PURPOSE: To determine the lower limit of dose reduction with hybrid and fully iterative reconstruction algorithms in detection of endoleaks and in-stent thrombus of thoracic aorta with computed tomographic (CT) angiography by applying protocols with different tube energies and automated tube current modulation. MATERIALS AND METHODS: The calcification insert of an anthropomorphic cardiac phantom was replaced with an aortic aneurysm model containing a stent, simulated endoleaks, and an intraluminal thrombus. CT was performed at tube energies of 120, 100, and 80 kVp with incrementally increasing noise indexes (NIs) of 16, 25, 34, 43, 52, 61, and 70 and a 2.5-mm section thickness. NI directly controls radiation exposure; a higher NI allows for greater image noise and decreases radiation. Images were reconstructed with filtered back projection (FBP) and hybrid and fully iterative algorithms. Five radiologists independently analyzed lesion conspicuity to assess sensitivity and specificity. Mean attenuation (in Hounsfield units) and standard deviation were measured in the aorta to calculate signal-to-noise ratio (SNR). Attenuation and SNR of different protocols and algorithms were analyzed with analysis of variance or Welch test depending on data distribution. RESULTS: Both sensitivity and specificity were 100% for simulated lesions on images with 2.5-mm section thickness and an NI of 25 (3.45 mGy), 34 (1.83 mGy), or 43 (1.16 mGy) at 120 kVp; an NI of 34 (1.98 mGy), 43 (1.23 mGy), or 61 (0.61 mGy) at 100 kVp; and an NI of 43 (1.46 mGy) or 70 (0.54 mGy) at 80 kVp. SNR values showed similar results. With the fully iterative algorithm, mean attenuation of the aorta decreased significantly in reduced-dose protocols in comparison with control protocols at 100 kVp (311 HU at 16 NI vs 290 HU at 70 NI, P ≤ .0011) and 80 kVp (400 HU at 16 NI vs 369 HU at 70 NI, P ≤ .0007). CONCLUSION: Endoleaks and in-stent thrombus of thoracic aorta were detectable to 1.46 mGy (80 kVp) with FBP, 1.23 mGy (100 kVp) with the hybrid algorithm, and 0.54 mGy (80 kVp) with the fully iterative algorithm.

The directional flow of lymph is maintained by hundreds of intraluminal lymphatic valves. Lymphatic valves are crucial to prevent lymphedema, accumulation of fluid in the tissues, and to ensure immune surveillance; yet, the mechanisms of valve formation are only beginning to be elucidated. In this chapter, we will discuss the main steps of lymphatic valve morphogenesis, the important role of mechanotransduction in this process, and the genetic program regulated by the transcription factor Foxc2, which is indispensable for all steps of valve development. Failure to form mature collecting lymphatic vessels and valves causes the majority of postsurgical lymphedema, e.g., in breast cancer patients. Therefore, this knowledge will be useful for diagnostics and development of better treatments of secondary lymphedema.

OBJECTIVE. Acute mesenteric venous thrombosis signs at MDCT are well described, but the literature lacks studies assessing their evolution. We aimed to describe the radiologic evolution of isolated acute mesenteric venous thrombosis and associated prognostic factors. MATERIALS AND METHODS. Patients with isolated acute mesenteric venous thrombosis with follow-up for a minimum of 1 month with MDCT were selected. Images at the acute phase and on follow-up were reviewed in consensus reading. For acute mesenteric venous thrombosis, we searched for low-attenuated intraluminal filling defect. For chronic mesenteric venous thrombosis, we searched for vessel stenosis or occlusion associated with collateral mesenteric veins. Treatment, thrombosis risk factor, symptoms, location, and length and diameter of mesenteric venous thrombosis were reported and correlated with evolution over time. RESULTS. Twenty patients (nine women and 11 men; mean age, 52 years) were selected. Four patients recovered without radiologic sequelae, and 16 developed chronic mesenteric venous thrombosis signs. Anticoagulation did not influence recovery (p = 1). Patients with recovery compared with patients with chronic mesenteric venous thrombosis showed more frequent central lesions (p = 0.03). At diagnosis, the thrombosed segment was shorter and larger in the complete radiologic recovery group compared with the chronic mesenteric venous thrombosis signs group: mean length (± SD) 6.25 ± 3.21 cm and 12.81 ± 5.96 cm, respectively (p = 0.01); mean transverse diameter 1.82 ± 0.42 cm and 1.12 ± 0.34 cm, respectively (p = 0.01). Mesenteric fat infiltration at diagnosis was more frequent in the chronic mesenteric venous thrombosis signs group than in the complete recovery group (p = 0.03). CONCLUSION. Most cases of acute mesenteric venous thrombosis evolve toward the chronic form with vein stenosis or occlusion and development of collateral veins. Location, length of mesenteric venous thrombosis, transverse diameter of the vein, and mesenteric fat infiltration at diagnosis are determinant factors for mesenteric venous thrombosis evolution.

PURPOSE OF THE STUDY: This prospective study reports our preliminary results with local anaesthesia (LA) for carotid endarterectomy (CEA). MATERIAL AND METHODS: Twenty CEA in nineteen patients were performed using a three-stage local infiltration technique. CEA were performed through a short Duplex-assisted skin incision (median length: 55 mm) using a retro-jugular approach and polyurethane patch closure (median length: 35 mm). RESULTS: There were 13 men and 6 women with a mean age of 71.2 years. The indications of CEA were asymptomatic lesions in 11 cases, stroke in 7 cases and transient ischaemic attack in 2 cases. The median degree of internal carotid artery stenosis was 90%. One patient (5%) required an intraluminal shunt. There were no peri-operative deaths, stroke or conversion to general anaesthesia (GA). The median length of stay was 3 days. CONCLUSIONS: LA is a good alternative to GA. It can be used after a feasibility study and a short teaching procedure. In our centre, it is a safe and effective procedure associated with low morbidity, high acceptance by patients and a short hospital stay.

Purpose: To work out certain, well‑defined aetiologies frequently associated with mesenteric venous thrombosis (MVT) in order to predict a typical population at risk, since MVT is nowadays often incidentally detected on cross‑sectional imaging. To demonstrate the MDCT features, frequency and extent of associated bowel ischemia according to the underlying pathology. Methods and Materials: Our electronic database revealed 71 patients (25 women, mean age 55) with thrombosis of the superior and/or inferior mesenteric vein detected by MDCT between 2000 and 2008. Two radiologists jointly reviewed the corresponding MDCT features including intraluminal extension, underlying aetiology and associated bowel ischemia, if present. Results: MVT was associated with carcinoma in 31 (43.7%) patients (pancreas 21.1%, liver 9.9%, others 12.7%). Concomitant inflammation was seen in 15 (21.1%) patients (pancreatitis 11.3%, diverticulitis 4.2%, others 5.6%), whereas coagulation/hematologic disorders were found in 7 (9.9%) patients, liver cirrhosis in 6 (8.5%), mixed/miscellaneous causes in 5 (7%) and still unknown aetiologies in 5 patients (7%). MVT resulted from recent operations in 2 (2.8%) patients. MDCT features of venous bowel ischemia were present in 15 patients (21.1%). 46.5% of MVT were (sub)acute, while 53.5% chronic. The luminal extension was complete in 52.1%, subtotal (50% of lumen) in 22.5% and partial (50% of lumen) in 25.4% of patients, consisting either of blood clots (76.1%) or tumoral tissue (23.9%), the latter mainly due to pancreas adenocarcinoma (76.4%). Conclusion: MDCT features of MVT are seen with a wide range of underlying diseases. Signs of intestinal ischemia are infrequently associated, mostly occurring with coagulation/hematologic disorders (40%).

BACKGROUND: Direct colonic electrical stimulation may prove to be a treatment option for specific motility disorders such as chronic constipation. The aim of this study was to provoke colonic contractions using electrical stimulation delivered from a battery-operated device. METHODS: Electrodes were inserted into the caecal seromuscular layer of eight anaesthetized pigs. Contractions were induced by a neurostimulator (Medtronic 3625). Caecal motility was measured simultaneously by video image analysis, manometry and a technique assessing colonic transit. RESULTS: Caecal contractions were generated using 8-10 V amplitude, 1000 micros pulse width, 120 Hz frequency for 10-30 s, with an intensity of 7-15 mA. The maximal contraction strength was observed after 20-25 s. Electrical stimulation was followed by a relaxation phase of 1.5-2 min during which contractions propagated orally and aborally over at least 10 cm. Spontaneous and stimulated caecal motility values were significantly different for both intraluminal pressure (mean(s.d.) 332(124) and 463(187) mmHg respectively; P < 0.001, 42 experiments) and movement of contents (1.6(0.9) and 3.9(2.8) mm; P < 0.001, 40 experiments). CONCLUSION: Electrical stimulation modulated caecal motility, and provoked localized and propagated colonic contractions.

AIM: The study aimed to analyse the currently available national and international guidelines for areas of consensus and contrasting recommendations in the treatment of diverticulitis and thereby to design questions for future research. METHOD: MEDLINE, EMBASE and PubMed were systematically searched for guidelines on diverticular disease and diverticulitis. Inclusion was confined to papers in English and those < 10 years old. The included topics were classified as consensus or controversy between guidelines, and the highest level of evidence was scored as sufficient (Oxford Centre of Evidence-Based Medicine Level of Evidence of 3a or higher) or insufficient. RESULTS: Six guidelines were included and all topics with recommendations were compared. Overall, in 13 topics consensus was reached and 10 topics were regarded as controversial. In five topics, consensus was reached without sufficient evidence and in three topics there was no evidence and no consensus. Clinical staging, the need for intraluminal imaging, dietary restriction, duration of antibiotic treatment, the protocol for abscess treatment, the need for elective surgery in subgroups of patients, the need for surgery after abscess treatment and the level of the proximal resection margin all lack consensus or evidence. CONCLUSION: Evidence on the diagnosis and treatment of diverticular disease and diverticulitis ranged from nonexistent to strong, regardless of consensus. The most relevant research questions were identified and proposed as topics for future research.

Os autores relatam o caso de uma paciente de 20 anos de idade, sem antecedentes patológicos, que apresentou quadro de abdome agudo obstrutivo. Realizou exames radiológicos, ultra-som e tomografia computadorizada, seguidos de laparotomia exploradora, sendo constatadas: a) rotação hepática completa para o hipocôndrio esquerdo; b) hipotensão severa por compressão da veia cava inferior; c) malformação congênita dos ligamentos falciforme e coronários; d) obstrução do cólon transverso por bridas constritivas congênitas; e) importante distensão gasosa de alças cólicas no hipocôndrio direito. Foram ressecadas as bandas pericólicas, com redução imediata da dilatação no hemiabdome direito, havendo retorno espontâneo do fígado à posição anatômica. Esta condição ímpar, e os outros três casos similares já descritos na literatura mundial, são comentados neste trabalho.

O pólipo fibrovascular do esôfago é um tumor benigno raro, pedunculado e intraluminal. Relata-se um caso desta entidade, discutindo seus achados ao esofagograma e tomografia computadorizada. Em virtude da excelente correlação entre as características patológicas e de imagem, os estudos de tomografia computadorizada e ressonância magnética são considerados específicos e úteis em sua elucidação diagnóstica.

OBJETIVO: Descrever os principais aspectos radiológicos encontrados nas fístulas pós-operatórias de anastomose superior em pacientes submetidos a derivação gastrintestinal em Y de Roux pela técnica de Higa. MATERIAIS E MÉTODOS: Foram estudados 24 pacientes com fístula de anastomose no pós-operatório de gastroplastia redutora, avaliados por tomografias computadorizadas e/ou seriografias esofagogastrojejunais. RESULTADOS: As fístulas de anastomose superior ocorreram até o 30º dia de pós-operatório. Dezenove pacientes realizaram exame radiológico no momento do diagnóstico, sendo observado extravasamento de contraste, considerado sinal direto de fístula de anastomose, em dez pacientes. Dos nove restantes, em sete foi evidenciado extravasamento em exames subseqüentes, sendo ainda identificados sinais indiretos de fístula em seis destes. Sinais indiretos foram observados também em pacientes com extravasamento de contraste nos exames iniciais, sendo o pneumoperitônio o aspecto mais freqüente. Dos cinco pacientes sem exame radiológico no momento do diagnóstico, exames subseqüentes evidenciaram extravasamento de contraste em um e sinais indiretos em quatro pacientes. CONCLUSÃO: O achado radiológico mais comum foi o extravasamento de contraste (sinal direto de fístula). Os sinais indiretos foram: nível líquido bizarro, coleção intracavitária, pneumoperitônio desproporcional ao tempo pós-operatório, líquido na cavidade peritoneal, edema da anastomose inferior e distensão de delgado.

OBJETIVO: Avaliar o grau de aceitação do paciente submetido a colonografia por tomografia computadorizada (CTC) em comparação com a colonoscopia, quando realizadas para rastreamento de doença colorretal. MATERIAIS E MÉTODOS: Cinquenta pacientes com suspeita de doença colorretal foram submetidos a CTC e colonoscopia. Questionários foram aplicados antes e após a realização da CTC e após a colonoscopia. Graduou-se o desconforto esperado e experimentado antes e após a realização da CTC e da colonoscopia, bem como a preferência do paciente por exame. RESULTADOS: Em relação à CTC, antes de iniciar o exame 18% dos pacientes afirmaram esperar pouco desconforto, 78%, desconforto moderado e 4%, muito desconforto. Após a realização do exame, 72% dos pacientes relataram pouco desconforto, 26%, desconforto moderado e apenas um (2%) dos pacientes referiu muito desconforto. Após a realização da colonoscopia, 86% dos pacientes relataram preferência pela CTC. O grau de distensão colônica e a quantidade de fluido residual não influenciaram na preferência dos pacientes. CONCLUSÃO: Os pacientes preferiram a CTC à colonoscopia, não havendo relação estatística com o grau de distensão colônica na CTC e a eficiência do preparo intestinal.

OBJETIVO: O objetivo deste estudo foi avaliar o desempenho de contrastes orais neutros, comparando a capacidade de distensão intestinal, a distinção da parede intestinal, a aceitação e os efeitos colaterais. MATERIAIS E MÉTODOS: Estudo prospectivo, randomizado e duplo-cego em 30 pacientes submetidos a tomografia computadorizada de abdome e pelve com administração de contraste oral neutro, divididos em três grupos: leite, água e polietilenoglicol. Os exames foram analisados quanto ao grau de distensão intestinal e distinção da parede intestinal por dois examinadores em consenso. Os pacientes responderam a um questionário referente ao sabor da solução ingerida e efeitos colaterais. Foram utilizados os testes Kruskal-Wallis e qui-quadrado para as análises estatísticas. RESULTADOS: Distensão intestinal adequada (calibre da alça maior que 2 cm) foi observada em 14 segmentos dos 40 estudados (35%) no grupo leite, em 10 segmentos (25%) no grupo água e em 23 segmentos (57%) no grupo polietilenoglicol (p = 0,01). O preparo com polietilenoglicol resultou na melhor distensão intestinal, porém apresentou o pior sabor e maior incidência de diarreia, referidos pelos pacientes. CONCLUSÃO: O preparo oral com polietilenoglicol promove maior grau de distensão intestinal do que quando se utiliza água ou leite, mas tem pior aceitação, relacionada ao seu sabor e frequência de diarreia.

A 49yrs old, male patient presented progressive dysphagia during the last two years, initially to solids and, since two weeks, liquids he had lost 15 kilos. Esophagogram showed an intraluminal lesion with regular mucosa in the middle third of esophagus. The esophagoscopy exhibits regular mucosa up to the lesion, which did not alow passage. There was no evidence of linfonodal disease at CT scan. The hypothesis of esophageal leiomyoma led this patient a right posterior thoracotomy and enucleation of a 30 x 30 mm mass from the esophageal wall. With this case report we intend to review esophageal leiomyomas and their diagnosis and treatment.

OBJETIVO: O objetivo deste estudo foi avaliar a ausência da descompressão gástrica como método seguro em pacientes submetidos a papilotomia transduodenal, anastomose colédoco-duodenal ou a hepático-jejunostomia em Y de Roux. MÉTODO: Trinta e quatro pacientes foram submetidos à anastomose bíliodigestiva transduodenal, anastomose colédoco-duodenal ou a hepático- jejunostomia em Y de Roux sem descompressão gástrica com sonda. RESULTADOS: Vinte e quatro (70%) pacientes não apresentaram nenhum episódio de vômito. Não houve casos de distensão abdominal ou vômitos incoercíveis que obrigasse o uso da sonda no pós-operatório. Nenhum paciente apresentou deiscência da duodenotomia, das anastomoses ou da parede abdominal. Não houve casos de complicações pulmonares. CONCLUSÕES: O resultado do presente estudo sugere que a papilotomia transduodenal, a coledocoduodenostomia e a hepático-jejunostomia em y de Roux sem SNG não têm incidência aumentada de complicações. Além disso, os pacientes não são expostos ao risco da utilização da sonda.
