OBJECTIVE: The objective of this study is to evaluate the benefits of drainage in the Stoppa procedure for inguinal repair. PATIENTS AND METHODS: The use of a suction drain was randomized at the end of the surgical intervention in 26 male patients undergoing inguinal hernia repair, divided into 2 groups: Group A, 12 patients undergoing drainage, and group B, 14 patients not undergoing drainage. On the second postoperative day, all patients underwent abdominal pelvic computed tomography scan examination to detect the presence of abdominal fluid collection. RESULTS: In group A, no patient developed fluid collection in the preperitoneal space, and 1 patient presented with an abscess in the preperitoneal space on the 15th postoperative day. In group B, 12 patients presented with fluid collections in the preperitoneal space on computed tomography scan evaluation. However, only 3 patients presented minor complications. None of the patients developed a major complication. CONCLUSION: The use of suction drainage with the Stoppa procedure does not provide any benefit.

I. This paper deals with an experiment carried out to evaluate the effect of the sugar cane upper end on the composition of the sugar cane harvest as a raw material for the sugar industry. The variety studied was Co 421. The authors intend to study other varieties in the future. The data were collected from plant cane, at intervals of two weeks, always from the same field, from a small central area of 3.000 square meters approximately. Sixty (60) stalks were cut in each occasion, randomly chosen from the whole area. They were afterwards separated into three groups of 20 stalks, one for each of the treatments, namely: a) Complete stalks, with no leaves or sheaths. b) Stalks harvested by the technique of REYNOSO, that is, as usually done in practice. c) Stalks with the tops completely cut out, that is, cut by technique of REYNOSO and then with 3 other top internodes eliminated. The treatments caused significant differences on the following technological characteristics: a) Weight b) Cane pol c) Available sucrose (pol) per cent cane d) Cane juice pol e) Saline coefficient of juice. II. Except for weight, all changes were favorable to treatment c, even if with differences relatively slight, in percentage. IIII. Treatment differences for cane fiber, brix, reducing sugars, juice ashes, coefficient of purity and glucose coefficient were not significant. IV. Time of harvest was an important factor affecting the composition of the cane and of the juice. V. On the average the available sucrose of cane for treatments, with an standard error of 0.13%, was: Treatment c 13.05% Treatment b 12.65% Treatment a 12.53% This shows that there is no sound basis for the heavy fines applied by some sugar mills to planters who do not cut low enough the tops of the cane stalks.

Para a determinação do acúmulo de microrutrientes em função da idade da planta, instalou-se um experimento em Latossolo Roxo eutrófico, no Centro Nacional de Pesquisas de Soja/ EMBRAPA, Londrina (PR). Foram aplicadas seis doses de adubo NPK, dos quais foram usadas duas para estudar o acúmulo de micronutrientes: 0-0-0 e 90-45-45 kg/ha para a cultivar Con tissol e 0-0-0 e 45-45-45 kg/ha para a cultivar Guayacan. Coletaram-se amostras para análise de 14 em 14dias a partir da emergência. Pode-se concluir: a. o acúmulo máximo de micronutrientes ocorreu aos 88 dias e o ponto de inflexão aos 56 dias após a emergência; b. as quantidades totais absorvidas pelas plantas obedeceu ã ordem: Fe > Mn > B > Zn > Cu; c. a exportação de micronutrientes a través da colheita de grãos segue a ordem: Fe > Mn > Zn > Cu > B; d. o acúmulo máximo, em g de micronutrientes para uma produção de 1.000 kg de grãos foi: Fe = 933; Mn = 787; B = 383; Zn = 59,3; Cu = 22,8 e B= 12,3.

Foi feita a determinaçio dos elementos minerais contidos nos frutos do cacaueiro, amêndoas e casca. Um quilo de amendoas secas contêm, em grama: N-33,4; P--2,1; K-8,1; Ca-0,8; Mg-1,9; S-0,9; em mg: B-12; Cu-16, Fe-80; Mn-28; Mo-0,04; Zn-47. A análise completa do casqueiro semi decomposto mostrou na matéria seca os seguintes teores porcentuais N-2,20; P-0,05; K-2,40; Ca-0,51; Mg-0,32; S--0,12; concentração dos micronutrientes, em ppm é B-16; Cu-16; Fe-368; Mn-56; Mo-0,06; Zn-93. Foram colhidas amostras de folhas de uma roça altamente produtiva (172 arrobas ou 2580 quilos//ha)cuja análise mostrou os sequintes teores: N-I ,98%; P-0,17; K-2,20; Ca--0,73; Mq-0,19; B-25 ppm; Cu-14; Fe-87; Mu-134; Mo-0,16; Zn-96.

Transcatheter arterial chemoembolization (TACE) offers a survival benefit to patients with intermediate hepatocellular carcinoma (HCC). A widely accepted TACE regimen includes administration of doxorubicin-oil emulsion followed by gelatine sponge-conventional TACE. Recently, a drug-eluting bead (DC Bead) has been developed to enhance tumor drug delivery and reduce systemic availability. This randomized trial compares conventional TACE (cTACE) with TACE with DC Bead for the treatment of cirrhotic patients with HCC. Two hundred twelve patients with Child-Pugh A/B cirrhosis and large and/or multinodular, unresectable, N0, M0 HCCs were randomized to receive TACE with DC Bead loaded with doxorubicin or cTACE with doxorubicin. Randomization was stratified according to Child-Pugh status (A/B), performance status (ECOG 0/1), bilobar disease (yes/no), and prior curative treatment (yes/no). The primary endpoint was tumor response (EASL) at 6 months following independent, blinded review of MRI studies. The drug-eluting bead group showed higher rates of complete response, objective response, and disease control compared with the cTACE group (27% vs. 22%, 52% vs. 44%, and 63% vs. 52%, respectively). The hypothesis of superiority was not met (one-sided P = 0.11). However, patients with Child-Pugh B, ECOG 1, bilobar disease, and recurrent disease showed a significant increase in objective response (P = 0.038) compared to cTACE. DC Bead was associated with improved tolerability, with a significant reduction in serious liver toxicity (P < 0.001) and a significantly lower rate of doxorubicin-related side effects (P = 0.0001). TACE with DC Bead and doxorubicin is safe and effective in the treatment of HCC and offers a benefit to patients with more advanced disease.

OBJECTIVES: To determine the relationship between infections and functional impairment in nursing home residents. DESIGN: Prospective cohort study (follow-up period, 6 months). SETTING: Thirty-nine nursing homes in western Switzerland. PARTICIPANTS: A total of 1,324 residents aged 65 and older (mean age 85.7; 76.6% female) who agreed to participate, or their proxies, by oral informed consent. MEASUREMENTS: Functional status measured every 3 months. Two different outcomes were used: (a) functional decline defined as death or decreased function at follow-up and (b) functional status score using a standardized measure. RESULTS: At the end of follow-up, mortality was 14.6%, not different for those with and without infection (16.2% vs 13.1%, P=.11). During both 3-month periods, subjects with infection had higher odds of functional decline, even after adjustment for baseline characteristics and occurrence of a new illness (adjusted odds ratio (AOR)=1.6, 95% confidence interval (CI)=1.2-2.2, P=.002, and AOR=1.5, 95% CI=1.1-2.0, P=.008, respectively). The odds of decline increased in a stepwise fashion in patients with zero, one, and two or more infections. The analyses predicting functional status score (restricted to subjects who survived) gave similar results. A survival analysis predicting time to first infection confirmed a stepwise greater likelihood of infection in subjects with moderate and severe impairment at baseline than in subjects with no or mild functional impairment at baseline. CONCLUSION: Infections appear to be both a cause and a consequence of functional impairment in nursing home residents. Further studies should be undertaken to investigate whether effective infection control programs can also contribute to preventing functional decline, an important component of these residents' quality of life.

We show that nuclear C*-algebras have a re ned version of the completely positive approximation property, in which the maps that approximately factorize through finite dimensional algebras are convex combinations of order zero maps. We use this to show that a separable nuclear C*-algebra A which is closely contained in a C*-algebra B embeds into B.

We identified two distinct groups of patients in the 91 documented cases of pancreatic trauma (median age 8.0 years, range 0.6-15.8 years; M:F 2.5:1.0): 59 had a history of abdominal trauma and elevated serum lipase but no CT or ultrasound evidence of pancreatic injury (Group A); 32 had a history of abdominal trauma, elevated serum lipase but also had CT scan and/or ultrasound evidence of pancreatic injury (Group B). Patients with "less severe" injury based on normal imaging had a lower initial lipase level [Group A, median 651 U/L (interquartile range 520-1,324) vs. Group B, 1,608 U/L (interquartile range 680-3,526); p = 0.005] and shorter admission time [Group A, 9.0 days (interquartile range 5.5-15.5) vs. Group B, 13.4 days (interquartile range 6.8-23.8); p = 0.04]. There were no differences with respect to mortality (Group A, 13.5% vs. Group B, 12.5%), but patients with evidence of injury on imaging were more likely to have surgical intervention (p = 0.0001). The single most important overall cause of pancreatic trauma was involvement in a motor vehicle accident as a passenger or pedestrian. However, in children with high-grade ductal injury, bicycle handlebar injuries were most common. Associated injuries were common in both groups.

Purpose of this randomized trial was to compare conventional TACE with DEB-TACE for the treatment of intermediate-stage HCC in patients with cirrhosis. An independent, blinded review of magnetic resonance imaging studies was conducted to assess tumor response according to amended RECIST criteria. Results: DEB-TACE with doxorubicin showed a higher rate of complete response, objective response and disease control compared with conventional TACE (27% vs 22%; 52% vs 44%; and 63% vs 52%, respectively; p>0.05). Patients with Child Pugh B, ECOG 1, bilobar disease and recurrence following curative treatment showed a significant increase in objective response (p=0.038) compared to the control. There was a marked reduction in serious liver toxicity in patients treated with DEB-TACE. The rate of doxorubicin related side effects was significantly lower (p=0.0001) in the DEB-TACE group compared with the conventional TACE group. Conclusions: DEB-TACE with doxorubicin is safe and effective in the treatment of intermediate-stage HCC and may offer benefit to patients with more advanced disease.

Objectives and Study: To document the demographics, mechanisms and outcome of traumatic pancreatitis in children at a single large tertiary referral centre in Australia. Methods: We undertook a 10-year retrospective audit of children admitted to the Royal Children's Hospital [RCH], Melbourne, Australia with a hospital coded diagnosis which included pancreatic injury between 1993 and 2002. Data included patient demographics, source of admission, mechanism of injury, pancreatic complications, associated injuries, Intensive Care Unit [ICU] admission, results of any operative findings, results of any acute computed tomography (CT) and/or ultrasound (US) imaging of pancreas, selected laboratory findings and length of stay. Pancreatic trauma commonly occurs in the context of multiple injuries after motor vehicle accidents in children and bicycle handlebar injuries, especially in boys. Most children can be treated conservatively, with surgical intervention being limited to high-grade ductal injury.

Zielsetzung: Vergleich von Drug Eluting Bead (DEB)-TACE mit konventioneller TACE bei der Behandlung von ,,intermediate stage-HCC bei Patienten mit Zirrhose. Material und Methodik: 212 Patienten (185 ♂, 27 ♀; mittleres Alter, 67 Jahre) mit Child-Pugh A oder B Leberzirrhose und großem und/oder multinodulärem, irresektablen HCC wurden randomisiert, um das Therapieansprechen nach der Behandlung mit DEB (DC Bead; Biocompatibles, UK) beladen mit Doxorubicin oder konventioneller TACE mit Doxorubicin zu vergleichen. Die Randomisierung wurde nach Child-Pugh Status (A oder B), Performance Status (ECOG 0 oder 1), bilobärer Erkrankung (ja/nein) und frühere kurative Behandlung (ja/nein) stratifiziert. Der primäre Studienendpunkt war das 6-Monats-Tumoransprechen. Eine unabhängige verblindete MRT-Studie wurde durchgeführt, um das Tumoransprechen nach den RECIST Kriterien zu beurteilen. Ergebnisse: DEB-TACE mit Doxorubicin zeigte eine höhere Rate an komplettem Tumoransprechen, objektivem Ansprechen und Tumorkontrolle im Vergleich zur konventionellen TACE (27% vs 22%; 52% vs 44%; and 63% vs 52%; P>0.05). Patienten mit Child-Pugh B Zirrhose, ECOG 1 Performance Status, bilobärer Erkrankung und Rezidiven nach kurativer Behandlung zeigte einen signifikanten Anstieg des objektiven Ansprechens (p = 0.038) im Vergleich zur Kontrollgruppe. Bei Patienten, die mit DEB-TACE behandelt wurden, konnte eine deutliche Reduktion der gravierenden Lebertoxizität erreicht werden. Die Doxorubicin-Nebenwirkungsrate war in der DEB-TACE Gruppe deutlich geringer (p = 0.0001) als in der konventionellen TACEGruppe. Schlussfolgerung: DEB-TACE mit Doxorubicin ist sicher und effektiv in der Behandlung von ,,intermediate-stage HCC und bietet einen signifikanten Vorteil bei Patienten mit fortgeschrittener Erkrankung.

Where the level of agricultural technology is higher, common bean cultivars with a higher yield potential possibly require greater amounts of micronutrients. In Brazil however, there is a lack of information about the micronutrient extraction and exportation by the main grown cultivars. The objective of this study was to evaluate micronutrient (B, Cu, Fe, Mn, and Zn) extraction and exportation by common bean cultivars Pérola and IAC Alvorada, under different levels of NPK fertilization, on a dystroferric Red Nitosol, in Botucatu, São Paulo State, Brazil. The experiment was arranged in a randomized complete block (split plot) design with four replications. The plots consisted of six treatments based on a 2 x 3 factorial model, represented by two cultivars and three NPK levels (PD0 - 'Pérola' without fertilization, PD1 - 'Pérola' with 50 % of recommended fertilization, PD2 - 'Pérola' with 100 % of recommended fertilization, AD0 - 'IAC Alvorada' without fertilization, AD1 - 'IAC Alvorada' with 50 % of recommended fertilization, and AD2 - 'IAC Alvorada' with 100 % of recommended fertilization) and subplots sampled seven times during the cycle. Higher levels of NPK fertilization increased micronutrient extraction by both cultivars, and treatments with 100 % of recommended NPK fertilization extracted on average 167 g B, 58 g Cu, 1,405 g Fe, 1,213 g Mn and 211 g Zn per hectare. Regardless of the treatment, the highest demand period for B, Cu, Fe, Mn and Zn in both cultivars occurred at the R7 stage (pod formation), i.e. 42 to 55 days after emergence (DAE). The amount of B, Cu, Fe, Mn and Zn exported depended mainly on the level of NPK fertilization used, with values per hectare ranging from 38 to 90 g of B, 12 to 26 g of Cu, 222 to 568 g of Fe 234 to 467 g of Mn, and 40 to 96 g of Zn.
