Background: Paradoxical cases of psoriatic lesions induced or exacerbated by anti-tumor necrosis factor (TNF)-α therapy have been reported more frequently in recent years, but data related to inflammatory bowel disease (IBD) are rare. A systematic literature review was performed to provide information about this adverse effect in patients with IBD who receive anti-TNF therapy. Methods: Published studies were identified by a search of Medline, Embase, Cochrane, SciELO, and LILACS databases. Results: A total of 47 studies (222 patients) fulfilled the inclusion criteria and were selected for analysis. Clinical and therapeutic aspects varied considerably among these reports. Of the 222 patients, 78.38% were diagnosed with Crohn's disease, and 48.20% were female. The mean patient age was 26.50. years, and 70.72% of patients had no history of psoriasis. Patients developed psoriasiform lesions (55.86%) more often than other types of psoriatic lesions, and infliximab was the anti-TNF-α therapy that caused the cutaneous reaction in most patients (69.37%). Complete remission of cutaneous lesions was observed in 63.96% of the cases. Conclusions: We found that psoriatic lesions occurred predominantly in adult patients with Crohn's disease who received infliximab and had no previous history of psoriasis. Most patients can be managed conservatively without discontinuing anti-TNF-α therapy. © 2012 European Crohn's and Colitis Organisation.

Background: Exposure to ultraviolet (UV) radiation causes various forms of acute and chronic skin damage, including immunosuppression, inflammation, premature aging and photodamage. Furthermore, it induces the generation of reactive oxygen species, produces proinflammatory cytokines and melanocyte-stimulating hormone (MSH) and increases tyrosinase activity. The aim of this study was to evaluate the potential photoprotective effects of Rheum rhaponticum L. rhizome extract on human UV-stimulated melanocytes.Methods: The effects of Rheum rhaponticum rhizome extract on tyrosine kinase activity, and on interleukin-1α (IL-1α), tumour necrosis factor α (TNF-α), and α-MSH production in human epidermal melanocytes were evaluated under UV-stimulated and non-stimulated conditions. Antioxidant activity was evaluated by lipid peroxidation and 1,1-dyphenyl-2-picryl-hydrazyl (DPPH) assays, while anti-tyrosinase activity was evaluated by the mushroom tyrosinase method.Results: Rheum rhaponticum L. rhizome extract showed in vitro antioxidant properties against lipid peroxidation, free radical scavenging and anti-tyrosinase activities, and inhibited the production of IL-1α, TNF-α, α-MSH, and tyrosine kinase activity in melanocytes subjected to UV radiation.Conclusions: These results support the inclusion of Rheum rhaponticum L. rhizome extract into cosmetic, sunscreen and skin care products for the prevention or reduction of photodamage. © 2013 Silveira et al; licensee BioMed Central Ltd.

Background and Purpose Bone resorption induced by interleukin-1β (IL-1β) and tumour necrosis factor (TNF-α) is synergistically potentiated by kinins, partially due to enhanced kinin receptor expression. Inflammation-induced bone resorption can be impaired by IL-4 and IL-13. The aim was to investigate if expression of B1 and B2 kinin receptors can be affected by IL-4 and IL-13. Experimental Approach We examined effects in a human osteoblastic cell line (MG-63), primary human gingival fibroblasts and mouse bones by IL-4 and IL-13 on mRNA and protein expression of the B1 and B2 kinin receptors. We also examined the role of STAT6 by RNA interference and using Stat6-/- mice. Key Results IL-4 and IL-13 decreased the mRNA expression of B1 and B2 kinin receptors induced by either IL-1β or TNF-α in MG-63 cells, intact mouse calvarial bones or primary human gingival fibroblasts. The burst of intracellular calcium induced by either bradykinin (B2 agonist) or des-Arg10-Lys-bradykinin (B1 agonist) in gingival fibroblasts pretreated with IL-1β was impaired by IL-4. Similarly, the increased binding of B1 and B2 ligands induced by IL-1β was decreased by IL-4. In calvarial bones from Stat6-deficient mice, and in fibroblasts in which STAT6 was knocked down by siRNA, the effect of IL-4 was decreased. Conclusions and Implications These data show, for the first time, that IL-4 and IL-13 decrease kinin receptors in a STAT6-dependent mechanism, which can be one important mechanism by which these cytokines exert their anti-inflammatory effects and impair bone resorption. © 2013 The Authors. OBJECTIVE: To analyze cytokine gene expression in keratinocytes from patients with systemic lupus erythematosus (SLE). INTRODUCTION: Keratinocytes represent 95% of epidermal cells and can secrete several cytokines. METHODS: Keratinocytes were obtained by laser microdissection from 21 patients with SLE (10 discoid and 11 acute lesions) at involved and uninvolved sites. All patients were receiving a low/moderate prednisone dose and 18 were receiving chloroquine diphosphate. IL-2, IL-5, TNF-α and IFN-γ gene expression was evaluated by real-time PCR and expressed as the ratio (R) to a pool of skin samples from 12 healthy volunteers. RESULTS: Heterogeneity in cytokine gene expression was found among patients with SLE. Eighteen of 38 valid SLE samples (47%) presented overexpression (R>1) of at least one cytokine. Lesional skin samples tended to show higher cytokine expression than samples from uninvolved skin (p = 0.06). IL-5 and IFN-γ were the most commonly overexpressed cytokines. Samples with cytokine overexpression corresponded to more extensive and severe lesions. Prednisone dose did not differ between samples without cytokine overexpression (15.71±3.45 mg/day) and those with overexpressed cytokines (12.68±5.41 mg/day) (p = 0.216). Samples from all patients not receiving diphosphate chloroquine had at least one overexpressed cytokine. CONCLUSIONS: The heterogeneous keratinocyte cytokine gene expression reflects the complex immunological and inflammatory background in SLE. Patients with severe/extensive skin lesions showed a higher frequency of cytokine gene overexpression. Increased IFN-γ and IL-5 expression suggests that Th1 and Th2 cells are involved in SLE skin inflammation. The possibility that prednisone and antimalarial drugs may have contributed to low cytokine gene expression in some samples cannot be ruled out.

Este trabalho apresenta os resultados da pesquisa intitulada Níveis de TNF–α e seu Polimorfismo Genético em Pacientes HIV Positivos, Portadores de Lipodistrofia e em Uso de Terapia Anti–retroviral. Objetivou–se, por meio da pesquisa ora apresentada, estudar os níveis séricos de TNF–α e seu Polimorfismo Genético em Pacientes HIV Positivos, Portadores de Lipodistrofia e em Uso de Terapia Anti–retroviral. Justifica esse objetivo a necessidade de melhor conhecer um dos fatores envolvidos no desenvolvimento de uma síndrome clínica conhecida como lipodistrofia, considerada um efeito adverso da terapia anti–retroviral, a fim de que esse conhecimento oportunize a exeqüidade de um regime terapêutico seguro, propiciando, desse modo, uma possível melhoria na qualidade de vida dos pacientes. A metodologia empreendida para a determinação quantitativa dos níveis séricos de TNF–α utilizou-se do método ELISA, enquanto, tendo em vista a pesquisa da ocorrência do polimorfismo, utilizou–se da reação de PCR para a extração do DNA, seguida da digestão pela enzima de restrição Ncol. Participaram da presente pesquisa 40 pacientes, dos quais 26 do sexo masculino e 14 do sexo feminino. Desses pacientes, procedeu–se a dosagem dos níveis de TNF–α, bem como pesquisa da presença ou ausência do polimorfismo genético (–308A).

A hepatite B crônica apresenta amplo espectro de manifestações clínicas, resultante de diversos fatores, tais como o padrão de secreção e polimorfismo nos genes de citocinas. Este trabalho objetiva correlacionar os polimorfismos TNF-α -308G/A, INF-γ +874A/T, TGF-β1 -509C/T e IL-10 -1081A/G e os níveis séricos destas citocinas com a apresentação clínica da hepatite B. Foram selecionados 53 casos consecutivos de hepatite B, sendo divididos em grupo A (portador inativo= 30) e B (hepatite crônica/cirrose= 23). Como grupo controle, selecionaram-se 100 indivíduos com anti-HBc e anti-HBs positivos. Os níveis séricos das citocinas foram determinados por ensaios imunoenzimáticos, tipo ELISA (eBiosceince, Inc. Califórnia, San Diego, USA). A amplificação gênica das citocinas se realizou pela PCR e a análise histopatológica obedeceu à classificação METAVIR. Identificou-se maior prevalência do genótipo TNF-α -308AG (43,3% vs. 14,4%) no grupo B do que nos controles e a presença do alelo A se correlacionou com risco de infecção crônica pelo VHB (OR= 2,6). Os níveis séricos de INF-γ e de IL-10 foram maiores (p< 0,001) nos controles do que os demais grupos e, inversamente, as concentrações plasmáticas de TGF-β1 foram menores no grupo controle (p< 0,01). Observou-se, na histopatologia hepática, que atividade inflamatória > 2 se correlacionou com maiores níveis de TNF-α e de INF-γ (p< 0,05), assim como a fibrose > 2 com maiores níveis de INF-γ (p< 0,01). Na população pesquisada, menores níveis séricos de INF-γ e de IL-10 e maiores de TGF-β1 estiveram associados com a hepatite B crônica, bem como a presença do alelo A no gene TNF-α - 308 aumentou em 2,6 o risco de cronificação.
