Background The Australian state of Victoria, with 5.2 million residents, enforced home quarantine during a H1N1 pandemic in 2009. The strategy was targeted at school children. The objective of this study was to investigate the extent to which parents’ access to paid sick leave or paid carer’s leave was associated with (a) time taken off work to care for quarantined children, (b) household finances, and (c) compliance with quarantine recommendations. Methods We conducted an online and telephone survey of households recruited through 33 schools (85% of eligible schools), received 314 responses (27%), and analysed the subsample of 133 households in which all resident parents were employed. Results In 52% of households, parents took time off work to care for quarantined children. Households in which no resident parent had access to leave appeared to be less likely to take time off work (42% vs 58%, p=0.08) although this difference had only borderline significance. Among parents who did take time off work, those in households without access to leave were more likely to lose pay (73% vs 21%, p<0.001). Of the 26 households in which a parent lost pay due to taking time off work, 42% experienced further financial consequences such as being unable to pay a bill. Access to leave did not predict compliance with quarantine recommendations. Conclusions Future pandemic plans should consider the economic costs borne by households and options for compensating quarantined families for income losses.

INTRODUCTION AND AIMS: Bars, pubs and taverns in cities are often concentrated in entertainment precincts that are associated with higher rates of alcohol-related crime. This study assessed public perception and experiences of such crime in two city entertainment precincts, and support for alcohol-related crime reduction strategies. DESIGN AND METHODS: A cross-sectional household telephone survey in two Australian regions assessed: perception and experiences of crime; support for crime reduction strategies; and differences in such perceptions and support. RESULTS: Six hundred ninety-four people completed the survey (32%). Most agreed that alcohol was a problem in their entertainment precinct (90%) with violence the most common alcohol-related problem reported (97%). Almost all crime reduction strategies were supported by more than 50% of participants, including visitors to the entertainment precincts, with the latter being slightly less likely to support earlier closing and restrictions on premises density. Participants in one region were more likely to support earlier closing and lock-out times. Those at-risk of acute alcohol harm were less likely to support more restrictive policies. DISCUSSION AND CONCLUSIONS: High levels of community concern and support for alcohol harm-reduction strategies, including restrictive strategies, provide policy makers with a basis for implementing evidence-based strategies to reduce such harms in city entertainment precincts. Aim: Poor nutritional status has negative effects on post-operative outcomes, further compounded by surgical stress and fasting, places gastrointestinal surgery patients at high risk of malnutrition. Recent published research has challenged historic surgical nutrition practices; however, changes to practice in Australia have been slow. The aim of this study was to investigate current nutritional management of gastrointestinal surgery patients and compare this with the best practice guidelines, while exploring enablers to implementation of best practice. Methods: A 30-question telephone survey was developed to explore demographics and nutritional management of gastrointestinal surgical patients during pre-admission, inpatient stay and post-operative care. Forty-one gastrointestinal surgery dietitians were identified and contacted from 31 public hospitals in Victoria, Australia, and invited to participate. Results: Twenty-five dietitians participated in the survey (response rate 61%). Very few dietitians (12%) were funded for pre-admission clinics or outpatient clinics, and, overwhelmingly, dietitians reported not being involved in nutritional decision-making, and reported feeling unsatisfied with current nutritional management of patients. Despite half the hospitals reporting following best practice guidelines, only 22% implemented guidelines completely. There was no correlation observed between dietitian experience, department size or full-time equivalents allocated to surgery and nutritional intervention; however, the presence of a care pathway made a significant difference to the dietitian's overall satisfaction with dietetic care (P = 0.002). Conclusions: Current nutritional management of gastrointestinal surgery patients in Victorian hospitals is far from best practice. The implementation of a care pathway is the most effective way of ensuring best practice nutritional management of gastrointestinal surgical patients.

OBJETIVO: Avaliar a reprodutibilidade e a validade de indicadores de atividade física e sedentarismo, obtidos por sistema de vigilância baseado em inquéritos telefônicos. MÉTODOS: Foram realizadas análises de reprodutibilidade e validade em duas subamostras aleatórias (n=110 e n=111, respectivamente) da amostra total (N=2.024) de adultos (>18 anos), estudada pelo sistema, no município de São Paulo, em 2005. Os indicadores avaliados incluíram a freqüência de "suficientemente ativos no lazer", "inativos em quatro domínios da atividade física (lazer, trabalho, transporte e atividades domésticas)" e "ver televisão por longos períodos". A reprodutibilidade foi estudada comparando-se resultados obtidos a partir da entrevista telefônica original do sistema e de outra entrevista idêntica repetida após sete a 15 dias e feita por entrevistador diferente do que fez a entrevista original. A validade foi estudada comparando-se resultados obtidos a partir da entrevista telefônica original e de três recordatórios de 24 horas (método de referência) realizados na semana seguinte à entrevista original. RESULTADOS: A freqüência dos três indicadores avaliados foi idêntica ou muito próxima entre a primeira e a segunda entrevistas telefônicas, e os coeficientes kappa se situaram entre 0,53 e 0,80, indicando boa reprodutibilidade de todos os indicadores. Relativamente ao método de referência, evidenciou-se especificidade de 80% ou mais para os três indicadores e sensibilidade de 69,7% para "ver televisão por longos períodos", 59,1% para "inativos em quatro domínios" e 50% para "suficientemente ativos no lazer". CONCLUSÕES: Os indicadores de atividade física e sedentarismo empregados pelo sistema aparentam ser reprodutíveis e suficientemente acurados. Se mantido em operação nos próximos anos, o sistema poderá oferecer ao Brasil um instrumento útil para avaliação de políticas públicas de promoção da atividade física e controle das doenças crônicas não transmissíveis relacionadas ao sedentarismo.

O objetivo deste trabalho foi avaliar a validade relativa dos indicadores de consumo de alimentos e bebidas obtidos pelo sistema de vigilância por entrevista telefônica (VIGITEL). Foi avaliada uma amostra aleatória (n = 100) da amostra total de aproximadamente dois mil adultos, estudada pelo sistema, em 2009, em Belém, Pará, Brasil. Os indicadores avaliados foram fatores de proteção (consumo adequado de frutas, legumes e verduras) e de risco (consumo de gordura saturada, refrigerante e bebidas alcoólicas) para doenças crônicas não transmissíveis. Compararam-se os resultados obtidos a partir de entrevista telefônica e de três recordatórios de 24 horas (padrão de referência). O padrão de referência evidenciou subestimação na frequência dos indicadores, à exceção de refrigerantes e bebidas alcoólicas. As médias das frequências do consumo foram, em geral, maiores no grupo exposto (entrevistados pelo VIGITEL). Não se pode concluir que o VIGITEL não seja um bom indicador de consumo, pois o padrão de referência também apresenta limitações, portanto o uso daquele como instrumento de monitoramento é justificável.

This chapter explores the characteristics of 114 American teenagers' Jewish identities using data from the National Study of Youth and Religion (NSYR). The NSYR includes a telephone survey of a nationally representative sample of 3,290 adolescents aged 13 to 17. Jewish teenagers were over-sampled, resulting in a total of 3,370 teenage participants. Of the NSYR teens surveyed, 141 have at least one Jewish parent and 114 of them identify as Jewish. The NSYR also includes in-depth face-to-face interviews with a total of 267 U.S. teens: 23 who have at least one Jewish parent and 18 who identify as Jewish. The following analysis draws upon quantitative data from the 114 teens who identified themselves as Jewish in the face-to-face interviews.

This study aimed to assess the prevalence of dental pain among adults and older people living in Brazil's State capitals. Information was gathered from the Telephone Survey Surveillance System for Risk and Protective Factors for Chronic Diseases (VIGITEL) in 2009 (n = 54,367). Dental pain was the outcome. Geographic region, age, gender, race, schooling, private health coverage, smoking, and soft drink consumption were the explanatory variables. Multilevel Poisson regression models were performed. Prevalence of dental pain was 15.2%; Macapa and Sao Luis had prevalence rates greater than 20%; all capitals in the South and Southeast, plus Cuiaba, Campo Grande, Maceio, Recife, and Natal had prevalence rates less than 15%. Factors associated with increased prevalence of dental pain were the North and Northeast regions, female gender, black/brown skin color, lack of private health insurance, smoking, and soft drink consumption. Dental pain is a public health problem that should be monitored by health surveillance systems.

BACKGROUND AND OBJECTIVES: Medical ecology is a conceptual framework introduced in 1961 to describe the relationship and utilization of health care services by a given population. We applied this conception to individuals enrolled in a private health maintenance organization (HMO) in Sao Paulo, Brazil, with the aim of describing the utilization of primary health care, verifying the frequency of various symptoms, and identifying the roles of different health care sources. METHODS: This was a cross-sectional telephone survey among a random sample of people enrolled in a private HMO. We interviewed a random sample of non-pregnant adults over age 18 using 10 questions about symptoms and health care use during the month prior to interview. RESULTS: The final sample consisted of 1,065 participants (mean age 68 years, 68% female). From this sample, 424 (39.8%) reported the presence of symptoms, 311 (29.2%) had a medical office consult, 104 (9.8%) went directly to an emergency medical department, 63 (5.9%) were hospitalized, 22 (2.1%) used complementary medicine resources, seven (0.7%) were referred to home care, and one (0.1%) was admitted to an academic hospital. CONCLUSIONS: The proportion of study participants referred to an academic care center was similar to that observed in previous "medical ecology" studies in different populations.

Analisar a tendência da frequência do consumo do feijão nos anos de 2006 a 2009 nas capitais brasileiras. Trata-se de uma série histórica utilizando a base de dados do Vigitel de todas as capitais brasileiras. O consumo de feijão foi descrito por meio da frequência relativa e a tendência foi avaliada utilizando-se regressão de Poisson. O consumo de feijão cinco ou mais vezes por semana variou de 71,85% (2006) a 65,79 (2009). Na maior faixa de frequência de consumo ao longo de todo o período estudado estão incluídas as capitais Goiânia, Belo Horizonte, Palmas, Cuiabá e Brasília. As pessoas com IMC na categoria adequado/baixo peso apresentaram as maiores frequências de consumo em relação aos indivíduos com sobrepeso e obesidade. Foi observada tendência significativa de redução do consumo regular de feijão segundo ano de realização da pesquisa, exceto para a categoria de idade entre os 45 e 54 anos. Ocorreu redução significativa da frequência do consumo do feijão pela população brasileira e a adoção de políticas de monitoramento e incentivo do consumo é necessária em função dos benefícios apresentados pela leguminosa.

OBJETIVO: Analisar os fatores de risco e proteção para Doenças Crônicas Não Transmissíveis - DCNT nas capitais do Brasil. METODOLOGIA: Foram analisadas informações provenientes do sistema de vigilância de fatores de risco e proteção para DCNT por inquérito telefônico - VIGITEL, em 2008. A amostra foi composta por 54 mil entrevistas sendo as frequências apresentadas para o conjunto das capitais por sexo, faixa etária e escolaridade. RESULTADOS: O estudo mostrou diferenças na prevalência de fatores de risco e proteção de DCNT entre sexos, idade e escolaridade. Os homens apresentaram maiores frequências de fatores de risco como fumo, excesso de peso, consumo de refrigerantes, carnes com excesso de gordura e bebidas alcoólicas. Os homens praticam mais atividade física no lazer. As mulheres se alimentam melhor e referem mais diagnóstico médico de doenças, como hipertensão arterial, dislipidemia e osteoporose, além de estado de saúde ruim. Em geral, os fatores de risco são mais frequentes na população de menor escolaridade. DISCUSSÃO: Estas informações devem redirecionar a implementação das políticas públicas com foco em um modo de viver mais saudável e escolhas individuais mais adequadas por parte da população adulta brasileira.

Background: School-based sex education is effective in reducing risky sexual behavior among adolescents that may lead to unintended pregnancies and sexually transmitted infections. However, most sex education policies in the US do not support evidence-based programs. Understanding parental attitudes around sex education is crucial to overcoming perceived barriers to implementing school-based sex education. Little research has been published on the opinions of parents in Texas, which accounts for 12% of the nation’s teen births. Purpose: The purpose of this study was to examine whether Texas parents favor teaching sex education in schools, in what grades they think sex education should be taught, what content they think should be taught, and who they think should make decisions regarding sex education. Methods: We commissioned a telephone survey of parents of children 18 years or younger in Harris County, Texas. Survey questions assessed demographic characteristics and opinions about sex education. We used chi-square tests to examine differences across sociodemographic characteristics. Results: 1,201 parents completed the survey. The majority of parents (80%) responded that sex education should begin in middle school or earlier, and two-thirds said that it should include information about condoms and contraception. Hispanic parents showed the highest support for teaching sex education and providing medically accurate information on condoms and contraception in middle school or earlier. Conclusion: Parents in Harris County overwhelmingly support sex education that includes medically accurate information about condoms and contraception beginning before high school. These data provide evidence to change sex education policies to better reflect parental opinions.

Chronic fatigue syndrome (CFS) is a recently defined condition characterized by severe disabling fatigue that persists for a minimum of six months, and a host of somatic and neurocognitive symptoms. Although conditions similar to CFS have been described in the medical literature for over 100 years, little is known about the epidemiology of CFS or of chronic fatigue generally. The San Francisco Fatigue Study was undertaken to describe the prevalence and characteristics of self-reported chronic fatigue and associated conditions in a diverse urban community. The study utilized a cross-sectional telephone survey of a random sample of households in San Francisco, followed by case/control interviews of fatigued and nonfatigued subjects. Respondents were classified as chronically fatigued (CF) if they reported severe fatigue lasting six months or longer, then further classified as having CFS-like illness if, based on self-reported information, their condition appeared to meet CFS case definition criteria. Subjects who reported idiopathic chronic fatigue that did not meet CFS criteria were classified as having ICF-like illness.^ 8004 households were screened, yielding fatigue and demographic information on 16970 residents. CF was reported by 635 persons, 3.7% of the study population. CFS-like illness was identified in 34 subjects (0.2%), and ICF-like illness in 259 subjects (1.6%). Logistic regression analysis indicated that prevalence odds ratios for CFS-like illness were significantly elevated for females compared to males (OR = 2.9), and in Blacks (OR = 2.9) and Native Americans (OR = 13.2) relative to Whites, but significantly lower in Asians (OR = 0.12). Above-average household income was protective for all categories of CF. CFS-like subjects reported more symptoms and were more severely disabled than ICF-like subjects, but the pattern of symptoms experienced by both groups was similar. In conclusion, unexplained chronic fatigue, including CFS-like illness, occurs in all sociodemographic groups, but may be most prevalent among persons with lower incomes and in some racial minorities. Future studies that include clinical evaluation of incident cases of CFS and ICF are required to further clarify the epidemiology of unexplained chronic fatigue in the population. ^

BACKGROUND Headache is one of the most common symptoms in primary care. To improve the quality of headache diagnosis and management with the largest possible benefit for the general population, headache and pain societies around the world have recently been devoting more attention to headache in primary care.The aim of the study was to investigate the potential contribution that national societies can make toward raising the awareness of primary headaches in general practice. FINDINGS In a qualitative telephone survey, targeting primary care practices (PCP), we asked about the frequency of headache patients in their practices and inquired about their treatment and referral strategies.A total of 1000 telephone interviews with PCP have been conducted. Three-hundred and fifty physicians have been directly interviewed, 95% of them see headache patients every week, 23% daily. Direct MRI referral is done by 84%. Sixty-two per cent of the physicians knew the Swiss headache society, 73% were interested in further education about headaches. CONCLUSION The survey yielded information about the physicians' awareness of the Swiss Headache Society and its activities, and about their desire for continuing education in the area of headache. National headache societies should work to improve the cooperation between headache specialists and PCP, aiming for a better care for our patients with headache.
