Le tamoxifène, un modulateur sélectif des récepteurs oestrogéniques, est un médicament largement utilisé depuis plus de vingt ans pour le traitement et la prévention du cancer du sein. Plusieurs études ont rapporté que l’administration aiguë du tamoxifène pouvait réduire certains courants K+ cardiaques. Cette observation suggère que les femmes traitées de façon chronique avec le tamoxifène risquent d’avoir une prolongation de leur intervalle QT, favorisant ainsi le développement de torsades de pointes. Puisque in vivo, le tamoxifène est largement métabolisé et son effet est attribué à celui du 4hydroxy-tamoxifène (4OH-tamoxifène), nous avons d'abord vérifié si les effets du tamoxifène sur la repolarisation pouvaient être dus au 4OH-tamoxifène. À l'aide de la méthode de patch-clamp, nous avons étudié l’effet aigu du 4OH-tamoxifène sur les courants K+ présents au niveau ventriculaire chez la souris femelle. En premier lieu, nous avons démontré que les souris traitées avec le 4OH-tamoxifène présentaient une diminution des courants K+ comparativement aux souris intactes. Fait intéressant, le prétraitement des myocytes avec l’antagoniste des récepteurs oestrogéniques, le ICI 182,780, ou l’inhibiteur de la synthèse protéique, l'actinomycine D, n’a pas modifié les effets du 4OH-tamoxifène. Ces résultats suggéraient que les effets du 4OH-tamoxifène sur les courants potassiques ne soient pas liés à la transcription génomique et n’implique pas les récepteurs aux œstrogènes. Bien que l’administration aiguë du 4OH-tamoxifène diminue les courants K+ cardiaques, l’absence de troubles au niveau du rythme cardiaque chez les femmes traitées à long terme exclu la possibilité de conclure que le traitement chronique avec le tamoxifène augmente la durée de l’intervalle QT. L'accès à des souris femelles et des cobayes nous a permis de démontrer que contrairement au traitement en aigu, les courants et les canaux K+ cardiaques sont augmentés en chronique. Les oestrogènes associés à une diminution des courants K+ d’une part et nos résultats obtenus avec le tamoxifène d’autre part suggèrent qu’en bloquant les récepteurs oestrogéniques, le tamoxifène puisse prévenir les effets inhibiteurs des oestrogènes sur les courants K+. Cette association œstrogènes- tamoxifène- récepteurs oestrogéniques et courants K+ nous a encouragées à approfondir encore nos études et vérifier l’influence des hormones sexuelles féminines sur la repolarisation ventriculaire. Une troisième étude a été ainsi réalisée chez des souris femelles ovariectomisées et des souris déficientes en récepteurs oestrogéniques α ou β afin de vérifier le rôle des oestrogènes et des récepteurs oestrogéniques sur la repolarisation ventriculaire. Nos résultats ont révélé clairement que l’absence des oestrogènes entraîne une augmentation de la densité du courant K+ transitoire indépendant du Ca2+ (Ito) et de l’expression du canal Kv4.3 et ces effets sont médiés par les REα. Ces données soutiennent davantage notre conclusion que l’inhibition des récepteurs oestrogéniques est responsable de l’augmentation des courants/canaux K+ et suggèrent fortement qu’ils jouent un rôle dans la régulation de la repolarisation ventriculaire. Elles soulignent aussi l'importance de vérifier le statut hormonal des animaux utilisés pour des études touchant l'électrophysiologie cardiaque. Dans la dernière partie de cette thèse nous avons vérifié les effets de la grossesse et du système nerveux autonome sur les différents paramètres électrocardiographiques et plus particulièrement sur le rythme cardiaque chez la souris. Nos données ont montré que, comme chez la femme enceinte, la grossesse est associée à une augmentation du rythme cardiaque. De plus, l'augmentation des niveaux des hormones féminines pourrait affecter l’automatisme et l’activité électrique cardiaque. Ces différentes études ont augmenté les connaissances sur la régulation hormonale de l'électrophysiologie cardiaque et aideront aux avancements des recherches chez les femmes.

Resumen Introducción Una posible opción de tratamiento para el manejo del trastorno depresivo mayor (TDM) es la estimulación magnética transcraneal (EMT) que ha mostrado propiedades antidepresivas superiores al placebo con un buen perfil de seguridad. El objetivo del presente trabajo es determinar la reducción en la severidad del TDM y la proporción de eventos adversos (EA) en pacientes con TDM refractario y no refractario, posterior al uso de EMT administrada en monoterapia o tratamiento coadyuvante comparado con terapia farmacológica. Metodología Se planteó una pregunta PICOT de la cual se realizó una búsqueda sistemática de estudios clínicos en las bases de datos Medline, EMBASE y Cochrane. Dos investigadores en forma independiente realizaron la selección de artículos, evaluación de calidad con la herramienta de la colaboración Cochrane y extracción de datos. Se extrajeron datos de eficacia como tasa de respuesta, porcentaje de remisión, calidad de vida, diminución sintomática del trastorno depresivo mayor en la escala de Hamilton y capacidad funcional. Igualmente, proporción de pacientes con EA. Se realizó un meta-análisis de estas variables teniendo en cuenta la heterogeneidad. Resultados La presente revisión sistemática incluyó 26 estudios clínicos aleatorizados de baja calidad metodológica mostrando que la EMT presentó una eficacia superior cuando es usada como coadyuvante a las terapias con que venían siendo tratados los pacientes con TDM refractario y no refractario en los desenlaces de tasa de respuesta y porcentaje de remisión. En el caso de intervenciones farmacológicas específicas, la EMT presento eficacia similar, tanto en terapia coadyuvante como en monoterapia comparado con las intervenciones farmacológicas. En cuanto a seguridad, la EMT presenta un buen perfil de seguridad debido a que en todos los escenarios estudiados los EA fueron no serios y baja frecuencia Conclusiones La evidencia disponible sugiere que la EMT mostró ser efectivo y seguro para el manejo del TDM refractario y no refractario. Sin embargo, la evidencia es débil por lo tanto se necesita mayor investigación clínica que soporte su uso.

Recent evidence suggests that an area in the dorsal medial prefrontal cortex (dorsal nexus) shows dramatic increases in connectivity across a network of brain regions in depressed patients during the resting state;1 this increase in connectivity is suggested to represent hotwiring of areas involved in disparate cognitive and emotional functions.1, 2, 3 Sheline et al.1 concluded that antidepressant action may involve normalisation of the elevated resting state functional connectivity seen in depressed patients. However, the effects of conventional pharmacotherapy for depression on this resting state functional connectivity is not known and the effects of antidepressant treatment in depressed patients may be confounded by change in symptoms following treatment.

Objectives This study established the value of the 6-sulfatoxymelatonin (aMT6s) urine concentration as a predictor of the therapeutic response to noradrenaline reuptake inhibitors in depressive patients. Methods Twenty-two women aged 18-60 years were selected. Depressive symptoms were assessed by using the Hamilton Depression Scale. Urine samples were collected at 0600-1200 h, 1200-1800 h, 1800-2400 h, and 2400-0600 h intervals, 1 day before and 1 day after starting on the nortriptyline treatment. Urine aMT6s concentration was analyzed by a one-way analysis of variance/Bonferroni test. Spearman`s rank correlation coefficient was used to analyze the correlation between depressive symptoms after 2 weeks of antidepressant treatment and the increase in aMT6s urine concentration. Results Higher and lower size effect groups were compared by independent Student`s t-tests. At baseline, the 2400- to 0600-h interval differed from all other intervals presenting a significantly higher aMT6s urine concentration. A significant difference in aMT6s urine concentrations was found 1 day after treatment in all four intervals. Higher size effect group had lower levels of depressive symptoms 2 weeks after the treatment. A positive correlation between depressive symptoms and the delta of aMT6s in the 2400-0600h interval was observed. Conclusion Our results reinforce the hypothesis that aMT6s excretion is a predictor of clinical outcome in depression, especially in regard to noradrenaline reuptake inhibitors. Background and purpose: The present study was designed to assess whether cyclooxygenase-2 (COX-2) activation is involved in the effects of chronic aldosterone treatment on endothelial function of mesenteric resistance arteries (MRA) from Wistar-Kyoto (WKY) and spontaneously hypertensive rats (SHR). Experimental approach: Relaxation to acetylcholine was measured in MRA from both untreated and aldosterone-treated strains. Vasomotor responses to prostacyclin and U46619 were also analysed. Release of 6-oxo-prostaglandin (PG)F(1 alpha) and thromboxane B(2) (TxB(2)) was determined by enzyme immunoassay. COX-2 protein expression was measured by western blot. Key results: Aldosterone reduced acetylcholine relaxation in MRA from both strains. In MRA from both aldosterone-treated strains the COX-1/2 or COX-2 inhibitor (indomethacin and NS-398, respectively), Tx2 synthesis inhibitor (furegrelate), prostacyclin synthesis inhibitor (tranylcypromine) or Tx2/PG2 receptor antagonist (SQ 29 548), but not COX-1 inhibitor SC-560, increased acetylcholine relaxation. In untreated rats this response was increased only in SHR. Prostacyclin elicited a biphasic vasomotor response: lower concentrations elicited relaxation, whereas higher concentrations elicited contraction that was reduced by SQ 29 548. Aldosterone increased the acetylcholine-stimulated production of 6-oxo-PGF(1 alpha) and TxB(2) in MRA from both strains. COX-2 expression was higher in both strains of rats treated with aldosterone. Conclusions and implications: Chronic treatment with aldosterone impaired endothelial function in MRA under normotensive and hypertensive conditions by increasing COX-2-derived prostacyclin and thromboxane A(2). As endothelial dysfunction participates in the pathogenesis of many cardiovascular disorders we hypothesize that anti-inflammatory drugs, specifically COX-2 inhibitors, could ameliorate vascular damage in patients with elevated aldosterone production.

The effect of glucose on the intracellular pH (pH(i)) recovery rate (dpH(i)/dt) and Na(+)-glucose transporter (SGLT) localization was investigated in HEK-293 cells, a cell line that expresses endogenous NHE1, NHE3, SGLT1, and SGLT2 proteins. The activity of the Na(+)/H(+) exchangers (NHEs) was evaluated by using fluorescence microscopy. The total and membrane protein expression levels were analyzed by immunoblotting. In cells cultivated in 5 mM glucose, the pH(i) recovery rate was 0.169 +/- A 0.020 (n = 6). This value did not change in response to the acute presence of glucose at 2 or 10 mM, but decreased with 25 mM glucose, an effect that was not observed with 25 mM mannitol. Conversely, the chronic effect of high glucose (25 mM) increased the pH(i) recovery rate (similar to 40%, P < 0.05), without changes in the total levels of NHE1, NHE3, or SGLT1 expression, but increasing the total cellular (similar to 50%, P < 0.05) and the plasma membrane (similar to 100%, P < 0.01) content of SGLT2. Treatment with H-89 (10(-6) M) prevented the stimulatory effect of chronic glucose treatment on the pH(i) recovery rate and SGLT2 expression in the plasma membrane. Our results indicate that the effect of chronic treatment with a high glucose concentration is associated with increased NHEs activity and plasma membrane expression of SGLT2 in a protein kinase A-dependent way. The present results reveal mechanisms of glucotoxicity and may contribute to understanding the diabetes-induced damage of this renal epithelial cell.

Nicotinic acetylcholine receptors (nAChR) exert pivotal roles in synaptic transmission, neuroprotection and differentiation. Particularly, homomeric alpha 7 receptors participate in neurite outgrowth, presynaptic control of neurotransmitter release and Ca(2+) influx. However, the study of recombinant alpha 7 nAChRs in transfected cell lines is difficult due to low expression of functional receptor channels. We show that PC12 pheochromocytoma cells induced to differentiation into neurons are an adequate model for studying differential nAChR gene expression and receptor activity. Whole-cell current recording indicated that receptor responses increased during the course of differentiation. Transcription of mRNAs coding for alpha 3, alpha 5, alpha 7, beta 2 and beta 4 subunits was present during the course of differentiation, while mRNAs coding for alpha 2, alpha 4 and beta 3 subunits were not expressed in PC12 cells. alpha 7 subunit expression was highest following 1 day of induction to differentiation. Activity of alpha 7 nAChRs, however, was most elevated on day 2 as revealed by inhibition experiments in the presence of 10 nM methyllycaconitine, rapid current decay and receptor responsiveness to the alpha 7 agonist choline. Increased alpha 7 receptor activity was noted when PC12 were induced to differentiation in the presence of choline, confirming that chronic agonist treatment augments nAChR activity. In summary, PC12 cells are an adequate model to study the role and pharmacological properties of this receptor during neuronal differentiation.

Arylpiperazine compounds are promising 5-HT1A receptor ligands that can contribute for accelerating the onset of therapeutic effect of selective serotonin reuptake inhibitors. In the present work, the chemometric methods HCA, PCA, KNN, SIMCA and PLS were employed in order to obtain SAR and QSAR models relating the structures of arylpiperazine compounds to their 5-HT1A receptor affinities. A training set of 52 compounds was used to construct the models and the best ones were obtained with nine topological descriptors. The classification and regression models were externally validated by means of predictions for a test set of 14 compounds and have presented good quality, as verified by the correctness of classifications, in the case of pattern recognition studies, and b, the high correlation coefficients (q(2) = 0.76, r(2) = 0.83) and small prediction errors for the PLS regression. Since the results are in good agreement with previous SAR studies, we can suggest that these findings can help in the search for 5-HT1A receptor ligands that are able to improve antidepressant treatment. Fencamfamine (FCF) is an indirect dopamine agent with effects similar to amphetamine and cocaine. In the present study, we investigate changes in Na,K-ATPase, cyclic AMP-dependent protein kinase (PKA) and nitric oxide synthase (NOS) activity and cyclic GMP levels in the nucleus accumbens (NAc) and striatum (ST) of animals acutely or repeatedly treated with FCF (3.5 mg/kg). Na,K-ATPase had a similar activity in control and repeatedly treated animals, but was reduced in the NAc of the acute group. This enzyme was reduced in the ST in acute and repeatedly treated animals, compared to the control group. Expression of the alpha(1,2,3)-Na,K-ATPase isoforms in the NAc and the ST was not altered in all groups studied. Acute FCF induced a significant increase in PKA activity in both the ST and the NAc. Repeatedly treated animals showed a higher increase in PKA activity in the NAc, but not in the ST, when compared to the acute group. There was also an increase in both NOS activity and cyclic GMP levels only in the NAc of FCF repeatedly treated animals compared to the acute and control groups. We suggest that chronic FCF treatment is linked to a modification in Na,K-ATPase activity through the PKA and NO-cyclic GMP pathway. (C) 2003 Elsevier Ltd. All rights reserved.

The caffeine is a mild psychostimulant that has positive cognitive effects at low doses, while promotes detrimental effects on these processes at higher doses. The episodic-like memory can be evaluated in rodents through hippocampus-dependent tasks. The dentate gyrus is a hippocampal subregion in which neurogenesis occurs in adults, and it is believed that this process is related to the function of patterns separation, such as the identification of spatial and temporal patterns when discriminating events. Furthermore, neurogenesis is influenced spatial and contextual learning tasks. Our goal was to evaluate the performance of male Wistar rats in episodic-like tasks after acute or chronic caffeine treatment (15mg/kg or 30mg/kg). Moreover, we assessed the chronic effect of the caffeine treatment, as well as the influence of the hippocampus-dependent learning tasks, on the survival of new-born neurons at the beginning of treatment. For this purpose, we used BrdU to label the new cells generated in the dentate gyrus. Regarding the acute treatment, we found that the saline group presented a tendency to have better spatial and temporal discrimination than caffeine groups. The chronic caffeine group 15 mg/kg (low dose) showed the best discrimination of the temporal aspect of episodic-like memory, whereas the chronic caffeine group 30mg/kg (high dose) was able to discriminate temporal order, only in a condition of greater difficulty. Assessment of neurogenesis using immunohistochemistry for evaluating survival of new-born neurons generated in the dentate gyrus revealed no difference among groups of chronic treatment. Thus, the positive mnemonic effects of the chronic caffeine treatment were not related to neuronal survival. However, another plastic mechanism could explain the positive mnemonic effect, given that there was no improvement in the acute caffeine groups

The biological effects of catecholamines in mammalian pigment cells are poorly understood. Our previous results showed the presence of α1-adrenoceptors in SK-Mel 23 human melanoma cells. The aims of this work were to (1) characterize catecholamine effects on proliferation, tyrosinase activity and expression, (2) identify the α1- adrenoceptor subtypes, and (3) verify whether chronic norepinephrine (NE) treatment modified the types and/or pharmacological characteristics of adrenoceptors present in SK-Mel 23 human melanoma cells. Cells treated with the aradrenergic agonist, phenylephrine (PHE, 10-5 or 10-4 M), for 24-72 h, exhibited decreased cell proliferation and enhanced tyrosinase activity, but unaltered tyrosinase expression as compared with the control. The proliferation and tyrosinase activity responses were inhibited by the α1-adrenergic antagonist prazosin, suggesting they were evoked by α1-adrenoceptors. The presence of actinomycin D, a transcription inhibitor, did not diminish PHE-induced effects. RT-PCR assays, followed by cloning and sequencing, demonstrated the presence of α1A- and α1B-adrenoceptor subtypes. NE-treated cells (24 or 72 h) were used in competition assays, and showed no significant change in the competition curves of α1-adrenoceptors as compared with control curves. Other adrenoceptor subtypes were not identified in these cells, and NE pretreatment did not induce their expression. In conclusion, the activation of SK-Mel 23 human melanoma α1- radrenoceptors elicit biological effects, such as proliferation decrease and tyrosinase activity increase. Desensitization or expression of other adrenoceptor subtypes after chronic NE treatment were not observed.

Este trabalho apresenta o estudo de caso de uma mulher encaminhada ao serviço de psicologia de um hospital universitário com diagnóstico de urticária crônica. A queixa principal foi de que, apesar da adesão ao tratamento, os sintomas permaneciam. O objetivo foi auxiliar a cliente na instalação de novos repertórios e na generalização de padrões adequados já instalados, buscando melhor controle da urticária. Realizaram-se dez sessões de atendimento ambulatorial utilizando-se o modelo construcional, da abordagem analítico-comportamental, por meio de análises funcionais de episódios relatados pela cliente. Resultados indica-ram multicausalidade no desencadeamento e manutenção dos sintomas. O treino em análise de contingências favoreceu melhor controle dos fatores ambientais. A cliente aderiu adequadamente ao uso do medicamento após estabelecer relações funcionais entre sintomas e estressores ambientais, promovendo redução dos sintomas e melhor controle da doença. Concluiu-se que o atendimento auxiliou na melhora da qualidade de vida da cliente, destacando-se a relevância da adesão ao tratamento.

Bacterial cellulose is a highly hydrated pellicle made up of a random assembly of ribbon shaped fibers less than 5 nm wide. The unique properties provided by the nanometric structure have led to a number of diagnostic biological probes, display devices due to their unique size-dependent medical applications. Bacterial cellulose matrix extracellular is a novel biotechnology and unique medicine indicated for ultimate chronic wound treatment management, drug delivery, tissue engineering, skin cancer and offers an actual and effective solution to a serious medical and social problem and to promote rapid healing in lesions caused by Diabetic burns, ulcers of the lower limbs or any other circumstance in which there's epidermal or dermal loss. In this work, it is reported novel antimicrobial peptides (AMPs) bacterial cellulose/polyhexanide biguanide (PHMB) which are produced by symbioses culture between polyhexanide biguanide and green tea culture medium resulting in the pure 3-D structure consisting of an ultra-fine network of novel biocellulose/PHMB nanofibres matrix (2-8 nm), highly hydrated (99% in weight), and with higher molecular weight, full biocompatibility.
