Objectives: To assess the accuracy of reporting from both a diet history and food record and identify some of the characteristics of more accurate reporters in a group of healthy adult volunteers for an energy balance study. Design: Prospective measurements in free-living people. Setting: Wollongong, Australia. Subjects: Fifteen healthy volunteers (seven male, eight female; aged 22 -59 y; body mass index (BMI) 19 - 33 kg/m(2)) from the local community in the city of Wollongong, Australia. Interventions: Measurement of energy intake via diet history interview and 7 day food records, total energy expenditure by the doubly labelled water technique over 14 days, physical activity by questionnaire, and body fat by dual-energy X-ray absorptiometry. Results: Increased misreporting of energy intake was associated with increased energy expenditure (r = 0.90, P < 0.0001, diet history; r(s)=0.79, P=0.0005, food records) but was not associated with age, sex, BMI or body fat. Range in number of recorded dinner foods correlated positively with energy expenditure (r(s)=0.63, P=0.01) and degree of misreporting (r(s)=0.71, P=0.003, diet history; r(s)=0.63, P=0.01, food records). Variation in energy intake at dinner and over the whole day identified by the food records correlated positively with energy expenditure (r=0.58, P = 0.02) and misreporting on the diet history (r=0.62, P=0.01). Conclusions: Subjects who are highly active or who have variable dietary and exercise behaviour may be less accurate in reporting dietary intake. Our findings indicate that it may be necessary to screen for these characteristics in studies where accuracy of reporting at an individual level is critical. Adipose tissue is a highly active endocrine organ secreting a range of soluble products with both local and distant actions. These hormones have important roles in metabolism, reproduction, cardiovascular function and immunity. It is now evident that adipose endocrine function directly influences other organ systems, including the brain, liver and skeletal muscle. The endocrine function of adipose tissue is significantly regulated by nutritional status, and both are inextricably linked to the energy storage role of adipose tissue. This chapter highlights the endocrinology of adipose tissue by concentrating on functional aspects of the secreted products. The data of particular relevance to humans are highlighted, and areas in need of future research are suggested.

O advento de novas drogas anti-retrovirais como os inibidores de protease provocou mudanças sensíveis na morbidade e mortalidade de pacientes infectados pelo HIV. OBJETIVOS: Avaliar o impacto das novas drogas anti-retrovirais (Highly Active Anti-retroviral Therapy - HAART) na prevalência de otite média crônica em população pediátrica infectada pelo HIV. MÉTODOS: Analisamos os prontuários de 471 crianças com idade entre zero e 12 anos e 11 meses portadoras de HIV atendidas no ambulatório de AIDS de Clínica Otorrinolaringológica do HCFMUSP. As crianças foram divididas em dois grupos, de acordo com a faixa etária: 0 a 5 anos e 11 meses e 6 a 12 anos e 11 meses, e classificadas como portadoras de otite média crônica, baseadas em achados de anamnese, otoscopia, audiometria e imitanciometria. As prevalências de otite média crônica apresentadas e as contagens de linfócitos T CD4+ foram comparadas entre as crianças em uso ou não de HAART. RESULTADOS: Das 459 crianças atendidas, 65 (14,2%) apresentavam otite média crônica. Observamos, nas crianças de 0 a 5 anos e 11 meses que o uso de HAART esteve associado a significante menor prevalência de otite média crônica (p = 0,02), e maior contagem de linfócitos T CD4+ (p < 0,001). CONCLUSÃO: O uso de HAART esteve associado à menor prevalência da forma crônica de otite média entre crianças menores de 6 anos infectadas pelo HIV, provavelmente como conseqüência do aumento promovido na contagem de linfócitos T CD4+.

OBJECTIVE: To assess the association between dietary intake and central obesity among people living with HIV/AIDS and receiving highly active antiretroviral therapy. METHODS: A cross-sectional study was conducted involving 223 adult individuals in the city of São Paulo city in 2002. The study population was classified according to central obesity, defined as waist-to-hip ratio >0.95 for men and >0.85 for women. The dietary variables studied were energy consumption (in calories and calories/kilo of body weight), macronutrients (in grams and % of energy intake), total fiber (grams) and fruit and vegetables intake (grams). The potential confounders examined were sex, skin color, age, schooling, income, body mass index, physical activity, smoking habits, peripheral CD4+ T lymphocyte count and length of protease inhibitor use. The multiple logistic regression model was performed in order to evaluate the association between central obesity and dietary intake. RESULTS: The prevalence of central obesity was 45.7% and it was associated with greater consumption of lipids: for every increase of 10g of lipid intake the odds of central obesity increased 1.28 times. Carbohydrate consumption showed negative association (OR=0.93) with central obesity after adjustment for control variables. CONCLUSIONS: The results suggest that the amount of carbohydrates and lipids in the diet, regardless of total energy intake, may modify the chance of developing central obesity in the studied population. Nutritional interventions may be beneficial for preventing central obesity among HIV/AIDS patients.

OBJECTIVE: To evaluate metabolic changes associated with highly active antiretroviral therapy (HAART) in HIV-positive patients, and to identify risk factors associated. METHODS: Retrospective study that included 110 HIV-positive patients who where on HAART in the city of Porto Alegre (Southern Brazil) between January 2003 and March 2004. Data on demographic variables, cigarette smoking, diabetes mellitus, cholesterol and triglyceride levels, stage of HIV infection, antiretroviral therapy and HCV coinfection were collected. General linear models procedure for repeated measures was used to test the interaction between HAART and HCV coinfection or protease inhibitor treatment. RESULTS: Total cholesterol, triglycerides, and glucose levels significantly increased after receiving HAART (p<0.001 for all variables), but no interaction with protease inhibitors was seen for total cholesterol, glucose and triglyceride levels (interaction treatment*protease inhibitors p=0.741, p=0.784, and p=0.081, respectively). An association between total cholesterol levels and HCV coinfection was found both at baseline and follow-up (effect of HCV coinfection, p=0.011). Glucose levels were increased by HAART (treatment effect, p=0.036), but the effect was associated to HCV coinfection (treatment*HCV effect, p=0.018). Gender, smoking habit, intravenous drug use and age were not significantly associated with cholesterol, triglyceride and glucose changes. CONCLUSIONS: HCV-infected patients at baseline were significantly less likely to develop hypercholesterolemia. The results provide further evidence of the role of HAART for the development of metabolic disturbances.

Foi realizada uma revisão da literatura científica sobre adesão terapêutica à highly active antiretroviral therapy e sobre a qualidade de vida dos pacientes portadores do HIV, indexada no MEDLINE no período entre 1998 e 2008. Foram incluídos estudos em pacientes maiores de 18 anos, publicados em português, espanhol ou inglês. Foram excluídos estudos de revisão, relatos de caso e cartas. Dos 21 estudos encontrados, 12 foram incluídos (três ensaios clínicos, três coortes prospectivos, seis transversais). A relação entre qualidade de vida e adesão terapêutica permanece controversa, embora estudos descritivos apontem a possibilidade de uma relação positiva. Os resultados podem ter sido influenciados por características específicas dos ensaios clínicos descritos e mostram não haver consenso sobre o impacto da adesão terapêutica sobre a qualidade de vida dos pacientes.

A investigação científica que se seguiu à descoberta da infecção pelo vírus da imunodeficiência humana (VIH) representou um marco histórico importante, permitindo a instituição de terapêutica e alterando a história natural da infecção, melhorando as expectativas dos indivíduos infectados.Se, por um lado, a terapêutica antirretroviral de alta eficácia (do inglês, Highly Active Antiretroviral Theraphy: HAART), nos países desenvolvidos, teve um enorme impacto na melhoria da qualidade de vida e esperança média de vida dos indivíduos infectados, permitindo-lhes encarar a infecção como uma doença crónica, por outro lado, introduziu-se uma nova dinâmica epidemiológica na transmissão de VIH e um novo desafio para a sua prevenção.Os comportamentos sexuais são o factor-alvo através do qual se pode, de uma forma mais eficaz, prevenir a possibilidade de infectar um qualquer indivíduo na comunidade. Parece, pois, importante haver necessidade de um olhar atento da Saúde Pública sobre a vida sexual dos indivíduos infectados, não só na tentativa de, promover a sua qualidade de vida, como também, de prevenir a disseminação da doença, e, ainda mais importante, prevenir a disseminação do vírus nas suas formas resistentes.O objectivo deste estudo foi analisar a consistência do uso de preservativo/práticas de sexo seguro, em indivíduos infectados por VIH, seguidos em Consulta de Imunodepressão, do Serviço de Doenças Infecciosas, do Hospital de Santa Maria. Pretende-se avaliar, no âmbito da “Educação para a Saúde”, a necessidade de aconselhamento aos indivíduos infectados, relativamente aos seus comportamentos sexuais.Para o efeito, realizou-se um estudo descritivo e transversal. A população envolvida compreendeu todos os indivíduos infectados seguidos na Consulta de Imunodepressão. A amostra foi seleccionada através de aleatorização simples e constituída por 98 elementos. A colheita de dados foi efectuada através do método de entrevista, a qual só teve início após a assinatura do consentimento informado. O estudo decorreu entre 14 de Março e 27 de Junho de 2008.Foram analisados os dados relativamente às variáveis: sócio-demográficas, estado serológico da infecção, história sexual, consumo de substâncias, comportamento sexual e percepção de risco da transmissão da infecção por VIH.Os comportamentos de sexo não seguro foram associados ao número de parceiros sexuais/ano (p=0,007), ter parceiro sexual habitual (p=0,010), ter múltiplos parceiros sexuais ocasionais (p=0,000) e à prática de sexo oral (sexo oral receptivo, p=0,001; sexo oral activo, p=0,006).Os resultados demonstraram a importância da prevenção secundária nos indivíduos infectados por VIH. A necessidade dos centros de atendimento estarem providos de mensagem de prevenção, ensinos e aconselhamento a todas as pessoas infectadas e, também, aos seus parceiros sexuais, aumentando-lhes a qualidade de vida e reduzindo o risco de transmissão da infecção por HIV. ABSTRACT: The scientific research that followed the finding of human immunodeficiency virus (HIV) infection represented an important historical mark, allowing therapeutic institution and changing the natural history of the infection, thus improving the expectations of infected individuals. If by one side the Highly Active Antiretroviral Therapy (HAART) had an enormous impact on the improvement of life quality and life expectancy of the infected individuals on the developed countries, allowing them to face the infection as a chronic disease, on the other side it was introduced a epidemiologic dynamic on the HIV transmission and a new challenge for its prevention. Sexual behaviour is the main factor through which is possible to prevent more efficiently the possibility of infection of a community’s individual. Thus it appears important the need of Public Health to have a careful look into the sexual life of infected individuals, not only to try to promote their life quality but as well to prevent the dissemination of the disease and more importantly prevent the dissemination of the virus in its resistant forms. The objective of this study was to analyse the consistency of the use of condoms/safe sex by individuals infected by HIV that are followed by the Infectious Disease Service of the Santa Maria Hospital through Immune-depression Appointments. The goal was to evaluate the need of advisement to the infected individuals relatively to their sexual behaviours. For that it was done a descriptive and transversal study. The involved population included all infected individuals followed in the Immune-depression Appointments. The sample was selected by simple randomisation and was composed by 98 elements. The data was obtained by interview method, which was initiated after signing the informed consent. The study was took place between 14 March and 27 June 2008. It was analysed the data relative to the variables: socio-demographics, serologic state of the infection, sexual history, substances consumption, sexual behaviour and risk perception on the transmission of HIV infection. The unsafe sexual behaviours were associated to the number of sexual partners/year (p=0,007), having steady partner (p= 0,010), having occasional sexual partners (p=0,000) and oral sex practice (receptive oral sex, p=0,001; active oral sex, p= 0,006).The results demonstrated the importance of the secondary prevention on individuals infected by HIV. It also demonstrated the need of providing the attendance centres with a prevention message, teachings and advises to all infected persons as well as to their sexual partners, increasing their life quality and reducing the transmission risk of HIV infection.

Resumo O objectivo geral deste trabalho foi contribuir para optimizar a terapêutica anti-retroviral e o seu impacto na qualidade de vida do indivíduo infectado pelo vírus da imunodeficiência humana. Pretendia-se definir se o análogo não-nucleósido inibidor da transcriptase reversa do vírus da imunodeficiência humana, efavirenz, cumpria os requisitos para ser monitorizado na prática clínica, estabelecer as condições para a sua eventual monitorização e, simultaneamente, investigar outras acções farmacodinâmicas do efavirenz em terapêuticas prolongadas. Os critérios que fundamentam a indicação da monitorização das concentrações plasmáticas de fármacos, em geral, incluem: correlação entre a concentração do fármaco e a eficácia/toxicidade; variabilidade inter-individual elevada; variabilidade intra-individual e janela terapêutica reduzidas e ainda a elevada probabilidade de interacções medicamentosas. A correlação entre concentração plasmática de efavirenz e eficácia/toxicidade era conhecida e o facto de o efavirenz ser substrato, indutor e inibidor do sistema enzimático citocromo P450 e ser utilizado em terapêuticas crónicas e nunca em monoterapia, constituíam fortes argumentos para a aplicação da monitorização terapêutica ao efavirenz. O presente trabalho contribuiu para o conhecimento de outros critérios, nomeadamente, a variabilidade nas concentrações plasmáticas deste fármaco, entre indivíduos e no mesmo indivíduo, e permitiu definir diferentes aspectos para a prática da monitorização terapêutica deste fármaco, entre eles, o volume de plasma necessário, o parâmetro farmacocinético a avaliar e a periodicidade das quantificações. Para se atingirem os objectivos definidos foi necessário, em primeiro lugar, proceder à instalação e validação de um método de quantificação de concentrações de efavirenz, em plasma de indivíduos infectados pelo vírus da imunodeficiência humana: ficou disponível no Laboratório de Farmacologia da Faculdade de Ciências Médicas, um método que permite a monitorização das concentrações plasmáticas de nove fármacos anti-retrovirais (nevirapina, indinavir, amprenavir, atazanavir, ritonavir, efavirenz, lopinavir, saquinavir e nelfinavir). O método desenvolvido está presentemente a ser utilizado na monitorização terapêutica destes fármacos e em estudos Farmacológicos. Esta quantificação é realizada numa única corrida analítica de cromatografia líquida de elevada eficiência, a partir de 0,4 mL de plasma de cada indivíduo e a sua qualidade é avaliada, bianualmente, por uma entidade externa. Posteriormente, com o objectivo de as comparar, procurou-se conhecer a variabilidade entre indivíduos e intra-individual das concentrações lasmáticas do fármaco e concluiu-se que a variabilidade entre indivíduos é superior à intra-individual, o que suporta a monitorização das suas concentrações. Uma vez encontrada uma variabilidade inter-individual elevada, surgiu um outro objectivo específico, que consistiu na identificação de possíveis factores a justificassem. Na presente dissertação foi mostrado que o sexo, idade, peso e etnia não justificam por si só esta variação, não sendo possível o ajuste de dose com base nestas variáveis. Esta conclusão constitui um factor adicional que reforça que a toma da dose recomendada de efavirenz poderá não ser apropriada para todos os indivíduos. A co-infecção pelos vírus da hepatite B e/ou C é comum nesta população e poderia ser um dos factores implicados nesta variabilidade farmacocinética. A realização do presente trabalho permitiu sugerir que a presença desta co-infecção per se não contribui para o aumento das concentrações plasmáticas do fármaco; que, em indivíduos co-infectados com função hepática normal, não há um risco acrescido de toxicidade dependente da concentração e que as indicações para a monitorização terapêutica de efavirenz em indivíduos co-infectados, com função hepática normal, são semelhantes aquelas descritas para indivíduos mono-infectados pelo vírus da imunodeficiência humana. Um outro objectivo específico deste trabalho surgiu quando foi descrito que os efeitos dos análogos não-nucleósidos inibidores da transcriptase reversa no perfil de lípidos e lipoproteínas dos indivíduos pareciam diferir dos efeitos descritos para os inibidores de protease, que eram frequentemente associados a deslipidémia. Os análogos não-nucleósidos inibidores da transcriptase reversa tinham sido associados a aumentos nos níveis de colesterol associado às lipoproteínas de elevada densidade. Esta observação, além de não ser consensual, podia ser imputada ao decréscimo na carga viral dos indivíduos em terapêutica e correspondia a estudos observacionais de curta-duração. Estes factos estimularam a realização de uma análise prospectiva dos valores da concentração de lípidos e lipoproteínas em doentes medicados com efavirenz e à avaliação da sua eventual relação com a concentração deste nti-retroviral, a curto e a longo-termo. Pela primeira vez, foi demonstrado que o efeito do efavirenz no colesterol associado às lipoproteínas de elevada densidade permaneceu durante 36 meses, que o aumento é dependente do valor basal destas lipoproteínas e da concentração plasmática do fármaco. Mostrou-se também que, em associação a este aumento quantitativo, o efavirenz estava associado a um aumento qualitativo, com uma melhoria da função antioxidante destas lipoproteínas, avaliada pela actividade do enzima paraoxonase-1. Em resumo, os diferentes estudos incluídos na presente dissertação têm como conclusão geral que é possível optimizar a resposta à terapêutica com efavirenz através da monitorização das suas concentrações plasmáticas. A realização deste trabalho contribuiu para o conhecimento científico através: 1. Da instalação e validação de um método de quantificação de concentrações de análogos não-nucleósidos inibidores da transcriptase reversa e inibidores da protease em plasma de indivíduos infectados pelo vírus da imunodeficiência humana. 2. Do estudo da variabilidade inter e intra-individual nas concentrações plasmáticas de efavirenz. A superioridade da variabilidade inter-individual relativamente à associada ao mesmo indivíduo comprova a importância de monitorizar as concentrações plasmáticas deste fármaco. 3. Da definição de procedimentos operativos para a monitorização terapêutica do efavirenz em geral e numa população particular: os indivíduos co-infectados pelos vírus da hepatite B e/ou C com função hepática normal. 4. Da descoberta de acções farmacodinâmicas do efavirenz, a longo prazo, nomeadamente o efeito benéfico (quantitativo e qualitativo) no colesterol associado às lipoproteínas de elevada densidade. Este efeito é mantido durante três anos e é dependente da concentração plasmática do fármaco, o que salienta a importância de monitorizar as suas concentrações.

A high incidence of cytomegalovirus (CMV) infections is observed in Brazil. These viruses are causatives of significant morbidity and mortality among patients with advanced human immunodeficiency virus (HIV) infection. This work, shows the application of a PCR on determination of CMV load in the buffy coat and plasma. We analyzed the samples of 247 HIV infected patients in order to diagnose CMV infection and disease. We developed a semi-quantitative PCR that amplifies part of the glycoprotein B (gB) gene of CMV. The semi-quantitative PCR was carried out only in positive clinical samples in a qualitative PCR confirmed by a nested-PCR. CD4 lymphocyte count, HIV viral load and CMV disease symptom were correlated with CMV load. CMV genome was detected in the buffy coat of 82 of 237 (34.6%) patients, in 10 of these the CMV load was determined varying between 928 and 332 880 viral copies/mug DNA. None of these 237 patients developed any suggestive manifestation of CMV disease. For the other 10 HIV infected patients selected based on the suspicion of CMV disease, CMV genome was detected in only one case. This patient presented a high CMV load, 8 000 000 copies/mug DNA, and developed a disseminated form of CMV disease including hepatitis and retinitis. Our results were greatly influenced by the impact of the highly active antiretroviral therapy that reduced incidence of CMV viremia and occurrence of CMV disease in the HIV infected patients.

Opportunistic diseases in HIV-infected patients have changed since the introduction of highly active anti-retroviral therapy (HAART). This study aims at evaluating the frequency of associated diseases in patients with AIDS admitted to an university hospital of Brazil, before and after HAART. The medical records of 342 HIV-infected patients were reviewed and divided into two groups: group 1 comprised 247 patients before HAART and, group 2, 95 patients after HAART. The male-to-female rate dropped from 5:1 to 2:1for HIV infection. There was an increase in the prevalence of tuberculosis and toxoplasmosis, with a decrease in Kaposi's sarcoma, histoplasmosis and cryptococcosis. A reduction of in-hospital mortality (42.0% vs. 16.9%; p = 0.00002) has also occurred. An agreement between the main clinical diagnoses and autopsy findings was observed in 10 out of 20 cases (50%). Two patients with disseminated schistosomiasis and 2 with paracoccidioidomycosis are reported. Overall, except for cerebral toxoplasmosis, it has been noticed a smaller proportion of opportunistic conditions related to severe immunosuppression in the post HAART group. There was also a significant reduction in the in-hospital mortality, possibly reflecting improvement in the treatment of the HIV infection.

INTRODUCTION: Information about HIV phenotypes of resistant to available ART and the influence of different risk factors on virological failures (VF) in Costa Rican HIV positive patients prior or during HAART is unknown. MATERIALS AND METHODS: Eighty nine samples, 72 VF and 17 basal (before treatment) were analyzed by examining resistant mutants in reverse transcriptase (RT) and protease (PT) regions using Trugene or LIPA genotyping kits. Sixty eight control patients were selected and relevant information was collected in a questionnaire. RESULTS: Poor adherence, presence of resistant mutations and number of treatment's changes were the only significant factors found (p = 0.006, 0.04 and 0.01 respectively). From 66 sequenced samples, 78%, 50% and 50% showed resistance to NRTI (nucleoside reverse transcriptase inhibitors), NNRT (non-nucleoside reverse transcriptase inhibitors) and PI (protease inhibitors), respectively. The most frequent mutations were M41L, M184V, and T215FY in RT and L62PI, L10FIRV and M36I in PT. DISCUSSION: The most important factor related to treatment response in this study was adherence to treatment. Mutations in RT were related to the treatment failure while the ones found in PT were secondary mutations which have been previously described to influence the selection of primary resistance mutations in these regions. The study reveals the urgency to detect resistant mutations in VF to be considered by physicians for selection of treatment schedule, to analyze basal HIV patients for monitoring of the spread of resistant mutations and the importance to reinforce the adherence in the patients for overall treatment outcome.

BACKGROUND: Before the introduction of highly active antiretroviral therapy (HAART), CMV retinitis was a common complication in patients with advanced HIV disease and the therapy was well established; it consisted of an induction phase to control the infection with ganciclovir, followed by a lifelong maintenance phase to avoid or delay relapses. METHODS: To determine the safety of CMV maintenance therapy withdrawal in patients with immune recovery after HAART, 35 patients with treated CMV retinitis, on maintenance therapy, with CD4+ cell count greater than 100 cells/mm³ for at least three months, but almost all patients presented these values for more than six months and viral load < 30000 copies/mL, were prospectively evaluated for the recurrence of CMV disease. Maintenance therapy was withdrawal at inclusion, and patients were monitored for at least 48 weeks by clinical and ophthalmologic evaluations, and by determination of CMV viremia markers (antigenemia-pp65), CD4+/CD8+ counts and plasma HIV RNA levels. Lymphoproliferative assays were performed on 26/35 patients. RESULTS: From 35 patients included, only one had confirmed reactivation of CMV retinitis, at day 120 of follow-up. No patient returned positive antigenemia tests. No correlation between lymphoproliferative assays and CD4+ counts was observed. CONCLUSION: CMV retinitis maintenance therapy discontinuation is safe for those patients with quantitative immune recovery after HAART.

The simultaneous presence of infectious organisms within cutaneous lesions of Kaposi sarcoma in persons with AIDS has been demonstrated. We describe a patient with concurrent leprosy and Kaposi sarcoma presenting as an immune reconstitution inflammatory syndrome in the setting of AIDS.

Neurological disorders caused by Cytomegalovirus (CMV) in patients with Acquired Immunodeficiency Syndrome (AIDS) are rarely reported in the Highly Active Antiretroviral Therapy (HAART) period. The objective of this study was to describe the main clinical and laboratory features of patients with CMV-related neurological complications in HIV-infected patients admitted to a referral center in São Paulo, Brazil. CMV disease requires the identification of the virus in the cerebrospinal fluid (CSF) using Polymerase Chain Reaction (PCR). Thirteen cases were identified between January, 2004 and December, 2008. The median age of patients was 38 years and nine (69%) were men. At admission all patients were aware of their HIV status and only four (31%) patients were on HAART. Patients who were not on antiretroviral therapy before admission received HAART while inpatients. CMV disease was the first AIDS-defining illness in eight (62%) patients. The neurologic syndromes identified were diffuse encephalitis (n = 7; 62%), polyradiculopathy (n = 7; 54%), focal encephalitis (rhombencephalitis) (n = 1; 8%), and ventriculo-encephalitis (n = 1; 8%). Seven (54%) patients presented extra-neural CMV disease and four (31%) had retinitis. The median of CD4+ T-cell count was 13 cells/µL (range: 1-124 cells/µL). Overall in-hospital mortality was 38%. Eight patients used ganciclovir or foscarnet (in-hospital mortality: 50%) and five patients used ganciclovir and foscarnet (in-hospital mortality: 20%). None of the patients fulfilled the diagnosis criteria of immune reconstitution inflammatory syndrome. Four patients were lost to follow-up, and three patients presented immune recovery and discontinued secondary prophylaxis. Although infrequent, distinct neurological syndromes caused by CMV continue to cause high mortality among AIDS patients. Survival depends upon the use of effective antiviral therapy against CMV and the early introduction of HAART.

Background: Data on human immunodeficiency virus (HIV) infected patients receiving dialysis in Portugal is scarce. Methods: This nationwide epidemiological survey retrospectively evaluates HIV-infected patients on chronic dialysis in Portugal between 1997 and 2002. Results: Sixty-six patients were evaluated (mean age: 39.1±1.6 years, 47 men, 35 black African). Sixty-two patients started dialysis and 4 patients who were receiving dialysis had HIV seroconversion. Eighty-five percent of patients were treated in Lisbon. The annual incidence of HIV-infected patients on chronic dialysis was 0.5% in 1997 and 0.9% in 2002. Seventy-eight percent of patients were HIV-1 infected , 13% had hepatitis B and 31% hepatitis C. Sexual contact was the mode of transmission of HIV in 53% of cases. Four patients had biopsy-proved HIV-associated nephropathy. Ninety-five percent of patients were on chronic hemodialysis. Fifty percent of patients had acquired immunodeficiency syndrome. At follow-up, 12 patients died. HIV-infected CKD patient survival after starting dialysis was 80% at 3 years. Conclusion: The incidence of HIV-infected patients on chronic dialysis in Portugal has almost doubled. Widespread use of highly active antiretroviral therapy and the increasing number of black Africans from former overseas Portuguese colonies now living in Portugal are possible reasons for this large increase.
