Collateral circulation, defined as the supplementary vascular network that maintains cerebral blood flow (CBF) when the main vessels fail, constitutes one important defense mechanism of the brain against ischemic stroke. In the present study, continuous arterial spin labeling (CASL) was used to quantify CBF and obtain perfusion territory maps of the major cerebral arteries in spontaneously hypertensive rats (SHRs) and their normotensive Wistar-Kyoto (WKY) controls. Results show that both WKY and SHR have complementary, yet significantly asymmetric perfusion territories. Right or left dominances were observed in territories of the anterior (ACA), middle and posterior cerebral arteries, and the thalamic artery. Magnetic resonance angiography showed that some of the asymmetries were correlated with variations of the ACA. The leptomeningeal circulation perfusing the outer layers of the cortex was observed as well. Significant and permanent changes in perfusion territories were obtained after temporary occlusion of the right middle cerebral artery in both SHR and WKY, regardless of their particular dominance. However, animals with right dominance presented a larger volume change of the left perfusion territory (23 +/- 9%) than animals with left dominance (7 +/- 5%, P<0.002). The data suggest that animals with contralesional dominance primarily safeguard local CBF values with small changes in contralesional perfusion territory, while animals with ipsilesional dominance show a reversal of dominance and a substantial increase in contralesional perfusion territory. These findings show the usefulness of CASL to probe the collateral circulation.

OBJECTIVE: Experimental studies on lung preservation have always been performed using animal models. We present ex vivo lung perfusion as a new model for the study of lung preservation. Using human lungs instead of animal models may bring the results of experimental studies closer to what could be expected in clinical practice. METHOD: Brain-dead donors whose lungs had been declined by transplantation teams were used. The cases were randomized into two groups. In Group 1, Perfadex (R) was used for pulmonary preservation, and in Group 2, LPDnac, a solution manufactured in Brazil, was used. An ex vivo lung perfusion system was used, and the lungs were ventilated and perfused after 10 hours of cold ischemia. The extent of ischemic-reperfusion injury was measured using functional and histological parameters. RESULTS: After reperfusion, the mean oxygenation capacity was 405.3 mmHg in Group 1 and 406.0 mmHg in Group 2 (p=0.98). The mean pulmonary vascular resistance values were 697.6 and 378.3 dyn.s.cm(-5), respectively (p=0.035). The mean pulmonary compliance was 46.8 cm H2O in Group 1 and 49.3 ml/cm H2O in Group 2 (p=0.816). The mean wet/dry weight ratios were 2.06 and 2.02, respectively (p=0.87). The mean Lung Injury Scores for the biopsy performed after reperfusion were 4.37 and 4.37 in Groups 1 and 2, respectively (p=1.0), and the apoptotic cell counts were 118.75/mm(2) and 137.50/mm(2), respectively (p=0.71). CONCLUSION: The locally produced preservation solution proved to be as good as Perfadex (R). The clinical use of LPDnac may reduce costs in our centers. Therefore, it is important to develop new models to study lung preservation.

Distances walked in walking tests are important functional markers, although they are not accepted as defining characteristics of Ineffective Peripheral Tissue Perfusion. The aims of this study were to verify the distances participants with and without this nursing diagnosis walked in the six-minute walk test and if these measures may be considered defining characteristics of this phenomenon. Participants with (group A; n=65) and without (group B; n=17) this nursing diagnosis were evaluated regarding physical examination, vascular function and functional capacity. Participants of group A seemed to have worse vascular function and functional capacity compared with those of group B. Pain-free travelled distance was predictive of the nursing diagnosis. These results are important for the refinement of this diagnosis. In conclusion, this study provides evidences that the distances walked in the six-minute walk test may be considered defining characteristics of Ineffective Peripheral Tissue Perfusion.

Objective To evaluate the changes in tissue perfusion parameters in dogs with severe sepsis/septic shock in response to goal-directed hemodynamic optimization in the ICU and their relation to outcome. Design Prospective observational study. Setting ICU of a veterinary university medical center. Animals Thirty dogs with severe sepsis or septic shock caused by pyometra who underwent surgery and were admitted to the ICU. Measurements and Main Results Severe sepsis was defined as the presence of sepsis and sepsis-induced dysfunction of one or more organs. Septic shock was defined as the presence of severe sepsis plus hypotension not reversed with fluid resuscitation. After the presumptive diagnosis of sepsis secondary to pyometra, blood samples were collected and clinical findings were recorded. Volume resuscitation with 0.9% saline solution and antimicrobial therapy were initiated. Following abdominal ultrasonography and confirmation of increased uterine volume, dogs underwent corrective surgery. After surgery, the animals were admitted to the ICU, where resuscitation was guided by the clinical parameters, central venous oxygen saturation (ScvO2), lactate, and base deficit. Between survivors and nonsurvivors it was observed that the ScvO2, lactate, and base deficit on ICU admission were each related independently to death (P = 0.001, P = 0.030, and P < 0.001, respectively). ScvO2 and base deficit were found to be the best discriminators between survivors and nonsurvivors as assessed via receiver operator characteristic curve analysis. Conclusion Our study suggests that ScvO2 and base deficit are useful in predicting the prognosis of dogs with severe sepsis and septic shock; animals with a higher ScvO2 and lower base deficit at admission to the ICU have a lower probability of death.

The pathophysiology of neurodegenerative diseases (ND) such as Alzheimer's disease (AD) and Parkinson's disease (PD) has not yet been completely elucidated. However, in the past few years, there have been great knowledge advances about intra-and extracellular proteins that may display impaired function or expression in AD, PD and other ND, such as amyloid beta (AB), alpha-synuclein, tau protein and neuroinfiammatory markers. Recent developments in the imaging techniques of positron emission tomography (PET) and single photon emission computed tomography (SPECT) now allow the non-invasive tracking of such molecular targets of known relevance to ND in vivo. This article summarizes recent findings of PET and SPECT studies using these novel methods, and discusses their potential role in the field of drug development for ND as well as future clinical applications in regard to differential diagnosis of ND and monitoring of disease progression.

Objective To evaluate the effects of intravenous regional limb perfusion (IRLP) administration of amphotericin B in horses to treat pythiosis after surgical excision and thermocautery. Study Design Case series. Animals Horses (n = 12) with Pythium insidiosum infection of the distal aspect of the thoracic or pelvic limbs. Methods After surgical excision of granulation tissue and thermocautery, 50 mg amphotericin B was administered by IRLP through a catheter placed in a superficial vein of the affected limb next to the lesion after placing a tourniquet above the injection site. The lesions and locomotor system were evaluated before treatment and at 7, 14, 21, 28, 35, and 60 days. Results Ninety-two percent of horses treated with amphotericin B had complete lesion resolution 35 or 60 days after 1 or 2 IRLP treatments, respectively. IRLP induced limb edema and pain during regional palpation in 42%, and inflammation of the injection site in 33% of horses; however these signs resolved after 14 days. Conclusions IRLP administration of amphotericin B was effective for treating pythiosis in equine limbs, resolving infection with manageable side effects.

Abstract Introduction We conducted the present study to investigate whether early large-volume crystalloid infusion can restore gut mucosal blood flow and mesenteric oxygen metabolism in severe sepsis. Methods Anesthetized and mechanically ventilated male mongrel dogs were challenged with intravenous injection of live Escherichia coli (6 × 109 colony-forming units/ml per kg over 15 min). After 90 min they were randomly assigned to one of two groups – control (no fluids; n = 13) or lactated Ringer's solution (32 ml/kg per hour; n = 14) – and followed for 60 min. Cardiac index, mesenteric blood flow, mean arterial pressure, systemic and mesenteric oxygen-derived variables, blood lactate and gastric carbon dioxide tension (PCO2; by gas tonometry) were assessed throughout the study. Results E. coli infusion significantly decreased arterial pressure, cardiac index, mesenteric blood flow, and systemic and mesenteric oxygen delivery, and increased arterial and portal lactate, intramucosal PCO2, PCO2 gap (the difference between gastric mucosal and arterial PCO2), and systemic and mesenteric oxygen extraction ratio in both groups. The Ringer's solution group had significantly higher cardiac index and systemic oxygen delivery, and lower oxygen extraction ratio and PCO2 gap at 165 min as compared with control animals. However, infusion of lactated Ringer's solution was unable to restore the PCO2 gap. There were no significant differences between groups in mesenteric oxygen delivery, oxygen extraction ratio, or portal lactate at the end of study. Conclusion Significant disturbances occur in the systemic and mesenteric beds during bacteremic severe sepsis. Although large-volume infusion of lactated Ringer's solution restored systemic hemodynamic parameters, it was unable to correct gut mucosal PCO2 gap.

Abstract Due to the ongoing efforts in transplanting b-cell mass there is also a great medical interest in specific b-cell imaging agents to quantify the acceptance of transplanted islets in humans in vivo. Additionally, in the context of type 1 diabetes mellitus the chronic and progressive loss of b-cells caused by autoimmune destruction has led to concerted efforts to prevent further loss of b-cells by autoantigen-specific immunotherapy of pre-diabetic patients. nateglinide and glibenclamide are SUR1 ligands used to stimulate insulin secretion in type 2 diabetic patients. They bind to a class of molecules known as the ATP-sensitive potassium channels, located on the insulin producing b-cells of the islets of Langerhans and are therefore excellent candidates as b-cell specific tracers. To obtain a precursor for a direct labelling of nateglinide with [18F]fluoride, the aromatic system of the phenylalanine structure element was derivatised to obtain a phenolic OH-group in 4-position which is capable of further derivatisation. The formed phenylether N-(trans-4-isopropylcyclohexanecarbonyl)-O-(2-hydroxyethyl)-D-tyrosin benzylester was tried to be tosylated according to several literature procedures but none of them was applicable. The catalytic influence of ytterbium(III)triflate in the reaction of toluenesulfonic acid anhydride and the alcohol was investigated. It was found that Yb(III) facilitates the tosylation of the alcohol under non-basic conditions and was extended to the tosylation of a great variety of different alcohols to prove its applicability in general. The radioactive labelling of N-(trans-4-isopropyl-cyclohexanecarbonyl)-O-(2-[18F]fluoroethyl)-D-tyrosine with [18F]F-/ Kryptofix® 222/ K2CO3-system was achieved in radiochemical yields (RCY) of 10 % after deprotection with Pd/ C and H2. In addition to the direct labelling approach, a labelling procedure applying 2[18F]fluoroethyltosylate and N-(trans-4-isopropyl-cyclohexanecarbonyl)-D-tyrosin was performed in 40 % RCY. Unfortunately the determination of the KD value of N-(trans-4-isopropylcyclohexanecarbonyl)-O-(2-fluoroethyl)-D-tyrosine revealed a significant decrease in affinity compared to original nateglinide. The in vivo evaluation of some 18F-labelled glibenclamide derivatives in humans and animals revealed that longer measuring times are warranted because a high liver uptake spoiles the data acquisition and the activity washout proceeds very slowly. Therefore glibenclamide was labelled with a radioisotope with a longer half life such as 99mTc (t1/2 = 6 h) to lengthen the possible time frame for image acquisition. The synthesis of a 99mTc labelled hydrophilic glibenclamide derivative was performed. It is hoped that gliben-clamide is internalised into the b-cell and there binds to the 95 % of intracellular SUR-1 receptors with eventual metablolisation and thus trapping in the cell. The KD-value of the corresponding Re-compound was determined to be 0.5 nM and the insulin secretion properties were similar to those of original glibenclamide. The labelling precursor N-{4-[N,N-bis-(carboxy-methyl)-aminoethyl)-5-chlorobenzene-carboxamido]-ethyl}-benzene-sulfonyl-N'-cyclohexyl urea tris sodium salt was reacted with [99mTc(I)(OH2)3(CO)3] Cl to yield the final N-{4-[99mTc(I)-tricarbonyl-N,N-bis-(carboxymethyl)-aminoethyl)-5-chloro-benzene-carboxamidoethyl]-benzene-sulfonyl}-N'-cyclo-hexyl-urea sodium salt in 70% RCY.

Myocardial perfusion quantification by means of Contrast-Enhanced Cardiac Magnetic Resonance images relies on time consuming frame-by-frame manual tracing of regions of interest. In this Thesis, a novel automated technique for myocardial segmentation and non-rigid registration as a basis for perfusion quantification is presented. The proposed technique is based on three steps: reference frame selection, myocardial segmentation and non-rigid registration. In the first step, the reference frame in which both endo- and epicardial segmentation will be performed is chosen. Endocardial segmentation is achieved by means of a statistical region-based level-set technique followed by a curvature-based regularization motion. Epicardial segmentation is achieved by means of an edge-based level-set technique followed again by a regularization motion. To take into account the changes in position, size and shape of myocardium throughout the sequence due to out of plane respiratory motion, a non-rigid registration algorithm is required. The proposed non-rigid registration scheme consists in a novel multiscale extension of the normalized cross-correlation algorithm in combination with level-set methods. The myocardium is then divided into standard segments. Contrast enhancement curves are computed measuring the mean pixel intensity of each segment over time, and perfusion indices are extracted from each curve. The overall approach has been tested on synthetic and real datasets. For validation purposes, the sequences have been manually traced by an experienced interpreter, and contrast enhancement curves as well as perfusion indices have been computed. Comparisons between automatically extracted and manually obtained contours and enhancement curves showed high inter-technique agreement. Comparisons of perfusion indices computed using both approaches against quantitative coronary angiography and visual interpretation demonstrated that the two technique have similar diagnostic accuracy. In conclusion, the proposed technique allows fast, automated and accurate measurement of intra-myocardial contrast dynamics, and may thus address the strong clinical need for quantitative evaluation of myocardial perfusion.

Perfusion CT imaging of the liver has potential to improve evaluation of tumour angiogenesis. Quantitative parameters can be obtained applying mathematical models to Time Attenuation Curve (TAC). However, there are still some difficulties for an accurate quantification of perfusion parameters due, for example, to algorithms employed, to mathematical model, to patient’s weight and cardiac output and to the acquisition system. In this thesis, new parameters and alternative methodologies about liver perfusion CT are presented in order to investigate the cause of variability of this technique. Firstly analysis were made to assess the variability related to the mathematical model used to compute arterial Blood Flow (BFa) values. Results were obtained implementing algorithms based on “ maximum slope method” and “Dual input one compartment model” . Statistical analysis on simulated data demonstrated that the two methods are not interchangeable. Anyway slope method is always applicable in clinical context. Then variability related to TAC processing in the application of slope method is analyzed. Results compared with manual selection allow to identify the best automatic algorithm to compute BFa. The consistency of a Standardized Perfusion Index (SPV) was evaluated and a simplified calibration procedure was proposed. At the end the quantitative value of perfusion map was analyzed. ROI approach and map approach provide related values of BFa and this means that pixel by pixel algorithm give reliable quantitative results. Also in pixel by pixel approach slope method give better results. In conclusion the development of new automatic algorithms for a consistent computation of BFa and the analysis and definition of simplified technique to compute SPV parameter, represent an improvement in the field of liver perfusion CT analysis.

Questa tesi si propone di innovare lo stato dell’arte dei metodi di analisi dell’eterogeneità in lesioni polmonari attualmente utilizzati, affiancando l’analisi funzionale (emodinamica) a quella morfologica, grazie allo sviluppo di nuove feature specifiche. Grazie alla collaborazione tra il Computer Vision Group (CVG) dell’Università di Bologna e l’Unità Operativa di Radiologia dell’IRCCS-IRST di Meldola (Istituto di Ricovero e Cura a Carattere Scientifico – Istituto Scientifico Romagnolo per lo Studio e la Cura dei Tumori), è stato possibile analizzare un adeguato numero di casi reali di pazienti affetti da lesioni polmonari primitive, effettuando un’analisi dell’eterogeneità sia su sequenze di immagini TC baseline sia contrast-enhanced, consentendo quindi un confronto tra eterogeneità morfologica e funzionale. I risultati ottenuti sono infine discussi sulla base del confronto con le considerazioni di natura clinica effettuate in cieco da due esperti radiologi dell’IRCCS-IRST.

ZusammenfassungrnDie häufigsten Todesfälle weltweit sind auf Herzerkrankungen zurückzuführen. Bei der koronaren Herzkrankheit (KHK) sammeln sich über Jahre arteriosklerotische Ablagerungen in den Herzkranzgefäßen an und führen so zu einer verminderten Durchblutung und Versorgung des Herzmuskelgewebes mit Sauerstoff und Nährstoffen. Zur nuklearmedizinischen Bildgebung finden am häufigsten das SPECT-Nuklid 201Tl sowie die beiden 99mTc-Radiopharmaka Sestamibi und Tetrofosmin Anwendung. Die PET-Technik ist der SPECT-Technik in Bezug auf absolute Quantifizierung sowie Auflösung überlegen. Ziel der vorliegenden Arbeit war es, ein mögliches PET-Radiopharmakon zur Diagnostik der KHK zu entwickeln. Um eine dem 99mTc-Nuklid vergleichbare Verfügbarkeit im klinischen Alltag zu erreichen, sollte als Basis des neuen Radiopharmakons das mittels Radionuklid-Generator verfügbare 68Ga dienen. Schiff’sche Basen-Verbindungen zeigten nach Komplexierung mit 67/68Ga eine deutliche Aufnahme in die Herzmuskelzellen. Auf dieser Grundlage wurden verschiedene Schiff’sche Basen-Strukturen synthetisiert. Diese unterscheiden sich einerseits durch das Substitutionsmuster der verwendeten Aldehyde und andererseits durch das verwendete Rückgrat. Alle synthetisierten Chelatoren wurden erfolgreich mit 68Ga radioaktiv markiert und konnten anschließend aufgereinigt werden. Die Evaluierung dieser Substanzen in vitro zeigte, dass sie in unterschiedlichen Medien stabil ist. Die Lipophilie der 68Ga-Verbindungen (log D) lag zwischen 0,87±0,24 und 2,72±0,14. Die Ladung der Verbindungen wurde mittels Papierelektrophorese bei pH= 7 als kationisch bestimmt. Zusätzlich fanden in vitro-Untersuchungen zur Bestimmung der Aufnahme der Komplexe in HL-1 Herzzellen statt. Um den Einfluss des Zellmembranpotentials bzw. des Mitochondrienmembranpotentials zu untersuchen, wurde ein Teil der Zellen dafür mit Valinomycin (Ionophor, zerstört das Potential) behandelt. Mittels ex vivo-Biodistributionen wurde die Organverteilung von zwei Schiff’schen Basen (68Ga-BADED-2 und 68Ga-BAPDMEN-2) mit dem routinemäßig in der Klinik eingesetzten Derivat 99mTc-Sestamibi sowie dem 18F-Flurpiridaz in Ratten verglichen. Alle Verbindungen zeigten dabei eine deutliche Herzaufnahme von mehr als 2 % der injizierten Dosis pro Gramm Gewebe. Durch in vivo-PET-Aufnahmen wurden die Zeit-Aktivitätskurven der 68Ga-Verbindungen sowie zum Vergleich des 18F-Flurpiridaz bestimmt. Die Aufnahmen lagen im Bereich von 0,63±0,15 für 68Ga-BAPEN-3 bis 2,72±0,86 für 68Ga-BADED-8.In dem zweiten Teil der Arbeit wurden die Vorteile des hochaffinen Herztracers Flurpiridaz mit dem lipophilen, positiv-geladenen Ga-Schiff’sche Base-Chelator kombiniert. Hierzu wurde zunächst das Insektizid Flurpiridaz synthetisiert und mit dem BAPEN-Rückgrat gekoppelt. Die entstandene Verbindung wurde erstmals mit 68Ga radioaktiv markiert und muss in weiterführenden Arbeiten evaluiert werden.

Herz-Kreislauf-Erkrankungen zählen weltweit zu den Hauptursachen, die zu frühzeitigem Tod führen. Pathophysiologisch liegt eine Gefäßwandverdickung durch Ablagerung arteriosklerotischer Plaques (Arteriosklerose) vor. Die molekulare Bildgebung mit den nuklearmedizinischen Verfahren SPECT und PET zielt darauf ab, minderperfundierte Myokardareale zu visualisieren, um den Krankheitsverlauf durch frühzeitige Therapie abschwächen zu können. Routinemäßig eingesetzt werden die SPECT-Perfusionstracer [99mTc]Sestamibi und [99mTc]Tetrofosmin. Zum Goldstandard für die Quantifizierung der Myokardperfusion werden allerdings die PET-Tracer [13N]NH3 und [15O]H2O, da eine absolute Bestimmung des Blutflusses in mL/min/g sowohl in der Ruhe als auch bei Belastung möglich ist. 2007 wurde [18F]Flurpiridaz als neuer Myokardtracer vorgestellt, dessen Bindung an den MC I sowohl in Ratten, Hasen, Primaten als auch in ersten klinischen Humanstudien eine selektive Myokardaufnahme zeigte. Um eine Verfügbarkeit des Radionuklids über einen Radionuklidgenerator gewährleisten zu können, sollten makrozyklische 68Ga-Myokard-Perfusionstracer auf Pyridaben-Basis synthetisiert und evaluiert werden. Die neue Tracer-Klasse setzte sich aus dem makrozyklischen Chelator, einem Linker und dem Insektizid Pyridaben als Targeting-Vektor zusammen. Struktur-Affinitätsbeziehungen konnten auf Grund von Variation des Linkers (Länge und Polarität), der Komplexladung (neutral und einfach positiv geladen), des Chelators (DOTA, NODAGA, DO2A) sowie durch einen Multivalenzansatz (Monomer und Dimer) aufgestellt werden. Insgesamt wurden 16 neue Verbindungen synthetisiert. Ihre 68Ga-Markierung wurde hinsichtlich pH-Wert, Temperatur, Vorläufermenge und Reaktionszeit optimiert. Die DOTA/NODAGA-Pyridaben-Derivate ließen sich mit niedrigen Substanzmengen (6 - 25 nmol) in 0,1 M HEPES-Puffer (pH 3,4) bei 95°C innerhalb 15 min mit Ausbeuten > 95 % markieren. Für die DO2A-basierenden Verbindungen bedurfte es einer mikrowellengestützen Markierung (300 W, 1 min, 150°C), um vergleichbare Ausbeuten zu erzielen. Die in vitro-Stabilitätstests aller Verbindungen erfolgten in EtOH, NaCl und humanem Serum. Es konnten keine Instabilitäten innerhalb 80 min bei 37°C festgestellt werden. Unter Verwendung der „shake flask“-Methode wurden die Lipophilien (log D = -1,90 – 1,91) anhand des Verteilungs-quotienten in Octanol/PBS-Puffer ermittelt. Die kalten Referenzsubstanzen wurden mit GaCl3 hergestellt und zur Bestimmung der IC50-Werte (34,1 µM – 1 µM) in vitro auf ihre Affinität zum MC I getestet. In vivo-Evaluierungen erfolgten mit den zwei potentesten Verbindungen [68Ga]VN160.MZ und [68Ga]VN167.MZ durch µ-PET-Aufnahmen (n=3) in gesunden Ratten über 60 min. Um die Organverteilung ermitteln zu können, wurden ex vivo-Biodistributionsstudien (n=3) vorgenommen. Sowohl die µ-PET-Untersuchungen als auch die Biodistributionsstudien zeigten, dass es bei [68Ga]VN167.MZ zwar zu einer Herzaufnahme kam, die jedoch eher perfusionsabhängig ist. Eine Retention des Tracers im Myokard konnte in geringem Umfang festgestellt werden.
