BACKGROUND: Lymphedema is an underdiagnosed pathology which in industrialized countries mainly affects cancer patients that underwent lymph node dissection and/or radiation. Currently no effective therapy is available so that patients' life quality is compromised by swellings of the concerned body region. This unfortunate condition is associated with body imbalance and subsequent osteochondral deformations and impaired function as well as with an increased risk of potentially life threatening soft tissue infections. METHODS: The effects of PRP and ASC on angiogenesis (anti-CD31 staining), microcirculation (Laser Doppler Imaging), lymphangiogenesis (anti-LYVE1 staining), microvascular architecture (corrosion casting) and wound healing (digital planimetry) are studied in a murine tail lymphedema model. RESULTS: Wounds treated by PRP and ASC healed faster and showed a significantly increased epithelialization mainly from the proximal wound margin. The application of PRP induced a significantly increased lymphangiogenesis while the application of ASC did not induce any significant change in this regard. CONCLUSIONS: PRP and ASC affect lymphangiogenesis and lymphedema development and might represent a promising approach to improve regeneration of lymphatic vessels, restore disrupted lymphatic circulation and treat or prevent lymphedema alone or in combination with currently available lymphedema therapies.

OBJETIVOS: analisar o efeito da glicose na hemodinâmica materno-fetal pela avaliação doplervelocimétrica da circulação materno-fetal e fetoplacentária. MÉTODOS: trata-se de estudo realizado por único observador, no qual foram incluídas 31 gestantes clinicamente sadias entre a 28ª e a 36ª semana. Os parâmetros foram avaliados imediatamente antes e 60 minutos após a ingestão de 50 g de glicose. Foram utilizados como critérios de inclusão a avaliação clínica e laboratorial normal, a presença de feto único, a idade gestacional entre a 28ª e a 36ª semana de gestação confirmada por exame de ultra-sonografia e/ou a data da última menstruação, a glicemia de jejum menor ou igual a 110 mg/dL e o teste de sobrecarga após 50 g de glicose menor de 140 mg/dL. Como critérios de exclusão, adotaram-se a presença de malformação ou alterações de desenvolvimento fetal, o trabalho de parto, os antecedentes familiares de diabetes, as patologias próprias ou intercorrentes à gestação e o uso de fumo, álcool ou outras drogas. Foram avaliados os vasos maternos da artéria carótida comum e artérias uterinas, os vasos placentários da artéria umbilical e os vasos fetais da artéria cerebral média e aorta abdominal. Foram analisados os seguintes parâmetros em cada vaso: índice de resistência, índice de pulsatilidade, velocidade sistólica máxima, velocidade diastólica final e tempo de aceleração. A freqüência cardíaca fetal foi avaliada pelo modo M da ultra-sonografia. Para análise estatística foi utilizado o teste t de Student quando a variável diferença de antes e depois da ingestão de glicose apresentou distribuição normal no teste de Kolmogorov-Smirnov. Quando a normalidade foi rejeitada, utilizamos o teste não-paramétrico de Wilcoxon, com o nível de significância sempre estabelecido de p<0,05. RESULTADOS: Observou-se a elevação da glicemia materna após a ingestão de 50 g de glicose (pré: 68,0±10,1 mg/dL e pós: 104,6±28,2 mg/dL; p<0,001) A freqüência cardíaca fetal diminuiu após a ingestão de glicose (pré: 137,9±6,1 bpm e pós: 134,5±6,9 bpm; p=0,01). Na artéria umbilical, houve aumento do índice de pulsatilidade (pré: 0,8±0,1 e pós: 0,9±0,2; p=0,03). Não foram encontradas alterações velocimétricas significativas nos demais vasos e dos demais índices investigados. CONCLUSÕES: apesar da variação dos níveis de glicemia materna e da freqüência cardíaca fetal, após a ingestão de glicose, não ocorreram alterações significativas de fluxo nos vasos arteriais: umbilical, cerebral média e aorta fetais e carótidas e uterina maternas. Concluímos que a concentração da glicose utilizada pode ser liberada sem interferência hemodinâmica no compartimento materno-fetal.

An experimental study was conducted in a pump-turbine model in pumping mode, in order to characterize the flow field structure in the region between stay and guide vanes, using mainly the laser-Doppler anemometry in a two-color and back-scattered light-based system. The structure of the steady and unsteady flow was analyzed. The measurements were performed at three operation points. The obtained data provide appropriate boundary conditions and a good base of validation for numerical codes, and for the understanding of main loss mechanisms of this complex flow.

The flow structure of cold and ignited jets issuing into a co-flowing air stream was experimentally studied using a laser Doppler velocimeter. Methane was employed as the jet fluid discharging from circular and elliptic nozzles with aspect ratios varying from 1.29 to 1.60. The diameter of the circular nozzle was 4.6 mm and the elliptic nozzles had approximately the same exit area as that of the circular nozzle. These non-circular nozzles were employed in order to increase the stability of attached jet diffusion flames. The time-averaged velocity and r.m.s. value of the velocity fluctuation in the streamwise and transverse directions were measured over the range of co-flowing stream velocities corresponding to different modes of flame blowout that are identified as either lifted or attached flames. On the basis of these measurements, attempts were made to explain the existence of an apparent optimum aspect ratio for the blowout of attached flames observed at higher values of co-flowing stream velocities. The insensitivity of the blowout limits of lifted flames to nozzle geometry observed in our previous work at low co-flowing stream velocities was also explained. Measurements of the fuel concentration at the jet centerline indicated that the mixing process was enhanced with the 1.38 aspect ratio jet compared with the 1.60 aspect ratio jet. On the basis of the obtained experimental data, it was suggested that the higher blowout limits of attached flames for an elliptic jet of 1.38 aspect ratio was due to higher entrainment rates.

The purpose of this investigation was to evaluate the possibility to enhance certain qualities of facial prostheses. Polymethyl methacrylate is still being used as base mate¬rial or clip carrier material, but it is hard and heavy, and debonding of the silicone from the acrylic base material is a frequent problem. This thesis aims to evaluate the use of fiber-reinforced composite (FRC) as framework material for maxillofacial silicone prostheses. FRC has been used as reinforcement in removable and fixed partial dentures since the 1990s. This material is lightweight and can be fabricated to compress the margins of the prosthesis slightly, to keep it tightly against the skin during jaw movements and facial expressions. Additionally, the use of a thermochromic pigment, colorless in room temperature and red in a cold environment, was studied in order to evaluate the possibility of using this color changing pigment in facial prostheses to mimic the color change of facial skin in cold weather. The tensile bond strength between pre-impregnated, unidirectional FRC and maxillofacial silicone elastomer was studied. Three different bonding agents or primers were compared. Bond strength was improved by one of the primers and by roughening the surface. The effect of a skin compressing glass fiber-reinforced composite framework on facial skin blood flow was studied by using a face mask, constructed with a compression pad corresponding to the outer margin of a glass fiber-reinforced framework beam of a facial prosthesis. The skin blood flow of ten healthy volunteers, aged 23-25 years, was measured during touch, light, and moderate compression of the skin, by using laser Doppler imaging technique. None of the compressions showed any marked effects on local skin blood flow. There were no significant differences between blood flow during compression and at baseline. Maxillofacial silicone elastomer was colored intrinsically with conventional color pigments: a control group containing only conventional pigments was compared to two test groups with 0.2 wt% and 0.6 wt% thermochromic pigment added. The color of the material was measured with a spectrophotometer in room temperature and after storage in a freezer. The color stability of the maxillofacial silicone elastomer colored with thermo¬chromic pigment was evaluated by artificial aging. The color dif¬ference of the L* (lightness) and a* values (redness), comparing color after the samples were stored at room temperature and in a freezer (-19°C), was statistically significant for both 0.2 wt% and 0.6 wt% thermo¬chromic pigment groups. The differences in the b* values (yellowness) were statistically significant for the 0.6 wt% group. Exposure to ultraviolet (UV) radiation led to visually noticeable and statistically signifi¬cant color changes (ΔE) in all color values in both test groups. The specimens containing thermochromic pigment were very sensitive to UV radiation. In conclusion, a framework of fiber-reinforced composite can successfully be bonded to maxillofacial silicone elastomer, and a framework beam, compressing the facial skin, did not remarkably alter the skin blood flow on healthy, young adults. The thermochromic pigment showed color change in maxillofacial silicone elastomer. However, artificial aging showed that it was too sensitive to UV radiation to be used, as such, in maxillofacial prostheses.

Although enteropathogenic Escherichia coli (EPEC) are well-recognized diarrheal agents, their ability to translocate and cause extraintestinal alterations is not known. We investigated whether a typical EPEC (tEPEC) and an atypical EPEC (aEPEC) strain translocate and cause microcirculation injury under conditions of intestinal bacterial overgrowth. Bacterial translocation (BT) was induced in female Wistar-EPM rats (200-250 g) by oroduodenal catheterization and inoculation of 10 mL 10(10) colony forming unit (CFU)/mL, with the bacteria being confined between the duodenum and ileum with ligatures. After 2 h, mesenteric lymph nodes (MLN), liver and spleen were cultured for translocated bacteria and BT-related microcirculation changes were monitored in mesenteric and abdominal organs by intravital microscopy and laser Doppler flow, respectively. tEPEC (N = 11) and aEPEC (N = 11) were recovered from MLN (100%), spleen (36.4 and 45.5%), and liver (45.5 and 72.7%) of the animals, respectively. Recovery of the positive control E. coli R-6 (N = 6) was 100% for all compartments. Bacteria were not recovered from extraintestinal sites of controls inoculated with non-pathogenic E. coli strains HB101 (N = 6) and HS (N = 10), or saline. Mesenteric microcirculation injuries were detected with both EPEC strains, but only aEPEC was similar to E. coli R-6 with regard to systemic tissue hypoperfusion. In conclusion, overgrowth of certain aEPEC strains may lead to BT and impairment of the microcirculation in systemic organs.

The interaction between local and reflexive control of skin blood flow (SkBF) is unclear. This thesis isolated the roles of rectal (Tre) and local (Tloc) temperature on forearm SkBF regulation at normal and elevated body temperatures, and to investigate the interaction between local and reflexive SkBF control. While either normothermic (Tre ~37.0°C) or hyperthermic (∆Tre +1.1°C), SkBF was assessed on the dorsal aspect of each forearm in 10 participants while Tloc was manipulated in an A-B-A-B fashion between neutral (33.0°C) and hot (38.5°C). Finally, local heating to 44°C was performed to elicit maximal SkBF. Data are presented as a percentage of maximal cutaneous vascular conductance (CVC), calculated as laser-Doppler flux divided by mean arterial pressure. Tloc manipulations performed during normothermia had significantly greater effects on CVC than during hyperthermia. The decreased modification to SkBF from the Tloc changes during hyperthermia suggests that strong reflexive vasodilation attenuates local SkBF control mechanisms.

Le couplage neurovasculaire (CNV) est un mécanisme d’homéostasie cérébrale régulant le débit sanguin cérébral (CBF) en fonction de l’activité neuronale. La manière dont il est altéré par l’angiotensine II (Ang II), une hormone synthétisée et relâchée dans la circulation systémique ou, alternativement, produite dans le cerveau grâce aux astrocytes, demeure à élucider. Ces cellules expriment le récepteur AT1 (rAT1) et participent à l’orchestration du CNV en relâchant des agents vasoactifs suivant la réponse calcique astrocytaire. Nous avons donc étudié le rôle de cette réponse dans l’altération du CNV induite par l’Ang II. Nous avons trouvé par fluxmétrie par laser Doppler que l’Ang II atténue (p<0.05) la réponse du CBF engendrée par l’activation des récepteurs métabotropes du glutamate du groupe I (mGluRI) du cortex chez la souris C57BL/6. De manière similaire, l’Ang II diminue l'élévation du CBF induite par la stimulation des vibrisses (p<0.05). Sur tranches de cerveaux en aiguë, la polarité de la réponse vasculaire induite par un agoniste mGluRI dans les artérioles parenchymateuses a été significativement renversée par l’Ang II de la vasodilatation vers la vasoconstriction. En parallèle, l’Ang II a augmenté les niveaux de calcium astrocytaire basaux et l’amplitude des réponses calciques (p<0.05). L’altération des réponses vasculaires et calciques maximales a été prévenue par le candesartan, antagoniste des rAT1. Nos résultats suggèrent que l’Ang II potentialise via les rAT1 la réponse calcique qui atteint un seuil favorisant la vasoconstriction par rapport à la vasodilatation, altérant ainsi l’augmentation du CBF en réponse à l’activité neuronale.

Speckle Pattern Shearing Interferometrie (Shearografie) ist eine speckle-interferometrische Messmethode und zeichnet sich durch die ganzflächige, berührungslose Arbeitsweise, hohe räumliche Auflösung und hohe Messempfindlichkeit aus. Diese Dissertation beinhaltet die neue bzw. weitere Entwicklung der Shearografie zur qualitativen Schwingungsbeobachtung und zur quantitativen Schwingungsmessung. Für die qualitative Schwingungsbeobachtung in Echtzeit werden die Optimierung des Zeitmittelungsverfahrens und die neue entwickelte Online-Charakterisierung von Streifenmustern mit statistischen Verfahren vorgestellt. Auf dieser Basis können sowohl eine genaue Fehlstellen-Detektion bei der zerstörungsfreien Materialprüfung als auch eine präzise Resonanzuntersuchung zeitsparend und vollautomatisch durchgeführt werden. Für die quantitative Schwingungsmessung wird eine sog. dynamische Phasenschiebe-Technik neu entwickelt, welche durch die Einführung eines synchron zum Objekt schwingenden Referenzspiegels realisiert wird. Mit dieser Technik ermöglicht das Zeitmittelungsverfahren die Amplituden und Phasen einer Objektschwingung quantitativ zu ermitteln. Auch eine Weiterentwicklung des stroboskopischen Verfahrens in Kombination mit zeitlicher Phasenverschiebung wird in der Arbeit präsentiert, womit der gesamte Prozess der Schwingungsmessung und -rekonstruktion beschleunigt und automatisch durchgeführt wird. Zur Bestimmung des Verschiebungsfeldes aus den gemessenen Amplituden und Phasen des Verformungsgradienten stellt diese Arbeit auch eine Weiterentwicklung des Summationsverfahrens vor. Das Verfahren zeichnet sich dadurch aus, dass die Genauigkeit des ermittelten Verschiebungsfelds unabhängig von der Sheargröße ist und gleichzeitig das praktische Problem - Unstetigkeit - gelöst wird. Eine quantitative Messung erfordert eine genaue Kalibrierung der gesamten Messkette. Ein auf dem Least-Square-Verfahren basierendes Kalibrierverfahren wird in der Arbeit zur Kalibrierung der statischen und dynamischen Phasenverschiebung vorgestellt. Auch die Ermittelung der Sheargröße mit Hilfe der 1D- bzw. 2D-Kreuz-Korrelation wird präsentiert. Zum Schluss wurde die gesamte Entwicklung durch eine Vergleichsmessung mit einem handelsüblichen Scanning-Laser-Doppler-Vibrometer experimentell verifiziert.

Hypothesis: The aim of this study was to measure the mass loading effect of an active middle-ear implant (the Vibrant Soundbridge) in cadaver temporal bones. Background: Implantable middle ear hearing devices such as Vibrant Soundbridge have been used as an alternative to conventional hearing aids for the rehabilitation of sensorineural hearing loss. Other than the obvious disadvantage of requiring implantation middle ear surgery, it also applies a direct weight on the ossicular chain which, in turn, may have an impact on residual hearing. Previous studies have shown that applying a mass directly on the ossicular chain has a damping effect on its response to sound. However, little has been done to investigate the magnitude and the frequency characteristics of the mass loading effect in devices such as the Vibrant Soundbridge. Methods: Five fresh cadaver temporal bones were used. The stapes displacement was measured using laser Doppler vibrometry before and after the placement of a Vibrant Sound-bridge floating mass transducer. The effects of mass and attachment site were compared with the unloaded response. Measurements were obtained at frequencies between 0.1 and 10 kHz and at acoustic input levels of 100 dB sound pressure level. Each temporal bone acted as its own control. Results: Placement of the floating mass transducer caused a reduction of the stapes displacement. There were variations between the bones. The change of the stapes displacement varied from 0 dB to 28 dB. The effect was more prominent at frequencies above 1,000 Hz. Placing the floating mass transducer close to the incudostapedial joint reduced the mass loading effect. Conclusion: The floating mass transducer produces a measurable reduction of the stapes displacement in the temporal bone model. The effect is more prominent at high frequencies.

Cercal hairs represent in cricket a wind sensitive escape system, able to detect the airflow generated from predating species. These sensors have been studied as a biomimetic concept to allow the development of MEMS for biomedical use. In particular, the behaviour of the hairs, including airflow response, resonant frequency and damping, has been investigated up to a frequency of 20 kHz. The microscopic nature of the hairs, the complex vibrations of excited hairs and the high damping of the system suggested that the use of Laser Doppler vibrometry could possibly improve the test performance. Two types of tests were performed: in the first case the hairs were indirectly excited using the signal obtained from a vibrating aluminium plate, whilst in the second case the hairs were directly excited using a white noise chirp. The results from the first experiment indicated that the hairs move in-phase with the exciting signal up to frequencies in the order of 10 kHz, responding to the vibration modes of the plate with a signal attenuation of 12 to 20 dB. The chirp experiment revealed the presence of rotational resonant modes at 6850 and 11300 Hz. No clear effect of hair length was perceivable on the vibration response of the filiform sensors. The obtained results proved promising to support the mechanical and vibration characterisation of the hairs and suggest that scanning Laser vibrometry can be used extensively on highly dampened biological materials.

Although chronic fish oil intervention had been shown to have a positive impact on vascular reactivity, very little is known about their acute effects during the postprandial phase. Our aim was to examine the impact of a fish oil-enriched test meal on postprandial vascular reactivity in healthy younger ( < 50 years) v. older ( ≥ 50 years) men. Vascular reactivity was measured at baseline (0 h), 2 and 4 h after the meal by laser Doppler iontophoresis and blood samples taken at 0 and 4 h for the measurement of plasma lipids, total nitrite, glucose and insulin. Acetylcholine- (ACh, endothelial-dependent vasodilator) and sodium nitroprusside (SNP, endothelial-independent vasodilator)-induced reactivities were greater at 4 h than at baseline or 2 h in the younger men (P < 0·04). These changes were not observed in the older men. Comparison of the male groups revealed significantly greater responses to ACh (P = 0·006) and SNP (P = 0·05) at 4 h in the younger compared with the older males. Postprandial NEFA concentrations were also greater at 4 h in the younger compared with the older men (P = 0·005), with no differences observed for any of the other analytes. Multiple regression analysis revealed age to be the most significant predictor of both ACh and SNP induced reactivity 4 h after the meal. In conclusion, the ingestion of a meal enriched in fish oil fatty acids was shown to improve postprandial vascular reactivity at 4 h in our younger men, with little benefit evident in our older men.

Diets low in fruit and vegetables are reportedly responsible for 2.7 million deaths annually from cardiovascular diseases (CVD) and certain cancers. A daily fruit and vegetable intake of five 80 g portions is recommended for chronic disease prevention. However, in the UK, average adult consumption is less than three portions. It is suggested that fruit juice should only count as one portion. However, fruit juices are a beneficial source of phytochemicals. The preliminary results of two randomized, controlled, crossover, dietary intervention studies investigating the effects of chronic and acute consumption of fruit and vegetable puree and juice based drinks (FVPJ) on bioavailability, antioxidant status, vascular reactivity, and risk factors for CVD are reported. In the first study, 39 volunteers consumed 200 ml FVPJ, or fruit-flavoured control, daily for six weeks. In the second study, 24 volunteers consumed 400 mL FVPJ, or sugar-matched control, on the morning of the study day. Blood and urine samples were collected throughout both studies and real-time measurements of vascular tone were performed using laser Doppler imaging with iontophoresis. Overall, the studies showed that the fruit and vegetable puree and juice based drink increased dietary phytochemicals. There was a trend towards increased vasodilation following both acute and chronic fruit juice consumption. Measurements of antioxidant status, oxidative stress and other cardiovascular disease risk factors are currently being determined.

The average UK adult consumes less than three portions of fruit and vegetables daily, despite evidence to suggest that consuming five portions daily could help prevent chronic diseases. It is recommended that fruit juice should only count as one of these portions, as juicing removes fibre and releases sugars. However, fruit juices contain beneficial compounds such as vitamin C and flavonoids and could be a useful source of dietary phytochemicals. Two randomised controlled cross-over intervention studies investigating the effects of chronic and acute consumption of commercially-available fruit- and vegetable-puree-based drinks (FVPD) on bioavailability, antioxidant status and CVD risk factors are described. Blood and urine samples were collected during both studies and vascular tone was measured using laser Doppler imaging. In the chronic intervention study FVPD consumption was found to significantly increase dietary carotenoids (P = 0.001) and vitamin C (P = 0.003). Plasma carotenoids were increased (P = 0.001), but the increase in plasma vitamin C was not significant. There were no significant effects on oxidative stress, antioxidant status and other CVD risk factors. In the acute intervention study FVPD were found to increase total plasma nitrate and nitrite (P = 0.001) and plasma vitamin C (P = 0.002). There was no effect on plasma lipids or uric acid, but there was a lower glucose and insulin peak concentration after consumption of the FVPD compared with the sugar-matched control. There was a trend towards increased vasodilation following both chronic and acute FVPD consumption. All volunteers were retrospectively genotyped for the eNOS G298T polymorphism and the effect of genotype on the measurements is discussed. Overall, there was a non-significant trend towards increased endothelium-dependent vasodilation following both acute and chronic FVPD consumption. However, there was a significant time x treatment effect (P < 0.05) of acute FVPD consumption in individuals with the GG variant of the eNOS gene.

Regular consumption of green tea polyphenols (GTP) is thought to reduce the risk of cardiovascular disease (CVD) but has also been associated with liver toxicity. The present trial aimed to assess the safety and potential CVD health beneficial effects of daily GTP consumption. We conducted a placebo-controlled parallel study to evaluate the chronic effects of GTP on liver function and CVD risk biomarkers in healthy men. Volunteers (treatment: n = 17, BMI 26.7 +/- 3.3 kg/m(2), age 41 +/- 9 y; placebo, n = 16, BMI 25.4 +/- 3.3 kg/m(2), age 40 +/- 10 y) consumed for 3 wk 6 capsules per day (2 before each principal meal) containing green tea extracts (equivalent to 714 mg/d GTP) or placebo. At the beginning and end of the intervention period, we collected blood samples from fasting subjects and measured vascular tone using Laser Doppler lontophoresis. Biomarkers of liver function and CVD risk (including blood pressure, plasma lipids, and asymmetric dimethylarginine) were unaffected by GTP consumption. After treatment, the ratio of total:HDL cholesterol was significantly reduced in participants taking GTP capsules compared with baseline. Endothelial-dependent and -independent vascular reactivity did not significantly differ between treatments. In conclusion, the present data suggests that the daily consumption of high doses of GTP by healthy men for 3 wk is safe but without effects on CVD risk biomarkers other than the total:HDL cholesterol ratio.
