in weighted Body-Mass-Index over the course of a year. Fifty-seven women in their first year of occupational life participated at baseline and thirty-eight at follow-up. Working conditions were assessed by self-reports and observer-ratings. Body-Mass-Index at baseline and change in Body-Mass-Index one year later were regressed on self-reported social stressors as well as observed work stressors, observed job control, and their interaction. Seen individually, social stressors at work predicted Body-Mass-Index. Moreover, increase in social stressors and decrease of job control during the first year of occupational life predicted increase in Body-Mass-Index. Work redesign that reduces social stressors at work and increases job control could help to prevent obesity epidemic.

Research has shown that chance events affect careers but has not established the nature of their effects. Moreover, the relationship between chance and career decidedness is not well understood. The present study used a person-centered approach with latent profile analysis to examine 312 Swiss adolescents in their first year of vocational training. We identified five qualitatively differing profiles according to levels of perceived chance events and career decidedness: balanced scorers, undecided with mean chance, undecided with high chance, decided with chance, and decided without chance. The groups differed significantly in work motivation (i.e., occupational self-efficacy beliefs, perceived person-job fit, and work engagement). Decided adolescents reported more favorable work motivation regardless of their level of perceived chance events. The results imply that promoting decidedness remains a valuable goal in career counseling despite the occurrence of unpredicted events.

Background. Injecting drug users (IDUs) are at risk of infection with Hepatitis C Virus (HCV) and Human Immunodeficiency Virus (HIV). Independently, each of these viruses is a serious threat to health, with HIV ravaging the body’s immune system, and HCV causing cirrhosis, liver cancer and liver failure. Co-infection with HIV/HCV weakens the response to antiretroviral therapy in HIV patients. IDUs with HIV/HCV co-infection are at a 20 times higher risk of having liver-related morbidity and mortality than IDUs with HIV alone. In Vietnam, studies to ascertain the prevalence of HIV have found high rates, but little is known about their HCV status. ^ Purpose. To measure the prevalence of HCV and HIV infection and identify factors associated with these viruses among IDUs at drug treatment centers in northern Vietnam. ^ Methods. A cross-sectional study was conducted from November 2007 to February 2008 with 455 injecting drug users aged 18 to 39 years, admitted no more than two months earlier to one of four treatment centers in Northern Vietnam (Hatay Province) (response rate=95%). Participants, all of whom had completed detoxification and provided informed consent, completed a risk assessment questionnaire and had their blood drawn to test for the presence of antibody-HCV and antibody-HIV with enzyme immuno assays. Univariate and multivariable logistic regression models were utilized to explore the strength of association using HIV, HCV infections and HIV/HCV co-infection as outcomes and demographic characteristics, drug use and sexual behaviors as factors associated with these outcomes. Unadjusted and adjusted odds ratios and 95% confidence intervals were calculated. ^ Results. Among all IDU study participants, the prevalence of HCV alone was 76.9%, HIV alone was 19.8%. The prevalence of HIV/HCV co-infection was 92.2% of HIV-positive and 23.7% of HCV-positive respondents. No sexual risk behaviors for lifetime, six months or 30 days prior to admission were significantly associated with HCV or HIV infection among these IDUs. Only duration of injection drug use was independently associated with HCV and HIV infection, respectively. Longer duration was associated with higher prevalence. Nevertheless, while HCV infection among IDUs who reported being in their first year of injecting drugs were lower than longer time injectors, their rates were still substantial, 67.5%. ^ Compared with either HCV mono-infection or HIV/HCV non-infection, HIV/HCV co-infection was associated with the length of drug injection history but was not associated with sexual behaviors. Higher education was associated with a lower prevalence of HIV/HCV co-infection. When compared with HIV/HCV non-infection, current marriage was associated with a lower prevalence of HIV/HCV co-infection. ^ Conclusions. HCV was prevalent among IDUs from 18 to 39 years old at four drug treatment centers in northern Vietnam. Co-infection with HCV was predominant among HIV-positive IDUs. HCV and HIV co-infection were closely associated with the length of injection drug history. Further research regarding HCV/HIV co-infection should include non-injecting drug users to assess the magnitude of sexual risk behaviors on HIV and HCV infection. (At these treatment centers non-IDUs constituted 10-20% of the population.) High prevalence of HCV prevalence among IDUs, especially among HIV-infected IDUs, suggests that drug treatment centers serving IDUs should include not only HIV prevention education but they should also include the prevention of viral hepatitis. In addition, IDUs who are HIV-positive need to be tested for HCV to receive the best course of therapy and achieve the best response to HIV treatment. These data also suggest that because many IDUs get infected with HCV in the first year of their injection drug career, and because they also engaged in high risk sexual behaviors, outreach programs should focus on harm reduction, safer drug use and sexual practices to prevent infection among drug users who have not yet begun injecting drugs and to prevent further spread of HCV, HIV and co-infection. ^

Acute promyelocytic leukemia (APL) is associated with reciprocal chromosomal translocations involving the retinoic acid receptor α (RARα) locus on chromosome 17. In the majority of cases, RARα translocates and fuses with the promyelocytic leukemia (PML) gene located on chromosome 15. The resulting fusion genes encode the two structurally unique PML/RARα and RARα/PML fusion proteins as well as aberrant PML gene products, the respective pathogenetic roles of which have not been elucidated. We have generated transgenic mice in which the PML/RARα fusion protein is specifically expressed in the myeloid-promyelocytic lineage. During their first year of life, all the PML/RARα transgenic mice have an abnormal hematopoiesis that can best be described as a myeloproliferative disorder. Between 12 and 14 months of age, 10% of them develop a form of acute leukemia with a differentiation block at the promyelocytic stage that closely mimics human APL even in its response to retinoic acid. Our results are conclusive in vivo evidence that PML/RARα plays a crucial role in the pathogenesis of APL.

The purpose of this study was to compare the efficacy and performance evaluations of alternatively certified first-year teachers to traditionally certified first-year teachers. The participants were 25 first-year teachers in the Broward County Public School District (FL) who completed the Transition to Teaching alternative certification program and a comparison group of 32 first-year teachers in the same school district who completed a traditional university teacher preparation program. ^ The study was a mixed methods design (Creswell, 2003; Tashakkori & Teddlie, 1998). The quantitative data were collected during the 2002–2003 school year using the Teachers' Sense of Efficacy Scale (Tschannen-Moran & Hoy, 2001) and the Florida Performance Measurement System formative and summative instruments. The qualitative data consisted of focus group interviews that were conducted at the end of the 2002–2003 school year. ^ Data were analyzed using independent samples t tests to compare the means of the two populations on their efficacy scores and performance evaluations. Paired samples t tests and analysis of covariance (ANCOVA) were used to compare the efficacy scores for each certification type at the beginning of the school year to the efficacy scores at the end of the school year. A repeated measures analysis of variance (ANOVA) was used to compare the change in the efficacy scores of the teachers from the beginning of the school year to the end of the school year. Focus group interviews were conducted and transcribed, and the content was analyzed and categorized based on the four sources of self-efficacy described by Bandura (1986, 1997). ^ The results of this study revealed that no statistically significant differences existed between the two groups of teachers in their teacher efficacy or performance evaluations and that they reported similar sources of their efficacy. These findings add to the research base that supports alternative certification as a viable and effective pathway into teaching. ^

Because it is widely accepted that providing information online will play a major role in both the teaching and practice of medicine in the near future, a short formal course of instruction in computer skills was proposed for the incoming class of students entering medical school at the State University of New York at Stony Brook. The syllabus was developed on the basis of a set of expected outcomes, which was accepted by the dean of medicine and the curriculum committee for classes beginning in the fall of 1997. Prior to their arrival, students were asked to complete a self-assessment survey designed to elucidate their initial skill base; the returned surveys showed students to have computer skills ranging from complete novice to that of a systems engineer. The classes were taught during the first three weeks of the semester to groups of students separated on the basis of their knowledge of and comfort with computers. Areas covered included computer basics, e-mail management, MEDLINE, and Internet search tools. Each student received seven hours of hands-on training followed by a test. The syllabus and emphasis of the classes were tailored to the initial skill base but the final test was given at the same level to all students. Student participation, test scores, and course evaluations indicated that this noncredit program was successful in achieving an acceptable level of comfort in using a computer for almost all of the student body.

New technologies have transformed teaching processes and enabled new ways of study and learning. In these activities, it is suspected that the students don't make good use of new available technologies or, in the best case, they are underused. The analysis of this issue with the design of strategies to correct any defects found is the motivation that supports the development of this work and the main purpose of it. Evaluate information search habits used by the student and analyse their deduct synthesis and processing capabilities of the results found. The researchers of this study are university teachers of first year subjects, which allows them to know the information search performances by students.
