As researchers and practitioners move towards a vision of software systems that configure, optimize, protect, and heal themselves, they must also consider the implications of such self-management activities on software reliability. Autonomic computing (AC) describes a new generation of software systems that are characterized by dynamically adaptive self-management features. During dynamic adaptation, autonomic systems modify their own structure and/or behavior in response to environmental changes. Adaptation can result in new system configurations and capabilities, which need to be validated at runtime to prevent costly system failures. However, although the pioneers of AC recognize that validating autonomic systems is critical to the success of the paradigm, the architectural blueprint for AC does not provide a workflow or supporting design models for runtime testing. ^ This dissertation presents a novel approach for seamlessly integrating runtime testing into autonomic software. The approach introduces an implicit self-test feature into autonomic software by tailoring the existing self-management infrastructure to runtime testing. Autonomic self-testing facilitates activities such as test execution, code coverage analysis, timed test performance, and post-test evaluation. In addition, the approach is supported by automated testing tools, and a detailed design methodology. A case study that incorporates self-testing into three autonomic applications is also presented. The findings of the study reveal that autonomic self-testing provides a flexible approach for building safe, reliable autonomic software, while limiting the development and performance overhead through software reuse. ^

Background: Chronic fatigue syndrome, also known as myalgic encephalomyelitis (CFS/ME), is characterized by chronic disabling fatigue and other symptoms, which are not explained by an alternative diagnosis. Previous trials have suggested that graded exercise therapy (GET) is an effective and safe treatment. GET itself is therapist-intensive with limited availability. Objective: While guided self-help based on cognitive behavior therapy appears helpful to patients, Guided graded Exercise Self-help (GES) is yet to be tested. Methods: This pragmatic randomized controlled trial is set within 2 specialist CFS/ME services in the South of England. Adults attending secondary care clinics with National Institute for Health and Clinical Excellence (NICE)-defined CFS/ME (N=218) will be randomly allocated to specialist medical care (SMC) or SMC plus GES while on a waiting list for therapist-delivered rehabilitation. GES will consist of a structured booklet describing a 6-step graded exercise program, supported by up to 4 face-to-face/telephone/Skype™ consultations with a GES-trained physiotherapist (no more than 90 minutes in total) over 8 weeks. The primary outcomes at 12-weeks after randomization will be physical function (SF-36 physical functioning subscale) and fatigue (Chalder Fatigue Questionnaire). Secondary outcomes will include healthcare costs, adverse outcomes, and self-rated global impression change scores. We will follow up all participants until 1 year after randomization. We will also undertake qualitative interviews of a sample of participants who received GES, looking at perceptions and experiences of those who improved and worsened. Results: The project was funded in 2011 and enrolment was completed in December 2014, with follow-up completed in March 2016. Data analysis is currently underway and the first results are expected to be submitted soon. Conclusions: This study will indicate whether adding GES to SMC will benefit patients who often spend many months waiting for rehabilitative therapy with little or no improvement being made during that time. The study will indicate whether this type of guided self-management is cost-effective and safe. If this trial shows GES to be acceptable, safe, and comparatively effective, the GES booklet could be made available on the Internet as a practitioner and therapist resource for clinics to recommend, with the caveat that patients also be supported with guidance from a trained physiotherapist. The pragmatic approach in this trial means that GES findings will be generalizable to usual National Health Service (NHS) practice.

OBJETIVO: Avaliar a qualidade global das refeições oferecidas por Unidades de Alimentação e Nutrição de empresas beneficiárias do Programa de Alimentação do Trabalhador, na cidade de São Paulo. MÉTODOS: Estudo transversal realizado com 72 empresas cadastradas no programa. Foram coletadas informações de três dias consecutivos das refeições oferecidas no almoço, no jantar e na ceia. A qualidade das refeições oferecidas foi avalia pelo Índice de Qualidade da Refeição, e sua análise foi feita de forma estratificada segundo o perfil da empresa obtido pela análise de cluster. RESULTADOS: A média do Índice de Qualidade da Refeição para as grandes refeições foi de 66,25. Foram obtidos dois grupos de empresas na análise de cluster. As empresas do primeiro, composto em sua maioria por empresas do setor de comércio de micro e pequeno porte, cadastradas na modalidade de autogestão e sem supervisão de nutricionista, obtiveram pior qualidade da refeição (Índice=56,23). As empresas do segundo cluster, constituído principalmente por empresas de médio e grande porte do setor industrial, com gestão terceirizada e supervisão de nutricionista, obtiveram pontuação média do Índice de 82,95. CONCLUSÃO: As refeições oferecidas pelas empresas participantes do Programa de Alimentação do Trabalhador não estavam adequadas, segundo o Índice de Qualidade da Refeição. As empresas de menor porte e estrutura tiveram refeições de pior qualidade quando comparadas com as demais, demonstrando que empresas deste perfil são prioritárias para intervenções dentro do Programa de Alimentação do Trabalhador.

Using a dynamic systems model specifically developed for Piracicaba, Capivari and Jundia River Water Basins (BH-PCJ) as a tool to help to analyze water resources management alternatives for policy makers and decision takers, five simulations for 50 years timeframe were performed. The model estimates water supply and demand, as well as wastewater generation from the consumers at BH-PCJ. A run was performed using mean precipitation value constant, and keeping the actual water supply and demand rates, the business as usual scenario. Under these considerations, it is expected an increment of about similar to 76% on water demand, that similar to 39% of available water volume will come from wastewater reuse, and that waste load increases to similar to 91%. Falkenmark Index will change from 1,403 m(3) person(-1) year(-1) in 2004, to 734 m(3) P(-1) year(-1) by 2054, and the Sustainability Index from 0.44 to 0.20. Another four simulations were performed by affecting the annual precipitation by 90 and 110%; considering an ecological flow equal to 30% of the mean daily flow; and keeping the same rates for all other factors except for ecological flow and household water consumption. All of them showed a tendency to a water crisis in the near future at BH-PCJ.

Attempts to understand why people with adequate communication skills do not always perform well have focused on personality or personal style variables. This research focuses on the situational context and the difficulty inherent in particular encounters. This paper reports two studies concerned with what makes difficult face-to-face communication in work settings difficult or demanding. The first study (Study 1) identifies the types of face-to-face communication encounters that people find difficult to manage in the workplace. Quantitative and qualitative data were gathered to define 41 difficult communication situations representing situations difficult for superiors, colleagues and subordinates, as well as generically difficult situations. In Study 2, quantitative data were analysed using multidimensional scaling techniques to reveal the underlying structure of the situations. Four dimensions were identified: protection/approach, vulnerability, self-management, and involvement/engagement. The results provide insight into the ways in which people construe these types of situations and also provide a taxonomy of difficult communication situations in the workplace. Theoretical and practical implications of the findings are discussed.

This review illustrates, through a series of case histories, how oral medicine insights aid the diagnosis and management of patients with excessive tooth wear. The cases reviewed are drawn from the records of 500 southeast Queensland patients referred to the author over a 12 year period. Patients most at risk of dental erosion have work and sports dehydration, caffeine addiction, gastro-oesophageal reflux, asthma, diabetes mellitus, hypertension or other systemic diseases or syndromes that predispose to xerostomia. Saliva protects the teeth from the extrinsic and intrinsic acids which cause dental erosion. Erosion, exacerbated by attrition and abrasion, is the main cause of tooth wear. These cases illustrate that teeth, oral mucosa, salivary glands, skin and eyes should be examined for evidence of salivary hypofunction and attendant medical conditions. Based on comprehensive oral medicine, dietary analyses and advice, it would seem patients need self-management plans to deal with incipient chronic tooth wear. The alternative is the expensive treatment of pain, occlusal damage and pulp death required to repair the effects of acute severe tooth wear.

The efficacy of psychological treatments emphasising a self-management approach to chronic pain has been demonstrated by substantial empirical research. Nevertheless, high drop-out and relapse rates and low or unsuccessful engagement in self-management pain rehabilitation programs have prompted the suggestion that people vary in their readiness to adopt a self-management approach to their pain. The Pain Stages of Change Questionnaire (PSOCQ) was developed to assess a patient's readiness to adopt a self-management approach to their chronic pain. Preliminary evidence has supported the PSOCQ's psychometric properties. The current study was designed to further examine the psychometric properties of the PSOCQ, including its reliability, factorial structure and predictive validity. A total of 107 patients with an average age of 36.2 years (SD = 10.63) attending a multi-disciplinary pain management program completed the PSOCQ, the Pain Self-Efficacy Questionnaire (PSEQ) and the West Haven-Yale Multidimensional Pain Inventory (WHYMPI) pre-admission and at discharge from the program. Initial data analysis found inadequate internal consistencies of the precontemplation and action scales of the PSOCQ and a high correlation (r = 0.66, P < 0.01) between the action and maintenance scales. Principal component analysis supported a two-factor structure: 'Contemplation' and 'Engagement'. Subsequent analyses revealed that the PSEQ was a better predictor of treatment outcome than the PSOCQ scales. Discussion centres upon the utility of the PSOCQ in a clinical pain setting in light of the above findings, and a need for further research. (C) 2002 International Association for the Study of Pain. In the 1996 baseline surveys of the Australian Longitudinal Study of Women's Health (ALSWH), 36.1% of mid-age women (45-50) and 35% of older women (70-75) reported leaking urine. This study aimed to investigate (a) the range of self-management strategies used to deal with urinary incontinence (UI); (b) the reasons why many women who report leaking urine do not seek help for UI; and (c) the types of health professionals consulted and treatment provided, and perceptions of satisfaction with these, among a sample of women in each age group who reported leaking urine often' at baseline. Five hundred participants were randomly selected from women in each of the mid-age and older cohorts of the ALSWH who had reported leaking urine often in a previous survey. Details about UI (frequency, severity, and situations), self-management behaviors and help-seeking for UI, types of health professional consulted, recommended treatment for the problem, and satisfaction with the service provided by health care professionals and the outcomes of recommended treatments were sought through a self-report mailed follow-up survey. Most respondents had leaked urine in the last month (94% and 91% of mid-age and older women, respectively), and 72.2% and 73. 1% of mid-aged and older women, respectively, had sought help or advice about their UI. In both age groups, the likelihood of having sought help significantly increased with severity of incontinence. The most common reasons for not seeking help were that the women felt they could manage the problem themselves or they did not consider it to be a problem.. Many women in both cohorts had employed avoidance techniques in an attempt to prevent leaking urine, including reducing their liquid consumption, going to the toilet just in case, and rushing to the toilet the minute they felt the need to. Strategies are needed to inform women who experience UI of more effective management techniques and the possible health risks associated with commonly used avoidance behaviors. There may be a need to better publicize existing incontinence services and improve access to these services for women of all ages.

The Bologna Process aimed to build a European Higher Education Area promoting student's mobility. The adoption of Bologna Declaration directives requires a self management distributed approach to deal with student's mobility, allowing frequent updates in institutions rules or legislation. This paper suggests a computational system architecture, which follows a social network design. A set of structured annotations is proposed in order to organize the user's information. For instance, when the user is a student its annotations are organized into an academic record. The academic record data is used to discover interests, namely mobility interests, among students that belongs the academic network. These ideas have been applied into a demonstrator that includes a mobility simulator to compare and show the student's academic evolution.

Introdução: A doença pulmonar obstrutiva crónica (DPOC) é considerada uma das principais causas de morbilidade e mortalidade em todo o mundo, atingindo substancialmente o contexto económico e social. As ações de educação permitem capacitar o doente na gestão e controlo da sua doença, no desenvolvimento do empowerment e na aquisição de estratégias para melhorar os vários outcomes da doença crónica. Têm sido propostas como medidas para limitar a crescente carga e custos de saúde. Contudo, ainda se encontra incerta a sua eficácia nesta patologia. Objetivo: Analisar as implicações das ações educativas de self-management sobre a DPOC, nomeadamente ao nível da recorrência aos serviços de saúde, nas exacerbações, na autoeficácia e na medicação. Métodos: Estudos controlados e randomizados, escritos em inglês, que incluíram ações de educação de selfmanagement foram pesquisados em bases de dados MEDLINE, CINAHL, ScienceDirect. Outcomes analisados: número de exacerbações, recorrência aos serviços de saúde, autoeficácia e conhecimento da DPOC, recurso a antibiótico e/ou corticosteróides. Resultados: 12 artigos foram incluídos na revisão sistemática. O período de intervenção variou entre 3 a 18 meses e o número de participantes randomizados de 40 a 743. Foi observada uma diminuição da recorrência aos serviços de saúde e no aumento da autoeficácia e conhecimento da DPOC. Não foram observadas alterações na frequência de exacerbações nem no recurso a antibióticos e/ou corticosteróides. Conclusão: A implementação de ações de educação na DPOC mostrou trazer benefícios a nível da recorrência dos serviços de saúde e na autoeficácia e conhecimento acerca da DPOC. Contudo, mais estudos controlados e randomizados rigorosos são necessários.

Objetivo: Este estudo avaliou os efeitos da implementação de um programa de autorregulação no aumento da autonomia de um aluno com Perturbação do Espetro do Autismo, em contexto de sala de aula, no segmento letivo de Português. Método: A metodologia adotada foi quantitativa e experimental, num desenho de investigação de sujeito único, dividido em três fases de investigação: linha de base, início do programa de intervenção e um mês depois. Os registos dos vídeos, e a construção e implementação do programa de intervenção em conjunto com a professora, tiveram como base os princípios e procedimentos da Análise Comportamental Aplica. Resultados / Discussão: Após a implementação do programa de intervenção observou-se um acentuado aumento do envolvimento do aluno e das suas respostas às instruções académicas da professora, essencialmente ao nível das instruções de grupo que, na linha de base, apresentavam valores baixos. A eficácia da intervenção reforça a importância de se capacitar professores na aplicação de técnicas comportamentais dado que, neste estudo e à semelhança de outros, a professora aprendeu rapidamente a implementar práticas que facilmente se ajustam à sala de aula, levando à melhoria sistemática do comportamento do aluno com PEA numa sala de aula inclusiva.

Introdução: Cada vez mais a Reabilitação Cardíaca (RC) tem um início precoce no tratamento e nas necessidades do paciente no sentido de promover a sua autonomia e responsabilização pela recuperação, através de uma abordagem multidisciplinar. Os programas home-based e a inclusão das tecnologias de informação e comunicação são soluções atrativas para o aumento da participação dos doentes selecionados e inclusão de grupos de doentes atualmente sub-representados. Objetivos: Sistematizar a evidência científica atual sobre a efetividade dos programas de reabilitação cardíaca home-based com controlo á distância através da aplicação de novas tecnologias, comparando-a com a reabilitação centre-based/hospital-based, ao nível da adesão e da atividade física. Métodos: Este trabalho consiste numa revisão sistemática da literatura publicada entre 2007 e 2014, através de uma pesquisa em diferentes bases de dados eletrónicas científicas (Elsevier – Science Direct, PEDro, PubMed, Scielo Portugal e B-on) com as palavras-chave: reabilitação cardíaca, home-based, centre-based, hospital-based, reabilitação exercise-based, telemonitorização, smartphone, internet, atividade física, em todas as combinações possíveis. Os estudos foram analisados independentemente por dois revisores quanto aos critérios de inclusão e qualidade dos estudos. Resultados: Dos 101 estudos identificados, apenas dez foram incluídos. Considerando a escala da PEDro, quatro estudos obtiveram um score 5, quatro, um score de 6, e 2 com um score de 7 em 10. Os estudos foram realizados em adultos com idades compreendidas entre os 18 e os 80 anos. Os programas de intervenção dividiram-se em planeamento de atividade física e em autogestão. Todos os programas de exercício físico conduziram a um aumento da capacidade de exercício e consequente, maior controlo de fatores de risco. Pelos níveis de adesão aos PRC home-based e pelos resultados positivos de diferentes parâmetros em relação a reabilitação centre-based/hospital-based é notável a efetividade da telemonitorização baseada em casa. Conclusão: A telemonitorização domiciliária constitui um elemento fundamental para a solução de numerosos problemas destes doentes, tornando-se em métodos simples e de fácil funcionamento para haver sucesso nas taxas de adesão. Com efeito, a utilização das tecnologias de informação e de comunicação permite uma prestação e gestão eficazes dos cuidados de saúde no domicílio.

Asthma in schoolchildren became a priority both for developing and developed countries. Two large multi-centre epidemiological studies(Portuguese Study of Allergic Diseases in Childhood and the International Study of Asthma and Allergies in Childhood) were implemented in the last decade. One of the main objectives was to compare prevalence of asthma and evaluate cultural, ethnic and environmental variables in schoolchildren of different continents. These studies can contribute to a global strategy (with national specificity’s) including self management programs in the control of asthma to reduce the morbidity and mortality, and promote better quality of life with better allocation of resources.
