Background The pattern of associations and the attributable fractions (AF) of atopic conditions due to specific sensitizations vary between countries. Objective To assess the level of associations and AF between sensitization to five allergens and atopic conditions in two settings. Methods We studied 2063 Brazilians and 1231 Chileans of both sexes using representative samples selected at birth in the 1970s. Information on asthma and rhinitis was based on the European Community Respiratory Health Survey questionnaire. We assessed bronchial hyperresponsiveness (BHR) to methacholine and sensitization to Dermatophagoides pteronyssinus, cat, dog, grass blend and Alternaria alternata. Results The prevalence of sensitization to one or more allergens was 50% in Brazilians and 22% in Chileans. The level of associations varied according to the outcome used. Strong associations between sensitization and asthma, defined as wheeze or awakening with breathlessness at night and positive BHR, were found for each of the five allergens in Chileans [varying from odds ratio (OR) 3.24, 95% confidence interval (CI) 1.47, 7.15 for D. pteronyssinus to 8.44, 95% CI 3.82, 18.66 for cat], whereas the level of associations was restricted to D. pteronyssinus, cat and dog in Brazilians and was somewhat weaker (highest OR 3.90, 95% CI 2.80-5.44). The AF of sensitization on asthma was 54% in Brazil and 44% in Chile. D. pteronyssinus and cat made an independent contribution to asthma in the two samples. The patterns of associations between sensitization and rhino-conjunctivitis were similar to those for asthma. Conclusion The associations between sensitization, and asthma and rhinitis were high in Chile and moderately high in Brazil, but the AF were higher in Brazil, reflecting a higher prevalence of sensitization. In Brazil, dust mite had the greatest impact on atopic conditions while in Chile several allergens had an impact. Sensitization is as serious a problem in Chile and Brazil as in developed countries.

Background: Asthma medication places patients at risk of dental erosion by reducing salivary protection against extrinsic or intrinsic acids. But patterns of lesions in asthmatics may differ from patterns in non-asthmatics, because gastro-oesophageal reflux (GOR) is found in 60 per cent of asthmatics. Methods: The lesions in 44 asthma cases were compared to those of age and sex match controls with no history of asthma or medications drawn from the dental records of 423 patients referred concerning excessive tooth wear. The subjects were 70 males age range 15 to 55 years and 18 females age range 18 to 45. Anamnestic clinical data were compared between the two groups. Models of all 88 subjects were examined by light microscopy, and wear patterns were recorded on permanent central incisor, canine, premolar and first molar teeth. Results: Clinical differences were a higher incidence of tooth hypersensitivity; xerostomia, salivary gland abnormalities, gastric complaints, and self induced vomiting in the cases. No differences were found between the cases and controls on citrus fruit and acid soft drink consumption. More occlusal erosion sites were found in cases, whereas more attrition sites were found in the controls. There were no significant differences in palatal erosion on maxillary anterior teeth found between cases and controls. Lingual erosion of the mandibular incisors, found only in GOR patients, was not observed. Conclusions: A higher incidence of erosion was found in asthmatics. Gastro-oesophageal reflux symptoms were not associated with the sign of lingual mandibular incisor erosion. The clinical significance is that asthmatics are at risk of dental erosion from extrinsic acid, but GOR does not appear to contribute in a site-specific manner.

Seventy four asthmatic children aged 7 to 11 years were examined along with controls matched by age and sex. Clinical and laboratory investigations preceded a 28-day follow-up where data about morning and evening peak expiratory flow rate (PEF), symptoms and treatment were recorded. The coefficient of variation of PEF was found to be an objective measurement of asthma severity that has statistically significant correlation with both symptoms (r s= .36) and treatment (r s= .60). Moreover, it separates mild and severe asthmatics, as confirmed by statistically significant differences (p= .008 or less) in symptoms, treatment, skin allergy and airways response to exercise. Skin allergy and airways responsiveness to exercise were found to be predictors of both disease and severity. By means of logistic regression analysis it was possible to establish the probabilities for both asthma and severe asthma when children presenting and not presenting these characteristics are compared. One single positive skin test represent a probability of 88% for the development of asthma and a probability of 70% for severe disease. A PEF reduction of 10% after an exercise test implies a probability of 73% for disease and a probability of 64% for severe disease. Increases in these variables imply geometrically increased risks and their presence together have a multiplicative effect in the final risk.

OBJECTIVE: A case-control study of patients with pneumonia was conducted to investigate whether wheezing diseases could be a risk factor. METHODS: A random sample was taken from a general university hospital in S. Paulo City between March and August 1994 comprising 51 cases of pneumonia paired by age and sex to 51 non-respiratory controls and 51 healthy controls. Data collection was carried out by two senior paediatricians. Diagnoses of pneumonia and presence of wheezing disease were independently established by each paediatrician for both cases and controls. Pneumonia was radiologically confirmed and repeatability of information on wheezing diseases was measured. Logistic regression analysis was used to identify risk factors. RESULTS: Wheezing diseases, interpreted as proxies of asthma, were found to be an important risk factor for pneumonia with an odds ratio of 7.07 (95%CI= 2.34-21.36), when the effects of bedroom crowding (odds ratio = 1.49 per person, 95%CI= 0.95-2.32) and of low family income (odds ratio = 5.59 against high family income, 95%CI= 1.38-22.63) were controlled. The risk of pneumonia attributable to wheezing diseases is tentatively calculated at 51.42%. CONCLUSION: It is concluded that at practice level asthmatics should deserve proper surveillance for infection and that at public health level pneumonia incidence could be reduced if current World Health Organisation's guidelines were reviewed as to include comprehensive care for this illness.

Introdução: Embora existam estratégias para coordenar as funções postural e ventilatória numa situação normal, isto pode não ser verdade quando a necessidade para uma das funções está aumentada, como por exemplo em patologia respiratória (asma) ou no exercício físico, em que subsistem maiores necessidades ventilatórias. O método Pilates, que foca a relação entre o corpo e a disciplina mental, visa prosperar a saúde e o bem-estar pelo enfatizar da boa postura, do alinhamento corporal e da coordenação da ventilação com o movimento. Objectivo: Comparar características de controlo motor e parâmetros ventilatórios em asmáticos controlados e indivíduos sem patologia, e verificar o efeito de um programa de exercícios segundo Pilates nesses outcomes em indivíduos com asma controlada. Métodos: Estudo quasi-experimental, com uma amostra constituída por 21 estudantes voluntários, 7 pertencentes ao “grupo sem patologia”, 7 ao “grupo controlo asmático” e 7 ao “grupo experimental asmático”. Para avaliação do timing de ativação e do padrão de recrutamento muscular no movimento rápido do membro superior foi utilizada eletromiografia de superfície do Diafragma, Eretor da Coluna, Multífidos, Oblíquo Externo, Reto Anterior e Transverso Abdominal/Oblíquo Interno. Foram também avaliados parâmetros de função ventilatória: a percentagem de volume expiratório forçado no primeiro segundo do previsto, o débito expiratório máximo instantâneo, a ventilação máxima voluntária, a pressão inspiratória máxima e a pressão expiratória máxima. As avaliações decorreram antes e após 8 semanas da aplicação de um programa de exercícios segundo Pilates no grupo experimental asmático, com exceção do grupo sem patologia que realizou apenas o primeiro momento de avaliação. Resultados: O grupo controlo asmático apresentou um timing de ativação significativamente maior do Transverso Abdominal/Oblíquo Interno e do Diafragma, em relação ao grupo sem patologia. Nos parâmetros ventilatórios, o grupo controlo asmático apresentou menores valores de percentagem de volume expiratório no primeiro segundo do previsto, de débito expiratório máximo instantâneo e de pressão expiratória máxima. Após a realização do programa de exercícios segundo Pilates verificaram-se alterações significativas no timing de activação do Eretor da Coluna, do Multífidos, do Transverso/Oblíquo Interno e do Diafragma, tendo ambos diminuído no grupo experimental asmático. Ainda, o grupo experimental asmático, em relação aos parâmetros ventilatórios, apresentou diferenças significativas no débito expiratório máximo instantâneo, na ventilação máxima voluntária e na pressão expiratória máxima, tendo ambos aumentado estes valores. Conclusão: Os asmáticos controlados parecem possuir características de controlo motor, especificamente no timing de ativação, e valores de parâmetros ventilatórios diferentes em comparação aos indivíduos sem patologia. O programa de exercícios segundo Pilates, implementado no grupo experimental asmático, parece ter influenciado positivamente esses outcomes.

Introduction: bronchial asthma is a chronic disease that affects a high percentage of adolescents, with a significant restriction of daily activities, and is a cause of school absenteeism. The relationships between adolescents and asthma disease in school were assessed, with a view to improving knowledge about the asthmatic adolescent. Methods: a survey was conducted in the Lisbon metropolitan area, covering urban (Lisbon) and rural (Lourinh˜a ) zones and including 1879 students and 81 teachers from the 7th to 9th high school years. The study groups were asthmatic students, their peers, and teachers. A self-administered questionnaire was applied to collect information. The results were compared with a reference group of 91 asthmatic students attending our Department of Immunoallergy-Hospital Dona Estefânia. Cotinine urinary measurements were made in a sample of asthmatics and a control group. Results: the prevalence of current asthma among students was 10%. Estimates of asthma annual burden among 7th to 9th year students from Lisbon and Lourinh˜a high schools included 4,307 days missed from school, 4,148 medical consultations and a minimum of 351 hospital emergency care and 80 hospital admissions. Exposure to passive smoking was not significantly different between asthmatic students and theirs peers. Cotinine urinary measurements did not discriminate between exposed and non-exposed individuals. Cigarette smoking was almost as common among adolescent asthmatics (5.4%) as it was in non-asthmatic subjects (6.7%). However, 55% of asthmatics mentioned active and passive smoking as an asthma exacerbating factor. Asthmatic students, theirs peers and teachers showed a deficient knowledge about asthma (mean group scores: 17.6; 14.2 and 17.7 of a possible 30), particularly in the areas related to asthma recognition and its management. Asthmatics attending our Allergy Department had the highest scores. All groups showed tolerance in the sense of a positive and understanding attitude toward a person with asthma. However, traditional beliefs about asthma disease (dependence, inferiority...) were confirmed. A positive correlation between knowledge levels and tolerance attitudes was found. Conclusion: in view of the dimension of the asthma problem in adolescence and its social and economic impact, it is justifiable to assess the need for the implementation of asthma education programs in schools in order to improve asthma management by the adolescents and their schools.

Background: Childhood asthma represents an increasing health problem and is the leading cause of hospital admission and absenteeism in children with chronic disease. It also compromises quality of life, eventually contributing to disturbances in self-concept. Self-concept is a recent and global perspective of “the self” and relates to skills, self-image and self-esteem. Little information is available on this topic and there are no data from Portuguese countryside towns. Objective: The aim of this study was to determine the prevalence of asthma among all school children in the 5th and 6th grades in a Portuguese countryside town and to establish its possible correlation with absenteeism and self-concept. Methods: In April 2002, two questionnaires were administered in the presence of the researcher to a group of 950 children attending different schools. The children completed the internationally renowned questionnaires: ISAAC and the Self-Concept Scale by Susan Harter. Results: Our sample (n = 818) had a mean age of 11 years (10-15 years) and a male-to-female ratio of 1/1. The cumulative prevalence of asthma was11.9% and that of active asthma was 8.8 %; 63.9% of asthmatics were male and 36.1 % were female. The mean age of asthmatics was 11.34 years and 74 % had active symptoms. Comparison of this group of 97 asthmatic children with the remaining children revealed a statistically significant correlation between the presence of asthma and school absenteeism (global: p = 0.04; gymnastics: 0.05). Regarding the Self-Concept Scale a statistically significant association was found between the presence of asthma and school achievement (p = 0.027), physical appearance (p = 0.015), behavior (p < 0.000) and self-esteem (p < 0.000). No statistically significant correlations were found in social acceptance (p = 0.289) or athletic competence (p = 0.085). Asthmatic boys had higher self-concept scores than girls, except in the domain of behavior. Conclusions: Twelve percent of the population studied was asthmatic. In asthmatic children, absenteeism was higher and self-concept was lower for almost all domains, except social acceptance and athletic achievement, probably due to overprotection.

Background: A new method for determining serum specific IgE (IMMULITE“ 2000 3gAllergy) has recently become available. Objective: To evaluate the clinical performance of IMMULITE 2000 in the diagnosis of cow’s milk allergy compared with that of UniCAP“. Additionally, we verified the behavior of both methods at two diagnostic decision points proposed by other authors. Methods: The study population consisted of 31 children with cow’s milk allergy (group A) and a control group of 19 atopic children without food allergy(group B). A blood sample from each child was tested using both methods and the results were compared. Results: In group A, the values for cow’s milk IgE ranged from 0.35 kU/L (the lowest common detection limit) to above 100 kU/L. In group B, the values were less than 1.1 kU/L for IMMULITE 2000 and less than 1.6 kU/L for UniCAP. An agreement of 90 % in IgE classes was obtained. Both methods demonstrated exactly the same diagnostic performance(sensitivity: 100 %; specificity: 78.9 %; negative predictive value: 100%; positive predictive value: 84.6%;efficiency: 90.2 %). The evaluation of the two methods at the two different decision points proposed in the literature showed a better positive predictive value with UniCAP, but we obtained equivalent performance with IMMULITE 2000 by choosing higher cutoff values. Conclusions: We conclude that IMMULITE 2000 is as effective as UniCAP in the diagnosis of cow’s milk allergy. Both methods can be used to obtain site-specific decision points that are population, age and disease dependent.

RESUMO: Relevância e Objectivos: O objectivo deste estudo consistiu em verificar a eficácia e segurança de uma técnica de higiene brônquica – Active Cycle breathing technique (ACBT), na remoção de secreções e desinsuflação em crianças asmáticas. É uma técnica recente, que ainda não foi estudada nesta população. Metodologia: A amostra foi constituída por um total de 14 crianças, com idades compreendidas entre os 6 e 18 anos, com o diagnóstico de asma. Foi dividida em dois grupos: grupo de controlo, constituído por asmáticos estáveis e grupo experimental constituído por asmáticos pós-crise. Ambos receberam o mesmo tratamento, que consistiu numa única sessão de ACBT. Realizaram um exame da função respiratória (variáveis FEV1, PEF, MEF75%, RV e FRC) antes e após a aplicação da técnica. Mediu-se a Saturação de O2 antes, a meio e no fim da aplicação da técnica. Por último, pesaram-se as secreções recolhidas durante a sessão de tratamento. Resultados: Como resultados, quando omparamos o antes e após aplicação do ACBT, obtivemos diferenças significativas no RV (p <0,05) e FRC (p <0,05); muito significativa na Sa02 (p <0,01) e peso de secreções, nos asmáticos pós crise. Ocorreu uma melhoria significativa da saturação (p <0,05) em asmáticos estáveis, não havendo secreções. Não houve efeitos adversos nem broncoespasmo em ambos os grupos.Conclusão: O ACBT é uma técnica que promove a remoção de secreções e diminuição da insuflação na população de asmáticos.--------------------ABSTRACT:Relevance and Goals: the goal of this study was to verify the efficiency and security of a bronchial hygienic technique - Active Cycle Breathing Technique (ACBT) – in the obstruction and insufflation of asthmatic children. This is a new technique that has not yet been studied in this population. Procedures: The subjects were 14 children aged between 6 and 18 years old with asthma diagnosis. They were divided in two groups: the control group with stable asthmatics and the experimental group with post-crisis asthmatics. Both received the same treatment that was a single session of ACBT. They made a lung function test before and after the application of the technique (variables FEV1, PEF, MEF75%, RV and FRC). The level of O2 saturation was measured before, during and after the procedure. In the end, the secretions collected during the treatment were weighted. Results: We observed a significant difference on RV (p ˂0,05), FRC (p ˂0,05), Sa02 (p ˂0,01) and in the weight of the secretions in the post-crisis asthmatics after ACBT. There was a significant improvement of saturation (p ˂0,05) only in the stable asthmatics. There were no adverse effects or bronchospasm in both groups.

The lack of specific laboratorial diagnosis methods and precise symptoms makes the toxocariasis a neglected disease in Public Health Services. This study aims to determine the frequency of Toxocara spp. infection in children attended by the Health Public Service of Hospital Municipal de Maringá, South Brazil. To evaluate the association of epidemiological and clinical data, an observational and cross-section study was carried out. From 14,690 attended children/year aged from seven month to 12 years old, 450 serum samples were randomly collected from September/2004 to September/2005. A questionnaire was used to evaluate epidemiological, clinical and hematological data. An ELISA using Toxocara canis larval excretory-secretory products as antigen detected 130 (28.8%) positive sera, mainly between children from seven month to five years old (p = 0.0016). Significant correlation was observed between positive serology for Toxocara, and frequent playing in sandbox at school or daycare center (p = 0.011) and the presence of a cat at home (p = 0.056). From the families, 50% were dog owners which exposed soil backyards. Eosinophilia (p = 0.776), and signs and symptoms analyzed (fever p = 0.992, pneumonia p = 0.289, cold-like symptoms p = 0.277, cough p = 0.783, gastrointestinal problems p = 0.877, migraine p = 0.979, abdominal pain p = 0.965, joint pain p = 0.686 and skin rash p = 0.105) could not be related to the presence of anti-Toxocara antibodies. Therefore, two asthmatics children showed titles of 1:10,240 and accentuated eosinophilia (p = 0.0001). The authors emphasize the needs of prevention activities.

A dermatite atópica é uma doença inflamatória crónica da pele, tendo por base diversos mecanismos etiopatogénicos. Considerando a sua heterogeneidade, foi, recentemente, introduzida outra designação para esta patologia - Síndroma Eczema / Dermatite Atópica (SEDA). A associação com alergia alimentar ou respiratória parece ser variável entre as diferentes populações. Objectivo: Analisar um grupo de doentes referenciados à Consulta de Imunoalergologia com o diagnóstico de SEDA, com o intuito de avaliar a associação desta síndrome com a alergia alimentar e doença respiratória nesta população. Métodos: Do número total de primeiras consultas do nosso Serviço durante os anos 2000-01 (n = 3436) foram seleccionados todos os doentes com história de SEDA. A população foi analisada quanto a idade, sexo, existência de alergia alimentar, doença respiratória e resultados de testes cutâneos (TC) por picada. Resultados: Foram encontrados 193 doentes com uma idade média de 7,5 anos de idade (1 -54 anos) e relação F/M = 1 / 1,5. Eram 68 (35,8%) os doentes com SEDA isolada. SEDA associada a doença respiratória foi identificada em 113 (58,5%) e a alergia alimentar em 19 (9,8%) - na maioria dos casos manifestando-se por urticária / angioedema. Os TC revelaram-se positivos para aeroalergénios em 74% e para alergénios alimentares em 18% da amostra. Os TC foram positivos em 58,9% dos doentes com SEDA isolada, 84,2% dos doentes com alergia alimentar e 92% com doença respiratória. Conclusão: Em contraste com outras séries, foi encontrada uma baixa prevalência de alergia alimentar, na maioria dos casos manifestada por reacções imediatas. Mais de metade dos doentes estudados apresentava doença respiratória alérgica associada a uma elevada prevalência de sensibilização a aeroalergénios. Estes resultados reflectem a heterogeneidade das populações com SEDA e a importância dos aeroalergénios na nossa população.

The role of atopy on cystic fibrosis (CF) progression remains unclear but evidence suggests that it may influence the appearance of co-morbid conditions such as CF asthma or allergic bronchopulmonary aspergillosis (ABPA). Recognising asthma in patients with CF is not always easy but the identification of atopic markers favours the diagnosis. Physicians should be aware of this fact in order to achieve a better control of respiratory symptoms in patients with CF. Bronchial mucosa inflammation and abnormal mucus predispose to mould colonisation. These patients are at higher risk of allergic sensitisation, especially when atopic susceptibility is present. In the particular case of A. fumigatus, allergic sensitisation precedes ABPA development, which occurs in up to 10% of CF patients. Progression of lung function deterioration is most strikingly pronounced in patients with ABPA. Therefore, sensitisation with A. fumigatus should be regularly tested in patients with CF, especially those at higher risk. Recombinant allergens constitute an important advance in differentiating Aspergillus sensitisation from ABPA itself.

Food allergy (FA) prevalence data in infants and preschool-age children are sparse, and proposed risk factors lack confirmation. In this study, 19 children’s day care centers (DCC) from 2 main Portuguese cities were selected after stratification and cluster analysis. An ISAAC’s (International Study of Asthma and Allergies in Childhood) derived health questionnaire was applied to a sample of children attending DCCs. Outcomes were FA parental report and anaphylaxis. Logistic regression was used to explore potential risk factors for reported FA. From the 2228 distributed questionnaires, 1217 were included in the analysis (54.6%). Children’s median age was 3.5 years, and 10.8% were described as ever having had FA. Current FA was reported in 5.7%. Three (0.2%) reports compatible with anaphylaxis were identified. Reported parental history of FA, personal history of atopic dermatitis, and preterm birth increased the odds for reported current FA. A high prevalence of parental-perceived FA in preschool-age children was identified. Risk factor identification may enhance better prevention.

RESUMO: A prevalência das doenças atópicas tem vindo a aumentar, em especial ao nível dos países ocidentalizados. Vários fatores têm sido apontados para justificar este aumento de prevalência,destacando-se o reduzido tamanho das famílias, o elevado uso de antibióticos, a melhoria das condições sanitárias, bem como a diminuição quer das infeções de helmintas, quer da contaminação orofecal. Alguns estudos têm também avaliado a influência do ambiente pré-natal no desenvolvimento de atopia e asma. Da análise da literatura, parece inegável a importância deste período para o desenvolvimento do sistema imunitário. Neste âmbito, a transmissão de atopia à descendência em mulheres atópicas, e concretamente com asma alérgica, poderá ser moldada desde este período. A possibilidade de identificar marcadores de risco precoces para o desenvolvimento de atopia poderá ser o primeiro passo para o desenvolvimento de estratégias de prevenção para os indivíduos em risco. Este trabalho pretendeu abordar o sistema imunitário materno de forma a enriquecer a sua caraterização desde o terceiro trimestre da gravidez até ao fim do puerpério. Para além da exploração de perfis celulares e citocínicos maternos (nos quais se incluiu sobretudo a avaliação de diferentes populações de células T e B, com funções efetoras e reguladoras), foi também considerada a sua eventual relação com o desenvolvimento de atopia nas crianças. Foram recrutadas 135 mulheres com critérios para serem incluídas num dos 4 grupos do estudo: grávidas atópicas – GA (n=24), não grávidas atópicas – NGA (n=32), grávidas saudáveis – GS (n=44) e não grávidas saudáveis – NGS (n=35). Foram caraterizadas por Citometria de Fluxo populações de leucócitos e linfócitos, com particular interesse nos perfis maturativos de linfócitos T e B, bem como nas subpopulações de células T e B reguladoras. Foi ainda efetuada uma análise funcional, para avaliar a capacidade de produção de citocinas pelos linfócitos T e B. Foram igualmente avaliadas as concentrações de citocinas séricas por ensaios imunoenzimáticos. Estes parâmetros imunológicos maternos foram acompanhados desde o terceiro trimestre de gestação, até depois do puerpério (primeiras 6 semanas pós parto), e aos seis meses de idade, foi efetuada uma avaliação clínica das crianças. As mulheres não grávidas atópicas apresentaram contagens celulares mais elevadas para a generalidade das populações leucocitárias e linfocitárias (em relação a mulheres não grávidas saudáveis). Destaca-se ainda uma maior presença de eosinófilos nas mulheres NGA (p=0,0009; teste de Mann-Whitney U), que tinham igualmente os seus compartimentos linfocitários T e B mais ricos em células de memória, em relação às mulheres NGS. Para os perfis de regulação, verificou-se que as células T reguladoras se encontravam percentualmente aumentadas (p≤0,003; teste de Mann-Whitney U), tal omo as células T produtoras de IL10 após estimulação (p≤0,03; teste de Mann-Whitney U) em mulheres NGA. Também se observou uma maior expressão de Foxp3 (p=0,0002; teste de Mann-Whitney U), e ainda a diminuição dos níveis séricos de IFN-γ nas mulheres NGA (p=0,0019; teste de Mann-Whitney U), em relação a mulheres NGS. De um modo geral, as alterações verificadas nos parâmetros imunológicos de mulheres grávidas atópicas no terceiro trimestre da gravidez foram semelhantes às observadas em mulheres grávidas saudáveis. Comparadas com mulheres NGA, nas mulheres grávidas atópicas ocorreu uma alteração substancial da fórmula leucocitária, com um importante incremento de neutrófilos (p<0,0001; teste de Mann-Whitney U) e diminuição dos valores das restantes populações leucocitárias. A diminuição nas contagens de linfócitos totais estendeu-se a grande parte das subpopulações linfocitárias caraterizadas. Nos compartimentos linfocitários T e B foi possível observar uma diminuição das subpopulações de células de memória. Verificou-se igualmente na gravidez uma menor expressão de Foxp3 em mulheres GA (p<0,0001; teste de Mann-Whitney U) e ainda menos células B CD24HiCD38Hi circulantes (p=0,0012; teste de Mann-Whitney U). Ocrreu ainda uma diminuição relativa das células T CD4 produtoras de IFN-γ em mulheres GA (p≤0,024; teste de Mann-Whitney U), e uma maior presença de células T CD8 produtoras de IL17 (p=0,0172; teste de Mann-Whitney U), em relação ao observado em mulheres NGA. Depois do puerpério, no compartimento T de mulheres do grupo GA, verificou-se um aumento das populações de células de memória. Em comparação com a gravidez, após o puerpério o compartimento B, apresentou nas mulheres GA um aumento significativo da subpopulação de células B de transição (p<0,0001; teste de Wilcoxon). Verificou-se, igualmente em mulheres GA após o puerpério, uma maior expressão de Foxp3 nas células T reguladoras (p<0,0001; teste de Wilcoxon) e o aumento das populações de células T circulantes produtoras de IFN-γ (p≤0,0234; teste de Wilcoxon). As modulações das populações T e B desde a gravidez até depois do puerpério ocorreram de forma semelhante nas mulheres dos grupos GA e GS. Apesar de as mulheres GA manterem um perfil imunológico próximo do das mulheres GS depois do puerpério, aconteceu também neste período um processo de reaproximação ao perfil observado nas mulheres NGA. As mulheres GA com manifestações de risco para atopia na descendência (comparadas com mulheres GA sem manifestações de risco para atopia na descendência até aos 6 meses de vida) apresentaram uma maior proporção de células T e menor proporção de células B, percentagens mais elevadas de células T CD8 de memória efetoras, de células B de transição e de células B CD24HiCD38Hi, e contagens mais baixas de células B de memória. Na avaliação destes parâmetros como marcadores de risco para o desenvolvimento de atopia verificou-se que o parâmetro com melhor desempenho foi a percentagem de células B de transição, com uma Odds-Ratio de 54,0 [IC 95%: 4,2-692,9; (p=0,0005)], sensibilidade de 90,0% [IC 95%: 55,5 – 99,8] e especificidade de 85,7% [IC 95%: 57,2 – 98,2]. Este estudo foi pioneiro em Portugal, e no mundo, no que se refere ao acompanhamento do compartimento linfocitário B circulante, abordando o seu perfil de maturação, e em particular as células B com funções reguladoras, desde a gravidez até ao fim do puerpério, em mulheres atópicas e não atópicas. A este nível, encontram-se estudos na literatura a documentar a alteração do compartimento B durante a gravidez. O presente trabalho reporta agora que alterações, como a diminuição do número de células B em circulação, são impostas também na mulher atópica. Em suma, demonstrou-se a existência de um perfil imunológico caraterístico em mulheres atópicas, que sofre alterações significativas durante a gravidez, tendendo os parâmetros imunológicos a normalizar após o puerpério. O compartimento T, para o qual a literatura é mais rica em estudos e abordagens, demonstrou também neste trabalho oscilações caraterísticas entre o período pré e pós-natal. Verificaram-se sobretudo variações nos compartimentos de células T de memória, sem grandes alterações ao nível das células Treg no que se refere à sua presença em circulação. Apenas a registar a menor expressão de Foxp3 nas células Treg durante a gestação observada em mulheres atópicas, tal como em mulheres saudáveis (como também já foi relatado em estudos anteriores). Apesar de muitos dos dados se encontrarem em concordância com a literatura, quer no que se refere às subpopulações de células de memória, quer no que se refere às células Treg, também se encontram resultados discordantes, por exemplo documentando variações numéricas nas células Treg em circulação em mulheres atópicas e mulheres atópicas grávidas. A importância de harmonizar protocolos e fenótipos, parece crucial na abordagem de estudos futuros. Ao nível do risco para a atopia na descendência de mulheres atópicas, acrescentou-se ainda a possibilidade de definir marcadores não invasivos para a criança, em particular as células B de transição. Estas células, cuja maior presença em circulação no recém-nascido foi recentemente associada com manifestações alérgicas subsequentes, são agora apontadas já na mulher atópica, grávida do terceiro trimestre, como um elemento de risco para o desenvolvimento de atopia. Os marcadores de risco descritos, para além de facilmente poderem vir a ser englobados no âmbito dos normais rastreios maternos durante a gravidez, apresentam ainda a vantagem da precocidade do diagnóstico, permitindo não só a possibilidade de prevenção pós-natal, mas estendendo esta possibilidade ao período gestacional.----------------------------ABSTRACT: The prevalence of atopic diseases has been increasing, especially in Westernized countries. Several factors have been suggested to justify this increase in prevalence, as the small size of families, the high use of antibiotics, the improvement in sanitation conditions, as well as the reduction of both helminth infections, and orofecal contamination. A few studies have adressed the influence of prenatal environment on the development of atopy and asthma. From literature, it seems undeniable the importance of the prenatal period for the development of the immune system. In this context, the transmission of atopy to the progeny in atopic women, and specifically in women with allergic asthma, can be modulated from this period on. The ability to detect early risk markers for the development of atopic diseases may be the first step in the development of prevention strategies for individuals at risk. This study aimed to approach the maternal immune system in order to enrich its characterization from the third trimester of pregnancy until the end of the puerperium period. In addition to the evaluation of the maternal cellular profiles (in which, mostly, diferente populations of T and B cells with effector and regulatory functions were included) and citokines, the relation between these profiles and the development of atopy in the progeny was also assessed. 135 women were recruited for this study, and fullfiled the inclusion criteria necessary to be included in one of the four groups preset: atopic pregnant women - GA (n = 24), atopic nonpregnant women - NGA (n = 32), healthy pregnant women - GS (n = 44) and healthy nonpregnant women - NGS (n = 35). Populations of leukocytes and lymphocytes, and particularty maturation profiles of T and B lymphocytes, as well as subpopulations of T and B cells with regulatory functions, were characterized by flow cytometry. Functional assays were also performed, to assess the ability of cytokine production by T and B lymphocytes. Serum cytokine concentrations were assessed as well by enzymatic immunoassays. These maternal imune parameters were monitored since the third trimester of pregnancy until the end of the puerperium period (first six weeks after delivery). A clinical evaluation of all the newborn children was performed at the age of six months. Non-atopic pregnant women presented higher cell counts for most leukocyte and lymphocyte populations (compared to healthy non-pregnant women). We should also highlight the increased presence of eosinophils in NGA women (p = 0,0009; Mann-Whitney U test). Again compared to NGS women, NGA women showed increased memory cells within the circulating T and B lymphocyte compartments. Considering the regulatory profiles, NGA women presented higher percentages of regulatory T cells (p≤0,003; Mann-Whitney U test) and IL10 producing T cells after stimulation (p≤0,03; Mann Whitney U), as well as increased expression of Foxp3 (p = 0,0002; Mann-Whitney U test), and also decreased serum levels of IFN-γ (p = 0,0019; test Mann-Whitney U test) compared to NGS women. In general, the changes observed in immune parameters of atopic pregnant women in the third trimester of gestation were similar to those observed in healthy pregnant women. Comparing pregnant and non-pregnant atopic women, an important change in leukocyte subsets was observed, with a significant increase of neutrophils (p <0,0001; Mann-Whitney U test) and the consequent diminution of the remaining leukocyte populations in the GA group. The decrease in total lymphocyte counts was extended to most of the lymphocyte subsets characterized. It was possible to detect a decrease in memory cell subsets within the T and B lymphocyte compartments, also. During pregnancy, a lower expression of Foxp3 was reported in GA women (p <0,0001; Mann-Whitney U test) and, besides, lesser CD24HiCD38Hi B cells were present in circulation in these women, compared to NGA women (p = 0,0012; Mann-Whitney U test). There was still a decrease in the percentages of IFN-γ-producing CD4 T cells in GA women (p≤0,024; Mann-Whitney U test) and a greater presence of IL17-producing CD8 T cells (p = 0,0172; Mann-Whitney U test), compared to the levels observed in NGA women. At the end of the puerperium, there was an increase in memory cell subpopulations within the T cell compartment of GA women. Compared with the pregnancy evaluation, after puerperium, the B cell compartment showed a significant increase in the transitional subpopulation (p<0,0001; Wilcoxon test), in GA women. Moreover, after puerperium, GA women exhibited a greater expression of Foxp3 in Treg cells (p <0,0001; Wilcoxon test) and there was an increase in circulating IFN-γ-producing T cells (p≤0,0234; Test Wilcoxon). The modulations of T and B cell subpopulations from pregnancy until the end of puerperium were similar in women of GA and GS groups. Although at the end of puerperium, GA women still kept an immune profile close the one observed in GS women, at this time point, there were also signs of rapprochement between the immune profiles observed in women of GA and NGA groups. GA women with atopic manifestations in the offspring (compared to GA women without atopic manifestations in the offspring at the age of 6 months) presented higher proportions of T cells and lower proportions of B cells, higher percentages of effector memory CD8 T cells, transitional B cells and CD24HiCD38Hi B cells, and, finally, lower absolute counts of memory B cells. In the evaluation of these parameters as risk markers for the development of atopy, the parameter which presented the best performance was the percentage of transitional B cells, with an Oddsratio of 54,0 [95% CI: 4,2 to 692,9; (p = 0,0005)], sensitivity of 90,0% [95% CI: 55,5 to 99,8] and a specificity of 85,7% [95% CI: 57,2 to 98,2]. This study was a pioneer in Portugal, and in the world, in what concerns the monitoring of the circulating B cell compartment, addressing not only the maturation profile, but, in particular, B cells with regulatory functions, from pregnancy untill after puerperium, in atopic and non-atopic women. Literature presents evidence of a typical change in circulating B cells during pregnancy. This study now reports that changes, such as the decrease in the number of circulating B cells,/ are also imposed by pregnancy in atopic woman. In brief, it demonstrated the existence of a characteristic immune profile in atopic women, which undergoes significant alterations during pregnancy, tending to normalize after the puerperium. As for the T cell compartment, for which the literature is richer in studies and approaches, this study also showed characteristic fluctuations between the pre- and postnatal periods. There were variations mostly in the memory subsets within the T cell compartment, without major changes in regulatory T cells regarding their presence in circulation. Only the expression of Foxp3 in Treg cells presented lower levels during pregnancy, in both atopic and healthy women (as previously reported in other studies). Although much of the data now reported are in agreement with literature, regarding either memory cell subsets or regulatory T cells, there are also conflicting results, for example documenting changes in the numbers of regulatory T cells circulating in atopic pregnant and atopic non-pregnant women. The importance of harmonizing protocols and phenotypes seems crucial for the establishement of future studies. Considering the risk for atopy in the offspring of atopic women, this study added the possibility to define non-invasive markers for the child, in particular transitional B cells. These cells, whose greater presence in circulation in newborns has recently been associated with subsequent allergy development, are here identified in atopic pregnant women in the third trimester of gestation as a risk factor in the development of atopy in their progeny. The risk factors described, besides having the capacity to easily become integrated within the normal maternal screening protocols during pregnancy, also have the advantage of an early diagnosis, allowing not only the possibility of postnatal prevention but extending this possibility to the prenatal period.
