The Building the Community-Pharmacy Partnership has worked to improve local health outcomes by encouraging members of the community to develop skills relating to managing their own health, and providing accessible information on the availability of services of which they can avail of. The aim is to facilitate local people to gain knowledge on various health issues through books, videos, leaflets and enable the most vulnerable and in need to access additional support through the pharmacy to complementary therapy. Outcome: A room in the pharmacy has been renovated and offers access to other support services. Funding has allowed the pharmacist to equip the room with videos, leaflets, books etc. There has been an increase in partnership working between the community group, pharmacist and more contact has been made with primary care. Further funding has allowed for the development of a subsidised support referral service for counselling and complementary therapies. As well as this, the pharmacist has worked with local groups to provide information sessions on lifestyle and on health issues identified by the local groups

This project will build on previous work carried out over the last 3 years. There are 4 main aspects: - intergenerational work focusing on the development of a vegetable garden with pharmacy input on health advice - develop and promote the lunch club which has been very successful to date and has targeted people previously not accessing support or community services in the village - annual health fair - local carer’s advice centre to provide practice information and signposting services. This last aspect is the result of ongoing identification of the need to support carers over the last 2 years of the BCPP project. This project will be driven forward by a lay health worker working closely with the pharmacist Outcome: The project built on previous work. Some of the work included: health fayre, intergenerational garden, and luncheon club. Results from questionnaires showed that the project increased people's awareness of the causes of ill-health and how they might deal with them. People now see the pharmacist as a local and not just as someone dispensing medicine.

This has been one of the first opportunities to get health on the agenda in the rural area of Larne. This builds on the work and experience of a previously funded BCPP project in Larne town. This project will carry out information sessions in order to; facilitate discussion, disseminate information and identify relevant health issues. The project will mainly target women in the rural areas of the Larne Borough. Other aims of the project include; supporting local women to share health message to the wider community. This project will mainly be based in the pharmacy, with the pharmacist working with the women to identify how the can develop additional rooms in the pharmacy to meet local community health needs

To address diet and lifestyle issues and improve the health of the over 50s in Gilford. Outcome: Health related issues for example heart disease, osteoporosis and diabetes were raised and members became more aware of their health, lifestyle and specifically diet through individual consultations with pharmacist and nutritionist and through group sessions. The pharmacist has worked well with the GP and is broadening links with other support agencies eg CAB. The GP / Pharmacist relationship has strengthened.

The key element of the HSE South’s Programme is to enhance and develop community mental health services in Carlow, Kilkenny and South Tipperary, to enable the service user to remain in the community to the greatest extent possible. HSE South has prioritised the implementation of the change programme and has allocated more than €20m capital funding and over €1.75m revenue funding to support this comprehensive development programme. Speaking at the briefings Mr. Pat Healy, Regional Director of Operations, HSE South said, “When this plan is delivered, clients will have access to the highest standards of services in all three counties, which should significantly improve these clients’ treatment programmes and quality of life. The National Service Users Executive are supporting the change programme, which is of immense importance to HSE South. The programme heralds the enhancement and development of community mental health services, the closure of old long stay institutions, the separation of North and South Tipperary acute inpatient mental health services and development of appropriate acute inpatient services, for the extended catchment area, in line with the national strategy for mental health “A Vision for Change”. Footprints is developing a Transition Community Initiative which will be another distinct area of work within the Healthy Living Project, as part of the group's ongoing endeavours to influence change within the Colin community, tackling health inequalities. The Building a Transition Community Initiative oversees the development of the grounds at Footprints Women's Centre and offers food growing training to local residents who will be encouraged to volunteer in the garden. A local gardener has been hired and oversees the development and training of the volunteers. The project will grow fruit and vegetables within the grounds of Footprints Women's Centre and this produce will be used within the Footprints Catering Services. Any supplies surplus to requirements will be used in schools in the neighbourhood. Skills learned by the volunteers will also be transferred to their gardens at home throughout the Colin neighbourhood. The aim is also to demonstrate how participation in this project will increase and improve the physical activity levels and mental wellbeing of residents. The third annual report from the World Health Organisation's (WHO) Healthy Promoting Hospitals (HPH) and Healthy Services network highlights a rich selection of the innovative developments and team-working achievements across services in Northern Ireland. The report provides a platform to showcase the five Health and Social Care Trusts and Cooperation and Working Together (CAWT)��'s commitment to health and wellbeing to the population and shows how hospitals can have an impact on the determinants of health as they are explained in the context of people��'s daily lives.

BACKGROUND: Despite the progress over recent decades in developing community mental health services internationally, many people still receive treatment and care in institutional settings. Those most likely to reside longest in these facilities have the most complex mental health problems and are at most risk of potential abuses of care and exploitation. This study aimed to develop an international, standardised toolkit to assess the quality of care in longer term hospital and community based mental health units, including the degree to which human rights, social inclusion and autonomy are promoted. METHOD: The domains of care included in the toolkit were identified from a systematic literature review, international expert Delphi exercise, and review of care standards in ten European countries. The draft toolkit comprised 154 questions for unit managers. Inter-rater reliability was tested in 202 units across ten countries at different stages of deinstitutionalisation and development of community mental health services. Exploratory factor analysis was used to corroborate the allocation of items to domains. Feedback from those using the toolkit was collected about its usefulness and ease of completion. RESULTS: The toolkit had excellent inter-rater reliability and few items with narrow spread of response. Unit managers found the content highly relevant and were able to complete it in around 90 minutes. Minimal refinement was required and the final version comprised 145 questions assessing seven domains of care. CONCLUSIONS: Triangulation of qualitative and quantitative evidence directed the development of a robust and comprehensive international quality assessment toolkit for units in highly variable socioeconomic and political contexts

OBJECTIVES: Within a strong interdisciplinary framework, improvement in the quality of care for children with autistic spectrum disorders through a 2 year implementation program of Practice Parameters, aimed principally at improving early detection and intervention. METHOD: We developed Practice Parameters (PPs) for Pervasive Developmental Disorders and circulated the PPs to all child and adolescent psychiatrists practicing in the region. RESULTS: PP development and parallel information strategies resulted in a significant decrease of 1.5 years in the mean-age-at-diagnosis. However, further analysis indicated that improvement was only transient. CONCLUSION: Despite the encouraging improvement in mean-age-at-diagnosis 2 years after PP implementation, other indicators showed a failure to maintain the improvements. A systematic screening program would be the most reliable method to reinforce the PPs.

Family impact (or family burden) is a concept born in the field of mental health that has successfully been exported to the ambit of intellectual disability (ID). However, differences in family impact associated with severe mental health disorders (schizophrenia), to ID or to mental health problems in ID should be expected. Seventy-two adults with intellectual disability clients of the Carmen Pardo-Valcarce Foundation's sheltered workshops and vocational employment programmes in Madrid (Spain), 203 adults diagnosed with schizophrenia from four Spanish Community Mental Health Services (Barcelona, Madrid, Granada and Navarra) and 90 adults with mental health problems in ID (MH-ID) from the Parc Sanitari Sant Joan de Déu Health Care Site in Sant Boi de Llobregat, Barcelona (Spain) were asked to participate in the present study along with their main caregivers. Family impact experienced by caregivers was assessed with the ECFOS-II/SOFBI-II scale (Entrevista de Carga Familiar Objetiva y Subjetiva/Objective and Subjective Family Burden Interview). In global terms, results showed that the higher family impact was found between caregivers to people with MH-ID. The interaction of both conditions (ID and mental health problems) results in a higher degree of burden on families than when both conditions are presented separately. There was also an impact in caregivers to people with schizophrenia, this impact being higher than the one detected in caregivers to people with intellectual disability. Needs of caregivers to people with disability should be addressed specifically in order to effectively support families.

Con la llegada del nuevo paradigma del “envejecimiento activo” planteado por la Organización Mundial de la Salud (OMS, 2002), estamos asistiendo a una nueva manera de enfocar el progresivo envejecimiento de la población. Este enfoque trata de fomentar la consecución de una esperanza de vida saludable y libre de discapacidad, gracias a la promoción de políticas y programas cuyos objetivos principales se centran en la independencia y en la autonomía a lo largo del proceso de envejecimiento. Acorde con este enfoque pensamos que las ocupaciones significativas son una buena herramienta para mantener la autonomía de estas personas al fomentar la prevención y la promoción de su salud, motivo por el cual desarrollamos este estudio. Los objetivos principales del presente estudio son conocer y comprender que ocupaciones adquieren un significado y satisfacción personal en las personas mayores-jóvenes, que han concluido su etapa laboral, que cuentan con buena salud y condiciones psicofísicas favorables y que viven de manera autónoma en la comunidad, así como explorar también si se establece en estas personas alguna vinculación entre las actividades que eligen desarrollar en la actualidad con su perfil e historia ocupacional previa. Por último, este estudio pretende conocer el tipo de gestión que cada persona prefiere para llevar a cabo las actividades cotidianas que les son más significativas, a partir de los contextos en los que se inscribe. El marco teórico se desarrolla a partir de la conceptualización aportada por diversos autores sobre el envejecimiento activo y la ciencia de la ocupación. La metodología utilizada es de carácter cualitativo, enfocado desde un paradigma constructivista. Los resultados del estudio muestran que se inicia tímidamente un cambio de tendencias en la elección y el significado que las personas mayores-jóvenes atribuyen a sus ocupaciones. Muestran también que las ocupaciones elegidas como más significativas por estas personas mantienen una vinculación con las que ya realizaban con anterioridad a la jubilación, con su historia o perfil 6 ocupacional previo. Y finalmente demuestran que el tipo de gestión que eligen para efectuarlas influye en la elección del entorno donde las llevan a término.

BACKGROUND: The impact of abnormal spirometric findings on risk for incident heart failure among older adults without clinically apparent lung disease is not well elucidated.METHODS: We evaluated the association of baseline lung function with incident heart failure, defined as first hospitalization for heart failure, in 2125 participants of the community-based Health, Aging, and Body Composition (Health ABC) Study (age, 73.6 +/- 2.9 years; 50.5% men; 62.3% white; 37.7% black) without prevalent lung disease or heart failure. Abnormal lung function was defined either as forced vital capacity (FVC) or forced expiratory volume in 1(st) second (FEV1) to FVC ratio below lower limit of normal. Percent predicted FVC and FEV1 also were assessed as continuous variables.RESULTS: During follow-up (median, 9.4 years), heart failure developed in 68 of 350 (19.4%) participants with abnormal baseline lung function, as compared with 172 of 1775 (9.7%) participants with normal lung function (hazard ratio [HR] 2.31; 95% confidence interval [CI], 1.74-3.07; P <.001). This increased risk persisted after adjusting for previously identified heart failure risk factors in the Health ABC Study, body mass index, incident coronary heart disease, and inflammatory markers (HR 1.83; 95% CI, 1.33-2.50; P <.001). Percent predicted (%) FVC and FEV 1 had a linear association with heart failure risk (HR 1.21; 95% CI, 1.11-1.32 and 1.18; 95% CI, 1.10-1.26, per 10% lower % FVC and % FEV1, respectively; both P <.001 in fully adjusted models). Findings were consistent in sex and race subgroups and for heart failure with preserved or reduced ejection fraction.CONCLUSIONS: Abnormal spirometric findings in older adults without clinical lung disease are associated with increased heart failure risk. The purpose of this qualitative study was to understand the client and occupational therapist experiences of a mental health group. A secondary aim was to explore the extent to which this group seemed to have reflected a client-centred approach. The topic emerged from personal and professional issues related to the therapist as teacher and to inconsistencies in practice with the profession's client-centred philosophy. This philosophy, the study's frame of reference, was established in terms of themes related to the client-therapist relationship and to client values. Typical practice was illustrated through an extensive literature review. Structured didacticexperiential methods aiming toward skill development were predominant. The interpretive sciences and, to a lesser extent, the critical sciences directed the methodology. An ongoing support group at a community mental health clinic was selected as the focus of the study; the occupational therapist leader and three members became the key participants. A series of conversational interviews, the . core method of data collection, was supplemented by observation, document review, further interviews, and fieldnotes. Transcriptions of conversations were returned to participants for verification and for further reflection Analysis primarily consisted of coding and organizing data according to emerging themes. The participants' experiences of group, presented as narrative stories within a group session vignette, were also returned to participants. There was a common understanding of the group's structure and the importance of having "air time" within the group; however, differences in perceptions of such things as the importance of the group in members' lives were noted. All members valued the therapeutic aspects of group, the role of group as weekly activity and, to a lesser extent, the learning that came from group. The researcher's perspective provided a critique of the group experience from a client-centred perspective. Some areas of consistency with client-centred practice were noted (e.g., therapist attitudes); however the group seemed to function far from a client-centred ideal. Members held little authority in a -relationship dominated by the leaders, and leader agendas rather than member values controlled the session. Possible reasons for this discrepancy ranging from past health care encounters through to co-leader discord emerged. The actual and potential significance of this study was discussed according to many areas of implications: to OT practice, especially client-centred group practice, to theory development, to further areas of research and methodology considerations, to people involved in the group and to my personal growth and development.

This thesis explores Aboriginal women's access to and success within universities through an examination of Aboriginal women's educational narratives, along with input from key service providers from both the Aboriginal and non-Aboriginal community. Implemented through the Wildfire Research Method, participants engaged in a consensusbased vision of accessible education that honours the spiritual, emotional, intellectual, and physical elements necessary for the success of Aboriginal women in university. This study positions Aboriginal women as agents of social change by allowing them to define their own needs and offer viable solutions to those needs. Further, it connects service providers from the many disconnected sectors that implicate Aboriginal women's education access. The realities of Aboriginal women are contextualized through historical, sociocultural, and political analyses, revealing the need for a decolonizing educational approach. This fosters a shift away from a deficit model toward a cultural and linguistic assets based approach that emphasizes the need for strong cultural identity formation. Participants revealed academic, cultural, and linguistic barriers and offered clear educational specifications for responsive and culturally relevant programming that will assist Aboriginal women in developing and maintaining strong cultural identities. Findings reveal the need for curriculum that focuses on decolonizing and reclaiming Aboriginal women's identities, and program outcomes that encourage balance between two worldviews-traditional and academic-through the application of cultural traditions to modern contexts, along with programming that responds to the immediate needs of Aboriginal women such as childcare, housing, and funding, and provide an opportunity for universities and educators to engage in responsive and culturally grounded educational approaches.
