Metabólitos ativos da cafeína apresentam toxicidade, podendo causar efeitos deletérios em muitos órgãos no período fetal pelo consumo materno. O estudo objetivou avaliar o papel da administração de cafeína durante a gestação em vários parâmetros importantes para a função testicular em camundongos adultos C57BL/6. Fêmeas grávidas foram divididas em: grupo controle (C), que receberam injeções de sol. salina (0,9% NaCl) e grupo cafeína (CF), que receberam diariamente injeções subcutâneas de 20 mg / kg de cafeína. Filhotes tiveram livre acesso à ração padrão desde o desmame até três meses de idade, quando foram mortos. O grupo CF apresentou uma redução no consumo alimentar (C = 4,36 0,14; CF = 3,79 0,05/g, p<0,05) e ganho de massa corporal (C = 25,73 0,14; CF = 21,08 0,41/g, p=0,0001). Ainda este grupo, apresentou alterações estruturais nos testículos da prole, como redução no peso relativo (C = 0,078 0,003; CF = 0,063 0,002/g, p=0,002), diâmetro tubular (C = 455,4 2,7; CF = 393,5 3,1/m, p=0,0001), lume tubular (C = 310,6 2,5; CF = 254,8 2,9/m, p=0.0001) e altura do epitélio seminífero (C = 144,8 0,9; CF = 138,7 1,3/m, p=0,0005) observados pela técnica de morfometria. Testosterona sérica avaliada por quimioluminescência foi reduzida (C = 6,6 2,8; CF = 0,5 0,2/ng/ml, p=0.04). Western Blotting foi utilizado para análise de expressão protéica. Houve aumento do receptor de leptina (C = 0,81 0,04; CF = 1,28 0,15/g, p=0,01), do receptor para o hormônio leteinizante (C = 0,92 0,05; CF = 0,74 0,05/g, p=0,01),da aromatase (C = 0,32 0,03; CF = 0,48 0,05/g, p=0,03) e do regulador pró apoptose (C = 0,27 0,02; CF = 0,42 0,03/g, p=0,01) enquanto o receptor para hormônio folículo estimulante (FSHR) (C = 0,76 0,06; CF = 0,56 0,04/g, p=0,02), o receptor para proteína reguladora da esteroidogênese (C = 1,009 0,065; CF = 0,584 0,060/g, p=0,0007), a proteína do fator de crescimento vascular endotelial (C = 0,33 0,08; CF = 0,05 0,01/g, p=0,009), o receptor de androgênio (C = 0,97 0,11; CF = 0,59 0,07/g, p=0,02) e o antígeno nuclear de proliferação celular (C = 0,93 0,11; CF = 0,48 0,07/g, p=0,01) foram reduzidos. Este estudo sugere que o consumo de cafeína durante a gravidez parece ser nocivo à fertilidade de animais machos, caracterizando o fenômeno de programação, o que gera alterações funcionais e estruturais nos testículos da prole adulta.

A leptina tem um papel importante na regulação do sistema reprodutivo além de seu papel principal na regulação do peso corporal e ingestão alimentar. O objetivo deste estudo foi avaliar o efeito da administração de leptina durante o período neonatal na função testicular da prole adulta. Vinte e quatro filhotes de 12 mães foram divididos em 2 grupos: Grupo leptina: injetados com 50 L de leptina (80ng/gPC, subcutânea) nos primeiros 10 dias de vida e Grupo Controle: injetados com o mesmo volume de solução salina. Todos os animais foram sacrificados aos 90 dias de vida. Parâmetros analisados: consumo alimentar, massa corporal, crescimento linear, data de início da puberdade, perfil lipídico, níveis séricos de estradiol e testosterona, expressão gênica (PCR em tempo real) e expressão proteica (Western blot) de ObRa, OBRb, aromatase, AR, ER, morfometria testicular, número, morfologia e viabilidade de espermatozoides. Os dados foram expressos como média erro padrão. A significância estatística foi determinada pelo teste t de Student. A Injeção de leptina levou a uma redução (p≤0,008) no consumo alimentar a partir do dia 26 ao 40 e do dia 70 em diante, enquanto que a massa corporal (p≤0,03) e o crescimento linear (p≤0,05) foram reduzidos do dia 26 até o dia 45. O peso da hipófise (p≤0,0006), hipotálamo (p≤0,01), próstata (p≤0,003), testículo (p≤0,008) e epidídimo (p≤0,004) e bexiga (p≤0,009) foram significativamente reduzidos pela injeção de leptina. A Injeção de leptina adiantou o início da puberdade (C=45,0 0,3; L=41,6 0,3; dias, P≤0,0001). Em relação ao perfil lipídico, a administração de leptina gerou um aumento nos níveis séricos de TG (C= 116,5 15,9; L=172,6 19,7; ng/dL, P≤0,05) e uma redução nos níveis de HDL (C=33 1,5; L = 24 3,5; ng/dL, P≤0,02). Os níveis séricos de testosterona também foram reduzidos pela leptina (C=5,2 1,0; L=1,1 0,3; ng/mL, P≤0,003). Todos os genes avaliados por PCR em tempo real mostraram um aumento na sua expressão: Obra (C=0,32 0,04; L=0,69 0,16; P≤0,04), OBRb (C=0,37 0,06; L=0,71 0,16; P≤0,03), AR (C=0,28 0,02; L=0,71 0,16; P≤0,02), Aromatase (C=0,31 0,04; L=0,53 0,09; P≤0,04), ER-α (C=0,79 0,03; L=0,93 0,03; P≤0,01), ER-β (C=0,29 0,03; L=0,73 0,16; P≤ 0,02). Por outro lado, a expressão proteica de OBR (C=4,4 0,29; L=6,6 0,84; P≤0,05), ER-α (C=0,4 0,02; L=0,6 0,05; P≤0,03) e aromatase (C=0,4 0,03; L=0,5 0,02; P≤0,04) aumentaram, enquanto que a expressão proteica de AR (C=0,16 0,01; L=0,09 0,01; P≤0,009) foi reduzida pela administração de leptina. A análise morfométrica mostrou que a leptina levou a um aumento da área total do túbulo seminífero (C=64,6 3,1; L=56,1 2,1;μm, P≤0,01), aumento da área luminal (C=40,6 2,3; L= 48,7 0,9; μm P≤ 0,004) e na altura do epitélio (C=21,5 1,2; L=24,6 1,1; μm, P≤0,03), enquanto que o comprimento do túbulo seminífero foi reduzido no grupo tratado (C=2200 350; L= 1100 110;cm, P≤0,006). A administração de leptina levou a um aumento no número total de espermatozoides (C=20x107 2x107; L=30x107 5x107;Cls/mL, P≤0,009) e no número de anormalidades (C=40,6 1,9; L=45,8 1,2, P≤0,049). Podemos concluir que a leptina tem um papel importante na morfologia e função testicular. A leptina parece ter efeito direto neste tecido uma vez que a expressão gênica e proteica de OBR, AR, ER e aromatase foram alterados pela administração da leptina.

Pela sua alta incidência, morbidade, mortalidade e custos ao sistema de saúde, a sepse se destaca entre as diversas indicações de internação em unidade de terapia intensiva (UTI). A disfunção da microcirculação tem papel central na gênese e manutenção da síndrome séptica, sendo um marco fisiopatológico desta síndrome. Pacientes críticos invariavelmente estão ansiosos, agitados, confusos, desconfortáveis e/ou com dor. Neste contexto, drogas sedativas são amplamente utilizadas na medicina intensiva. A dexmedetomidina, um agonista potente e altamente seletivo dos receptores alfa-2 adrenérgicos, vem conquistando espaço como o sedativo de escolha nas UTIs por seus efeitos de sedação consciente, redução da duração e incidência de delirium e preservação da ventilação espontânea. Apesar destas possíveis vantagens, a indicação de uso da dexmedetomidina na síndrome séptica ainda carece de conhecimentos sobre seus efeitos na microcirculação e perfusão orgânica. Com o intuito de caracterizar os efeitos microcirculatórios da dexmedetomidina em um modelo murino de endotoxemia que permite estudos in vivo da inflamação e disfunção da perfusão microvascular, hamsters Sírios dourados submetidos à endotoxemia induzida por administração intravenosa de lipopolissacarídeo de Escherichia coli (LPS, 1,0 mg.kg-1) foram sedados com dexmedetomidina (5,0 μg.kg.h-1). A microscopia intravital da preparação experimental (câmara dorsal) permitiu a realização de uma análise quantitativa das variáveis microvasculares e do rolamento e adesão de leucócitos à parede venular. Também foram analisados os parâmetros macro-hemodinâmicos e gasométricos (arterial e venoso portal), as concentrações de lactato arterial e venoso portal, a água pulmonar total e a sobrevivência do animal. Animais não-endotoxêmicos e/ou tratados com solução salina a 0,9% serviram como controles neste experimento. O LPS aumentou o rolamento e a adesão de leucócitos à parede venular, diminuiu a densidade capilar funcional e a velocidade das hemácias nos capilares e induziu acidose metabólica. O tratamento com dexmedetomidina atenuou significativamente estas respostas patológicas (p < 0,05). A frequência de pulso dos animais foi significativamente reduzida pela droga (p < 0,05). Outros resultados não foram tão expressivos (estatisticamente ou clinicamente). Estes resultados indicam que a utilização de dexmedetomidina produz um efeito protetor sobre a microcirculação da câmara dorsal de hamsters endotoxêmicos. Efeitos anti-inflamatórios da dexmedetomidina sobre os leucócitos e o endotélio poderiam melhorar a perfusão capilar e representar o mecanismo in vivo de ação da droga na microcirculação.

This paper describes an investigation of the behavior of suction surface boundary layers in a modern multistage Low-Pressure turbine. An array of 18 surface-mounted hot-film anemometers was mounted on a stator blade of the third stage of a four-stage machine. Data were obtained at Reynolds numbers between 0.9 × 105 and 1.8 × 105. At the majority of the test conditions, wakes from upstream rotors periodically initiated transition at about 40 percent surface length. In between these events, laminar separation occurred at about 75 percent surface length. Because the wake-affected part of the flow appeared to be only intermittently turbulent, laminar separation also occurred at about 75 percent surface length while this flow was instantaneously laminar. At all but the lowest Reynolds numbers, the time-mean boundary layer appeared to have re-attached by the trailing edge even though it was not fully turbulent. It is inferred that the effect of the wakes on the performance of the blade row is limited and that steady flow design methods should provide an adequate assessment of LP turbine performance during design.

Aim: To study the pharmacokinetics of sifuvirtide, a novel anti-human immunodeficiency virus (HIV) peptide, in monkeys and to compare the inhibitory concentrations of sifuvirtide and enfuvirtide on HIV-1-infected-cell fusion. Methods: Monkeys received 1.2 mg/kg iv or sc of sifuvirtide. An on-line solid-phase extraction procedure combined with liquid chromatography tandem mass spectrometry (SPELC/MS/MS) was established and applied to determine the concentration of sifuvirtide in monkey plasma. A four-I-127 iodinated peptide was used as an internal standard. Fifty percent inhibitory concentration (IC50) of sifuvirtide on cell fusion was determined by co-cultivation assay. Results: The assay was validated with good precision and accuracy. The calibration curve for sifuvirtide in plasma was linear over a range of 4.88-5000 mu g/L, with correlation coefficients above 0.9923. After iv or sc administration, the observed peak concentrations of sifuvirtide were 10626 +/- 2886 mu g/L and 528 +/- 191 mu g/L, and the terminal elimination half-lives (T,12) were 6.3 +/- 0.9 h and 5.5 +/- 1.0 h, respectively. After sc, T-max was 0.25-2 h, and the absolute bioavailability was 49% +/- 13%. Sifuvirtide inhibited the syncytium formation between HIV-1 chronically infected cells and uninfected cells with an IC50 of 0.33 mu g/L. Conclusion: An on-line SPE-LC/MS/MS approach was established for peptide pharmacokinetic studies. Sifuvirtide was rapidly absorbed subcutaneously into the blood circulation. The T-1/2 of sifuvirtide was remarkably longer than that of its analog, enfuvirtide, reported in healthy monkeys and it conferred a long-term plasma concentration level which was higher than its IC50 in vitro.

Investigating the activities of the prefrontal cortex (PFC) in the process of addiction is valuable for understanding the neural mechanism underlying the impairments of the PFC after drug abuse. However, limited data are obtained from primate animals and few studies analyze Electroencephalogram (EEG) in the gamma band, which plays an important role in cognitive functions. In addition, it is yet unclear whether drug abuse affects the orbitofrontal cortex (OFC) and dorsolateral PFC (DLPFC) - the two most important subregions of the PFC - in similar ways or not. The aim of this study is to address these issues. We recorded EEG in the OFC and DLPFC in three rhesus monkeys. All animals received a course of saline (NaCl 0.9%, 2 ml) injection (5 days) followed by 10 days of morphine injection (every 12 h), and then a further series of saline injection (7 days). A main finding in the present study was that morphine decreased EEG power in all frequency bands in a short period after injection in both the OFC and DLPFC in monkeys. And gamma power decreased not just in short period after morphine injection but lasted to 12 h after injection. Moreover, we found that although the changes in EEG activities in the OFC and DLPFC at 30-35 min after injection were similar, the DLPFC was more sensitive to the effect of morphine than the OFC. This survay has been done from Januray 2000 till May 2002 in Khouzestan costal waters. Four species of grouper were identified from which orange spotted grouper (Epinephelus coioides) was the dominant species. For studing environmental parameters and reproductive biology, age, growth parameters and mortality rate samples were collected by fishing ship. Samples were taken montly in 4 days by fishing traps and trawls. In addition, some samples were obtained from Khozestan fish landing centres. Environmental factors such as PH, 02, salinity, water temperature and depth of traping areas, were measured. To identify species, morphometric characteries of 452 individal fishes were measured. Stomach contents of 394 fish were has survaid, from which stomach of 226 fish, and 168 fish had empty stomachs. Percentage of empty stomachs (cv) in males was more than females. Food items found in 73 percent of stomach were crab (11%), shrimps (8.8%) , squids (3.9%), gastropods (17%) and bivalves (0.4%). Feeding intensity in year classes did not obay logic trends The importance relatively indicator (I.R.I) were 81, 9.9, 4, 1.5 and 0.3 percent for fish, crab, shrimp, squid, gastropod and bivalve respectively. For age determination, sagita otoliths of 450 fish were taken and countable sections were obtained from 425 specimens. Relative frequency distribution of opaque and transparent rings showed that each opaque growth ring generates once a year from November to September. It seemed that generation of opaque rings is affected by temperature and photoperiod changes. Correlation between length and age was calculated using Von Bertalanffy's least square method. Following equasion was obtaind: L(t) : 122.27 (1 e 0.146 (t+0.482)) Growth parameters were determined through by Ford Walford equasion and Response Surface and Shepherd subcommands in Elefan program and L00 and K amounts were have determined. Correlation between length and age of 635 fish was determined by gender . Length and age correlation was calculated by exponential model and between total length and standard length by straghit line model. Correlation between age and weight of sagita was calculated by total length and age. The most Correlation was between sagita weight and fish age (r=0.876). Total mortality rate (z) was estimated by Length Converthed Method , Jones and Vanzaling and Powel Wetherall. Total mortality rate was z=0.39. Natural mortality rate, using Pauly method was calculated M=0.32. Fishing mortality (F) was 0.08. Gonads of 425 fishes were surveid within 18 month, from which 363 were female, 46 were male and 16 were sex reversing individuals .Total length of females varied from 26 to 95.5 centimeters while males length varied from 56.5 to 107 centimeters. Sex reversing individuals had a length of 47.5 centimeters, when two years old and 62.5 centimeters at age of 3 years. From the mentioned 425 fish, 401 individuals were matured, containing 339 females and 62 males, 5.47 females against each male. Montly changes of Gonadosomatic Index (GSI) by total body weight and standard length and total body length showed that this index increases from march to May and maximum increase was in May . This experiment was adapted in spawning season. Potential, relative, and absoulate fecundity was estimated by counting eggs in three samples. Total amount of traped fish using special traps was 16182.18 kg from which Epinephelus coioides provided catching 15353.43 kg of it (91.27 %) and By catch was 141.18 kg (8.24 %). Total average CPUE for whole catch was 123.33 kg/day/vessel. Total amount of catch was estimated 232.04 tons, considering CPUE of total catch and total Khuzestan trap ships effort.

The magnitude and frequency of vertical fluctuations of the top of an axisymmetric miscible Boussinesq fountain forms the focus of this work. We present measurements of these quantities for saline-aqueous fountains in uniform quiescent surroundings. Our results span source Froude numbers 0.3 ≤ Fr 0 ≤ 40 and, thereby, encompass very weak, weak, intermediate and forced classes of fountain. We identify distinct scalings, based on known quantities at the fountain source, for the frequency of fountain height fluctuations which collapse our data within bands of Fr0. Notably, our scalings reveal that the (dimensionless) frequency takes a constant value within each band. These results highlight characteristic time scales for the fluctuations which we decompose into a single, physically apparent, length scale and velocity scale within each band. Moreover, within one particular band, spanning source Froude numbers towards the lower end of the full range considered, we identify unexpectedly long-period fluctuations indicating a near balance of inertia and (opposing) buoyancy at the source. Our analysis identifies four distinct classes of fluctuation behaviour (four bands of Fr 0) and this classification matches well with existing classifications of fountains based on rise heights. As such, we show that an analysis of the behaviour of the fountain top alone, rather than the entire fountain, provides an alternative approach to classifying fountains. The similarity of classifications based on the two different methods confirms that the boundaries between classes mark tangible changes in the physics of fountains. For high Fr0 we show that the dominant fluctuations occur at the scale of the largest eddies which can be contained within the fountain near its top. Extending this, we develop a Strouhal number, Strtop, based on experimental measures of the fountain top, defined such that Strtop = 1 would suggest the dominant fluctuations are caused by a continual cycle of eddies forming and collapsing at this largest physical scale. 研究了Ti_(0.8)Zr_(0.2)V_(1.1)Mn_(0.9-x)Ni_(0.4)Cr_x(x为0~0.4)固溶体电极合金的结构和电化学性能。结果表明,Ti_(0.8)Zr_(0.2)V_(1.1)Mn_(0.9-x)Ni_(0.4)Cr_x系列合金是由具有体心立方(BCC)结构的V基固溶体相和C14Laves第二相组成的双相合金。

The construction of the shuttle, expression vector of human tumor necrosis factor alpha (hTNF-alpha) gene and its expression in a cyanobacterium Anabaena sp. PCC 7120 was reported. The 700-bp hTNF cDNA fragments have been recovered from plasmid pRL-rhTNF, then inserted downstream of the promoter PpsbA in the plasmid pRL439. The resultant intermediary plasmid pRL-TC has further been combined with the shuttle vector pDC-8 to get the shuttle, expression vector pDC-TNF. The expression of the rhTNF gene in Escherichia coil has been analyzed by SDS-PAGE and thin-layer scanning, and the results show that the expressed TNF protein with these two vectors is 16.9 percent (pRL-TC) and 15.0 percent (pDC-TNF) of the total proteins in the cells, respectively, while the expression level of TNF gene in plasmid pRL-rhTNF is only 11.8 percent. Combined with the participation of the conjugal and helper plasmids, pDC-TNF has been introduced into Anabaena sg PCC 7120 by triparental conjugative transfer, and the stable transgenic strains have been obtained. The existence of the introduced plasmid pDC-TNF in recombinant cyanobacterial cells has been demonstrated by the results of the agarose electrophoresis with the extracted plasmid samples and Southern blotting with alpha-(32)p labeled hTNF cDNA probes, while the expression of the hTNF gene in Anabaena sp. PCC 7120 has been confirmed by the results of Western blotting with extracted protein samples and human TNF-alpha monoclonal antibodies. The cytotoxicity assays using the mouse cancer cell line L929 proved the cytotoxicity of the TNF in the crude extracts from the transgenic cyanobacterium Anabaena sp. PCC 7120.

Increasing attentions have been paid to the subsurface geological storage for CO2 in view of the huge storage capacity of subsurface reservoirs. The basic requirement for subsurface CO2 storage is that the CO2 should be sequestrated as supercritical fluids (physical trapping), which may also interact with ambient reservoir rocks and formation waters, forming new minerals (chemical trapping). In order to the effective, durable and safe storage for CO2, enough storage space and stable sealing caprock with strong sealing capacity are necessitated, in an appropriate geological framework. Up till now, hydrocarbon reservoirs are to the most valid and appropriate CO2 storage container, which is well proven as the favorable compartment with huge storage capacity and sealing condition. The thesis focuses on two principal issues related to the storage and sealing capacity of storage compartment for the Qingshankou and Yaojia formations in the Daqingzijing block, Southern Songliao Basin, which was selected as the pilot well site for CO2-EOR storage. In the operation area, three facies, including deltaic plain, deltaic front and subdeep-deep lake facies associations, are recognized, in which 11 subfacies such as subaqueous distributary channel, river- mouth bar, interdistributary bay, sheet sandbody, crevasse splay and overflooding plain are further identified. These subfacies are the basic genetic units in the reservoir and sealing rocks. These facies further comprise the retrogradational and progradational depositional cycles, which were formed base- level rise and fall, respectively. During the regressive or lake lowstand stage, various sands including some turbidites and fans occurred mostly at the bottom of the hinged slope. During the progradation stage, these sands became smaller in size and episodically stepped backwards upon the slope, with greatly expanded and deeped lake. However, most of Cretaceous strata in the study area, localized in the basin centre under this stage, are mainly composed of grey or grizzly siltstones and grey or dark grey mudstones intercalated with minor fine sandstones and purple mudstones. On the base of borehole and core data, these siltstones are widespread, thin from 10 to 50 m thick, good grain sorting, and have relative mature sedimentary structures with graded bedding and cross- lamination or crossbeds such as ripples, which reflect strong hydrodynamic causes. Due to late diagenesis, pores are not widespread in the reservoirs, especially the first member of Qingshankou formation. There are two types of pores: primary pore and secondary cores. The primary pores include intergranular pores and micropores, and the secondary pores include emposieus and fracture pores. Throat channels related to pores is also small and the radius of throat in the first, second and third member of Qingshankou formation is only 0.757 μm, 0.802 μm and 0.631 μm respectively. In addition, based on analyzing the probability plot according to frequency of occurrence of porosity and permeability, they appear single- peaked distribution, which reflects strong hetero- geneity. All these facts indicate that the conditions of physical property of reservoirs are not better. One reason may be provided to interpret this question is that physical property of reservoirs in the study area is strong controlled by the depositional microfacies. From the statistics, the average porosity and permeability of microfacies such as subaqueous distributary channel, channel mouth bar, turbidites, is more than 9 percent and 1md respectively. On the contrary, the average porosity and permeability of microfacies including sand sheet, flagstone and crevasse splay are less than 9 percent and 0.2md respectively. Basically, different hydrodynamic environment under different microfacies can decide different physical property. According to the reservoir models of the first member of Qingshankou formation in the No. well Hei47 block, the character of sedimentary according to the facies models is accord to regional disposition evolution. Meantime, the parameter models of physical property of reservoir indicate that low porosity and low permeability reservoirs widespread widely in the study area, but the sand reservoirs located in the channels are better than other places and they are the main sand reservoirs. The distribution and sealing ability of fault- fractures and caprock are the key aspects to evaluate the stable conditions of compartments to store CO2 in the study area. Based on the core observation, the fractures widespread in the study area, especially around the wells, and most of them are located in the first and second member of Qingshankou formation, almost very few in the third member of Qingshankou formation and Yaojia formation instead. In addition, analyzing the sealing ability of eleven faults in the three-dimensional area in the study area demonstrates that most of faults have strong sealing ability, especially in the No. well Hei56 and Qing90-27. To some extent, the sealing ability of faults in the No. well Hei49, Qing4-6 and Qing84-29 are worse than others. Besides, the deposition environment of most of formations in the study area belongs to moderately deep and deep lake facies, which undoubtedly take advantage to caprocks composed of mudstones widespread and large scale under this deposition environment. In the study area, these mudstones distribute widely in the third member of Qingshankou formation, Yaojia and Nenjiang formation. The effective thickness of mudstone is nearly ~550m on an average with few or simple faults and fractures. In addition, there are many reservoir beds with widely- developed insulated interbeds consist of mudstones or silty mudstone, which can be the valid barrier to CO2 upper movement or leakage through diffusion, dispersion and convection. Above all, the closed thick mud caprock with underdeveloped fractures and reservoir beds can be taken regard as the favorable caprocks to provide stable conditions to avoid CO2 leakage.

BACKGROUND: Several trials have demonstrated the efficacy of nurse telephone case management for diabetes (DM) and hypertension (HTN) in academic or vertically integrated systems. Little is known about the real-world potency of these interventions. OBJECTIVE: To assess the effectiveness of nurse behavioral management of DM and HTN in community practices among patients with both diseases. DESIGN: The study was designed as a patient-level randomized controlled trial. PARTICIPANTS: Participants included adult patients with both type 2 DM and HTN who were receiving care at one of nine community fee-for-service practices. Subjects were required to have inadequately controlled DM (hemoglobin A1c [A1c] ≥ 7.5%) but could have well-controlled HTN. INTERVENTIONS: All patients received a call from a nurse experienced in DM and HTN management once every two months over a period of two years, for a total of 12 calls. Intervention patients received tailored DM- and HTN- focused behavioral content; control patients received non-tailored, non-interactive information regarding health issues unrelated to DM and HTN (e.g., skin cancer prevention). MAIN OUTCOMES AND MEASURES: Systolic blood pressure (SBP) and A1c were co-primary outcomes, measured at 6, 12, and 24 months; 24 months was the primary time point. RESULTS: Three hundred seventy-seven subjects were enrolled; 193 were randomized to intervention, 184 to control. Subjects were 55% female and 50% white; the mean baseline A1c was 9.1% (SD = 1%) and mean SBP was 142 mmHg (SD = 20). Eighty-two percent of scheduled interviews were conducted; 69% of intervention patients and 70% of control patients reached the 24-month time point. Expressing model estimated differences as (intervention--control), at 24 months, intervention patients had similar A1c [diff = 0.1 %, 95 % CI (-0.3, 0.5), p = 0.51] and SBP [diff = -0.9 mmHg, 95% CI (-5.4, 3.5), p = 0.68] values compared to control patients. Likewise, DBP (diff = 0.4 mmHg, p = 0.76), weight (diff = 0.3 kg, p = 0.80), and physical activity levels (diff = 153 MET-min/week, p = 0.41) were similar between control and intervention patients. Results were also similar at the 6- and 12-month time points. CONCLUSIONS: In nine community fee-for-service practices, telephonic nurse case management did not lead to improvement in A1c or SBP. Gains seen in telephonic behavioral self-management interventions in optimal settings may not translate to the wider range of primary care settings.

Background: Small molecule inhibitors of the zinc finger domain (ZFI) in the nucleocapsid protein (NCp7) of HIV-1 are potent inhibitors of HIV and SIV
replication and may have utility as topical products to prevent infection. Furthermore, intravaginal rings (IVRs) were developed as coitally-independent,
sustained release devices which could be used for administration of HIV microbicides. The aims of these studies were to demonstrate that IVRs sized for
macaques are practical and compatible with the current generation of thioester-based NCp7 inhibitors.
Methods: Non-medicated silicone elastomer vaginal rings of various sizes thought to be applicable for macaques were prepared and tested for vaginal fit in Pigtailed and Chinese Rhesus macaques. Macaques were monitored for 8 weeks for mucosal disruption by colposcopy and proinflammatory cytokine markers in cervical vaginal lavages (CVL) using Luminex bead-based technology. Three different ZFIs (compounds 52, 89 and 122, each derived from an N-substituted S-acyl-2-mercaptobenzamide thioester scaffold) were loaded at 50 mg into an optimal matrix-type ring design. In vitro continuous release studies were then conducted over 28 days and analyzed by HPLC. Rate of release was determined by linear regression analysis.
Results: Qualitative evaluation at the time of ring insertion suggested that the 25 mm ring provided optimal fit in both macaque species. All rings remained in
place during the study period (2 to 4 weeks), and the animals did not attempt to remove the rings. No tissue irritation was observed, and no signs of physical
discomfort were noted. Also, no significant induction of cervicovaginal proinflammatory markers was observed during the 8-week period during and following ring insertion. One Pigtailed macaque showed elevated IL-8 levels in the CVL during the period when the ring was in place; however, these levels were comparable to those observed in two control macaques. In vitro release of the ZFIs peaked at day 1 and then continually declined to near steady-state rates between 20-30 mcg/day. The percent release after 14 days was 2.9, 2.0 and 0.9 for ZFI 89, 52 and 122, respectively.
Conclusions: IVRs of 25mm diameter, determined to be the optimal size for macaques, were well tolerated and did not induce inflammation. Release of all ZFI compounds followed t 0.5 kinetics. These findings suggest that efficacy testing in primate models is warranted to fully evaluate the potential to prevent
transmission.

Growing evidence suggests that significant motor problems are associated with a diagnosis of Autism Spectrum Disorders (ASD), particularly in catching tasks. Catching is a complex, dynamic skill that involves the ability to synchronise one's own movement to that of a moving target. To successfully complete the task, the participant must pick up and use perceptual information about the moving target to arrive at the catching place at the right time. This study looks at catching ability in children diagnosed with ASD (mean age 10.16 ± 0.9 years) and age-matched non-verbal (9.72 ± 0.79 years) and receptive language (9.51 ± 0.46) control groups. Participants were asked to "catch" a ball as it rolled down a fixed ramp. Two ramp heights provided two levels of task difficulty, whilst the sensory information (audio and visual) specifying ball arrival time was varied. Results showed children with ASD performed significantly worse than both the receptive language (p =.02) and non-verbal (p =.02) control groups in terms of total number of balls caught. A detailed analysis of the movement kinematics showed that difficulties with picking up and using the sensory information to guide the action may be the source of the problem. © 2013 Elsevier Ltd.

Suitable ester prodrugs of 17b-estradiol are identified, thus permitting effective sustained and controlled estrogen replacement therapy (ERT) from an elastomeric, silicone intravaginal ring (IVR). IVR devices of reservoir design were prepared by blending silicone elastomer base with n-propylorthosilicate (cross-linker) and 10% w/w of 17b-estradiol or an ester prodrug, the mix being activated with 0.5% w/w stannous octoate and cured at 808C for 2 min. A rate-controlling membrane was similarly prepared, without the active agent. IVR devices were of cross-sectional diameter 9 mm, outer diameter 54 mm, with core cross-sectional diameter of 2 mm and core length varied as required. Sink conditions were evident for the 17b-estradiol esters in 1.0% aqueous benzalkonium chloride solution. The low release rates into 0.9% w/v saline of the lipophilic valerate and benzoate esters were due to their intrinsically low aqueous solubilities. In vivo, these esters failed to raise plasma estradiol above baseline levels in postmenopausal human volunteers, despite good in vitro release characteristics under sink conditions. The best release rates under sink conditions, in combination with substantial aqueous solubilities as indicated by the release rates into saline, were observed for the acetate and propionate esters. A
combination of drug release characteristics, short plasma half-life and a toxicologically acceptable hydrolysis product indicated that 17b-estradiol-3-acetate was the prodrug of choice for IVR delivery of ERT. In vivo, an IVR device releasing
100 mg/day of estradiol as its 3-acetate ester maintained over 84 days a circulating plasma concentration in the region of 300 pmol l , within the clinically desirable range for ERT.
