Objectives Extending the roles of nurses, pharmacists and allied health professionals to include prescribing has been identified as one way of improving service provision. In the UK, over 50 000 non-medical healthcare professionals are now qualified to prescribe. Implementation of non-medical prescribing ( NMP) is crucial to realise the potential return on investment. The UK Department of Health recommends a NMP lead to be responsible for the implementation of NMP within organisations. The aim of this study was to explore the role of NMP leads in organisations across one Strategic Health Authority (SHA) and to inform future planning with regards to the criteria for those adopting this role, the scope of the role and factors enabling the successful execution of the role. Methods Thirty-nine NMP leads across one SHA were approached. Semi-structured telephone interviews were conducted. Issues explored included the perceived role of the NMP lead, safety and clinical governance procedures and facilitators to the role. Transcribed audiotapes were coded and analysed using thematic analytical techniques. Key findings In total, 27/39 (69.2%) NMP leads were interviewed. The findings highlight the key role that the NMP lead plays with regards to the support and development of NMP within National Health Service trusts. Processes used to appoint NMP leads lacked clarity and varied between trusts. Only two NMP leads had designated or protected time for their role. Strategic influence, operational management and clinical governance were identified as key functions. Factors that supported the role included organisational support, level of influence and dedicated time. Conclusion The NMP lead plays a significant role in the development and implementation of NMP. Clear national guidance is needed with regards to the functions of this role, the necessary attributes for individuals recruited into this post and the time that should be designated to it. This is important as prescribing is extended to include other groups of non-medical healthcare professionals.

The efficacy of breast-conserving Surgery for the local control of early breast cancer has been repeatedly evidenced. Although immediate reconstruction following breast-conserving Surgery has been described, little information is available regarding surgical management in reoperative settings due to positive margins. We studied the influence of intraoperatively assessed and postoperatively controlled Surgical margin status on the type of breast-conserving Surgery and report our results regarding complications in a reoperative breast reconstruction scenario. All patients were seen by a multidisciplinary team who recommended breast-conserving surgery. According to the breast volume, ptosis and tumor size/location, the patients were also evaluated by a plastic Surgeon, who recommended reconstruction with the appropriate technique. Intraoperative assessment Of Surgical margins was determined by histological examination of frozen sections. The mean follow-up time was 48 months. Two hundred and eighteen patients (88.5%) underwent breast-conserving Surgery and immediate reconstruction. Twelve (5.5%) patients had a positive tumor margin after review of the permanent section. All patients underwent re-exploration. In 1.3%, a second reconstructive technique was indicated and in 2.2% a skin-sparing mastectomy with total reconstruction was performed. Our findings support the important role of the intraoperative assessment of surgical margins and its interference in the selection of reconstruction techniques and negative margins: however, it will not guarantee complete excision of the tumor. Promoting the inclusion of students with disabilities in e-learning systems has brought many challenges for researchers and educators. The use of synchronous communication tools such as interactive whiteboards has been regarded as an obstacle for inclusive education. In this paper, we present the proposal of an inclusive approach to provide blind students with the possibility to participate in live learning sessions with whiteboard software. The approach is based on the provision of accessible textual descriptions by a live mediator. With the accessible descriptions, students are able to navigate through the elements and explore the content of the class using screen readers. The method used for this study consisted of the implementation of a software prototype within a virtual learning environment and a case study with the participation of a blind student in a live distance class. The results from the case study have shown that this approach can be very effective, and may be a starting point to provide blind students with resources they had previously been deprived from. The proof of concept implemented has shown that many further possibilities may be explored to enhance the interaction of blind users with educational content in whiteboards, and further pedagogical approaches can be investigated from this proposal. Managing software maintenance is rarely a precise task due to uncertainties concerned with resources and services descriptions. Even when a well-established maintenance process is followed, the risk of delaying tasks remains if the new services are not precisely described or when resources change during process execution. Also, the delay of a task at an early process stage may represent a different delay at the end of the process, depending on complexity or services reliability requirements. This paper presents a knowledge-based representation (Bayesian Networks) for maintenance project delays based on specialists experience and a corresponding tool to help in managing software maintenance projects. BACKGROUND: National quality registries (NQRs) purportedly facilitate quality improvement, while neither the extent nor the mechanisms of such a relationship are fully known. The aim of this case study is to describe the experiences of local stakeholders to determine those elements that facilitate and hinder clinical quality improvement in relation to participation in a well-known and established NQR on stroke in Sweden. METHODS: A strategic sample was drawn of 8 hospitals in 4 county councils, representing a variety of settings and outcomes according to the NQR's criteria. Semi-structured telephone interviews were conducted with 25 managers, physicians in charge of the Riks-Stroke, and registered nurses registering local data at the hospitals. Interviews, including aspects of barriers and facilitators within the NQR and the local context, were analysed with content analysis. RESULTS: An NQR can provide vital aspects for facilitating evidence-based practice, for example, local data drawn from national guidelines which can be used for comparisons over time within the organisation or with other hospitals. Major effort is required to ensure that data entries are accurate and valid, and thus the trustworthiness of local data output competes with resources needed for everyday clinical stroke care and quality improvement initiatives. Local stakeholders with knowledge of and interest in both the medical area (in this case stroke) and quality improvement can apply the NQR data to effectively initiate, carry out, and evaluate quality improvement, if supported by managers and co-workers, a common stroke care process and an operational management system that embraces and engages with the NQR data. CONCLUSION: While quality registries are assumed to support adherence to evidence-based guidelines around the world, this study proposes that a NQR can facilitate improvement of care but neither the registry itself nor the reporting of data initiates quality improvement. Rather, the local and general evidence provided by the NQR must be considered relevant and must be applied in the local context. Further, the quality improvement process needs to be facilitated by stakeholders collaborating within and outside the context, who know how to initiate, perform, and evaluate quality improvement, and who have the resources to do so.

A competitividade das empresas apresenta forte ligação com a qualidade do relacionamento com o cliente. Para competir em tempo real é preciso dotar a empresa de meios que permitam conhecer e relacionar-se de forma cada vez mais eficiente com os clientes. O uso intensivo da tecnologia é fator que contribui para que o relacionamento com o cliente se torne mais efetivo. CRM (Customer Relationship Management) faz uso da tecnologia de informação com o objetivo de desenvolver e gerenciar relações entre empresas e seus clientes, de forma individualizada e mutuamente benéfica. O presente trabalho refere-se à implementação de um sistema CRM, abrangendo as etapas de seleção da solução de CRM, definição do organograma, cronograma e metodologia de trabalho, redesenho dos processos de atendimento ao cliente e configuração do software. Também é objeto deste trabalho a verificação do nível de satisfação do cliente resultante. O estudo foi desenvolvido junto a uma empresa do setor petroquímico, caso pioneiro de implantação de CRM no país, considerando o segmento de mercado em análise. O texto discute a relação existente entre um sistema de CRM e a satisfação do cliente, apresentando os resultados de uma pesquisa de satisfação aplicada aos clientes da empresa em estudo.

Este trabalho apresenta a implantação do processo de Planejamento Estratégico e o conseqüente sistema de medição com sugestão de inclusão de novos indicadores críticos ao sucesso da gestão em Empresa da Indústria Têxtil. Primeiramente foi elaborado o Planejamento Estratégico da Empresa em questão, pois é indispensável à análise de competitividade das organizações bem como necessário à elaboração de seus Sistemas de Medidas de Desempenho, decorrentes das estratégias estabelecidas. Em seguida, foi revisto o Sistema de Medidas de Desempenho utilizado, que tem o objetivo de estimular as primeiras ações de ajuste do planejado com a situação real. Por último o Sistema de Medidas de Desempenho existente foi comparado ao proposto no Balanced Scorecard para a adoção de ações de ajuste do sistema atual, contemplando a introdução de novos indicadores. Ficaram evidentes os benefícios decorrentes do processo de Planejamento Estratégico com a definição de objetivos e a formulação de estratégias para alcançá-los, desdobradas em todos os níveis da organização – requerendo um Sistema adequado de Medidas de Desempenho para o monitoramento da implementação das ações resultantes.

Este trabalho apresenta um estudo sobre as políticas de educação escolar e saúde indígena no Brasil, buscando verificar a presença das tendências de modificação observadas nas políticas sociais do país, a partir das décadas de 1980 e 1990: descentralização das políticas para as esferas subnacionais de governo; criação de mecanismos de participação da sociedade civil nos processos decisórios; estabelecimento de parcerias com instituições privadas para a provisão de serviços públicos; institucionalização de canais de controle; alteração no conteúdo das políticas e ampliação de seu alcance. O estudo foi realizado a partir de uma análise comparativa das duas políticas no nível federal e no nível local de governo. A análise no nível local foi realizada a partir do estudo de caso do Parque Indígena do Xingu. Buscou-se verificar, na trajetória das políticas, a influência dos fatores relacionados aos processos de Redemocratização e Reforma do Estado, das dinâmicas próprias das áreas de saúde e educação, da questão indígena e da agenda estatal indigenista. A partir da análise, verificamos, no nível federal, um avanço na legislação de ambos os campos, comparado com os princípios existentes antes da Constituição Federal de 1988. No que se refere à legislação, a educação escolar indígena apresenta-se mais consolidada quando comparada à saúde que ainda apresenta muitas indefinições. No Parque Indígena do Xingu percebemos uma inflexão nas duas políticas, a partir da década de 1990, que passam a buscar um crescente protagonismo indígena e a valorização de uma abordagem intercultural. No Xingu estes avanços foram resultado, sobretudo, da iniciativa das comunidades indígenas da região, em parceria com a Universidade Federal de São Paulo, na área da saúde, e com o Instituto Socioambiental, na área da educação, e com recursos, a principio, de fundações internacionais.

O planejamento provoca um fascínio e deslumbramento no homem desde os primórdios do seu aparecimento, quando procura antever o futuro, que se lhe apresenta desconhecido e misterioso. Uma das características do ser humano é fazer planos, sendo essa atividade uma atitude inerente ao homem e também às sociedades. A literatura brasileira, disponível sobre o tema, apresenta-se de uma certa forma dicotômica, quase sempre sem uma unidade orgânica, ora produzindo parcos estudos teóricos, ora fornecendo análises de experiências específicas de países, regiões e estados em períodos de tempo determinados. A dissertação de mestrado para a Escola Brasileira de Administração Pública - EBAP Fundaçâo Getúlio Vargas - FGV,Planejamento Governamental. Aspectos teóricos e uma análise das experiências mundial, brasileira e cearence, consolida num só estudo os aspectos teóricos e práticos dentro de um encadeamento lógico,didático com o objetivo de revisitar/esclarecer o tema e servir de mate rial de consulta/referência para cursos de graduação e pós-graduação da disciplina Planejamento Governamental. Para uma melhor compreensão das experiências de planejamento, apresenta inicialmente os aspectos teóricos, conceituando o tema e relacionando suas funções ~ características. O trabalho mostra o inter-relacionamento do, planejamento com o poder e a política, incluindo a discussão do mito da neutralidade e racionalidade, assim como' sua utilização como instrumento nas mãos dos que detêm o poder, Demonstra também o caráter do plnejamento como processo com definições e a descrição de três modelos teóricos - Bromley, Tristão e Hilhorst. Apresenta o dilema liberdade do setor privado versus intervenção governamental, desde as primeiras manifestações de planejamento na Rússia, após a revolução de outubro de 1917 passando pelas experiências da Europa e Estados Unidos da América, até a dos países subdesenvolvidos, cada qual com suas condições históricas e institucionais peculiares. Faz uma análise da experiências mundial e brasileira (dividida em fases: ocasional - até 1930, empírica 1930-1964 e científica 1964-1986), sendo que para esta última adota uma abordagem síntese baseada nos pontos de vista de diversos autores examinados, principalmente Benedicto Silva, Werner Baer, Issac Kerstenetzky, Anniba1 V. Villela, Jorge Vianna Monteiro e Luiz Roberto Aze vedo Cunha. No caso da experiência cearense ( 1963-1986 ), a descrição factual e linear dos planos tem uma seqUência temporal (a exemplo dos planos brasileiros), forma meramente didática escolhi da e visa uma melhor compreensão do assunto. Paralelamente, analisa os planos em seus aspectos econômico, social, político e cultural de forma integrada, numa tentativa de dimensionar a história total do planejamento estadual. O enfoque tradicional de comparação objetivos propostos/objetivos alcançados, muito comum nas análises das experiências de planejamento, é deixado de lado stricto-senso. Em contrapartida, dá ênfase a um aspecto que julga mais relevante: a identificação da evolução dos arranjos organizacionais, visto ser o planejamento algo que neles se traduz e cuja exequibilidade depende, em alto grau, do contexto institucional. Isentando o conceito do planejamento de culpa pelo seu fracasso no atingimento da maioria de seus objetivos, o trabalho aponta uma série de fatores particularmente desfavoráveis que configura uma situação de crise: Objetivos apologéticos e triunfalistas; retórica inócua; uso do planejamento no sentido de fortalecedor e viabilizador dos interesses da classe dominante; exarcebação da técnica e tese da neutralidade do planejamento para efeito de obtenção de poder; manipulação do planejamento como instrumento de mistificação, assim como seu uso como fetiche e servidor de "trampolim" para a política (principalmente no Ceará); a eliminação do caráter federativo da União Republicana, provocando a perda de autonomia dos estados com o sistema tributário, conduzindo a uma centralização das decisões ( e do poder )'; ausência de continuidade nas políticas econômicas de longo prazo tendo como corolário a descontinuidade administrativa e, por último, a própria natureza do planejamento posto em prática no Brasil e no Ceará que, sendo indicativo, não detém o controle de todas as variáveis do processo. O aparecimento de um novo paradigma para o planejamento governamental passaria, necessariamente, pela antítese dos entraves apontados no estudo.

O presente projeto de dissertação apresenta uma metodologia para avaliar a relação de causa e efeito da forma de gestão adotada pelos Conselhos Municipais de Defesa dos Direitos da Criança e do Adolescente, em especial no que se refere à utilização de ferramentas de monitoria e avaliação para acompanhar os projetos executados, no aumento do orçamento público captado através de doações aos Fundos para a Infância e Adolescência – FIA. Este projeto busca identificar a existência de possíveis gargalos na gestão dos Conselhos, principalmente no que diz respeito aos processos de monitoria dos projetos desenvolvidos com recursos advindos de doações ao Fundo Municipal para a Infância e Adolescência como uma oportunidade de avançar no processo de melhoria da gestão pública, neste caso, de garantia de direitos infanto-juvenil. Para tanto, será necessário criar grupos de tratamento e controle considerando os Conselhos Municipais de Garantia de Direitos da Criança e do Adolescente que utilizam ferramentas de monitoria e avaliação daqueles que não adotam essa prática no seu cotidiano profissional. A identificação de tais Conselhos será feita a partir de três filtros correlacionados a saber, tamanho da população do município, índice de desenvolvimento humano municipal e cadastro em situação regular do Fundo Municipal para a Infância e a Adolescência. Não se trata de um trabalho que visa apontar erros na gestão dos Conselhos, ao contrário, uma proposta de mapeamento capaz de verificar fragilidades e propor uma linha de atuação alternativa - capaz de romper com práticas meramente paternalistas ou formalismos que produzam disfunções burocráticas - que gere transformação no espaço de atuação.

The coatings mortars are essential elements of building structures because they execute an important role in protecting walls and are particularly exposed to aggressive action responsible for its degradation over time. The importance of wall coverings has been the subject of discussion and analysis in the conservation and rehabilitation of old buildings. Are sometimes removed and replaced with inappropriate solutions of constructive point of view or architecture. The most commonly used coatings on walls of old buildings is based on traditional hydraulic lime mortars. The present study aims at the formulation of new lime- based mortars and aerial fine aggregate, in order to contribute to a better field of conservation and restoration mortar coating of old buildings. Residue was used for polishing porcelain as fine aggregate, replacing the aggregate (sand), in percentages 05-30% by mass. We conducted a thorough evaluation of the mortar properties in fresh and hardened state by comparing the performance of the same with a reference mortar. The residue used was characterized as the density, bulk density, and particle size laser, scanning electron microscopy, X-ray diffraction and X-ray fluorescence. Formulations were produced 7, 6 with residue and one commonly used formulation, which served as a reference. In the formulations of lime mortars air (hydrated lime powder CH-I) has been adopted a stroke volume (1:3) with constant binder, was varied and the water / binder and aggregate and waste. For evaluation of mortars fresh, proceeded to consistency analysis, specific gravity, water retention and air content embedded. In the hardened state assays were performed in specific gravity, water retention, modulus of elasticity, tensile strength in bending, compressive strength, water absorption by capillary action, adhesion, tensile strength, resistance to shrinkage and salts by of crystallization trials with resources chloride solution, nitrate and sulfate all sodium in prismatic at 90 days of age, in addition to the micro structural analysis of mortars. Based on the results we can see that the mortar formulated with 10% content of waste and the reference free retraction feature more stable closer to neutrality. The composition of 10% was obtained better performance against the action of the salt crystallization. The mortar with 15% residue obtained better density, lower air content embedded and high capacity for water retention developing good workability. The replacement of 20% of waste generates a satisfactory utilization of resistance to compression, flexion and traction grip the base. And, finally, it can be seen that the mortar with 10, 15 and 20% residual show, in principle, good suitability as coatings, thus enabling a final result consistent with durability, workability and aesthetics developing therefore a material with better performance to repair or replace existing mortars in old buildings

The state of Rio Grande do Norte presents a great potentiality for the production of ceramic tiles because of having natural raw material in quantity and quality making its economical exploration possible, beyond the great energetic differential of the state, the natural gás. This works aims to study the influence of the dolomite and granulometry concentration and calcinations temperature in the obtaining of formulations for porous coverings which have to be coherent to the project,s specifications. The experiments have involved the physical-chemical and mineralogical characterizations of raw materials and mechanical tests in the dry and burnt proof bodies preceding a mixture experiment planning with the use of the response surface methodology, in order to get the best raw materials combinations to produce a ceramic mass with specific properties. The twelve ceramic masses studied in this work were prepared by the via dry process, characterized, shaped by uniaxial pressing and sinterized in the temperatures of 940ºC, 1000ºC, 1060ºC, 1120ºC and 1180ºC, using a fast burning cycle. The crystalline phases formed during the sintering in the temperatures in study have revealed the presence of anorthite and diopside beyond quartz with a remaining phase. These phases were the main responsible ones by the physical- mechanical properties of the sinterized proof bodies. The proof bodies after the sintering stage have presented water absorption higher than 10% and a good dimensional stability in all studied temperatures. However, the flexural breaking strength results in the temperatures of 940ºC, 1000ºC and 1060ºC, under the temperature zone of the vitrification of ceramic whiteware do not reach the flexural breaking strength specific for the porous wall tile (15 MPa), but in the temperature of 1120ºC next to the vitrification temperature zone, some whiteware ceramic (formulations) has reached the specified value for the porous wall tile. The results of this work have showed that the studied raw materials have great importance for used in the production of porous wall tiles (BIII)

This dissertation focus, as main objective, to address the issue of fundamental rights and political freedoms of the individuals, guaranteed by the Constitution of 1988, with emphasis of study in the constitutional guarantee of freedom of speech, as well as in national related constitutional law institutions and its derivatives, and the connection with the historical and political affirmation of fundamental human rights and its importance for the construction, maintenance and consolidation of constitutional democracy in the Federative Republic of Brazil. This paper mainly deals with aspects of juspostive nature, focused mainly within its doctrinal aspect, making, for such, references both to the patrian doctrine and the foreign one, without forgetting the necessary jurisprudencial focus and analysis of the positive patrian planning with references to comparative law, in order to describe and analyze the emergence, evolution and dissemination of the institute, both in the major countries of the Western World and along the Brazilian constitutional history.

This work aims at developing an evaluation of the implementation of the Program Escola Ativa as a public policy for rural schools with combined grade classrooms in Jardim do Seridó RN (1998 2009) focusing on the following dimensions: school s physical environment, training, follow up, and didactic usage of the methodology. In order to develop this research we refer to the literature that analyzes the cycle of policies (FREY, 2000). In this approach, evaluation represents an important step in the process of analyzing the implementation of public policies, as a way of measuring up their performance, as well as a guide for realignments and redefinitions (PRESSMAN; WILDAVSKY, 1998). In order to accomplish this function, the evaluator of policies must be acquainted with scientific concepts and methods that consist of describing, interpreting and analyzing the policies in the governmental sphere (MENY; THOENIG, 1992; LIMA JÚNIOR, 1978). In this perspective, we intend to investigate whether in its proposition of minimizing the blanks in the Brazilian educational system, the implementation of the Program would be contributing to the improvement of the political-pedagogical practices in the rural schools with combined grade classrooms in Jardim do Seridó RN. In order to do this research, we have developed a theoretical-methodological matrix made of analysis dimensions, variables, indicators and instruments, such as literary revision, documental analysis, semi-structured interviews with four teachers and three supervisors that work and/or have worked in Escola Ativa in that town in the period comprised among 1998 2009, besides notes taken from field observation and photographs from four rural schools with combined grade classrooms. With this research we have identified that the Program, at a national level, has gone through different phases in its implementation process, for the town was not ready to fully take the responsibilities of the autonomous expansion, in 2002. From that period on, the execution of Escola Ativa has suffered several discontinuities, such as the lack of professional training and supervising. It is also noted that the methodology contributes to the dynamization of the didactic-pedagogical activities and promotes the cooperation and autonomy of the students in the organization and the applicability of the components of the curriculum, especially of Governo Estudantil and Cantinhos de Aprendizagem. Although the directions of the Program (BRASIL, 2005) point out that Escola Ativa has among its principles social transformation, we identified that, isolated, the initiative is not capable of promoting the changes that the rural schools need, namely investments in the physical, material, pedagogical and technological infra-structure, besides the estimation and a career plan for the teachers. In a general draft of the results of this research, we realized that some aspects presented about the peculiarities of Escola Ativa in Jardim do Seridó as a governmental Program, reinforce the need for the public policies to be evaluated, in order to confront critic and operationally the planning with the practice, revising action, whenever necessary
