In the United States, composites accounted for nearly 70% of the 173.2 million composite and amalgam restorations placed in 2006 (Kingman et al., 2012), and it is likely that the use of composite will continue to increase as dentists phase out dental amalgam. This trend is not, however, without consequences. The failure rate of composite restorations is double that of amalgam (Ferracane, 2013). Composite restorations accumulate more biofilm, experience more secondary decay, and require more frequent replacement. In vivo biodegradation of the adhesive bond at the composite-tooth interface is a major contributor to the cascade of events leading to restoration failure. Binding by proteins, particularly gp340, from the salivary pellicle leads to biofilm attachment, which accelerates degradation of the interfacial bond and demineralization of the tooth by recruiting the pioneer bacterium Streptococcus mutans to the surface. Bacterial production of lactic acid lowers the pH of the oral microenvironment, erodes hydroxyapatite in enamel and dentin, and promotes hydrolysis of the adhesive. Secreted esterases further hydrolyze the adhesive polymer, exposing the soft underlying collagenous dentinal matrix and allowing further infiltration by the pathogenic biofilm. Manifold approaches are being pursued to increase the longevity of composite dental restorations based on the major contributing factors responsible for degradation. The key material and biological components and the interactions involved in the destructive processes, including recent advances in understanding the structural and molecular basis of biofilm recruitment, are described in this review. Innovative strategies to mitigate these pathogenic effects and slow deterioration are discussed.

This paper reports a master research that aimed to analyze the patterns of institutional relationships between local officials and communities of agrarian reform settlements in four counties of the State of São Paulo – Apiaí, Iaras, Promissão and Mirante do Paranapanema – particularly with regard to the types of support offered by the first to the economic activities in the settlements. In terms of methodology, we conducted a qualitative, exploratory and descriptive research, outlined in the format of multiple case study. Having like data sources the local officials responsible for institutional relations with the agrarian reform settlements, technicians agents of agencys linkeds to the land question, as INCRA and ITESP, and settled in leadership positions in rural settlements, we sought to evaluate the intensity, the types and quality of the (possible or actual) support offered by municipalities to economic activities inside the settlements. We sought to evaluate the results from theoretical lens formed around the notions of local development and the significance assumed by settlements in different locations. The results showed a fairly heterogeneous and, often, disparate in particular circumstances. It was evident, from the information collected, that a positive action of the municipal government in supporting the economic activities of the settlements is essential for the release of social energy based in settlements, enabling the realization of the "amalgam of possibilities" constructed from the implementation of rural settlements.

Due to mechanical and aesthetic improvement properties, continuous fiber-reinforced composites have been developed to replace the metal framework in fixed partial dentures becoming an interesting alternative to conventional treatments. A male patient, 57 years old, attended at Fixed Partial Denture Clinic of Araraquara Dental School - UNESP, complaining about upper right first molar absence. After clinical examination, it was observed: upper right second molar with amalgam restoration and periodontal bone reduction and upper right second premolar unsatisfactory treated. Following the clinical conditions and the patient expectations, it was decided to use a fiber-reinforced composite resin to make a three-element fixed bridge. The patient showed full satisfaction with the aesthetic and functional results. The case has been followed up for 60 months.

AIM: To evaluate the adherence of Streptococcus mutans to the surface of the amalgam and copper/aluminum alloy samples and also evaluate the release of metallic ions. METHODS: The prepared medium was changed every 72 h and analyzed by atomic absorption spectrophotometer. Samples were removed from the prepared medium at 15, 30, 48 and 60 days. RESULTS: The result shows that ions released were statistically different among all groups, and so were both biofilm and pits formation and the corrosion induced by the S. mutans in both types of samples. SEM observation of the samples immersed in the prepared medium with S. mutans showed adherence of microorganisms on the whole surface, in all groups. CONCLUSIONS: The S. mutans adhere to both amalgam and copper/aluminum alloy causing corrosion of those restorations. S. mutans produced a greater ions release in Cu/Al alloy; in amalgam, the ions release was not influenced by exposure to S. mutans.

Composite restorations have higher failure rates, more recurrent caries and increased frequency of replacement as compared to dental amalgam. Penetration of bacterial enzymes, oral fluids, and bacteria into the crevices between the tooth and composite undermines the restoration and leads to recurrent decay and failure. The gingival margin of composite restora tions is particularly vulnerable to decay and at this margin, the adhesive and its seal to dentin provides the primary barrier between the prepared tooth and the environment. The intent of this article is to examine physico-chemical factors that affect the integrity and durability of the adhesive/dentin interfacial bond; and to explore how these factors act synergistically with mechanical forces to undermine the composite restoration. The article will examine the various avenues that have been pursued to address these problems and it will explore how alterations in material chemistry could address the detrimental impact of physico-chemical stresses on the bond formed at the adhesive/dentin interface.

The purpose of this study was to evaluate the apical sealing ability and the marginal adaptation of five dental materials used in retrofillings or applied to the bevelled root surface. One hundred and forty extracted single rooted human teeth were used, divided into seven groups of twenty each. ln the first, second, third and fourth groups, the teeth were apicoectomized, submitted to cavity preparations and retrofilled with one of the following materiais: zinc free silver amalgam, a dentin bonding system plus composite resin, a glass ionomer cement ora compomer. In the fifth, sixth and seventh groups, the teeth were apicoectomized and capped on the bevelled root surface with one of the following materiais: a dentin bonding system plus composite resin, a glass ionomer cement or a compomer. Two specimens of each experimental group were evaluated for the marginal adaptation using scanning electron microscopy. The remaining 126 specimens were immersed in 2% methylene blue dye, stored for one week at 37ºC and the infiltration was evaluated with a stereomicroscope. The results showed that the glass ionomer cement presented the lowest values of marginal infiltration when used as retrofilling material, with a significant statistical difference when compared with the others tested materials. When used as apical capping, the glass ionomer cement and the compomer were equivalent and significantly better than the dentin bonding system plus composite resin. Using scanning electron microscopy, all the materials showed some slight adjustment problem. ln the retrofilling, the smallest marginal gaps were observed with the compomer and the dentin bonding system plus composite resin, while the largest were observed with the glass ionomer cement and zinc free silver amalgam. ln the apical capping, the smallest marginal gaps were observed with the compomer and the dentin bonding system plus composite resin and ...

The activation of aluminum surface has the most various purposes as for example the search for the surface activation mechanism and the corrosion products by mercury ions. The objective of this work is to study the reactivity of the surface of aluminum metal when activated by mercury ions (Hg2+), with the consequent formation of an Al-Hg amalgam. Results demonstrate that the kinetics of the reaction, by measuring the mass change with time of the corrosion product formed between Al and Hg, and analysis by infrared spectroscopy (IR) that the product of the reaction between the amalgam, located on the surface, and the atmospheric oxygen is Al2O3 (aluminum hydroxide). The results also indicate that the kinetics of the reaction between the amalgam (Hg-Al) and atmospheric oxygen is of first order and reach a region where there is no more formation of product

Purpose: To investigate the periapical tissue response of 4 different retrograde root-filling materials, ie, intermediate restorative material, thermoplasticized gutta-percha, reinforced zinc oxide cement (Super-EBA), and mineral trioxide aggregate (MTA), in conjunction with an ultrasonic root-end preparation technique in an animal model. Materials and Methods: Vital roots of the third and fourth right mandibular premolars in 6 healthy mongrel dogs were apicectomized and sealed with 1 of the materials using a standardized surgical procedure. After 120 days, the animals were sacrificed and the specimens were analyzed radiologically, histologically, and scanning electron microscopically. The Fisher exact test was performed on the 2 outcome values. Results: Twenty-three sections were analyzed histologically. Evaluation showed better re-establishment of the periapical tissues and generally lower inflammatory infiltration in the sections from teeth treated with the intermediate restorative material and the MTA. New root cement on the resected dentin surfaces was seen on all sections regardless of the used material. New hard tissue formation, directly on the surface of the material, was seen only in the MTA sections. There was no statistical difference in outcome among the tested materials. Conclusions: The results from this dog model favor the intermediate restorative material and MTA as retrograde fillings when evaluating the bone defect regeneration. MTA has the most favorable periapical tissue response when comparing the biocompatibility of the materials tested. Objectives. To verify the hypothesis that crack analysis and a mechanical test would rank a series of composites in a similar order with respect to polymerization stress. Also, both tests would show similar relationships between stress and composite elastic modulus and/or shrinkage. Methods. Soda-lime glass discs (2-mm thick) with a central perforation (3.5-mm diameter) received four Vickers indentations 500 mu m from the cavity margin. The indent cracks were measured (500x) prior and 10 min after the cavity was restored with one of six materials (Kalore/KL, Gradia/GR, Ice/IC, Wave/WV, Majesty Flow/MF, and Majesty Posterior/MP). Stresses at the indent site were calculated based on glass fracture toughness and increase in crack length. Stress at the bonded interface was calculated using the equation for an internally pressurized cylinder. The mechanical test used a universal testing machine and glass rods (5-mm diameter) as substrate. An extensometer monitored specimen height (2 mm). Nominal stress was calculated dividing the maximum shrinkage force by the specimen cross-sectional area. Composite elastic modulus was determined by nanoindentation and post-gel shrinkage was measured using strain gages. Data were subjected to one-way ANOVA/Tukey or Kruskal-Wallis/Mann-Whitney tests (alpha: 5%). Results. Both tests grouped the composites in three statistical subsets, with small differences in overlapping between the intermediate subset (MF, WV) and the highest (MP, IC) or the lowest stress materials (KL, GR). Higher stresses were developed by composites with high modulus and/or high shrinkage. Significance. The available options for restoring multiple surface cavities are: amalgam, composite resin, or indirect restorations. Adhesive system and intradentinal pin-retained composite resin restorations should have a similar performance to pin-retained amalgam, regarding resistance to support occlusal forces. Polymerization shrinkage is a major concern when performing direct posterior composite resin restorations and the incremental insertion technique can provide less stress and outstanding margin behavior. Intradentinal pins can potentially enhance composite resin's retention, while reducing gaps caused by polymerization shrinkage. This article reports a clinical case involving an extensive restoration on a posterior tooth with cusp loss that was successfully treated using an intradentinal pin and direct nano-hybrid composite resin restoration.
