La dysfonction endothéliale vasculaire constitue un marqueur précoce des maladies cardiovasculaires car l’endothélium est l’une des premières cibles des facteurs de risque cardiovasculaire. La présence d'un stress chronique engendré par les facteurs de risque cardiovasculaire sollicite les mécanismes de défense endogènes, tels que les enzymes antioxydantes, qui servent au maintien de la fonction endothéliale. L’environnement vasculaire auquel l’endothélium est exposé a un effet direct sur son fonctionnement à long terme. Certaines habitudes de vie sont ainsi associées à une bonne santé cardiovasculaire. Par exemple, la diète méditerranéenne et/ou la pratique régulière de l’exercice physique aident à maintenir une fonction endothéliale adéquate et à réduire l’incidence des maladies cardiovasculaires. D'autre part, certains gènes clés, comme le gène suppresseur de tumeurs p53, régulent plusieurs voies métaboliques importantes pour préserver l’intégrité des cellules endothéliales. Nous posons l’hypothèse que l’environnement vasculaire post-natal influence la mise en place de mécanismes de défenses endogènes tels que les enzymes antioxydantes afin de faire face à des stress plus tard dans la vie. Notre objectif global était d’évaluer les impacts d’interventions post-natales bénéfiques et d’une diminution endogène du gène suppresseur de tumeurs p53, sur la fonction endothéliale vasculaire et sur sa capacité à faire face à un stress métabolique. Dans une première étude, nous avons soumis des souris saines C57Bl/6 dès leur sevrage et jusqu’à l’âge de 9 mois, à un programme d’exercice physique volontaire (course dans une roue) ou à un antioxydant (catéchine), comparé à un groupe de souris sédentaires et sans antioxydant. Puis les interventions ont été stoppées et une diète riche en gras a été introduite, ou non, pour une période de 3 mois; les souris ont été sacrifiées à l'âge de 9 ou 12 mois. Nous avons observé que l’exercice a protégé les cellules endothéliales des effets délétères induits par la diète riche en gras en préservant la fonction endothéliale par le maintien d’un profil rédox sain et en évitant la hausse de l’inflammation. La catéchine a maintenu la fonction endothéliale aortique, mais n’a pas prévenu le profil inflammatoire en présence de la diète riche en gras. Finalement, chez les souris sédentaires, la fonction endothéliale a été détériorée en présence de la diète riche en gras, sans indice d’inflammation vasculaire. Dans une seconde étude, des souris partiellement déficientes en p53 (p53+/-) et contrôles C57Bl/6 ont été exposées à la même diète riche en gras à partir de 3 mois et ce jusqu’à l’âge de 6 mois. Notre raisonnement était basé sur la démonstration que p53 est un régulateur de l’expression des enzymes antioxydantes in vitro. Chez les souris p53+/-, les cellules endothéliales ont été protégées du stress induit par l’hypercholestérolémie engendrée par la diète riche en gras. Cependant, chez les souris p53+/- cette protection pourrait être secondaire à un métabolisme accru des acides biliaires, qui en prévenant la hausse de cholestérol, protègerait indirectement l'endothélium. Nous avons donc pu démontrer l’importance de l’environnement vasculaire sur la fonction endothéliale. La diète riche en gras a stimulé certains mécanismes de défense vasculaires tels que la voie des EDHF et la superoxyde dismutase afin de maintenir la fonction endothéliale malgré les conditions pro-athérosclérotiques. Nous avons observé que l’exercice et la catéchine influencent différemment l’endothélium malgré leurs capacités antioxydantes. Ces études soulignent la sensibilité de l’endothélium aux changements dans l’environnement vasculaire. En accord avec le vieillissement de la population et la progression des maladies cardiovasculaires, la proportion de personnes ayant une dysfonction endothéliale augmente. Ainsi, une meilleure compréhension des mécanismes ou d’interventions qui permettent le maintien de la fonction endothéliale à long terme s’avère utile.

Plusieurs études montrent que les acides gras (AG) oméga-3 sont bénéfiques pour la santé cardiovasculaire. Une étude antérieure dans notre laboratoire a montré que l’administration des acides gras oméga-3 réduit la taille de l'infarctus du myocarde (IM). Cependant, la question demeure de savoir si les deux principaux acides gras oméga-3 à longue chaîne, l'acide eicosapentaénoïque (EPA) et l'acide docosahexaénoïque (DHA) possèdent la même efficacité à réduire la taille de l'infarctus. Le but de ce projet sera de déterminer l’efficacité relative de chacun de ces acides gras oméga-3 à protéger le cœur dans un modèle d’ischémie/reperfusion et d’étudier certaines voies de cardioprotection. Des rats mâles adultes Sprague-Dawley ont été nourris pendant 14 jours avec une diète comprenant l'un: 1- aucun AG oméga-3; 2- 5 g d'EPA / kg de nourriture; 3- 5 g de DHA / kg de nourriture; 4- 2,5 g de chaque oméga-3 AG / kg de nourriture; 5- 5 g chaque AG oméga-3 / kg de nourriture. Par la suite, les animaux ont été soumis à une ischémie pendant 40 minutes, causée par l'occlusion de l'artère coronaire gauche descendante. Au bout de 24 heures de reperfusion, la taille de l'infarctus est déterminée. Dans un sous-groupe d'animaux, l'activité d’Akt et des caspase-3 sont mesurées dans la région ischémique après 30 minutes de reperfusion. Finalement, à 15 minutes de reperfusion, l'ouverture du pore de transition de perméabilité mitochondriale mPTP est déterminée dans un autre sous-groupe. Les résultats indiquent que les diètes EPA ou DHA réduisent de manière significative la taille de l'infarctus par rapport à la diète sans AG oméga-3, tandis que la combinaison de deux acides gras oméga-3 n'a pas montré de diminution de la taille de l'infarctus. L'activité de la caspase-3 est réduite pour le groupe DHA puis, l'activité d'Akt est augmentée avec les diètes EPA et DHA seules. Finalement, en présence d’une diète enrichie uniquement de DHA, l'ouverture des mPTP est retardée comparativement aux autres diètes.

Introducción: La hipertensión arterial es un problema de salud pública tanto en países industrializados como en vía de desarrollo. Su prevalencia en la infancia viene en aumento por lo que es relevante determinarla en niños preescolares a nivel local. Objetivo: Determinar la prevalencia de hipertensión arterial en niños de tres a cinco años de una cohorte de 14 hogares infantiles del ICBF de la localidad de Usaquén en Bogotá. Materiales y métodos: Se realizó un estudio de corte transversal analítico, utilizando la base de datos de un ensayo aleatorizado y controlado del año 200913, y se evaluaron las cifras de tensión arterial de acuerdo a sexo, edad, talla y su correlación con el IMC con un nivel de confianza del 95% y precisión del 1%. Se calcularon las medias, desviaciones estándar, percentiles y prevalencia. Resultados: Se obtuvo una muestra de 1035 casos, encontrándose una prevalencia de 4,5% de HTA sistólica, 10,4% de diastólica, ambas en estadio I; teniendo en cuenta tanto sistólica como diastólica, fue de 11,6% en estadio I. Se determinaron los valores de presión arterial sistólica y diastólica en cuartiles de acuerdo a edad, sexo y talla. El coeficiente de correlación entre el IMC y los niveles de presión arterial sistólica y diastólica fueron de 0.0992 y 0.0362 respectivamente. Conclusión: La prevalencia de HTA general fue de 11,6%, predominando la diastólica en estadio I en niños preescolares. No se encontró correlación entre el IMC y las cifras de tensión arterial sistólica y diastólica.

A relação entre aptidão cardiorrespiratória e um menor perfil de risco cardiovascular tem sido demonstrado em crianças e adolescentes (Janssen & Leblanc, 2010; 2006; Kriemler et al., 2008). Objetivo: verificar a associação entre o consumo máximo de oxigénio ( 2max) e a circunferência da cintura (CC), pressão arterial sistólica (PAS), pressão arterial diastólica (PAD) e glicemia em adolescentes utilizando a base de dados do projeto Rastresaúde. Conclusão: os resultados do estudo sugerem uma associação inversa entre o 2max e a PAS, PAD e CC em adolescentes do sexo feminino e entre 2max e a CC em adolescentes do sexo masculino. Dada a escassez de estudos e derivado ao aumento da prevalência dos fatores de risco associados às doenças cardiovasculares na infância e adolescência é relevante realizar mais estudos na população jovem.

Increasing recognition of the importance of the long-chain n-3 PUFA, EPA and DHA, to cardiovascular health, and in the case of DHA to normal neurological development in the fetus and the newborn, has focused greater attention on the dietary supply of these fatty acids. The reason for low intakes of EPA and DHA in most developed countries (0 center dot 1-0 center dot 5hairspg/d) is the low consumption of oily fish, the richest dietary source of these fatty acids. An important question is whether dietary intake of the precursor n-3 fatty acid, alpha-linolenic acid (alpha LNA), can provide sufficient amounts of tissue EPA and DHA by conversion through the n-3 PUFA elongation-desaturation pathway. alpha LNA is present in marked amounts in plant sources, including green leafy vegetables and commonly-consumed oils such as rape-seed and soyabean oils, so that increased intake of this fatty acid would be easier to achieve than via increased fish consumption. However, alpha LNA-feeding studies and stable-isotope studies using alpha LNA, which have addressed the question of bioconversion of alpha LNA to EPA and DHA, have concluded that in adult men conversion to EPA is limited (approximately 8%) and conversion to DHA is extremely low (< 0 center dot 1%). In women fractional conversion to DHA appears to be greater (9%), which may partly be a result of a lower rate of utilisation of alpha LNA for beta-oxidation in women. However, up-regulation of the conversion of EPA to DHA has also been suggested, as a result of the actions of oestrogen on Delta 6-desaturase, and may be of particular importance in maintaining adequate provision of DHA in pregnancy. The effect of oestrogen on DHA concentration in pregnant and lactating women awaits confirmation.

Nutrigenetics and personalised nutrition are components of the concept that in the future genotyping will be used as a means of defining dietary recommendations to suit the individual. Over the last two decades there has been an explosion of research in this area, with often conflicting findings reported in the literature. Reviews of the literature in the area of apoE genotype and cardiovascular health, apoA5 genotype and postprandial lipaemia and perilipin and adiposity are used to demonstrate the complexities of genotype-phenotype associations and the aetiology of apparent between-study inconsistencies in the significance and size of effects. Furthermore, genetic research currently often takes a very reductionist approach, examining the interactions between individual genotypes and individual disease biomarkers and how they are modified by isolated dietary components or foods. Each individual possesses potentially hundreds of 'at-risk' gene variants and consumes a highly-complex diet. In order for nutrigenetics to become a useful public health tool, there is a great need to use mathematical and bioinformatic tools to develop strategies to examine the combined impact of multiple gene variants on a range of health outcomes and establish how these associations can be modified using combined dietary strategies.

Background and aims: Epidemiological evidence indicates that cereal dietary fibre (DF) may have several cardiovascular health benefits. The underlying mechanisms have not yet been elucidated. Here, the potential nutritional effects of physico-chemical. properties modifications of durum wheat dietary fibre (DWF) induced by enzyme treatment have been investigated. Methods and results: The conversion of the highly polymerised insoluble dietary fibre into soluble feruloyl oligosaccharides of DWF was achieved by a tailored enzymatic treatment. The in vitro fermentation and release of ferulic acid by intestinal microbiota from DWF before and after the enzymatic treatment were assessed using a gut model validated to mimic the human colonic microbial environment. Results demonstrated that, compared to DWF, the enzyme-treated DWF (ETD-WF) stimulated the growth of bifidobacteria and lactobacilli. Concurrently, the release of free ferulic acid by ET-DWF was almost three times higher respect to the control. No effect on the formation of short chain fatty acids was observed. Conclusions: The conversion of insoluble dietary fibre from cereals into soluble dietary fibre generated a gut microbial fermentation that supported bifidobacteria and lactobacilli. The concurrent increase in free ferulic acid from the enzyme-treated DWF might result in a higher plasma ferulic acid concentration which could be one of the reasons for the health benefits reported for dietary fibre in cardiovascular diseases. The deleterious impact of cigarette smoking on cardiovascular health may be in part attributable to a free radical mediated proinflammatory response in circulating monocytes. In the current investigation, the impact of vitamin C supplementation on monocyte gene expression was determined in apoE4 smokers versus non-smokers. A total of 10 smokers and 11 non-smokers consumed 60 mg/day of vitamin C for four weeks and a fasting blood sample was taken at baseline and post-intervention for the determination of plasma vitamin C and monocyte gene expression profiles using cDNA array and real time PCR. In apoE4 smokers, supplementation resulted in a 43% increase in plasma vitamin C concentrations. Furthermore, a number of genes were differentially expressed more than 2-fold in response to treatment, including a downregulation of the proinflammatory mediators tumor necrosis factor (TNF) beta, TNF receptor, neurotrophin-3 growth factor receptor, and monocyte chemoattractant protein I receptor. The study has identified a number of molecular mechanisms underlying the benefit of vitamin C supplementation in smokers. (c) 2005 Elsevier Inc. All rights reserved.

Dietary isoflavones are currently receiving much attention because of their potential role in preventing coronary artery disease and other chronic diseases. Accumulating evidence from cell culture and laboratory animal experiments indicates that isoflavones have the potential to prevent or delay atherogenesis. Suggested mechanisms of action include: a reduction in low-density lipoprotein (LDL) cholesterol and a potential reduction in the susceptibility of the LDL particle to oxidation; (2) an improvement in vascular reactivity; (3) an inhibition of pro-inflammatory cytokines, cell adhesion proteins and nitric oxide (NO) production; and (4) an inhibition of platelet aggregation. These mechanisms are consistent with the epidemiological evidence that a high consumption of isoflavone-rich soy products is associated with a reduced incidence of coronary artery disease. Biological effects of isoflavones are dependent on many factors, including dose consumed, duration of use, protein-binding affinity, and an individual's metabolism or intrinsic oestrogenic state. Further clinical studies are necessary to determine the potential health effects of isoflavones in specific population groups as we currently know little about age-related differences in exposure to these compounds and there are few guidelines on optimal dose for cardiovascular health benefits.

The UK Food Standards Agency convened a group of expert scientists to review current research investigating whether n-3 polyunsaturated fatty acids (PUFA) from plant oils (a-linolenic acid; ALA) were as beneficial to cardiovascular health as the n-3 PUFA from the marine oils, eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). The workshop also aimed to establish priorities for future research. Dietary intake of ALA has been associated with a beneficial effect on CHD; however, the results from studies investigating the effects of ALA supplementation on CHD risk factors have proved equivocal. The studies presented as part of the present workshop suggested little, if any, benefit of ALA, relative to linoleic acid, on risk factors for cardiovascular disease; the effects observed with fish-oil supplementation were not replicated by ALA supplementation. There is a need, therefore, to first prove the efficacy of ALA supplementation on cardiovascular disease, before further investigating effects on cardiovascular risk factors. The workshop considered that a beneficial effect of ALA on the secondary prevention of CHD still needed to be established, and there was no reason to look further at existing CHD risk factors in relation to ALA supplementation. The workshop also highlighted the possibility of feeding livestock ALA-rich oils to provide a means of increasing the dietary intake in human consumers of EPA and DHA.

Epidemiological studies have shown an inverse relationship between risk of CVD and intake of whole grain (WG)-rich food. Regular consumption of breakfast cereals can provide not only an increase in dietary WG but also improvements to cardiovascular health. Various mechanisms have been proposed, including prebiotic modulation of the colonic microbiota. In the present study, the prebiotic activity of a maize-derived WG cereal (WGM) was evaluated in a double-blind, placebo-controlled human feeding study (n 32). For a period of 21 d, healthy men and women, mean age 32 (sd 8) years and BMI 23·3 (sd 0·58) kg/m2, consumed either 48 g/d WG cereal (WGM) or 48 g placebo cereal (non-whole grain (NWG)) in a crossover fashion. Faecal samples were collected at five points during the study on days 0, 21, 42, 63 and 84 (representing at baseline, after both treatments and both wash-out periods). Faecal bacteriology was assessed using fluorescence in situ hybridisation with 16S rRNA oligonucleotide probes specific for Bacteroides spp., Bifidobacterium spp., Clostridium histolyticum/perfringens subgroup, Lactobacillus–Enterococcus subgroup and total bacteria. After 21 d consumption of WGM, mean group levels of faecal bifidobacteria increased significantly compared with the control cereal (P = 0·001). After a 3-week wash-out period, bifidobacterial levels returned to pre-intervention levels. No statistically significant changes were observed in serum lipids, glucose or measures of faecal output. In conclusion, this WG maize-enriched breakfast cereal mediated a bifidogenic modulation of the gut microbiota, indicating a possible prebiotic mode of action

Endothelial dysfunction and an associated increase in vascular tone are risk factors for cardiovascular disease and highly predictive of future cardiovascular events. A part of the benefits associated with increased intake of the long chain (LC) n-3 polyunsaturated fatty acids (PUFA), eicosapentaenoic acid and docosahexaenoic acid, found in fish oils is a positive impact on cardiovascular health. Here, the recent evidence from human observational and intervention trials are reviewed, and an insight into potential mechanisms underlying the impact of LC n-3 PUFA on vascular reactivity is provided.

CVD are the leading cause of death worldwide. Hypertension, a major controllable risk factor of CVD, is intimately associated with vascular dysfunction, a defect which is also now recognised to be a major, modifiable risk factor for the development of CVD. The purpose of the present review was to critically evaluate the evidence for the effects of milk proteins and their associated peptides on blood pressure (BP) and vascular dysfunction. After a detailed literature search, the number of human trials evaluating the antihypertensive effects of casein-derived peptides (excluding isoleucine-proline-proline and valine-proline-proline) was found to be limited; the studies were preliminary with substantial methodological limitations. Likewise, the data from human trials that examined the effects of whey protein and peptides were also scarce and inconsistent. To date, only one study has conducted a comparative investigation on the relative effects of the two main intact milk proteins on BP and vascular function. While both milk proteins were shown to reduce BP, only whey protein improved measures of arterial stiffness. In contrast, a growing number of human trials have produced evidence to support beneficial effects of both milk proteins and peptides on vascular health. However, comparison of the relative outcomes from these trials is difficult owing to variation in the forms of assessment and measures of vascular function. In conclusion, there is an accumulating body of evidence to support positive effects of milk proteins in improving and/or maintaining cardiovascular health. However, the variable quality of the studies that produced this evidence, and the lack of robust, randomised controlled intervention trials, undermines the formulation of firm conclusions on the potential benefits of milk proteins and peptides on vascular health.

The complex relationship between flavonoid-based nutrition and cardiovascular disease may be dissected by understanding the activities of these compounds in biological systems. The aim of the present study was to explore a hierarchy for the importance of dietary flavonoids on cardiovascular health by examining the structural basis for inhibitory effects of common, dietary flavonoids (quercetin, apigenin, and naringenin) and the plasma metabolite, tamarixetin. Understanding flavonoid effects on platelets in vivo can be informed by investigations of the ability of these compounds to attenuate the function of these cells. Inhibition of platelet function in whole blood and plasma was structure-dependent. The order of potency was apigenin > tamarixetin > quercetin = naringenin indicating that in vivo, important functional groups are potentially a methylated B ring, and a non-hydroxylated, planar C ring. Apigenin and the methylated metabolite of quercetin, tamarixetin significantly reduced thrombus volume at concentrations (5 μM) that suggested their reported physiological levels (0.1-1 μM) may exert low levels of inhibition. Flavonoid interactions with erythrocytes, leukocytes and human serum albumin in whole blood reduce their inhibitory activities against platelet function. The diminished inhibitory activity of flavonoids that we observed in whole blood and plasma indicated that these interactions do not overcome the attenuating effects of these compounds. Furthermore, inhibition of platelet aggregation by flavonoids was enhanced with increases in exposure time, indicating the potential for measurable inhibitory effects during resident plasma times. We conclude that flavonoid structures may be a major influence of their activities in vivo with methylated metabolites and those of flavones being more potent than those of flavonols and flavanones.
