A Síndrome da Imunodeficiência Adquirida (Aids) é uma doença do sistema imunológico humano causada pelo vírus da imunodeficiência humana (HIV). O HIV ataca células humanas responsáveis por defender o organismo de doenças, sendo os linfócitos T CD4+ os mais atingidos. A dor abdominal em paciente imunodeprimido evolui com difícil manejo diagnóstico, sendo mandatório ao cirurgião estar familiarizado com os diversos diagnósticos diferenciais e complicações secundárias da Aids. O presente trabalho teve como objetivo descrever os aspectos clínico-epidemiológicos de pacientes com Aids que evoluíram com abdome agudo e receberam tratamento cirúrgico no período de janeiro de 2001 a janeiro de 2011 no Hospital Universitário João de Barros Barreto. Foi um estudo observacional, retrospectivo, do tipo caso-controle, onde o grupo de casos foi constituído por pacientes com Aids que evoluíram com abdome agudo e o grupo controle, por pacientes que também evoluíram com abdome agudo, porém sem condição imunossupressora associada. Houve predominância do sexo masculino na proporção 4,5 homens para cada mulher no grupo com aids, porém com proporção similar nos controles. A maioria dos pacientes (87%) do grupo controle apresentou alguma alteração laboratorial, diferentemente do grupo com Aids, onde 38,5% dos pacientes tiveram resultado normal. A anemia esteve presente em 75% dos pacientes com Aids e a leucocitose em 80% do grupo controle. A causa mais frequente de abdome agudo na população com Aids foi perfuração intestinal (82,1%), enquanto no grupo controle foi obstrução intestinal (39,1%). Somente o quadro clínico de defesa abdominal e diminuição de ruídos hidroaéreos apresentaram diferença estatisticamente significativa (p<0.01). As alterações radiológicas mais frequentes foram distensão de alças em 87,2% dos pacientes com Aids e níveis hidroaéreos em 65,2% dos pacientes do grupo controle. A principal cirurgia realizada no grupo Aids foi a ressecção intestinal com reconstrução primária do trânsito (65,5%). As complicações cirúrgicas foram mais frequentes no grupo com Aids (87,2% com infecção de ferida operatória) e a causa predominante de óbito em ambos os grupos foi sepse a partir de foco abdominal (81% nos casos e 87,5% controles), inclusive nos pacientes ostomizados. A probabilidade de óbito nos casos com Aids foi superior em cerca de 2 vezes em relação aos controles. O tempo de internação e o tempo de pós-operatório até o óbito foi menor nos pacientes com Aids em comparação aos controles. Sendo fundamental a realização do estudo para melhorar o manejo e sobrevida dos pacientes com Aids.

OBJETIVO Analisar mudanças espaciais no risco de Aids e a relação entre incidência da doença e variáveis socioeconômicas. MÉTODOS Estudo caso-controle espacial, de base populacional, realizado em Rondônia, Brasil, com 1.780 casos notificados pelo Sistema de Vigilância Epidemiológica e os controles a partir de dados demográficos de 1987 a 2006. Os casos foram agrupados em cinco períodos de cinco anos consecutivos. Um modelo aditivo generalizado foi ajustado aos dados. O status dos indivíduos (caso ou controle) foi considerado como a variável dependente e independente: um alisamento ( spline ) bidimensional das coordenadas geográficas e variáveis socioeconômicas municipais. Os valores observados para o teste Moran I foram comparados com a distribuição de referência dos valores obtidos em condições de aleatoriedade espacial. RESULTADOS O risco de Aids apresentou padrão espacial e temporal marcado. A incidência associou-se a indicadores socioeconômicos municipais, como urbanização e capital humano. As maiores taxas de incidência de Aids ocorreram em municípios ao longo da rodovia BR-364; os resultados do teste Moran I mostram correlação espacial positiva associada à contiguidade dos municípios com a rodovia, no terceiro e quarto períodos (p = 0,05). CONCLUSÕES A incidência da doença foi maior em municípios de maior riqueza econômica e urbanização e naqueles cortados pelas estradas principais de Rondônia. O rápido desenvolvimento associado à ocupação de regiões remotas pode ser acompanhado por aumento de riscos à saúde.

OBJETIVO:traçar o perfil de mulheres vivendo com o vírus da imunodeficiência humana/Aids em municípios do interior do Estado de São Paulo, buscando-se identificar características relacionadas à vulnerabilidade individual, social e programática e analisar as condições em que tiveram conhecimento de seu status sorológico.MÉTODO:entre outubro de 2008 e dezembro de 2010, foi realizado estudo transversal, envolvendo 184 mulheres atendidas em serviço especializado. Os dados foram obtidos por entrevista e exame ginecológico, com coleta de amostras para diagnóstico etiológico de doenças sexualmente transmissíveis.RESULTADOS:predominaram mulheres brancas, entre 30 e 49 anos de idade, com companheiro, baixo nível escolar, múltiplos parceiros sexuais durante a vida e prática de sexo inseguro. A prevalência de doenças sexualmente transmissíveis foi de 87,0%.CONCLUSÃO:o estudo sugere a necessidade de ofertar assistência ginecológica em serviços especializados e realização de ações multiprofissionais que reforcem a autonomia feminina na tomada de decisões protetoras.

Toxoplasmosis, a zoonosis of worldwide distribution, has importance in human and veterinary medicine. Animals can be direct or indirect source of infection to man, and this intermediate host, the disease may be responsible for encephalitis and deaths due to congenital form as coinfection in neonates and patients with acquired immunodeficiency syndrome. The man and animals can acquire the disease by eating undercooked meat or cures, infected with tissue cysts, as well as food and water contaminated with oocysts. Iatrogenic, such as, blood transfusion and organ transplantation are other less frequent routes of transmission. The causative agent of this disease is Toxoplasma gondii, a protozoan obligate intracellular coccidian. In small animals, the infection has been reported in several countries, promoting varied clinical manifestations and uncommon but severe and fatal, which is a challenge in the clinical diagnosis of small animals, especially when the nervous system involvement. Thus, constitute the purpose of this review address the participation of small animals in the spread of the disease, clinical aspects related to it, as well as discuss methods of diagnosis, therapeutic measures, prophylaxis and control of this disease.

Objective: To identify vulnerabilities of elderly people with HIV/AIDS and the trajectory that they follow until reaching the diagnosis of the disease. Method: Qualitative research conducted in specialized clinics in the state of Sao Paulo, from January to June 2011. Semi-structured interviews were conducted with 11 elderly people who were found to be infected with the virus at the age of 60 years or older. The interviews were analyzed using content analysis. Results: In this process four categories emerged, then analyzed with reference to the theoretical framework of vulnerability. Conclusion: Late diagnosis of HIV infection or AIDS among the elderly happens in the secondary or tertiary service. Issues related to sexual life of the elderly are only questioned by health professionals after the diagnosis, also the time that condom use becomes absolute. It is believed that the investigation of the vulnerability of the elderly to HIV/AIDS allows for carrying out appropriate interventions for this population.

Human fungal infections have increased at an alarming rate in recent years, particularly in immunocompromised individuals. Cryptococcosis is the second most prevalent systemic fungal infection worldwide, and the most prevalent systemic infection in immunocompromised individuals, representing more than 70% of cases. The incidence of cryptococcosis is high in people with HIV/acquired immunodeficiency syndrome (AIDS), with recent estimates indicating that there are one million cases of cryptococcal meningitis globally per year in AIDS patients. The aim of this research was to develop a rapid flow cytometric antifungal susceptibility test and to compare the results with the standard methods. A reference strain and clinical isolates of Cryptococcus neoformans and Cryptococcus gattii were tested for susceptibility to amphotericin B by flow cytometry using propidium iodide as indicator of viability. Flow cytometry (FC) results were compared with the minimum inhibitory concentration (MIC) values determined by microdilution. The antifungal activity of amphotericin B ranged from MICs of 0.06 to 2μg/ml for the 11 isolates studied. The same results were found by FC. The FC method allows same-day results, assisting in the selection of appropriate antifungal therapies. These results demonstrate an excellent correlation between FC and the classic methods of testing for susceptibility to antifungal agents. This rapid diagnosis method makes it possible to quickly administer effective therapeutic interventions, often saving lives.

Introduction: The discrimination experienced by people with HIV / AIDS, not only by society but also by health professionals is one of the major problems observed relative the epidemic. Objective: To verify and to analyze the occurrence of discriminatory attitudes in the assistance to the health of people living with HIV/AIDS. Methods: It was a quantitative research was carried out with the participation of sixty-eight HIV-positive individuals from four Brazilian cities. The participants answered auto-administrate questionnaires that contained open and closed questions including the considered subject. Results: 41.2% of the total participants, HIV + people, said they had been discriminated against by health professionals. Among the discriminatory situations experienced by HIV patients, 34.2% nursing professionals were involved, in 34.2% dentists and in 31.6% doctors were involved. Those who have suffered discrimination, 78.6% said they had been discriminated against in public health service. Conclusion: The occurrence of discrimination in the assistance to the health of HIV-positive patients was high. The majority of discrimination situations occurred in the public health service. It is necessary the institution of strategies aiming at human attendance to these patients.

Introduction: health professionals, in many cases, have defensive and exclusionary practices, patterns of prejudice and discrimination in response to HIVpositive patients. Objective: to verify the perception of patients seropositives from a reference center for STD/HIV/aids in relation to health care. Methods: there were interviews with questions related to discriminatory attitudes suffered, access and health care. Results: 130 individuals participated in the survey, from which 30 (23.1%) reported events of discrimination suffered outside the center of reference, predominantly from surgeon-dentists and 112 (86.2%) believed to have the same access and health care as other citizens. 62 (47.7%) treated with the dental center of reference, of which 56 (90.3%) felt good being treated in a dental unit only for HIV positive and six (9.7%) were indifferent, believed to be more exposed, or have a bias to attend dental seropositive individuals. Conclusion: it is concluded that many patients have already been discriminated against outside the reference center. However, most of them have a positive feeling towards the treatment received at a specifi c place for treating HIV-positive patients, due to the feeling of being welcome at that place due to the humanization and access to health care.

Objectives: To describe the epidemiological profile, risk behaviors, and the prior history of sexually transmitted diseases (STDs) in women living with acquired immunodeficiency syndrome (AIDS). Methods: Cross-sectional study, performed at the Centro de Referencia e Treinamento em DST/AIDS of Sao Paulo. The social, demographic, behavioral, and clinical data such as age, schooling, marital status, age at first sexual intercourse, number of sexual partners, parity, use of drugs, time of HIV diagnosis, CD4 count, and viral load determination were abstracted from the medical records of women living with AIDS who had gynecological consultation scheduled in the period from June 2008 to May 2009. Results: Out of 710 women who were scheduled to a gynecological consultation during the period of the study, 598 were included. Previous STD was documented for 364 (60.9%; 95% CI: 56.9%-64.8%) women. The associated factors with previous STDs and their respective risks were: human development index (HDI) <0.50 (ORaj = 5.5; 95% CI: 2.8-11.0); non-white race (ORaj = 5.2; 95% CI: 2.5-11.0); first sexual intercourse at or before 15 years of age (ORaj = 4.4; 95% CI: 2.3-8.3); HIV infection follow-up time of nine years or more (ORaj = 4.2; 95% CI: 2.3-7.8)]; number of sexual partners during the entire life between three and five partners (ORaj = 2.2; 95% CI: 1.1-4.6), and six or more sexual partners (ORaj = 3.9; 95% CI: 1.9-8.0%); being a sex worker (ORaj = 1.9; 95% CI: 1.1-3.1). Conclusions: A high prevalence of a prior history of STDs in the studied population was found. It is essential to find better ways to access HIV infection prevention, so that effective interventions can be more widely implemented. (C) 2012 Elsevier Editora Ltda. All rights reserved.

Introduction: Several presentations of neurologic complications caused by JC virus (JCV) in human immunodeficiency virus (HIV)-infected patients have been described and need to be distinguished from the "classic" form of progressive multifocal leukoencephalopathy (PML). The objectives of this study were: 1) to describe the spectrum and frequency of presentations of JCV-associated central nervous system (CNS) diseases; 2) identify factors associated with in-hospital mortality of patients with JCV-associated CNS disease; and 3) to estimate the overall mortality of this population. Material and methods: This was a retrospective study of HIV-infected patients admitted consecutively for JCV-associated CNS diseases in a referral teaching center in Sao Paulo, Brazil, from 2002 to 2007. All patients with laboratory confirmed JCV-associated CNS diseases were included using the following criteria: compatible clinical and radiological features associated with the presence of JCV DNA in the cerebrospinal fluid. JCV-associated CNS diseases were classified as follows: 1) classic PML; 2) inflammatory PML; and 3) JC virus granule cell neuronopathy (GCN). Results: We included 47 cases. JCV-associated CNS diseases were classified as follows: 1) classic PML: 42 (89%); 2) inflammatory PML: three (6%); and 3) JC virus GCN: four (9%). Nosocomial pneumonia (p = 0.003), previous diagnosis of HIV infection (p = 0.03), and imaging showing cerebellar and/or brainstem involvement (p = 0.02) were associated with in-hospital mortality. Overall mortality during hospitalization was 34%. Conclusions: Novel presentations of JCV-associated CNS diseases were observed in our setting; nosocomial pneumonia, previous diagnosis of HIV infection, and cerebellar and/or brainstem involvement were associated with in-hospital mortality; and overall mortality was high.
