IntroductionThe influx and efflux of military personnel in the possible endemic areas of leishmaniasis provided the impetus for research on the sandflies on Marambaia Island.MethodsSandflies were collected with light traps installed in the 3 ecotypes of 3 areas chosen for their particular landscape aspects.ResultsIn 2009, were collected 32,006 specimens of sandflies belonging to 13 species. The species that showed highest density were Nyssomyia intermedia and Migoneimyia migonei.ConclusionsN. intermedia and M. migonei are the principal vectors of American tegumentary leishmaniasis (ATL) in Brazil; thus, extension studies related to the eco-epidemiology of these species of Marambaia Island are essential.

Introduction To determine the faunal composition of immature culicids inhabiting a percolation tank in the landfill of Sapucaia, State of Rio de Janeiro, Brazil, immature mosquitoes were collected over a two-day period during the third weeks of April, August and October 2011. Results The species found were Culex usquatus, Lutzia bigoti, Anopheles argyritarsis and Limatus durhamii. This study is the first to report the colonization of eutrophic breeding sites by these species. Conclusions The oviposition behavior observed in this study suggests a secondary adaptation or change in habit to select eutrophic environments during the developmental stages of the observed species.

Introduction Over the last recent years, the number of Q fever cases have has increased throughout the world. An epidemiological investigation was performed in the area in which the first molecular documentation of Q fever in Brazil was previously reported. Methods Indirect immunofluorescence assay (IFA) and PCR of Coxiella burnetii targeting the htpAB gene were performed in samples from 14 dogs (blood); 1 cat (blood); 10 goats (blood, milk, vaginal swab and anal swab); 3 sheep (blood); and 2 horses (blood). Results Two dogs, two sheep and five goats were seroreactive. DNA was amplified from 6 milk and 2 blood samples from goats and from dogs, respectively. The sequence of the amplicons exhibited 99% sequence similarity with the homologous sequence of the htpAB gene of C. burnetii RSA 331 (GenBank - CP000890). Conclusions The results confirm C. burnetii infection in animals in Rio de Janeiro and reinforce the need for the surveillance of Q fever in Brazil.

Introduction During a diagnostic evaluation of canine visceral leishmaniasis (VL), two of seventeen dogs were found to be co-infected by Leishmania (Viannia) braziliensis and Leishmania (Leishmania) chagasi. Methods Specific polymerase chain reaction (PCR) and restriction fragment length polymorphism-PCR (RFLP-PCR) assays were performed. Results PCR assays for Leishmania subgenus identification followed by RFLP-PCR analysis in biopsies from cutaneous lesions and the spleen confirmed the presence of Leishmania (Viannia) braziliensis and Leishmania (Leishmania) chagasi in those fragments. Conclusions This report reinforces the importance of using serological and molecular techniques in the epidemiological surveillance of canine populations in endemic areas in which both diseases are known to co-exist. In such cases, a reassessment of the control measures is required.

Introduction Sporotrichosis is a mycosis affecting both humans and animals. Within the context of the ongoing sporotrichosis epidemic in the State of Rio de Janeiro, Brazil, sick cats plays an important role in the zoonotic transmission. The aim of this study was to update the number of feline cases diagnosed at the Fundação Oswaldo Cruz (2005-2011). Methods The medical records of the cats followed were reviewed; the inclusion criterion was the isolation of Sporothrix spp. in culture. Results In total, 2,301 feline cases were identified. Conclusions These results should alert sanitary authorities to the difficulties associated with sporotrichosis control.

Introduction Nine cases of visceral leishmaniasis occurred recently in Barra Mansa, State of Rio de Janeiro, with a high mortality rate. Methods We reviewed the medical records of the patients. Results Eight were male; 7 were adults. Patients who died progressed to death quickly and presented with aggravating factors: systemic steroid therapy before diagnosis, bleeding, severe liver involvement, infection, and/or refusal to receive transfusion. Conclusions We warn clinicians to be aware of the emergence of visceral leishmaniasis in new areas and to keep in mind the possibility of atypical clinical pictures and aggravating factors, so timely diagnosis can be made and prompt and adequate treatment can be initiated.

Introduction This study aimed to analyze the relationship between the incidence of severe dengue during the 2008 epidemic in Rio de Janeiro, Brazil, and socioeconomic indicators, as well as indicators of health service availability and previous circulation of the dengue virus serotype-3 (DENV-3). Methods In this ecological study, the units of analysis were the districts of Rio de Janeiro. The data were incorporated into generalized linear models, and the incidence of severe dengue in each district was the outcome variable. Results The districts with more cases of dengue fever in the 2001 epidemic and a higher percentage of residents who declared their skin color or race as black had higher incidence rates of severe dengue in the 2008 epidemic [incidence rate ratio (IRR)= 1.21; 95% confidence interval (95%CI)= 1.05-1.40 and IRR= 1.34; 95%CI= 1.16-1.54, respectively]. In contrast, the districts with Family Health Strategy (FHS) clinics were more likely to have lower incidence rates of severe dengue in the 2008 epidemic (IRR= 0.81; 95%CI= 0.70-0.93). Conclusions At the ecological level, our findings suggest the persistence of health inequalities in this region of Brazil that are possibly due to greater social vulnerability among the self-declared black population. Additionally, the protective effect of FHS clinics may be due to the ease of access to other levels of care in the health system or to a reduced vulnerability to dengue transmission that is afforded by local practices to promote health.

Introduction In Brazil, visceral leishmaniasis (VL) has spread to various regions. This study reports canine cases of VL in Barra Mansa, where human VL cases were recently reported. Methods Using the human index case, a canine survey was performed by dual-path platform immunochromatography and enzyme-linked immunosorbent assay. Seropositive animals were euthanized. Cultures were collected to detect Leishmania parasites. Results Serological tests detected 141 canine VL cases, and Leishmania chagasi were isolated from 82.2% animals. Conclusions Leishmania chagasi is in circulation in Barra Mansa. This study broadens information on the parasite's distribution in the State of Rio de Janeiro.

INTRODUCTION: The presence of American cutaneous leishmaniasis (ACL) in the communities of the Campus FIOCRUZ Mata Atlântica (CFMA) in the City of Rio de Janeiro initiated the investigation of the Phlebotominae fauna in the Atlantic Forest to determine the occurrence of putative ACL vectors associated with the enzootic cycle. METHODS: For 24 consecutive months, sand flies were captured inside the forest and in the border area near the communities. RESULTS: The following sand fly species were identified: Brumptomyia brumpti, Brumptomyia cunhai, Brumptomyia nitzulescui, Lutzomyia edwardsi, Lutzomyia pelloni, and Lutzomyia quinquefer. Other identified sand fly vectors, such as Lutzomyia intermedia (the predominant species), Lutzomyia migonei, Lutzomyia whitmani, Lutzomyia fischeri, and Lutzomyia hirsuta hirsuta, are associated with ACL transmission, and the vector for American visceral leishmaniases (AVL), Lutzomyia longipalpis, was also found. CONCLUSIONS: All sand fly vectors were found in both studied environments except for Lutzomyia whitmani, which was only identified in the forest. This study represents the first identification of Lutzomyia longipalpis in the CFMA, and the epidemiological implications are discussed.

INTRODUCTION: Strongyloides stercoralis is a soil-transmitted helminth that produces an infection that can persist for decades. The relationships between certain clinical conditions and strongyloidiasis remains controversial. This study aims to identify the clinical conditions associated with intestinal strongyloidiasis at a reference center for infectious diseases in Rio de Janeiro, Brazil. METHODS: The clinical conditions that were assessed included HIV/AIDS, HTLV infection, cardiovascular diseases, diabetes, obstructive respiratory diseases, viral hepatitis, tuberculosis, cancer, chronic renal disease, nutritional/metabolic disorders, psychiatric conditions, rheumatic diseases and dermatologic diseases. We compared 167 S. stercoralis-positive and 133 S. stercoralis-negative patients. RESULTS: After controlling for sex (male/female OR = 2.29; 95% (CI): (1.42 - 3.70), rheumatic diseases remained significantly associated with intestinal strongyloidiasis (OR: 4.96; 95% CI: 1.34-18.37) in a multiple logistic regression model. With respect to leukocyte counts, patients with strongyloidiasis presented with significantly higher relative eosinophil (10.32% ± 7.2 vs. 4.23% ± 2.92) and monocyte (8.49% ± 7.25 vs. 5.39% ± 4.31) counts and lower segmented neutrophil (52.85% ± 15.31 vs. 61.32% ± 11.4) and lymphocyte counts (28.11% ± 9.72 vs. 30.90% ± 9.51) than S. stercoralis-negative patients. CONCLUSIONS: Strongyloidiasis should be routinely investigated in hospitalized patients with complex conditions facilitate the treatment of patients who will undergo immunosuppressive therapy. Diagnoses should be determined through the use of appropriate parasitological methods, such as the Baermann-Moraes technique.

OBJETIVO: Identificar a autopercepção da imagem corporal entre estudantes de nutrição no município do Rio de Janeiro. MÉTODOS: Estudo seccional junto a estudantes de nutrição do sexo feminino. Para dimensionar a prevalência de distorção de imagem corporal, utilizou-se o Body Shape Questionaire (BSQ) em sua versão para o português, adotando-se o intervalo de confiança de 95%. Para o estudo das associações, empregou-se o teste qui-quadrado. RESULTADOS: A amostra foi de193 estudantes, tendo como média de idade 20,9 anos (± 2). O escore médio do BSQ foi 81,2 pontos (± 33,6), correspondendo à preocupação leve com a auto-imagem corporal. A prevalência de universitárias com distorção grave da imagem corporal foi de 6,2% (IC 95%; 3,2%-10,6%). Observou-se associação estatisticamente significativa (p < 0,026) entre o BSQ e o índice de massa corporal (IMC) categorizado. Também foi significativa a associação entre BSQ moderado ou grave e insatisfação com o peso (p < 0,001). CONCLUSÃO: Os resultados encontrados evidenciam que o ideal de corpo magro imposto pela sociedade prevalece, pois mulheres com peso adequado apresentaram insatisfação com sua imagem corporal, desejando alterá-la para se adequar aos padrões sociais. Em se tratando de futuras nutricionistas, o impacto desse achado é ainda mais relevante, tendo em vista seu papel no manejo desses quadros.

OBJETIVO: Investigar a existência de diferença na forma de apresentação clínica e evolutiva da cardiomiopatia hipertrófica apical (CMHA), vista em uma amostra na cidade do Rio de Janeiro e a observada no oriente, onde foi descrita. MÉTODOS: Entre 156 pacientes com cardiomiopatia hipertrófica diagnosticados em nossa instituição, foram identificados 13 (8,34%) com a forma apical, sendo estudadas as suas manifestações clínicas, os meios de diagnóstico e a sua evolução. RESULTADOS: Eram 8 homens e 5 mulheres, com idades entre 19 a 75 anos, todos da raça branca, a maioria sintomática. O eletrocardiograma revelou ondas T gigantes em precordiais em 10 pacientes (76,92%), o ecocardiograma (ECO), a presença de hipertrofia apical em todos os casos, sendo em 10 (76,92%) restrita a ponta do ventrículo esquerdo (VE), em dois (15,4%) a ponta do ventrículo direito (VD) e, em 1 (7,68%), acometendo ambos os ventrículos. A cineventriculografia confirmou os achados do ECO. A evolução variou de 6 a 264 meses (x=95,4). Ocorreram dois óbitos (15,4%) em pacientes com o comprometimento associado ao VD, devido ao aumento significativo da dimensão dos átrios, ocorrência de fibrilação atrial, insuficiência valvar mitral e/ou tricúspide e tromboembolismo. Entre os vivos, a paciente com acometimento do VD evolui com restrição diastólica, e os restantes com lesão localizada do VE , 9 estão assintomáticos em uso de propranolol (8) ou amiodarona (1) e um permanece assintomático sem medicação. CONCLUSÃO: presentação, diagnóstico e evolução, porém notamos em 3 pacientes ( 23,08%) o acometimento da ponta do VD, não descrito no oriente, mas com péssimo prognóstico evolutivo.

OBJETIVO: Avaliar retrospectivamente, implante de stent (IS) em paciente com doença arterial coronária (DAC), realizado em 7 hospitais do Rio de Janeiro. MÉTODOS: De junho/94 a dezembro/96, foram realizados 2.220 procedimentos, e desses, analisados 783 IS em 660 (29,7%) pacientes, através da coronariografia sem subtração digital. Os diversos tipos de stent, mais freqüentemente, usados foram: Palmaz-Schatz (40,9%), Gianturco-Roubin (29,1%) e NIR (22,0%). As indicações para o IS foram: lesão de novo, 67,9%; lesão reestenótica, 16,0%; lesão com resultado subótimo da angioplastia transluminal coronária (ATC), 8,2%; oclusão aguda ou provável pós-ATC, 4,9%; oclusão crônica, 3,0%. Todos os stents foram implantados com alta pressão, sem controle pelo ultra-som intracoronário. A prevenção da trombose subaguda, na maioria dos pacientes (87,8%), foi feita com o uso de ticlopidina e ácido acetil-salicílico. RESULTADOS: Evolução imediata: a) sucesso no IS em 770 lesões: 98,0% em 646 (97,9%) pacientes; b) sucesso clínico em 634 (96,0%) casos; c) complicações maiores: infarto agudo do miocárdio (IAM) - 1,1%; cirurgia de revascularização miocárdica (CRM) - 1,4% e óbito - 0,8%; d) complicações vasculares com correção cirúrgica e/ou sangramento - 3,0%. Evolução tardia: a) dos 399 (60,4%) pacientes acompanhados clinicamente, a coronariografia de 121 (30,3%) mostrou retorno da lesão no IS em 79 (19,8%) casos; b) freqüência de outros eventos: IAM - 1,5%; CRM - 2,3%; óbito -1,0% e outra ATC ou procedimento similar - 12,5%. CONCLUSÃO: O estudo multicêntrico mostrou que o IS na DAC pode ser realizado com segurança, alto índice de sucesso imediato, poucas complicações e baixa taxa de eventos cardíacos na evolução tardia.
