A total of 177 patients with primary dislocation of the patella (PDP) were admitted to two trauma centers in Helsinki, Finland during 1991 to 1992. The inclusion criteria were: 1. Acute (≤14 days old) first-time lateral dislocation of the patella. 2. No previous knee operations or major knee injuries. 3. No ligament injuries to be repaired. 4. No osteochondral fractures requiring fixation. 50 patients were excluded. 30 of these excluded patients would have met the inclusion criteria, 19 patients received treatment by consultants not involved in the study, 7 refused to participate and 4 had an erroneous randomization. 127 patients including, 82 females, were then randomized to have either tailor-made operative procedure (group O) or conservative treatment (group C). The aftercare was similar for both groups. The mean age of the patients was 20 (9-47) years. All patients were subjected to analysis of trauma history (starting position and knee movement during the dislocation), examination under anesthesia (EUA) and arthroscopy. 70 patients (52 females) were randomized by their odd year of birth to operative group O and 57 patients (30 females) by their even year of birth to conservative group C. The diagnosis of PDP was based on locked dislocation in 68 patients, on dislocatability in EUA in 47 patients, and on subluxation in EUA combined with typical intra-articular lesions in 12 patients. In group O, 63 patients had exploration of the injuries on the medial side of the knee and tailor made reconstruction added with lateral release in 54 cases. The medial injury was operated by suturing in 39 patients, by duplication in 18 patients and by additional augmentation of the medial patellofemoral ligament (MPFL) with adductor magnus tenodesis in 6 patients. 7 patients, without locking in trauma history and only subluxation in EUA had only lateral release for realignment. In adductor magnus tenodesis the proximal end of the distal tendinous part was rerouted to the upper medial border of the patella. In the conservative group C, the treatment was adjusted to the extent of patellar displacement in EUA. Patients with dislocation in EUA had 3 weeks’ immobilization with the knee in slight flexion. Mobilization was started with a soft patellar stabilizing orthosis (PSO) used for additional three weeks. The patients with subluxation in EUA wore an orthosis for six weeks. The aftercare was similar in group O. The outcome was similar in both groups. After an average of 25 (20-45) months´ follow-up, the subjective result was better in group C in respect of the mean Hughston VAS knee score (87 for group O and 90 for group C, p=0.04, visual analog scale), but similar in terms of the patient’s own overall opinion and the mean Lysholm II knee score. Recurrent instability episodes occurred in 18 patients in group O and in 20 patients in group C. After an average of 7 (6-9) years´ follow-up, the groups did not show statistical difference either in respect of the patient’s own overall opinion, or the mean Hughston VAS and Kujala knee scores. The proportions of stable patellae was 25/70 (36%) in group O and 17/57 (30%) in group O (p=0.5). In a multivariate risk analysis, there was a correlation between low Kujala score (<90) as dependent parameter and female gender (OR: 3.5; 95% CI: 1.4-9.0), and loose body on primary radiographs (OR: 4.1; 95% CI: 1.2-15). Recurrent instability correlated with young age at the time of PDP (OR: 0.9; 95% CI: 0.8-1.0/year). Girls with open tibial apophysis had the worst prognosis for instability (88%; 95% CI: 77-98). The most common mechanisms in trauma history of the patients were movement to flexion from a straight start (78%) and movement to extension from a well-bent start (8%). Spontaneous relocation of the patella had taken place in 13/39 of girls, in 11/21 of boys, in 26/42 of women and in 17/24 of men with skeletal maturity of the tibia. The dislocation in EUA was non-rotating in 96/126 patients followed by outward rotating dislocation in 14/126 patients. Operative treatment policy in PDP is not recommended. Locking tendency of the patella in PDP depended on the skeletal maturation. Recurrence rate after PDP was higher than expected.

Triclosan, a well-known inhibitor of Enoyl Acyl Carrier Protein Reductase (ENR) from several pathogenic organisms, is a promising lead compound to design effective drugs. We have solved the X-ray crystal structures of Plasmodium falciparum ENR in complex with triclosan variants having different substituted and unsubstituted groups at different key functional locations. The structures revealed that 4 and 2' substituted compounds have more interactions with the protein, cofactor, and solvents when compared with triclosan. New water molecules were found to interact with some of these inhibitors. Substitution at the 2' position of triclosan caused the relocation of a conserved water molecule, leading to an additional hydrogen bond with the inhibitor. This observation can help in conserved water-based inhibitor design. 2' and 4' unsubstituted compounds showed a movement away from the hydrophobic pocket to compensate for the interactions made by the halogen groups of triclosan. This compound also makes additional interactions with the protein and cofactor which compensate for the lost interactions due to the unsubstitution at 2' and 4'. In cell culture, this inhibitor shows less potency, which indicates that the chlorines at 2' and 4' positions increase the ability of the inhibitor to cross multilayered membranes. This knowledge helps us to modify the different functional groups of triclosan to get more potent inhibitors. (C) 2010 IUBMB IUBMB Life, 62(6): 467-476.

Control is central to management and there is already a considerable body of research on control. However, the emergence and growth of multinational corporations (MNCs) has renewed the interest in control, as MNCs are complex (often large) organizations that face circumstances beyond those of national business organizations. The geographical dispersion of MNC activities means that the headquarters controls subsidiaries that differ with regard to power and that are embedded in different cultural, political, legal and educational systems. Foreign subsidiary control also takes place across language boundaries and physical (i.e. geographical) distances. In face of these challenges, how are foreign subsidiaries controlled? The thesis explores different types of control mechanisms and attempts to explain the degree to which they are used to control foreign subsidiaries. It contributes to existing knowledge on control by exploring how five different control mechanisms are related to each other. Previous research has tended to focus only on one or two control mechanisms and seldom has their effect on each other been explored. The thesis also contributes by including two central aspects of the MNC that have been neglected in much of the research on foreign subsidiary control: language competence of subsidiary staff and physical distance between the headquarters and its subsidiaries. The findings indicate that specific control mechanisms should not be studied in isolation as there are intricate relationships among the different control mechanisms. Language competence of the subsidiary staff can furthermore affect the type and degree of control that the headquarters can exercise over a subsidiary. The findings also indicate that changes in the physical distance between subsidiaries and its headquarters (i.e. a relocation of the headquarters as part of a restructuring process) can have great consequences for the headquarters-subsidiary relationship.

Brachial plexus birth injury (BPBI) is caused by stretching, tearing or avulsion of the C5-C8 or Th1 nerve roots during delivery. Foetal-maternal disproportion is the main reason for BPBI. The goal of this study was to find out the incidence of posterior subluxation of the humeral head during first year of life in BPBI and optimal timing of the ultrasonographic screening of the glenohumeral joint. The glenohumeral congruity and posterior subluxation of the humeral head associated to muscle atrophy were assessed and surgical treatment of the shoulder girdle as well as muscle changes in elbow flexion contracture were evaluated. The prospective, population based part of the study included all neonates born in Helsinki area during years 2003-2006. Patients with BPBI sent to the Hospital for Children and Adolescents because of decreased external rotation, internal rotation contracture or deformation of the glenohumeral joint as well as patients with elbow flexion contracture were also included in this prospective study. The incidence of BPBI was calculated to be 3.1/1000 newborns in Helsinki area. About 80% of the patients with BPBI recover totally during the follow-up within the first year of life. Permanent plexus injury at the age of one year was noted in 20% of the patients (0.64/1000 newborns). Muscle imbalance resulted in sonographically detected posterior subluxation in one third of the patients with permanent BPBI. If muscle imbalance and posterior subluxation are left untreated bony deformities will develop. All patients with internal rotation contracture of the glenohumeral joint presented muscle atrophy of the rotator cuff muscles. Especially subscapular and infraspinous muscles were affected. A correlation was found particularly between greatest thickness of subscapular muscle and subluxation of the humeral head, degree of glenoid retroversion, as well as amount of internal rotation contracture. Supinator muscle atrophy was evident among all the studied patients with elbow flexion contracture. Brachial muscle pathology seemed to be an important factor for elbow flexion contracture in BPBI. Residual dysfunction of the upper extremity may require operative treatment such as tendon lengthening, tendon transfers, relocation of the humeral head or osteotomy of the humerus. Relocation of the humeral head improved the glenohumeral congruency among patients under 5 years of age. Functional improvement without remodeling of the glenohumeral joint was achieved by other reconstructive procedures. In conclusion: Shoulder screening by US should be done to all patients with permanent BPBI at the age of 3 and 6 months. Especially atrophy of the subscapular muscle correlates with glenohumeral deformity and posterior subluxation of the humeral head, which has not been reported in previous studies. Permanent muscle changes are the main reason for diminished range of motion of the elbow and forearm. Relocation of the humeral head, when needed, should be performed under the age of 5 years.

Two common goals of this meeting are to arrest the effects of sea level rise and other phenomena caused by Greenhouse Gases from anthropogenic sources ("GHG",) and to mitigate the effects. The fundamental questions are: (1) how to get there and (2) who should shoulder the cost? Given Washington gridlock, states, NGO's and citizens such as the Inupiat of the Village of Kivalina have turned to the courts for solutions. Current actions for public nuisance seek (1) to reduce and eventually eliminate GHG emissions, (2) damages for health effects and property damage—plus hundreds of millions in dollars spent to prepare for the foregoing. The U.S. Court of Appeals just upheld the action against the generators of some 10% of the CO2 emissions from human activities in the U.S., clearing the way for a trial featuring the state of the art scientific linkage between GHG production and the effects of global warming. Climate change impacts on coastal regions manifest most prominently through sea level rise and its impacts: beach erosion, loss of private and public structures, relocation costs, loss of use and accompanying revenues (e.g. tourism), beach replenishment and armoring costs, impacts of flooding during high water events, and loss of tax base. Other effects may include enhanced storm frequency and intensity, increased insurance risks and costs, impacts to water supplies, fires and biological changes through invasions or local extinctions (IPCC AR4, 2007; Okmyung, et al., 2007). There is an increasing urgency for federal and state governments to focus on the local and regional levels and consistently provide the information, tools, and methods necessary for adaptation. Calls for action at all levels acknowledge that a viable response must engage federal, state and local expertise, perspectives, and resources in a coordinated and collaborative effort. A workshop held in December 2000 on coastal inundation and sea level rise proposes a shared framework that can help guide where investments should be made to enable states and local governments to assess impacts and initiate adaptation strategies over the next decade. Esta tese tem como objeto a regulação política da sexualidade no âmbito da família por saberes e instituições médicas brasileiras (1838-1940). Orienta-se pelo interesse em analisar continuidades e descontinuidades na construção de objetos, estratégias e táticas políticas direcionados para a regulação higiênica e eugênica do casamento e da sexualidade infantil. De inspiração foucaultiana, inscreve-se no campo da história dos saberes e está subsidiada por um conjunto heterogêneo de documentos (teses, artigos de periódicos, livros, anais etc.) circunscritos, majoritariamente, ao campo da medicina. Analisa a constituição de uma defesa higiênica dos casamentos no pensamento médico novecentista, voltada para remanejamentos das figuras de esposa e marido na nova configuração de família que começava a se esboçar no Brasil, contrastando-a com a regulação católica da moral sexual colonial. Em seguida, descreve a visibilidade higiênica que a medicina dará a infância no século XIX, problematizando especificamente o interesse pelo tema da masturbação, que articula simultaneamente a família, centrada na figura da mãe, e a escola na convocação de zelar pela criança. Partindo das contradições sociais que se apresentaram na construção do projeto liberal nacional a partir da década de 1870, discute a apropriação do discurso da degenerescência pelo saber médico-psiquiátrico brasileiro, que propiciou uma leitura da brasilidade marcada pelo excesso sexual e pela condição degenerada da miscigenação, a fim de pensar as condições de possibilidade para a emergência do projeto de eugenia matrimonial institucionalizado nas primeiras décadas do século XX e toma como táticas a campanha pela compulsoriedade do exame pré-nupcial, o combate aos casamentos consanguíneos, o controle do contágio venéreo e o aconselhamento sexual dos casais. Analisa a campanha de educação sexual, cuja pretensão de instituir uma sciencia sexual no Brasil, de legitimidade controversa, tinha como horizonte viabilizar uma profilaxia sexual que mitigasse a produção da criminalidade, das perversões sexuais e das doenças nervosas, bem como os desajustes familiares, a partir da fabricação de um novo objeto, qual seja, a sexualidade infantil, no qual incidirá uma nova pedagogia. Nesse particular, aponta particularidades discursivas da difusão das idéias freudianas entre higienistas brasileiros. Finalmente, sinaliza a constituição da higiene mental da criança como um novo domínio para a psiquiatria brasileira, que tomou a intensa circulação afetiva intrafamiliar como ponto de ancoragem para um projeto de normalização social, ainda centrado na eugenia, mas já atravessado por uma psicologia da adaptação.

蓝光光存储是信息存储领域发展的一个重要方向。测试系统是研究蓝光光存储材料的必备条件,为建立蓝光光存储研究平台,设计和搭建了一套模块化蓝光光存储性能静态测试系统。激光工作波长为406.7nm。系统采用调制解调技术降低系统噪声,提高测量粘度:采用光点移动模式降低对样品尺寸的要求;采用纳米平台提高记录点的反复定位性能。实验结果证明测试系统性能良好,可以满足蓝光光存储性能静态测试要求。

Blue ray optical storage is one of the important trends in the area of information storage, and attracts a number of researchers. Static tester for blue ray optical storage plays an important role in storage media research. We designed and constructed a static tester, in which modularization makes it very convenient to expand function. Employment of modulation/demodulation technique weakens test error so as to increase test precision. Focus move mode and the nano-positioners facilitate the relocation of recording marks. Only one laser with wavelength 406.7 nm is used. In this paper, system design and characteristics are represented in detail, and some experimental results are also given to show that the static tester can perform successfully, acting as research platform for blue ray optical storage. A violência é considerada uma questão social, ou ainda, um fenômeno social e histórico, que ocorre nas diversas formas de relações humanas e que pode se manifestar em atos individuais ou institucionais, ou seja, realizados por pessoas, grupos, nações, com o objetivo de provocar algum dano físico ou psicológico em outrem. O termo violência, no presente trabalho, refere-se às mortes por causas externas, que incluem as mortes intencionais e as mortes não intencionais, ou seja, agressões, suicídios e acidentes em geral. O objetivo é analisar a evolução da carga de mortalidade no estado do Rio de Janeiro e propor a realocação dos óbitos cuja intenção é indeterminada através de uma nova metodologia. Os dados utilizados são provenientes do Sistema de Informações sobre Mortalidade. Neste estudo, foi utilizado o indicador YLL (Years of Life Lost Anos de Vida Perdidos) na avaliação do comportamento das causas violentas ao longo do tempo, no período de 1996 a 2009, para as macrorregionais de saúde do estado do Rio de Janeiro, através de modelos de efeitos mistos. Foi aplicada a regressão logística multinomial nos óbitos com causa básica conhecida, utilizando as informações como lesões e características individuais das vítimas, para prever qual seria a causa básica de morte nos registros indeterminados com características semelhantes aos óbitos com causas conhecidas. Os resultados encontrados mostram que a violência aumentou em regiões do interior do estado, com destaque para a macrorregional Norte. Na capital e nas regiões metropolitanas, houve uma estabilização das taxas, com exceção para as mortes por agressão que sofreram queda, porém as taxas de YLL permaneceram elevadas. As duas metodologias de realocação, da Carga de Doença e desta nova proposta, aumentam todas as taxas de mortalidade por grupo de causas, porém o grupo que sofreu maior impacto foi o de quedas. Os resultados encontrados, apesar das limitações, apontam para uma proposta de combinação das duas metodologias. Para os óbitos com causa básica de Y10 (Envenenamento [intoxicação] por e exposição a analgésicos, antipiréticos e anti-reumáticos nãoopiáceos, intenção não determinada) a Y33 (Outros fatos ou eventos especificados, intenção não determinada), seria utilizada a metodologia da Carga de Doenças e, para os óbitos de Y34 (Fatos ou eventos não especificados e intenção não determinada), seria utilizado o método proposto.

Esta tese busca analisar os significados da reatualização de práticas e discursos sobre a remoção de favelas atualmente no Rio de Janeiro. Para a realização da pesquisa que resultou neste trabalho acompanhei diversas situações de realocação conduzidas pela prefeitura. Analiticamente, esta tese se articula a partir de três níveis: no primeiro, busco retraçar as condições de possibilidade, na atual conjuntura, que permitiram a retomada do termo/ação "remoção" como forma específica de intervenção estatal nas favelas cariocas. No segundo, analiso como operaram, concretamente, estas intervenções, a partir da observação dos inúmeros contatos entre agentes estatais e moradores daquelas localidades em processo de realocação. Por fim, trato das dinâmicas de ação coletiva constituídas a partir da crítica que estes moradores, bem como outros atores, individuais e coletivos, realizam a estes processos.

Estimates of the abundance of American horseshoe crabs (Limulus polyphemus) are important to determine egg production and to manage populations for the energetic needs of shorebirds that feed on horseshoe crab eggs. In 2003, over 17,500 horseshoe crabs were tagged and released throughout Delaware Bay, and recaptured crabs came from spawning surveys that were conducted during peak spawning. We used two release cohorts to test for a temporary effect of tagging on spawning behavior and we adjusted the number of releases according to relocation rates from a telemetry study. The abundance estimate was 20 million horseshoe crabs (90 % confidence interval: 13−28 million), of which 6.25 million (90% CI: 4.0−8.8 million) were females. The combined harvest rate for Delaware, New Jersey, Virginia, and Maryland in 2003 was 4% (90% CI: 3−6%) of the abundance estimate. Over-wintering of adults in Delaware Bay could explain, in part, differences in estimates from ocean-trawl surveys. Based on fecundity of 88,000 eggs per female, egg production was 5.5×1011 (90% CI: 3.5×1011, 7.7×1011), but egg availability for shorebirds also depended on overlap between horseshoe crab and shorebird migrations, density-dependent bioturbation, and wave-mediated vertical transport.

Background: In complex with its cofactor UAF1, the USP1 deubiquitinase plays an important role in cellular processes related to cancer, including the response to DNA damage. The USP1/UAF1 complex is emerging as a novel target in cancer therapy, but several aspects of its function and regulation remain to be further clarified. These include the role of the serine 313 phosphorylation site, the relative contribution of different USP1 sequence motifs to UAF1 binding, and the potential effect of cancer-associated mutations on USP1 regulation by autocleavage. Methods: We have generated a large set of USP1 structural variants, including a catalytically inactive form (C90S), non-phosphorylatable (S313A) and phosphomimetic (S313D) mutants, deletion mutants lacking potential UAF1 binding sites, a mutant (GG/AA) unable to undergo autocleavage at the well-characterized G670/G671 diglycine motif, and four USP1 mutants identified in tumor samples that cluster around this cleavage site (G667A, L669P, K673T and A676T). Using cell-based assays, we have determined the ability of these mutants to bind UAF1, to reverse DNA damage-induced monoubiquitination of PCNA, and to undergo autocleavage. Results: A non-phosphorylatable S313A mutant of USP1 retained the ability to bind UAF1 and to reverse PCNA ubiquitination in cell-based assays. Regardless of the presence of a phosphomimetic S313D mutation, deletion of USP1 fragment 420-520 disrupted UAF1 binding, as determined using a nuclear relocation assay. The UAF1 binding site in a second UAF1-interacting DUB, USP46, was mapped to a region homologous to USP1(420-520). Regarding USP1 autocleavage, co-expression of the C90S and GG/AA mutants did not result in cleavage, while the cancer-associated mutation L669P was found to reduce cleavage efficiency. Conclusions: USP1 phosphorylation at S313 is not critical for PCNA deubiquitination, neither for binding to UAF1 in a cellular environment. In this context, USP1 amino acid motif 420-520 is necessary and sufficient for UAF1 binding. This motif, and a homologous amino acid segment that mediates USP46 binding to UAF1, map to the Fingers sub-domain of these DUBs. On the other hand, our results support the view that USP1 autocleavage may occur in cis, and can be altered by a cancer-associated mutation.
