B-type natriuretic peptide (BNP) levels increase in systolic heart failure (HF). However, the value of BNP in hypertensive patients with suspected diastolic HF (symptoms suggestive of HF but normal ejection fraction) and its relation to myocardial function in these patients is unclear. We prospectively studied 72 ambulatory hypertensive subjects (40 women, mean age 58 +/- 8 years) with exertional dyspnea and ejection fraction greater than or equal to50%. Diastolic function was evaluated with transmitral and pulmonary venous Doppler, mitral annular velocities (pulsed-wave tissue Doppler), and flow propagation velocity (color M-mode). Systolic function was assessed with strain and strain rate derived from color tissue Doppler imaging. BNP was related to myocardial function and the presence or absence of global diastolic dysfunction. By conventional Doppler criteria, 34 patients had normal left ventricular diastolic function and 38 had isolated diastolic dysfunction. BNP values were higher in patients with diastolic dysfunction (46 +/- 48 vs 20 +/- 20 pg/ml, p = 0.004) and were related independently to blood pressure, systolic strain rate, left atrial function (p < 0.01 for all), and age (p = 0.015). Patients with diastolic dysfunction and pseudonormal filling had higher BNP levels compared with impaired relaxation (89 +/- 47 vs 35 +/- 42 pg/ml, p = 0.001). However, 79% of patients with diastolic dysfunction had BNP levels within the normal range. We conclude that in ambulatory hypertensive patients with symptoms suggestive of mild HF and normal ejection fraction, BNP is related to atrial and ventricular systolic parameters, blood pressure, and age. Although elevated in the presence of diastolic dysfunction, the BNP level mostly is in the normal range and, therefore, has limited diagnostic value in stable patients with suspected diastolic HF. Introdução – A monitorização da exposição ocupacional a doses de radiação ionizante pode ser complementada por dosímetros eletrónicos individuais que permitem uma leitura direta da dose de radiação recebida. Dada a dependência energética e de débito de dose já reportada para estes dosímetros, este trabalho pretende determinar a linearidade da resposta de um dosímetro eletrónico individual e estudar o comportamento da sua resposta em função da energia de radiação e do débito de dose. Metodologia – Para estudar a dependência da energia da radiação do dosímetro eletrónico pessoal Vertec Bleeper Sv procedeu‑se à sua irradiação com um equivalente de dose individual, Hp(10), de 500 μSv de radiação gama do Cobalto – 60 (60C) e Césio – 137 (137Cs) e das qualidades de radiação X da série Narrow (N): N‑30, N‑40, N‑60, N‑80, N‑100 e N‑120. Para investigar a dependência da resposta em função do débito de dose aplicaram‑se à ampola de raios X as intensidades de corrente elétrica de 1 mA, 5 mA, 10 mA, 15 mA e 20 mA. Resultados – Não existe uma relação entre a resposta do detetor e a energia de radiação a que este é exposto. Ocorre uma subestimação superior a 50% na grandeza medida para energias inferiores a 33 keV, mas ostenta uma medida relativamente linear da grandeza Hp(10) para doses inferiores a 100 μSv. Também se constata que, à medida que o débito de dose aumenta, existe uma diminuição na resposta do dosímetro. O menor decréscimo na resposta deste dosímetro eletrónico individual dá‑se para as qualidades de radiação N‑30 (1,1%), N‑40 (4,1%) e N‑120 (20,0%). Conclusão – Verifica‑se que a resposta do dosímetro individual Vertec Bleeper Sv depende fortemente da energia da radiação e do débito de dose. ABSTRACT: Introduction – The measurement of occupational exposure to radiation doses can be completed with an electronic personal dosemeter that allows a direct reading and alarm function of the received radiation dose. Due to the energy and dose rate dependence already reported for this type of dosemeter, it is intended, with this work, to determine the response linearity of an Electronic Personal Dosemeter and to study its response behavior to the dose rate and radiation energy. Methodology – The electronic personal dosemeter Vertec Bleeper Sv energy dependency was evaluated by its irradiation with 500 μSv from the radionuclides Cobalt – 60 (60C) and Cesium – 137 (137Cs) as well as by the radiation qualities of the Narrow (N) series: N‑30, N‑40, N‑60, N‑80, N‑100 e N‑120. To investigate the dose rate dependency, the intensities of electric current of 1 mA, 5 mA, 10 mA, 15 mA and 20 mA were applied to the X‑ray tube. Results – There is no relationship between the response of the detector and the radiation energy. For energies below 33 keV there is an underestimation over 50% of the radiation dose measured but the detector presents a linear response for energies under 100 μSv. A dependency on the dose rate is perceived since as the dose rate increases, the response of the individual monitor decreases. There is a smaller decrease for the radiation qualities of N‑30 (1.1%), N‑40 (4.1%) and N‑120 (20.0%). Conclusion – It is concluded that there is a strong dependence of radiation energy and dose rate on the response of an electronic personal dosemeter.

INTRODUCTION: Adults with repaired tetralogy of Fallot (TOF) may be at risk for progressive right ventricular (RV) dilatation and dysfunction, which is commonly associated with arrhythmic events. In frequently volume-overloaded patients with congenital heart disease, tissue Doppler imaging (TDI) is particularly useful for assessing RV function. However, it is not known whether RV TDI can predict outcome in this population. OBJECTIVE: To evaluate whether RV TDI parameters are associated with supraventricular arrhythmic events in adults with repaired TOF. METHODS: We studied 40 consecutive patients with repaired TOF (mean age 35 +/- 11 years, 62% male) referred for routine echocardiographic exam between 2007 and 2008. The following echocardiographic measurements were obtained: left ventricular (LV) ejection fraction, LV end-systolic volume, LV end-diastolic volume, RV fractional area change, RV end-systolic area, RV end-diastolic area, left and right atrial volumes, mitral E and A velocities, RV myocardial performance index (Tei index), tricuspid annular plane systolic excursion (TAPSE), myocardial isovolumic acceleration (IVA), pulmonary regurgitation color flow area, TDI basal lateral, septal and RV lateral peak diastolic and systolic annular velocities (E' 1, A' 1, S' 1, E' s, A' s, S' s, E' rv, A' rv, S' rv), strain, strain rate and tissue tracking of the same segments. QRS duration on resting ECG, total duration of Bruce treadmill exercise stress test and presence of exercise-induced arrhythmias were also analyzed. The patients were subsequently divided into two groups: Group 1--12 patients with previous documented supraventricular arrhythmias (atrial tachycardia, fibrillation or flutter) and Group 2 (control group)--28 patients with no previous arrhythmic events. Univariate and multivariate analysis was used to assess the statistical association between the studied parameters and arrhythmic events. RESULTS: Patients with previous events were older (41 +/- 14 vs. 31 +/- 6 years, p = 0.005), had wider QRS (173 +/- 20 vs. 140 +/- 32 ms, p = 0.01) and lower maximum heart rate on treadmill stress testing (69 +/- 35 vs. 92 +/- 9%, p = 0.03). All patients were in NYHA class I or II. Clinical characteristics including age at corrective surgery, previous palliative surgery and residual defects did not differ significantly between the two groups. Left and right cardiac chamber dimensions and ventricular and valvular function as evaluated by conventional Doppler parameters were also not significantly different. Right ventricular strain and strain rate were similar between the groups. However, right ventricular myocardial TDI systolic (Sa: 5.4+2 vs. 8.5 +/- 3, p = 0.004) and diastolic indices and velocities (Ea, Aa, septal E/Ea, and RV free wall tissue tracking) were significantly reduced in patients with arrhythmias compared to the control group. Multivariate linear regression analysis identified RV early diastolic velocity as the sole variable independently associated with arrhythmic history (RV Ea: 4.5 +/- 1 vs. 6.7 +/- 2 cm/s, p = 0.01). A cut-off for RV Ea of < 6.1 cm/s identified patients in the arrhythmic group with 86% sensitivity and 59% specificity (AUC = 0.8). CONCLUSIONS: Our results suggest that TDI may detect RV dysfunction in patients with apparently normal function as assessed by conventional echocardiographic parameters. Reduction in RV early diastolic velocity appears to be an early abnormality and is associated with occurrence of arrhythmic events. TDI may be useful in risk stratification of patients with repaired tetralogy of Fallot.

FUNDAMENTO: A avaliação da função Ventricular Direita (VD) pelo ecocardiograma em pacientes com Tromboembolismo Pulmonar (TEP) é complexa, frequentemente qualitativa; o Doppler tecidual tem sido utilizado para avaliação semiquantitativa dessa câmara, com algumas limitações. OBJETIVO: Avaliar a função do VD no TEP pelo ecocardiograma com Doppler tecidual, complementando com o peptídeo atrial natriurético (BNP). MÉTODOS: Foram estudados pacientes com TEP pelo ecocardiograma com Doppler tecidual e BNP até 24 horas do diagnóstico, obtendo-se as velocidades miocárdicas (s'), strain, strain rate e índice de performance miocárdica do VD; disfunção do VD foi iagnosticada por hipocinesia da câmara, movimento anormal septal e relação VD/VE >1. De acordo com o BNP os pacientes foram divididos em Grupo I, BNP < 50 pg/mL e Grupo II, BNP > 50 pg/mL. RESULTADOS: De 118 pacientes, 100 (60 homens, idade = 55 ± 17 anos) foram analisados; observou-se disfunção do VD em 28%, mais frequentemente no grupo II (19 vs. 9 pacientes, p < 0,001). O grupo II era mais idoso (64 ± 19 vs. 50 ± 15 anos), apresentava menor velocidade de s' (10,5 ± 3,5 vs. 13,2 ± 3,1 cm/s) e maior pressão pulmonar (48 ± 11 vs. 35 ± 11 mmHg), p < 0,001 para todos. O ponto de corte de s' para disfunção do VD foi de 10,8 cm/s (especificidade = 85%, sensibilidade = 54%), com moderada correlação entre o BNP e a onda s'(r = -0,39). CONCLUSÃO: No TEP, a disfunção do VD pelo ecocardiograma se acompanha de elevação do BNP; apesar confirmar adequadamente a presença de disfunção do VD, o Doppler tecidual apresenta sensibilidade limitada para este diagnóstico.

Diagnosis, prognosis and evaluation of death risk in Chagas cardiomyopathy still constitute a challenge due to the diversity of manifestations, which determine the importance of using echocardiography, tissue Doppler and biomarkers. To evaluate, within a systematic review, clinical and echocardiographic profiles of patients with chronic chagasic cardiomyopathy, which may be related to worse prognosis and major mortality risk. To perform the systematic review, we used Medline (via PubMed), LILACS and SciELO databases to identify 82 articles published from 1991 to 2012, with the following descriptors: echocardiography, mortality and Chagas disease. We selected 31 original articles, involving diagnostic and prognostic methods. The importance of Chagas disease has increased due to its emergence in Europe and United States, but most evidence came from Brazil. Among the predictors of worse prognosis and higher mortality risk are morphological and functional alterations in the left and right ventricles, evaluated by conventional echocardiography and tissue Doppler, as well as the increase in brain natriuretic peptide and troponin I concentrations. Recently, the evaluations of dyssynchrony, dysautonomia, as well as strain, strain rate and myocardial twisting were added to the diagnostic arsenal for the early differentiation of Chagas cardiomyopathy. Developments in imaging and biochemical diagnostic procedures have enabled more detailed cardiac evaluations, which demonstrate the early involvement of both ventricles, allowing a more accurate assessment of the mortality risk in Chagas disease.
Development of an optimized methodology for tensile testing of carbon steels in hydrogen environment

The study was performed at OCAS, the Steel Research Centre of ArcelorMittal for the Industry market. The major aim of this research was to obtain an optimized tensile testing methodology with in-situ H-charging to reveal the hydrogen embrittlement in various high strength steels. The second aim of this study has been the mechanical characterization of the hydrogen effect on hight strength carbon steels with varying microstructure, i.e. ferrite-martensite and ferrite-bainite grades. The optimal parameters for H-charging - which influence the tensile test results (sample geometry type of electrolyte, charging methods effect of steel type, etc.) - were defined and applied to Slow Strain Rate testing, Incremental Step Loading and Constant Load Testing. To better understand the initiation and propagation of cracks during tensile testing with in-situ H-charging, and to make the correlation with crystallographic orientation, some materials have been analyzed in the SEM in combination with the EBSD technique. The introduction of a notch on the tensile samples permits to reach a significantly improved reproducibility of the results. Comparing the various steel grades reveals that Dual Phase (ferrite-martensite) steels are more sensitive to hydrogen induced cracking than the FB (ferritic-bainitic) ones. This higher sensitivity to hydrogen was found back in the reduced failure times, increased creep rates and enhanced crack initiation (SEM) for the Dual Phase steels in comparison with the FB steels.

We study the damage enhanced creep rupture of disordered materials by means of a fiber bundle model. Broken fibers undergo a slow stress relaxation modeled by a Maxwell element whose stress exponent m can vary in a broad range. Under global load sharing we show that due to the strength disorder of fibers, the lifetime ʧ of the bundle has sample-to-sample fluctuations characterized by a log-normal distribution independent of the type of disorder. We determine the Monkman-Grant relation of the model and establish a relation between the rupture life tʄ and the characteristic time tm of the intermediate creep regime of the bundle where the minimum strain rate is reached, making possible reliable estimates of ʧ from short term measurements. Approaching macroscopic failure, the deformation rate has a finite time power law singularity whose exponent is a decreasing function of m. On the microlevel the distribution of waiting times is found to have a power law behavior with m-dependent exponents different below and above the critical load of the bundle. Approaching the critical load from above, the cutoff value of the distributions has a power law divergence whose exponent coincides with the stress exponent of Maxwell elements

OBJECTIVES: In vitro mechanical injury of articular cartilage is useful to identify events associated with development of post-traumatic osteoarthritis (OA). To date, many in vitro injury models have used animal cartilage despite the greater clinical relevance of human cartilage. We aimed to characterize a new in vitro injury model using elderly human femoral head cartilage and compare its behavior to that of an existing model with adult bovine humeral head cartilage. DESIGN: Mechanical properties of human and bovine cartilage disks were characterized by elastic modulus and hydraulic permeability in radially confined axial compression, and by Young's modulus, Poisson's ratio, and direction-dependent radial strain in unconfined compression. Biochemical composition was assessed in terms of tissue water, solid, and glycosaminoglycan (GAG) contents. Responses to mechanical injury were assessed by observation of macroscopic superficial tissue cracks and histological measurements of cell viability following single injurious ramp loads at 7 or 70%/s strain rate to 3 or 14 MPa peak stress. RESULTS: Confined compression moduli and Young's moduli were greater in elderly human femoral cartilage vs adult bovine humeral cartilage whereas hydraulic permeability was less. Radial deformations of axially compressed explant disks were more anisotropic (direction-dependent) for the human cartilage. In both cartilage sources, tissue cracking and associated cell death during injurious loading was common for 14 MPa peak stress at both strain rates. CONCLUSION: Despite differences in mechanical properties, acute damage induced by injurious loading was similar in both elderly human femoral cartilage and adult bovine humeral cartilage, supporting the clinical relevance of animal-based cartilage injury models. However, inherent structural differences such as cell density may influence subsequent cell-mediated responses to injurious loading and affect the development of OA.

Carbonate mylonites with varying proportions of second-phase minerals were collected at positions of increasing metamorphic grade along the basal thrust of the Morcles nappe (Helvetic nappes, Switzerland). Variations of temperature, stress, and strain rate, changes in chemistry of solid and fluid phases, and differing degrees of strain localization and annealing were tracked by measuring the shapes, mean sizes, and size distributions of both matrix and second-phase grains, as well as crystal preferred orientation (CPO) of the matrix. Field structures suggest that strain rate was constant along the fault. The mean and distribution of the calcite grain sizes were affected most profoundly by temperature: Increased temperature, presumably accompanied by decreased stress, correlated with larger mean sizes and wider size distributions. At a given location, the matrix grains in mylonites with more second-phase particles are, on average, smaller, have narrower size distributions, and have more elongate shapes. For example, mylonites with 50 vol.% of second phases have matrix grain sizes half that of pure mylonites. Changes in calcite chemistry and the presence of synkinematic fluids seemed to influence microfabric only weakly. Temporal variations in conditions, such as exhumation-induced cooling, apparently provoke changes in temperature, stress, and strain rate along the nappe. These changes result in further strain localization during retrograde conditions and cause the grain size to be reduced by an additional 50%. The matrix CPO strengthens with increasing temperature or strain, but weakens and rotates with increasing second-phase content, These fabric changes suggest differing rates of grain growth, grain size reduction, and development of CPO owing to variations in the deformation conditions and, perhaps, mechanisms. To interpret natural mylonite structures or to extrapolate mechanical data to natural situations requires careful characterization of the microfabric, and, in particular, second-phase minerals.
