Introduction. Fibromyalgia is a chronic pain syndrome with unknown etiology. Recent studies have shown some evidence demonstrating that oxidative stress may have a role in the pathophysiology of fibromyalgia. However, it is still not clear whether oxidative stress is the cause or the effect of the abnormalities documented in fibromyalgia. Furthermore, the role of mitochondria in the redox imbalance reported in fibromyalgia also is controversial. We undertook this study to investigate the role of mitochondrial dysfunction, oxidative stress, and mitophagy in fibromyalgia. Methods. We studied 20 patients (2 male, 18 female patients) from the database of the Sevillian Fibromyalgia Association and 10 healthy controls. We evaluated mitochondrial function in blood mononuclear cells from fibromyalgia patients measuring, coenzyme Q10 levels with high-performance liquid chromatography (HPLC), and mitochondrial membrane potential with flow cytometry. Oxidative stress was determined by measuring mitochondrial superoxide production with MitoSOX™ and lipid peroxidation in blood mononuclear cells and plasma from fibromyalgia patients. Autophagy activation was evaluated by quantifying the fluorescence intensity of LysoTracker™ Red staining of blood mononuclear cells. Mitophagy was confirmed by measuring citrate synthase activity and electron microscopy examination of blood mononuclear cells. Results. We found reduced levels of coenzyme Q10, decreased mitochondrial membrane potential, increased levels of mitochondrial superoxide in blood mononuclear cells, and increased levels of lipid peroxidation in both blood mononuclear cells and plasma from fibromyalgia patients. Mitochondrial dysfunction was also associated with increased expression of autophagic genes and the elimination of dysfunctional mitochondria with mitophagy. Conclusions. These findings may support the role of oxidative stress and mitophagy in the pathophysiology of fibromyalgia.

Cette thèse avait pour but premier d’évaluer la douleur chronique endométriosique et ses concomitants (dépression, anxiété et stress), les conséquences de la douleur sur le physique, les activités et le travail, sur la relation maritale et les séquelles sur la qualité de vie chez des participantes souffrant de douleurs pelviennes chroniques diagnostiquées endométriose (laparoscopie). En deuxième lieu, il s’agissait d’évaluer et de comparer l’efficacité des techniques psychologiques de contrôle de la douleur (Hypnose, Cognitif-behavioral) en ajout aux traitements médicaux à un groupe contrôle (Attention thérapeute). L’échantillon était composé de 60 femmes réparties aléatoirement soit à l’un des deux groupes de traitement ou au groupe contrôle. Les instruments de mesure étaient tous des questionnaires déjà traduits en français et validés pour la population francophone québécoise. Des tests du khi-carré ont été effectués pour les variables nominales et des analyses de variances (ANOVA) ont été faites pour les variables continues. Dans des modèles ANOVA estimant l’effet du traitement, du temps et de leur interaction, une différence significative (effet de Groupe ou traitement) a été trouvée pour les variables suivantes : Douleur (McGill :composante évaluative p = 0.02), au moment « présent » de l’Échelle visuelle analogique (EVA, p = 0.05) et dans l’Échelle de Qualité de vie (douleur, p = 0,03) ainsi qu’à la dimension Fonctionnement social de cette dernière échelle (SF-36; p = 0,04). En comparant les données en pré et post-traitement, des résultats significatifs au niveau du Temps ont aussi été mis en évidence pour les variables suivantes : Douleur McGill: Score total, (p = 0,03), Affective (p = 0,04), Évaluative (p = 0,01); Douleur (ÉVA) moment Fort (p < 0,0005), Dépression (p = 0,005), Anxiété (situationnelle/état (p = 0,002), Anxiété/trait (p < 0,001), Stress (p = 0, 003) ainsi que pour quatre composantes de la Qualité de vie (Fonctionnement social, (p = 0,05), Vitalité (p = 0,002), Douleur, (p = 0,003) et Changement de la santé (p < 0,001) et ceci pour les trois groupes à l’exception du groupe Hypnose sur cette dernière variable. Des effets d’Interaction (Groupe X Temps) sont ressortis sur les variables « Conséquences physiques » de la douleur mais sur la dimension « Activités » seulement (p = 0,02), sur l’anxiété situationnelle (État : p = 0,007). Un effet d’interaction se rapprochant de la signification (p = 0,08) a aussi été analysé pour la variable Fonctionnement social (SF-36). L’étude montre une légère supériorité quant au traitement Cognitif-behavioral pour l’anxiété situationnelle, pour le Fonctionnement social et pour la douleur mesurée par le SF-36. L’étude présente des forces (groupe homogène, essai clinique prospectif, répartition aléatoire des participantes et groupe contrôle) mais aussi des lacunes (faible échantillon et biais potentiels reliés à l’expérimentateur et à l’effet placebo). Toute future étude devrait tenir compte de biais potentiels quant au nombre d’expérimentateur et inclure un groupe placebo spécifique aux études à caractère psychologique. Une future étude devrait évaluer le schème cognitif « catastrophisation » impliqué dans la douleur, les traits de personnalité des participantes ainsi que le rôle du conjoint. De plus, des techniques psychologiques (entrevues motivationnelles) récentes utilisées dans plusieurs études devraient aussi être prises en considérations. Tout de même des résultats significatifs offrent des pistes intéressantes pour un essai clinique comportant un échantillon plus élevé et pour un suivi à long terme.

Conflit présenté comme gelé, la guerre du Haut-Karabagh n’en est pas moins réelle, s’inscrivant dans une géopolitique régionale complexe et passionnante, nécessitant de s’intéresser à l’histoire des peuples de la région, à l’histoire des conquêtes et politiques menées concomitamment. Comprendre les raisons de ce conflit situé aux limites de l’Europe et de l’Asie, comprendre les enjeux en cause, que se soit la problématique énergétique ou l’importance stratégique de la région du Caucase à la fois pour la Russie mais également pour les Etats-Unis ou l’Union Européenne ; autant de réflexions que soulève cette recherche. Au delà, c’est l’influence réciproque du droit international et du politique qui sera prise en compte, notamment concernant l’échec des négociations actuelles. Les difficultés d’application du droit international à ce conflit sui generis se situent à plusieurs niveaux ; sur le statut de la région principalement, mais également sur les mécanismes de sanctions et de réparations devant s’appliquer aux crimes sur les personnes et les biens et qui se heurtent à la classification difficile du conflit.

La prothèse totale du genou (PTG) est une chirurgie couramment pratiquée pour traiter les patients souffrant d’arthrose sévère du genou. Bien que cette technique chirurgicale soit efficace pour diminuer la douleur, améliorer la fonction du genou et rentable d’un point de vue socio-économique, un pourcentage non négligeable de patients n’est pas satisfait suite à la chirurgie, principalement due à une douleur persistante ou due à une perception d’avoir une mauvaise fonction articulaire, sans cause identifiée. Cependant, l’impact de cette chirurgie sur la cinématique tridimensionnelle (3D) du genou demeure mal compris. Dans le but de mieux comprendre pourquoi certains patients ressentent toujours de la douleur suite à cette chirurgie, cette étude analysera, dans un premier temps, l’effet prospectif de la chirurgie sur la cinématique 3D du genou. Puis dans un second temps, comparera la cinématique 3D de sujet souffrant de douleur à celle de sujets asymptomatiques suite à la prothèse. Pour parvenir à ces deux objectifs, deux études distinctes ont été entreprises. Une première étude prospective a porté sur l’évolution de la cinématique 3D du genou d’un groupe de 19 sujets, recrutés sur la liste d’attente pour prothèse totale du genou de deux chirurgiens du CHUM, hôpital Notre-Dame, puis la cinématique a été comparée avec un groupe contrôle de 17 sujets avec des genoux sains. Une seconde étude a comparé la cinématique 3D de 20 sujets souffrant de douleur post-PTG avec 20 sujets avec des genoux asymptomatiques suite à leur chirurgie. La première étude a permis de montrer que la cinématique dans le plan frontal suite à la prothèse totale du genou était corrigée vers celle des sujets sains. Contrairement à celle mesurée dans les autres plans (sagittal et axial) qui, malgré de petites corrections, demeure différente de la cinématique des sujets sains. La seconde étude a permis d’identifier un marqueur biomécanique de la douleur chez les sujets souffrant de douleur post-PTG. Effectivement, contrairement aux sujets asymptomatiques, suite à leur chirurgie, les patients souffrants de douleur marchent avec une contracture en flexion plus importante tout au long de la phase d’appui. Les résultats de ces deux études tendent à montrer que la prothèse totale du genou modifie la cinématique 3D du genou, sans toutefois redevenir semblable à celle d’un genou normal. De plus, certains marqueurs biomécaniques peuvent être associés à de la douleur suite à la chirurgie. Une meilleure compréhension de l’impact de la PTG sur la cinématique 3D du genou permettra d’offrir de meilleurs traitements en préparation et après la chirurgie et pourrait mener à de nouveaux designs de prothèses.

The Bocaina Plateau, which is situated on the eastern flank of the continental rift of southeastern Brazil, is the highest part of the Serra do Mar. Topographic relief in this area is suggested to be closely related to its complex tectono-magmatic evolution since the breakup of Western Gondwana and opening of the South Atlantic Ocean. Apatite fission track ages and track length distributions from 27 basement outcrops were determined to assess these hypotheses and reconstruct the denudation history of the Bocaina Plateau. The ages range between 303 +/- 32 and 46 +/- 5 Ma, and are significantly younger than the stratigraphic ages. Mean track lengths vary from 13.44 +/- 1.51 to 11.1 +/- 1.48 mu m, with standard deviations between 1.16 and 1.83 mu m. Contrasting ages within a single plateau and similar ages at different altitudes indicate a complex regional tectonothermal evolution. The thermal histories inferred from these data imply three periods of accelerated cooling related to the Early Cretaceous continental breakup, Early Cretaceous alkaline magmatism, and the Paleogene evolution of the continental rift of southeastern Brazil. The oldest fission track ages (>200 Ma) were obtained in the Serra do Mar region, suggesting that these areas were a long-lived source of sediments for the Parana, Bauru, and Santos basins. The Patellofemoral pain syndrome is defined as a fore or retro patellar pain and it has multifactorial etiology, where the bad patellar alignment is the most acceptable hypothesis. However proximal factors to the knee, as the debility of the muscles of the hip, have been demonstrated as a contributing factor to the appearing of that syndrome. Purpose: To evaluate if exists a relation between the hip muscles performance and the development of the SDPF. Methods: Thirty women took part in this study. They were divided in two groups; a control group (fifteen asymptomatic subjects) and an experimental group (fifteen subjects with the diagnosis of SDPF). The muscle performance was evaluated in an isokinetic dynamometer, where it was verified the peak torque (PT), PT to body weight, PT time and the agonist/antagonist relation. It was also analyzed the electromyographic activity of the middle gluteus. The data was analyzed by the not paired t test at a significance level of 5%. Results:. Didn t have significant difference to the PT of the abductor muscles (p = 0,46) and lateral rotators of the hip (p = 0,17) between groups. Also didn t have significant difference to the PT values by the body weight, to these muscle groups either (p = 0,10 e p = 0,11, respectively). Didn t have significant difference between the amplitude of the signal (p = 0,05) and the onset of medium gluteus (p = 0,25) between the groups. Conclusion: In the experimental conditions realized, the study didn t demonstrate a relation between performance the hip muscles behavior and the development of the SDPF

Patellofemoral pain syndrome (PFPS) is described as anterior or retropatellar pain knee in the absence of other pathologies and is frequently associated with dysfunction of the vastus medialis oblique (VMO). However, several studies have demonstrated the inability to selectively activate this muscle through exercise. To evaluate the effect of Neuromuscular Electrical Stimulation (NMES) selective VMO in women with syndrome. We evaluated thirty-eight women: twenty in the control group (24.15 ± 2.60 years) and eighteen diagnosed with PFPS (25.56 ± 3.55 years). Both groups were evaluated before and after a protocol of electro stimulation. To measure for comparing groups before and after treatment, we assessed the extensor torque concentric and eccentric knee through an isokinetic dynamometer, the intensity (Root Mean Square - RMS) and the onset of activation (onset) of VMO compared to the vastus lateralis (VL) in two types of exercise: open and closed kinetic chain. . Statistical analysis was performed using SPSS 15.0, with a significance level of 5%. Results: Our data showed an increase in the intensity of activation (RMS) of the VMO muscle after NMES in both study groups. During concentric contraction the RMS of the VMO before the NMES was 105.69 ± 32.26 μV and after a single intervention was 122.10 ± 39.62 μV (p = 0.048) for the control group. In the group with PPS, we found a similar behavior, with RMS of the VMO before NMES of 96.25 ± 18.83 μV and 139.80 ± 65.88 μV after the intervention (p = 0.0001). However, there was no evidence in the RMS value of VL muscle. The onset was calculated by subtracting the onset of VL by the onset of VMO. For the group with PFPS, the onset before the intervention was -0.007 ± 0.14 ms, indicating a delay of the VMO relative to VL, and after NMES was 0.074 ± 0.09 ms (p = 0.016), showing an activation previous VMO to VL. The same occurred for the control group. We also observed that NMES increased knee extensor power during the concentric contraction in both groups. Before the intervention the mean power was 28.97 ± 9.01 W for the PPS group and after NMES was 34.38 ± 7.61 W (p = 0.0001). Conclusion: We observed an increase in electromyographic activity of the VMO and also an anticipatory effect of this muscle

BACKGROUND: The N-methyl-D-aspartate receptor antagonist ketamine and its active enantiomer, S(+)-ketamine, have been injected in the epidural and subarachnoid spaces to treat acute postoperative pain and relieve neuropathic pain syndrome. In this study we evaluated the effects of a single dose of preservative-free S(+)-ketamine, in doses usually used in clinical practice, in the spinal cord and meninges of dogs.METHODS: Under anesthesia (IV etomidate (2 mg/kg) and fentanyl (0.005 mg/kg), 16 dogs (6 to 15 kg) were randomized to receive a lumbar intrathecal injection (L5/6) of saline solution of 0.9% (control group) or S(+)-ketamine 1 mg/kg(-1) (ketamine group). All doses were administered in a volume of 1 mL over a 10-second interval. Accordingly, injection solution ranged from 0.6% to 1.5%. After 21 days of clinical observation, the animals were killed; spinal cord, cauda equine root, and meninges were removed for histological examination with light microscopy. Tissues were examined for demyelination (Masson trichrome), neuronal death (hematoxylin and eosin) and astrocyte activation (glial fibrillary acidic protein).RESULTS: No clinical or histological alterations of spinal tissue or meninges were found in animals from either control or ketamine groups.CONCLUSION: A single intrathecal injection of preservative-free S(+)-ketamine, at 1 mg/kg-1 dosage, over a concentration range of 6 to 15 mg/mL injected in the subarachnoid space in a single puncture, did not produce histological alterations in this experimental model. INTRODUÇÃO: A síndrome de dor femoropatelar (SDFP) é um problema comum que afeta uma em cada quatro pessoas. A alteração no tempo de ativação e a intensidade de contração dos músculos vasto medial oblíquo (VMO) e vasto lateral (VL) são consideradas fatores importantes na etiologia da SDFP. No entanto, existem poucos estudos sobre a função da porção oblíqua do vasto lateral (VLO) e nenhum sobre o tempo de ativação (onset) do VLO em atividades funcionais em sujeitos normais e com SDFP. OBJETIVO: Assim, o objetivo do estudo foi investigar o tempo de início de atividade eletromiográfica nos músculos VMO, VLO e VL longo (VLL) durante a marcha. MATERIAIS E MÉTODOS: A amostra foi formada por 15 sujeitos sem e 12 com SDFP. Dados eletromiográficos foram obtidos dos músculos VMO, VLL e VLO durante caminhada na esteira sem inclinação. A diferença relativa no onset (DRO) entre VMO-VLL e VMO-VLO foi determinada a partir da média de três passadas. RESULTADOS: Houve diferença entre os sujeitos com e sem SDFP em relação à DRO entre VMO-VLL. Nos sujeitos com SDFP, a ordem de início da atividade elétrica foi VLL seguida por VLO e após VMO. Nos indivíduos sem a patologia, a ordem foi diferente: primeiro VMO após VLO e, por último, VLL. CONCLUSÃO: Os achados sugerem que a ativação do VMO após o VLL poderia auxiliar no desenvolvimento e na manutenção da SDFP, enquanto o tempo de ativação do VLO possui menor participação.

The patellofemoral pain syndrome (PFPS) is defined as a retropatellar or anterior knee pain, without another disease. It affects until 25% of the population, being more common in women and trained persons. As others pathologies, PFPS have been affected the training of elite and amateurs athletes. Thereby, the general purpose of this study was discuss the occurrence of PFPS as a sports injury, there prevention possibilities and the appropriate recovery training after injury. It had been developed a literature review addressing the specific characteristics of the syndrome, its diagnosis, its target population, its development, how it affects the training and which are their possibilities of prevention and treatment. © FTCD/FIP-MOC.

Newborn infants exposed to preeclampsia (PE) present increased short-term morbidity, mainly respiratory diseases such as respiratory distress syndrome and bronchopulmonary dysplasia. Gastrointestinal problems are also frequent, although a higher risk of necrotizing enterocolitis has not been confirmed. These problems could be related not just to PE itself but also to prematurity or intrauterine growth restriction, which frequently occur in this maternal disease. Other findings, however, seem to be due to the direct effect of the maternal disease; these findings include an increased frequency of neutropenia and thrombocytopenia and a lower incidence of cerebral disorders, such as periventricularintraventricular hemorrhage and cerebral palsy. The evaluation of long-term outcome shows increasing evidence that PE has important implications for the future health of the mother and her child, suggesting that PE is not a simple gestational disorder but a clinical syndrome with an unclear etiology, a genetic component, and a complex pathophysiology. This syndrome involves important maternal and fetal vascular alterations that can persist and cause diseases in later life. The divergence in results on outcomes for children exposed to PE could, in part, be due to methodological differences in the studies, most of which are retrospective case-control studies. Better evidence on prognosis is obtained from cohort studies. Even in the cohort studies, differences in patient characteristics and severity of maternal disease, as well as sample size, follow-up time, and main outcome measures certainly contribute to the variability in results reported in the literature. © 2012 by the American Academy of Pediatrics.
