Situado no contexto da qualidade em saúde, este estudo versa sobre a decisão clínica e autonomia do paciente. Parte-se da premissa que, demais da competência técnica profissional e utilização de tecnologia adequada, o respeito aos direitos dos pacientes é atributo essencial à boa qualidade do atendimento médico. Tomando como exemplo a abordagem terapêutica do climatério, foi feita análise qualitativa do processo de decisão clínica, com base nas informações obtidas através de entrevistas semi-estruturadas com médico ginecologistas e com pacientes em fase de climatério. O propósito foi buscar apreender os valores dos médicos e dos pacientes e tentar compreender a lógica de seus comportamentos e atitudes, no que se refere especificamente aos papéis desempenhados por eles nesse processo. Com base nos resultados da análise, discute-se a complexidade da aplicação do princípio da autonomia na prática clínica e apresenta-se uma reflexão sobre a acreditação, como estratégia possível de contribuição a esse processo e à melhoria da qualidade do atendimento médico, por sua grande identificação como os aspectos relativos aos direitos dos pacientes, aos processos de educação permanente e à melhoria contínua da qualidade.

Este artigo pretende apresentar um estudo de caso realizado no serviço de imagiologia do hospital fernando da fonseca (HFF). Com o objectivo de elaborar-se e implementar-se um modelo de aplicação dos cinco sensos (5s) no hff, pretendendo-se contribuir para a melhoria contínua da qualidade. Concluiu-se que a implementação dos 5s no serviço de imagiologia do hff contribuiu para a melhoria contínua da qualidade. This article aims to present a case study conducted in the imagiology service at fernando fonseca hospital (ffh). In order to prepare and to implement an application model of the five senses (5s) in ffh, aiming to contribute for a continuous quality improvement. It was concluded that the implementation of the 5s in the imagiology service of ffh contributed to a continuous quality improvement.

Systemic challenges within child welfare have prompted many states to explore new strategies aimed at protecting children while meeting the needs of families, but doing so within the confines of shrinking budgets. Differential Response has emerged as a promising practice for low or moderate risk cases of child maltreatment. This mixed methods evaluation explored various aspects of North Carolina's differential response system, known as the Multiple Response System (MRS), including: child safety, timeliness of response and case decision, frontloading of services, case distribution, implementation of Child and Family Teams, collaboration with community-based service providers and Shared Parenting. Utilizing Child Protective Services (CPS) administrative data, researchers found that compared to matched control counties, MRS: had a positive impact on child safety evidenced by a decline in the rates of substantiations and re-assessments; temporarily disrupted timeliness of response in pilot counties but had no effect on time to case decision; and increased the number of upfront services provided to families during assessment. Qualitative data collected through focus groups with providers and phone interviews with families provided important information on key MRS strategies, highlighting aspects that families and social workers like as well as identifying areas for improvement. This information is useful for continuous quality improvement efforts, particularly related to the development of training and technical assistance programs at the state and local level.

Cette thèse s’intéresse à l’amélioration des soins et des services de santé et touche aux relations entre 3 grands thèmes de l’analyse des organisations de santé : la gouvernance, le changement et les pratiques professionnelles. En nous appuyant sur l’analyse organisationnelle contemporaine, nous visons à mieux comprendre l’interface entre l’organisation et les pratiques cliniques. D’une part, nous souhaitons mieux comprendre comment l’organisation structure et potentialise les pratiques des acteurs. D’autre part, dans une perspective d’acteurs stratégiques, nous souhaitons mieux comprendre le rôle des pratiques des professionnels dans l’actualisation de leur profession et dans la transformation et l’évolution des organisations. Notre étude se fonde sur l’hypothèse qu’une synergie accrue entre l’organisation et les pratiques des professionnels favorisent l’amélioration de la qualité des soins et des services de santé. En 2004, le gouvernement ontarien entreprend une importante réforme des soins et services dans le domaine du cancer et revoit les rôles et mandats du Cancer Care Ontario, l’organisation responsable du développement des orientations stratégiques et du financement des services en cancer dans la province. Cette réforme appelle de nombreux changements organisationnels et cliniques et vise à améliorer la qualité des soins et des services dans le domaine de l’oncologie. C’est dans le cadre de cette réforme que nous avons analysé l’implantation d’un système de soins et de services pour améliorer la performance et la qualité et analysé le rôle des pratiques professionnelles, spécifiquement les pratiques infirmières, dans la transformation de ce système. La stratégie de recherche utilisée correspond à l’étude approfondie d’un cas correspondant à l’agence de soins et de services en oncologie en Ontario, le Cancer Care Ontario, et des pratiques professionnelles infirmières évoluant dans ce modèle. Le choix délibéré de ce cas repose sur les modalités organisationnelles spécifiques à l’Ontario en termes de soins en oncologie. La collecte de données repose sur 3 sources principales : les entrevues semi-structurées (n=25), l’analyse d’une abondante documentation et les observations non participatives. La thèse s’articule autour de trois articles. Le premier article vise à définir le concept de gouvernance clinique. Nous présentons l’origine du concept et définissons ses principales composantes. Concept aux frontières floues, la gouvernance clinique est axée sur le développement d’initiatives cliniques et organisationnelles visant à améliorer la qualité des soins de santé et la sécurité des patients. L’analyse de la littérature scientifique démontre la prédominance d’une vision statique de la gouvernance clinique et d’un contrôle accentué des pratiques professionnelles dans l’atteinte de l’efficience et de l’excellence dans les soins et les services. Notre article offre une conception plus dynamique de la gouvernance clinique qui tient compte de la synergie entre le contexte organisationnel et les pratiques des professionnels et soulève les enjeux reliés à son implantation. Le second article s’intéresse à l’ensemble des leviers mobilisés pour institutionnaliser les principes d’amélioration continue de la qualité dans les systèmes de santé. Nous avons analysé le rôle et la portée des leviers dans l’évolution du système de soins en oncologie en Ontario et dans la transformation des pratiques cliniques. Nos données empiriques révèlent 3 phases et de nombreuses étapes dans la transformation du système. Les acteurs en position d’autorité ont mobilisé un ensemble de leviers pour introduire des changements. Notre étude révèle que la transformation du Cancer Care Ontario est le reflet d’un changement radical de type évolutif où chacune des phases est une période charnière dans la transformation du système et l’implantation d’initiatives de qualité. Le troisième article pose un regard sur un levier spécifique de transformation, celui de la communauté de pratique, afin de mieux comprendre le rôle joué par les pratiques professionnelles dans la transformation de l’organisation des soins et ultimement dans le positionnement stratégique de la profession infirmière. Nous avons analysé les pratiques infirmières au sein de la communauté de pratique (CDP) des infirmières en pratique avancée en oncologie. En nous appuyant sur la théorie de la stratégie en tant que pratique sociale, nos résultats indiquent que l’investissement de la profession dans des domaines stratégiques augmente les capacités des infirmières à transformer leurs pratiques et à transformer l’organisation. Nos résultats soulignent le rôle déterminant du contexte dans le développement de capacités stratégiques chez les professionnels. Enfin, nos résultats révèlent 3 stratégies émergentes des pratiques des infirmières : une stratégie de développement de la pratique infirmière en oncologie, une stratégie d’institutionnalisation des politiques de la CDP dans le système en oncologie et une stratégie de positionnement de la profession infirmière. Les résultats de notre étude démontrent que l’amélioration de la qualité des soins et des services de santé est située. L’implantation de transformations dans l’ensemble d’un système, tel que celui du cancer en Ontario, est tributaire d’une part, des capacités d’action des acteurs en position d’autorité qui mobilisent un ensemble de leviers pour introduire des changements et d’autre part, de la capacité des acteurs à la base de l’organisation à s’approprier les leviers pour développer un projet professionnel, améliorer leurs pratiques professionnelles et transformer le système de soins.

El presente estudio evalúa si hay cambios importantes en el indicador DNA, sin pretender demostrar causalidad, durante dos años antes y tres años después de la participación del EE, mediante un análisis comparativo pareado entre grupos con recolección retrospectiva. Durante el desarrollo de la formación de la primera cohorte de residentes del programa de especialización en Medicina de Emergencias en Bogotá, el hospital base del posgrado contrató un especialista en emergencias graduado en Medellín del Instituto de Ciencias de la Salud (CES), como Jefe del SU, lo que fue visto como una oportunidad única para notar los cambios posteriores a su participación, si bien el estudio no se diseñó para evaluar causalidad directa, pues los múltiples factores que intervendrían y el riesgo de modificación del factor a estudiar eran muy altos como para proponerse un análisis multivariado prospectivo.

Universities are striving to enhance the quality of the educational experience in the professions in response to external and internal pressures. The professional field of public relations (PR) is not immune to these forces. Previously, enhancements were often pursued through particular initiatives relating to curriculum, pedagogical or assessment redesign at the unit level. While such initiatives are valuable we argue for a strategic, integrated, programmatic approach. This requires the design of learning environments, with integrated virtual and physical dimensions, based on a relevant and meaningful curriculum, and student-centred approaches to learning. These learning environments enable quality learning in fields like public relations with diverse student cohorts studying on- and off-campus. The challenges involved in designing what we term ‘contemporary learning environments’ are illuminated through a case study of Deakin University’s Public Relations Program. Over the last three years redesigning PR online has led to changed curriculum, and pedagogical and assessment practices. We conclude by suggesting that a commitment to continuous quality improvement will be required to ensure the program’s learning environment remains relevant to the needs of students studying in the field.

Identification of all diabetic patients in the population is essential if diabetic care is to be effective in achieving the targets of the St Vincent Declaration.1 The challenge therefore is to establish population based monitoring and control systems by means of state of the art technology in order to achieve quality assurance in the provision of care for patients with diabetes. 2,3 Disease management receives extensive international support as the most appropriate approach to organising and delivering healthcare for chronic conditions like diabetes.4 This approach is achieved through a combination of guidelines for practice, patient education, consultations and follow up using a planned team approach and a strong focus on continuous quality improvement using information technology. 5,6 The current software (Medical Director) could not easily meet these requirements which led us to adopt a trial of Ferret. In designing this project we used change management7 and the plan, do, study, act cycle8 illustrated in Diagram 1.

Aims & Rationale/Objectives
Taking a capacity building approach to research and evaluation within the context of a federally funded national program challenges the traditional paradigms of both research and evaluation. The objective of this approach was to foster attitudes and behaviours of reflection, critical inquiry and collaborative action amongst participants responsible for health care integration activities.

Methods
A series of workshops focusing on different elements of health care integration was conducted. Each workshop offered skill development in research and evaluation methods relevant to the participants' clinical practise. The workshops were multidisciplinary and cross-sectoral in order to promote discussion about shared patient care issues.

Principal Findings
Participatory action research facilitated by external agents can build the capacity of participants to identify and make changes that improve health care integration at local levels. A capacity building approach to research and evaluation can mediate tensions between top-down initiatives and on-the-ground practitioners.

Discussion
A capacity building approach was crucial to the success of this project particularly as the project proposal was developed at the corporate level. The workshops played an important role in engaging the participants and fostering the development of solutions for locally identified clinical issues. The opportunities for discussion with other health care service providers were both readily embraced and appreciated by the participants. The networks formed during the workshops are likely to be vital in sustaining integration efforts.

Implications
Education sessions such as the workshops held within this project ensure that health care integration remains on the agenda of the relevant organisations. These workshops fostered a continuous quality improvement approach whilst focusing on the skills required and the systemic barriers to achieving health care integration. The success of these workshops is evidence that the need and desire for shared education opportunities exists and the interdisciplinary focus is a powerful tool for developing an appreciation of the cultures within disciplines as well as linkages.

Objective
The Australian lifestyle intervention program Life! is only the second reported, large-scale diabetes prevention program. This paper describes the genesis and the successful establishment of Life! and its key outcomes for participants and implementation.

Research
Design and Methods Life!, a behavior change intervention, comprises six group sessions over eight months. The Victorian Department of Health funded Diabetes Australia-Victoria to implement the program. Experience of the Greater Green Triangle diabetes prevention implementation trial was used for intervention design, workforce development, training and infrastructure. Clinical and anthropometric data from participants, used for program evaluation, was recorded on a central database.

Results
Life! has a state-wide workforce of 302 trained facilitators within 137 organizations. 29,000 Victorians showed interest in Life! and 15,000 individuals have been referred to the program. In total, 8,412 participants commenced a Life! program between October 2007 and June 2011. 37% of the original participants completed the eight month program. Participants completing sessions one to five lost an average of 1·4 kg weight (p<0·001) and waist circumference of 2·5cm (p<0.001). Those completing six sessions lost an average of 2·4 kg weight (p<0·001) and waist circumference of 3·8 cm (p<0·001). The weight loss of 2.4 kg represents 2.7% of participants’ starting body weight.

Conclusion
The impact of Life! is attributable to applying available evidence for the systems design of the intervention, and collaboration between policy makers, implementers and evaluators using the principles of continuous quality improvement to support successful, large scale recruitment and implementation.

INTRODUCTION: Patient participation in healthcare is recognised internationally as essential for consumer-centric, high-quality healthcare delivery. Its measurement as part of continuous quality improvement requires development of agreed standards and measurable indicators. AIM: This systematic review sought to identify strategies to measure patient participation in healthcare and to report their reliability and validity. In the context of this review, patient participation was constructed as shared decision-making, acknowledging the patient as having critical knowledge regarding their own health and care needs and promoting self-care/autonomy. METHODS: Following a comprehensive search, studies reporting reliability or validity of an instrument used in a healthcare setting to measure patient participation, published in English between January 2004 and March 2014 were eligible for inclusion. RESULTS: From an initial search, which identified 1582 studies, 156 studies were retrieved and screened against inclusion criteria. Thirty-three studies reporting 24 patient participation measurement tools met inclusion criteria, and were critically appraised. The majority of studies were descriptive psychometric studies using prospective, cross-sectional designs. Almost all the tools completed by patients, family caregivers, observers or more than one stakeholder focused on aspects of patient-professional communication. Few tools designed for completion by patients or family caregivers provided valid and reliable measures of patient participation. There was low correlation between many of the tools and other measures of patient satisfaction. CONCLUSION: Few reliable and valid tools for measurement of patient participation in healthcare have been recently developed. Of those reported in this review, the dyadic Observing Patient Involvement in Decision Making (dyadic-OPTION) tool presents the most promise for measuring core components of patient participation. There remains a need for further study into valid, reliable and feasible strategies for measuring patient participation as part of continuous quality improvement.

Quase um em cada dez pacientes é lesionado ao receber cuidados de saúde e, destes, muitos sofrem lesões incapacitantes ou morte todos os anos. Entendendo a importância e o impacto negativo das falhas na segurança do paciente em âmbito global e a influência que a cultura e o clima de segurança exercem sobre a adoção de ações e decisões mais seguras, este estudo teve por objetivo a analise da cultura de segurança do paciente em instituições hospitalares, por meio da mensuração do clima de segurança. Trata-se de pesquisa quantitativa, transversal, do tipo Survey, em que para a realização da coleta de dados foi aplicado o Questionário de Atitudes de Segurança, adaptação transcultural para o Brasil do Safety Attitudes Questionnaire (SAQ) - Short Form 2006. O estudo ocorreu em dois hospitais gerais do estado de São Paulo, localizados em diferentes regiões metropolitanas, sendo um público e o outro privado. Os profissionais Médicos, Enfermeiros, Técnicos e Auxiliares de Enfermagem, Fisioterapeutas, Farmacêuticos e Nutricionistas, que atuavam nestes hospitais há pelo menos 6 meses, com carga horária de trabalho semanal de no mínimo 20 horas, constituíram a população deste estudo. Foi realizado um estudo piloto com 25 profissionais em cada hospital e a prevalência resultante deste teste foi utilizada no cálculo do tamanho amostral com nível de significância de 5%, erro relativo de 10% e perda de 20%, resultando em um total de 235 participantes. Os profissionais escolhidos como parte da amostra foram sorteados empregando-se amostragem aleatória simples computadorizada. As variáveis de cada domínio da escala quando testadas pelo Teste Kolmogorov-Smirnov não apresentaram normalidade. Deste modo, foi aplicado o Teste Mann-Whitney para comparar os valores das pontuações entre os hospitais e entre as categorias profissionais. Com relação aos resultados houve índice de participação de 86,8% da amostra sorteada, prevalecendo os sujeitos com 5 a 20 anos de tempo na especialidade, do gênero feminino, e trabalhadores da enfermagem. Não houve diferenças significantes dentre as pontuações obtidas pelos dois hospitais. Os participantes do estudo apresentaram percepção negativa quanto ao clima de segurança do paciente, com domínios Reconhecimento do Estresse e Percepção da Gestão apresentando resultados negativos, tanto para a amostra como um todo quanto por hospital. Os domínios Clima de Trabalho em Equipe, Satisfação no Trabalho e Comportamento Seguro/Práticas Seguras resultaram em percepções positivas para todas as categorias profissionais. Já o domínio Percepção da Gestão do Hospital resultou em percepção negativa para todas estas. Os Médicos e os Técnicos e Auxiliares de Enfermagem apresentaram percepções negativas em mais domínios. Em contrapartida, os Enfermeiros foram os únicos a apresentar atitude de segurança do paciente positiva, com escore total do SAQ exibindo diferença significante quando comparado a todas as outras categorias, apresentando também percepção positiva em maior número de domínios. Concluiu-se que existe a necessidade de abordagem relacionada ao Reconhecimento do Estresse dos profissionais, além dos aspectos do Gerenciamento. As categorias profissionais diferiram entre si com relação às percepções sobre a atitude de segurança do paciente. Desta forma, o desenvolvimento da cultura de segurança deve englobar todas as categorias profissionais, uma vez que esta abrange toda a organização, destacando-se a necessidade de enfoque de ações com relação a categoria dos Médicos e dos Técnicos e Auxiliares de Enfermagem. Além disso, ficou evidente o papel de destaque e liderança dos profissionais Enfermeiros nos processos de melhoria da qualidade, e colocando-os em posição privilegiada para conduzir os esforços de melhoria contínua da qualidade nos serviços de saúde

Innovation is part and parcel of any service in today's environment, so as to remain competitive. Quality improvement in healthcare services is a complex, multi-dimensional task. This study proposes innovation management in healthcare services using a logical framework. A problem tree and an objective tree are developed to identify and mitigate issues and concerns. A logical framework is formulated to develop a plan for implementation and monitoring strategies, potentially creating an environment for continuous quality improvement in a specific unit. Se presentan los resultados de la aplicación de una metodología integradora de auditoría de información y conocimiento, llevada a cabo en un Centro de Investigación del Ministerio de Ciencia, Tecnología y Medio Ambiente de la provincia de Holguín, Cuba, conformada por siete etapas con un enfoque híbrido dirigida a revisar la estrategia y la política de gestión de información y conocimiento, identificar e inventariar y mapear los recursos de I+C y sus flujos, y valorar los procesos asociados a su gestión. La alta dirección de este centro, sus especialistas e investigadores manifestaron la efectividad de la metodología aplicada cuyos resultados propiciaron reajustar la proyección estratégica en relación con la gestión de la I+C, rediseñar los flujos informativos de los procesos claves, disponer de un directorio de sus expertos por áreas y planificar el futuro aprendizaje y desarrollo profesional.
