BACKGROUND: Sialorrhoea, the symptom of apparent excessive secretion of saliva is a relatively uncommon complaint. Some authors consider that in the absence of clinical findings, then these patients have a psychiatric disorder masquerading as a physical illness. However, there is little evidence in the literature to support this conclusion and a detailed psychological assessment of this population has not previously been reported. METHODS: In total, 18 patients and 18 age- and sex-matched controls were studied. All had a history of a complaint of excess salivation in the absence of any oral mucosal or systemic abnormality. All patients completed an Eysenck Personality Questionnaire. RESULTS: There were no differences in the extroversion of psychoticism scores between the study and control group. However, the result showed significant increases in the neuroticism and Lie Scale score in the patient group. CONCLUSIONS: The overall results of this study indicate that the complaint of sialorrhoea in otherwise healthy individuals does not have an organic basis and suggest that sialorrhoea is associated with high levels of neuroticism and a tendency to dissimulate. © Blackwell Munksgaard 2006. Quantitative examination of prostate histology offers clues in the diagnostic classification of lesions and in the prediction of response to treatment and prognosis. To facilitate the collection of quantitative data, the development of machine vision systems is necessary. This study explored the use of imaging for identifying tissue abnormalities in prostate histology. Medium-power histological scenes were recorded from whole-mount radical prostatectomy sections at × 40 objective magnification and assessed by a pathologist as exhibiting stroma, normal tissue (nonneoplastic epithelial component), or prostatic carcinoma (PCa). A machine vision system was developed that divided the scenes into subregions of 100 × 100 pixels and subjected each to image-processing techniques. Analysis of morphological characteristics allowed the identification of normal tissue. Analysis of image texture demonstrated that Haralick feature 4 was the most suitable for discriminating stroma from PCa. Using these morphological and texture measurements, it was possible to define a classification scheme for each subregion. The machine vision system is designed to integrate these classification rules and generate digital maps of tissue composition from the classification of subregions; 79.3% of subregions were correctly classified. Established classification rates have demonstrated the validity of the methodology on small scenes; a logical extension was to apply the methodology to whole slide images via scanning technology. The machine vision system is capable of classifying these images. The machine vision system developed in this project facilitates the exploration of morphological and texture characteristics in quantifying tissue composition. It also illustrates the potential of quantitative methods to provide highly discriminatory information in the automated identification of prostatic lesions using computer vision.

This study aimed to examine the structure of the statistics anxiety rating scale. Responses from 650 undergraduate psychology students throughout the UK were collected through an on-line study. Based on previous research three different models were specified and estimated using confirmatory factor analysis. Fit indices were used to determine if the model fitted the data and a likelihood ratio difference test was used to determine the best fitting model. The original six factor model was the best explanation of the data. All six subscales were intercorrelated and internally consistent. It was concluded that the statistics anxiety rating scale was found to measure the six subscales it was designed to assess in a UK population.

We present the results of an initial investigation into the efficacy of using testate amoebae for the discrimination of soils from wet ground and puddles, as little attention has been given to these organisms in forensic science. The preservation of testate amoebae in these sediments is generally good, although test concentrations are low. Statistical analysis suggests that restate amoebae assemblages are somewhat spatially distinct and have potential to be used for soil discrimination. A case study is presented where mineralogical (X-ray diffraction) and restate amoebae analyses are used in conjunction to clarify the scene of crime in a 'cold case' murder enquiry. Testate amoebae were recovered from dried sediment residues on clothing 10 years after the murder. Despite these promising results, further experimental work is crucial to examine the spatial and temporal variation of amoebae assemblages in water films, wet ground and puddles before they can be added to the armoury of methods available to the forensic biologist.

The recent emergence of high-throughput arrays for methylation analysis has made the influence of tumor content on the interpretation of methylation levels increasingly pertinent. However, to what degree does tumor content have an influence, and what degree of tumor content makes a specimen acceptable for accurate analysis remains unclear. Taking a systematic approach, we analyzed 98 unselected formalin-fixed and paraffin-embedded gastric tumors and matched normal tissue samples using the Illumina GoldenGate methylation assay. Unsupervised hierarchical clustering showed 2 separate clusters with a significant difference in average tumor content levels. Cribriform-morular variant (C-MV) of papillary thyroid carcinoma (PTC) is a rare and unusual neoplasm composed of multiple histologic components, including cribriform, papillary, solid, tall columnar, and morular patterns. Analyses of gross C-MV of PTC lesions has linked adenomatous polyposis coli (APC) mutations to its pathogenesis; however, the extent of involvement of mutations in the development Of individual components is unclear We report on bidirectional sequencing of the mutation cluster region (codons 1032-1565) of the APC gene in individually laser-microdissected components of a previously unreported C-MV of PTC. A silent Thr1493Thr gene variant was found in all tumoral components, whereas a 5-base-pair frameshift deletion at codon 1309 was identified only in the morules. Neither variant was observed in matched normal thyroid tissue. These results show the histologic components of C-MV of PTC to have some common mutational background, although additional somatic mutations may be involved in the development of morular structures.

BRCA1 encodes a tumour suppressor protein that plays pivotal roles in homologous recombination (HR) DNA repair, cell-cycle checkpoints, and transcriptional regulation. BRCA1 germline mutations confer a high risk of early-onset breast and ovarian cancer. In more than 80% of cases, tumours arising in BRCA1 germline mutation carriers are oestrogen receptor (ER)-negative; however, up to 15% are ER-positive. It has been suggested that BRCA1 ER-positive breast cancers constitute sporadic cancers arising in the context of a BRCA1 germline mutation rather than being causally related to BRCA1 loss-of-function. Whole-genome massively parallel sequencing of ER-positive and ER-negative BRCA1 breast cancers, and their respective germline DNAs, was used to characterize the genetic landscape of BRCA1 cancers at base-pair resolution. Only BRCA1 germline mutations, somatic loss of the wild-type allele, and TP53 somatic mutations were recurrently found in the index cases. BRCA1 breast cancers displayed a mutational signature consistent with that caused by lack of HR DNA repair in both ER-positive and ER-negative cases. Sequencing analysis of independent cohorts of hereditary BRCA1 and sporadic non-BRCA1 breast cancers for the presence of recurrent pathogenic mutations and/or homozygous deletions found in the index cases revealed that DAPK3, TMEM135, KIAA1797, PDE4D, and GATA4 are potential additional drivers of breast cancers. This study demonstrates that BRCA1 pathogenic germline mutations coupled with somatic loss of the wild-type allele are not sufficient for hereditary breast cancers to display an ER-negative phenotype, and has led to the identification of three potential novel breast cancer genes (ie DAPK3, TMEM135, and GATA4).

Drug abuse is a widespread problem affecting both teenagers and adults. Nitrous oxide is becoming increasingly popular as an inhalation drug, causing harmful neurological and hematological effects. Some gas chromatography-mass spectrometry (GC-MS) methods for nitrous oxide measurement have been previously described. The main drawbacks of these methods include a lack of sensitivity for forensic applications; including an inability to quantitatively determine the concentration of gas present. The following study provides a validated method using HS-GC-MS which incorporates hydrogen sulfide as a suitable internal standard allowing the quantification of nitrous oxide. Upon analysis, sample and internal standard have similar retention times and are eluted quickly from the molecular sieve 5Å PLOT capillary column and the Porabond Q column therefore providing rapid data collection whilst preserving well defined peaks. After validation, the method has been applied to a real case of N2O intoxication indicating concentrations in a mono-intoxication.

This article extends existing discussion in literature on probabilistic inference and decision making with respect to continuous hypotheses that are prevalent in forensic toxicology. As a main aim, this research investigates the properties of a widely followed approach for quantifying the level of toxic substances in blood samples, and to compare this procedure with a Bayesian probabilistic approach. As an example, attention is confined to the presence of toxic substances, such as THC, in blood from car drivers. In this context, the interpretation of results from laboratory analyses needs to take into account legal requirements for establishing the 'presence' of target substances in blood. In a first part, the performance of the proposed Bayesian model for the estimation of an unknown parameter (here, the amount of a toxic substance) is illustrated and compared with the currently used method. The model is then used in a second part to approach-in a rational way-the decision component of the problem, that is judicial questions of the kind 'Is the quantity of THC measured in the blood over the legal threshold of 1.5 μg/l?'. This is pointed out through a practical example.

L’agression sexuelle (AS) commise envers les enfants est un sujet complexe à enquêter et les allégations reposent souvent exclusivement sur le témoignage de l’enfant. Cependant, même quand l’enfant divulgue une AS, il peut être réticent à révéler certains détails personnels et gênants de l’AS à un étranger. Étant donné qu’il n'est pas toujours possible d'obtenir le consentement de filmer et qu’il est relativement difficile de mesurer l’attitude non verbale de l’enfant et celui de l’enquêteur au cours des entrevues d’investigations, cette recherche a été novatrice dans sa création d’échelles verbales de telles attitudes. Afin de déterminer la corrélation de l’attitude des enquêteurs et la collaboration des enfants, 90 entrevues d’enfants âgés de 4 à 13 ans ont été analysées. Les entrevues ont été enregistrées sur bande audio, transcrites et codifiées à l'aide des sous-échelles verbales d'attitudes soutenantes et non-soutenantes des enquêteurs ainsi que d’attitudes de résistance et de coopération de la part de l'enfant. La proportion des détails sur l’AS fournie par les enfants a également été calculée. Afin de comparer les entrevues avec et sans le protocole du National Institute of Child Health and Human Development (NICHD), une MANCOVA, contrôlant pour l’âge de l’enfant et la proportion de questions ouvertes, démontre tel qu’attendu que les entrevues avec le protocole obtiennent plus de détails fournis à la suite des questions ouvertes que les entrevues sans le protocole. Cependant, aucune différence ne ressort quant aux attitudes de l’enfant et celle de l’enquêteur. Afin de trouver le meilleur prédicteur de la quantité de détails dévoilés par les enfants, une analyse de régression multiple hiérarchique a été faite. Après avoir contrôlé pour l'âge de l’enfant, l’utilisation du protocole et la proportion de questions ouvertes, la résistance de l’enfant et l’attitude non-soutenante de l’enquêteur expliquent 28 % supplémentaire de la variance, tandis que la variance totale expliquée par le modèle est de 58%. De plus, afin de déterminer si la collaboration de l’enfant et l’attitude de l’enquêteur varient en fonction de l’âge des enfants, une MANOVA démontre que les enquêteurs se comportent similairement, quel que soit l'âge des enfants. Ceci, malgré le fait que les jeunes enfants sont généralement plus réticents et coopèrent significativement moins bien que les préadolescents. Finalement, une régression multiple hiérarchique démontre que le soutien de l'enquêteur est le meilleur prédicteur de la collaboration des enfants, au-delà des caractéristiques de l'enfant et de l’AS. Bien que l’utilisation du protocole NICHD ait permis des progrès considérables dans la manière d’interroger les enfants, augmentant la proportion de détails obtenus par des questions ouvertes/rappel libre et amplifiant la crédibilité du témoignage, l’adhésion au protocole n’est pas en soi suffisante pour convaincre des jeunes enfants de parler en détail d’une AS à un inconnu. Les résultats de cette thèse ont une valeur scientifique et contribuent à enrichir les connaissances théoriques sur les attitudes de l'enfant et de l'enquêteur exprimées lors des entrevues. Même si les enquêteurs de cette étude offrent plus de soutien aux enfants résistants, indépendamment de leur âge, pour promouvoir la divulgation détaillée de l’AS, de meilleures façons de contrer les attitudes de résistance exprimées par les jeunes enfants et une minimisation des attitudes non-soutenantes lors des entrevues sont nécessaires.
