The aim of this study was to evaluate two root canal filling techniques used in teeth that had their apical foramen disrupted and compare the apical infiltration with an ideal clinical situation. Twenty-seven freshly extracted single-rooted teeth were selected and radiographed to confirm the existence of a single and straight root canal. The crowns were removed at a mean distance of 11 mm from the apex. The teeth had the root canals instrumented and were randomly assigned to 3 groups (n=9): ND group-root canals were filled using the lateral compaction technique and no disruption was performed; DRF group-the apical constriction was disrupted by advancing a #40 K-file 1 mm beyond the original working length, the canals were reinstrumented to create an apical ledge at 1 mm from the apical foramen and were obturated with a master gutta-percha cone with same size as the last file used for reinstrumentation; DF group - the teeth had the apical constriction disrupted and the canals were obturated with a master gutta-percha cone that fit at 1 mm from the apex. The teeth were submitted to dye leakage test with Rhodamine B for 7 days, using vaccum on the initial 5 min. The teeth were sectioned longitudinally and the leakage was measured in a linear fashion from apex to crown. There was no statistically significant difference (p>0.05) between the groups that had the apical foramen disrupted (DF, DRF), but significant difference was found between the disrupted groups and the non-disrupted one (p<0.01). In conclusion, none of the evaluated techniques was able to prevent apical infiltration, so working length so the working length determination has to be established and maintained carefully.

The aim of this study was to evaluate the periapical healing after the use of membrane, bone graft, and mineral trioxide aggregate (MTA) in apical surgery of dogs' teeth. Apical lesions were induced in 48 roots of 6 dogs after coronal access and pulpal removal. Apical surgery consisted of osteotomy with trephine bur for the standardization of the critical surgical cavities, followed by apicoectomy, curettage, preparation of the root-end cavities with the aid of the ultrasonic device, and retrofilling with MTA. The surgical sites were divided into: group 1-filled with blood; group 2-filled with blood and recovered with membrane; group 3-filled with bone graft; and group 4-filled with bone graft and recovered with membrane. The results showed that the inflammatory infiltrate, the periapical healing process, and the behavior of MTA was the same in all groups, including the mineralization stimulation. It was concluded that the use of membranes and bone graft materials isolated or associated in apical surgery did not alter the periapical healing process after the root-end filling with MTA. The aim of this study was to evaluate the apical sealing of dentinal tubules after root-end surface cutting by using Er:YAG and Nd:YAG lasers. After root-canal instrumentation and filling, apices of 50 extracted maxillary canine human teeth were resected by Er: YAG with 400 mJ, 10 Hz, for 30 sec. The samples were randomly assigned to five groups (n = 10): (GI) treated without root-end cavity, but with Nd: YAG (1.0W, 10 Hz, 20 sec) for dentinal tubules sealing; (GII) treated with root-end cavity without the use of Nd: YAG; (GIII) treated with root-end cavity and Nd: YAG application; (GIV) treated with root-end cavity made by Er: YAG with no focus and without Nd: YAG application; and (GV) treated without root-end cavity and without Nd: YAG application. The root-end cavities were performed by using Er: YAG at 300 mJ, 10 Hz, for 20 sec. Subsequently, all teeth were waterproofed and immersed in 2% methylene blue for 48 h in a vacuum environment. The samples were longitudinally sectioned, and microleakage was measured. ANOVA and the Fisher LSD test showed that GIV was less susceptible to microleakage than were the other groups (p < 0.05). Interestingly, the use of the Er: YAG with no focus showed superior dentinal tubule sealing in comparison with the other groups, even with or without root-end cavity and Nd: YAG application.

Este trabalho avaliou o selamento marginal apical de canais radiculares obturados com os cimentos endodônticos Sealapex, Apexit, Sealer 26 e Ketac Endo. Utilizaram-se 136 raízes, cujos canais radiculares, após o preparo biomecânico, foram obturados pela técnica da condensação lateral ativa com os cimentos em estudo. Metade das amostras, imediatamente após as obturações, foram imersas na solução de azul de metileno a 2% e a outra metade após 6 meses de armazenamento em plasma sangüíneo humano. Observou-se que os cimentos Sealapex e Sealer 26 apresentaram infiltrações médias estatisticamente iguais entre si e menores que as observadas para os demais cimentos (p < 0,05). Amostras imersas no corante imediatamente após a obturação dos canais apresentaram infiltração média menor (0,829 mm) do que aquelas mantidas por 6 meses em plasma sangüíneo humano (1,275 mm). Estas diferenças foram estatisticamente significantes (p < 0,05).

O objetivo deste estudo foi avaliar o selamento de ápices radiculares tratados com diferentes agentes desmineralizantes e retrobturados com agregado de trióxido mineral (MTA), mediante infiltração marginal por corante. Cinqüenta e seis dentes unirradiculares humanos extraídos foram instrumentados, obturados e seccionados apicalmente. Os preparos cavitários apicais foram confeccionados com pontas ultra-sônicas e os agentes desmineralizantes foram aplicados previamente à retrobturação com Pro Root MTA. Os espécimes foram divididos aleatoriamente em 4 grupos (n=14): grupo 1 (sem agente desmineralizante); grupo 2 (ácido fosfórico 35% durante 15 s); grupo 3 (solução de EDTA 17%, pH 7, durante 3 min); grupo 4 (gel de EDTA 24%, pH 7, durante 4 min). A extensão da infiltração de corante (rodamina B 2% a 37°C, por 24 h) foi avaliada em milímetros utilizando-se um estereomicroscópio. Os resultados foram analisados estatisticamente por meio de análise de variância a um critério e do teste Tukey com nível de significância de 5%. Dentre os grupos experimentais, a menor extensão de infiltração do corante foi verificada no grupo 1 (1,89 mm), seguido pelos grupos 2 (2,18 mm), 4 (2,54 mm) e 3 (2,64 mm). Não houve diferenças estatisticamente significante (p>0.05) na infiltração marginal pelo corante entre os grupos 1, 2 e 4 e os grupos 2, 3 e 4. Com base nos resultados obtidos, pode-se concluir que a aplicação de agentes desmineralizantes não pode ser recomendada quando da utilização do MTA em cirurgias parendodônticas.

Information about orthodontic movement of teeth with hypercementosis is scarce. As cementum deposition continues to occur, cementum is expected to change the shape of the root and apex over time, but this has not yet been demonstrated. Nor has it ever been established whether it increases or decreases the prevalence of root resorption during orthodontic treatment. The unique biological function of the interconnected network of cementocytes may play a role in orthodontic movement and its associated root resorptions, but no research has ever been conducted on the topic. Unlike cementum thickness and hypercementosis, root and apex shape has not yet been related to patient age. A study of the precise difference between increased cementum thickness and hypercementosis is warranted. Hypercementosis refers to excessive cementum formation above and beyond the extent necessary to fulfill its normal functions, resulting in abnormal thickening with macroscopic changes in the tooth root, which may require the delivery of forces that are different from conventional mechanics in their intensity, direction and distribution. What are the unique features and specificities involved in moving teeth that present with hypercementosis? Bodily movements would be expected to occur, since inclination might prove difficult to achieve, but would the root resorption index be higher or lower?

Foi propósito deste trabalho observar se o uso de cones de guta-percha contendo Ca(OH)2, promove melhora no selamento marginal apical e, também, se apenas o cone principal contendo essa droga produz esse efeito. Assim, dentes humanos extraídos foram preparados biomecânicamente e obturados pela técnica da condensação lateral com OZE e cones de guta-percha contendo ou não Ca(OH)2. Após imersão dos espécimes em azul de metileno a 2%, em ambiente com vácuo, observou-se diferença estatisticamente significante entre os espécimes obturados com cones contendo Ca(OH)2, comparativamente aos casos obturados com cones de guta percha comuns (p=0.01). Os resultados obtidos permitiram concluir que esses cones tornam as obturações mais herméticas e que esse efeito também pode ser obtido com o emprego do cone principal da mesma fórmula, aliado a cones acessórios comuns (p=0.05).

This study was conducted to observe the healing process in dogs' teeth with apical periodontitis after root canal treatment in one or two appointments. Premolars and anterior dogs' teeth had their root canals opened to the oral environment for 6 months before being treated. After root canal negotiation they were filled by the lateral condensation technique with gutta-percha points and Sealapex in one appointment or after a dressing with calcium hydroxide for 7 and 15 days. Six months after the treatment the animals were killed and the tissues prepared for histomorphological analysis. Scores attributed to the different histomorphological events were submitted to statistical analysis, which resulted in ranking the experimental groups from the best to the worst in the following way: (a) calcium hydroxide 14 days; (b) calcium hydroxide 7 days; and (c) one appointment. It was concluded that the use of a calcium hydroxide dressing helps to achieve better results (p < 0.01) than the treatment in one appointment.

Objective. The objective of this study was to evaluate the histopathologic response of periapical tissues after root canal treatment of necrotic dog teeth with chronic apical periodontitis by using 2 calcium hydroxide-based root canal dressings and 2 root canal sealers.Study design. Seventy-eight root canals were instrumented by using 5.25% sodium hypochlorite as the irrigating solution, after which a calcium hydroxide paste (Calen/PMCC or Calasept) was placed for 30 days as a dressing. The root canals were then filled by using cold lateral gutta-percha condensation and an enclodontic sealer (Sealapex or AH Plus). After 360 days, the animals were killed by anesthetic overdose; then, the teeth were histologically prepared, sectioned, and stained with hematoxylin and eosin for optical microscopic analysis of apical and periapical tissue repair.Results. Statistical analysis showed that the poorest histopathologic results were observed in the Calasept/AH Plus group and that the Sealapex sealer overall resulted in better apical repair than the AH Plus sealer. The histopathologic results of Calen/PMCC paste with both AH Plus and Sealapex and Calasept paste with only Sealapex were statistically similar but were different from the results of Calasept with AH Plus.Conclusions. The results of this study in the dog showed differences in apical and periapical tissue repair of teeth with chronic apical periodontitis by using 2 calcium hydroxide root canal dressings and 2 sealers. More research is necessary to determine the best combination of dressings and sealers.
