Diabetes Mellitus (DM) and osteoposes are chronic diseases with great socioeconomic consequences, mainly due to the late complications and consequent disabilities. The potential effects of DM on bone metabolism remain a very conroversial issue, and disagreement exists with regard to the clinical implications of diabetic osteopenia and the mechanism of its ocurrence. The issue is further complicated by the contribuicion of the especific factors, such as duration of disease an dthe degree of metabolic control. The objective of this study is to identify the osteopathy in children and adolescents with DM 1 assisted in the hospital of pediatrics, UFRN, through biochemical markers of bone and mineral metabolism and the extent of bone mineral density. The study was composed by 74 diabetics type 1 patients (DM1) of both gender and aged 6 to 20 yars. Normoglicêmic group was composed by 97 healthy subjects of both genders, which showed the same age range of DM1, in addition to same socioeconomic class. These individuals qere students from the networks of public education in the city of Natal-RN, randomly invited to paticipate in our study. Both groups DM1 and NG were divided intofour subgroups, according to the classification of tanner , T1, T2, T3, T4 for achieving a benchmark. Diabetic individuals showed up with a poor glycemic control. the group DN1 T4 showed an incresead value for total protein, albumin, urea and microalbumiuria are predictors of grumelura injury in DM1 patients . The total alkaline phosphatase activitywas kept on high levels for both groups because they are in a stature development age. For osteocalcin there were decreased levels for groups Dm1 T1, T2, and T3 when compared to their NG (s), suggesting that this decrease could be associated with reduction in the number and/or differentiation os osteoblasts thereby contributing to reducing bone formation. There were no changes in the activity of TRAP. The serum concentrations of total and ionized calcium, phosphorus and magnesium were included within the RV. It was observed that the BMD (Z- SCORE ) has always been within the RV for both groups, despite to DM1 T4. Taking all together, our results support the hypothesis that children and adolescents with type 1 DM present the risk in the long run to suffer a reduction in the bone mass, associated to poor glicemic control and disease duration. It could limit the bone growth and increase the probality of development of osteopenia, as well as other complications surch as retinopathy and renal failure

Social behavior of Guiana dolphins, Sotalia guianensis, at Pipa Beach, RN, Brazil: dynamics, sequence, breathing synchrony, and responses to dolphin watching. Social animals form groups that can range from temporary to permanent. Depending on the nature of the social relationships developed between individuals, groups present a particular social organization and the effect of these interactions shapes the activity patterns of these animals. This study investigates: (i) fission-fusion dynamics of Guiana dolphins, through the analysis of three dimensions of the social system (variation in spatial cohesion, variation in size and composition of groups), (ii) sequence, routine and behavioral stability, (iii) breathing intervals in synchronized groups and (iv) behavioral responses of the animals to dolphin watching. Systematic observations of Guiana dolphins were made from a platform located in cliffs about 25 m above sea level that surround Madeiro Bay, Pipa Beach. Sampling occurred from December 2007 to February 2009 between 0600 h and 1600 h, and the groups of Guiana dolphins were investigated according to their size (alone and group) and composition (adults, adults and juveniles, and adults and calves). According to the analysis of fission-fusion dynamics, Guiana dolphin groups frequently changed their composition, modifying their patterns of spatial grouping and cohesion every 20 minutes on average. More than 50% of the individuals maintained a distance of up to 2 m from other group members and new individuals were attracted to the group, especially during feeding, leaving it for foraging. Large groups were more unstable than small, while groups containing only adults were more stable than groups of adults and juveniles. According to the Z-score analysis to investigate the sequence and behavioral routine, lone individuals were more ! .7! ! involved in foraging and feeding, while resting was more common in groups. Foraging and feeding were more common in homogeneous groups (individuals of the same age class), while heterogeneous groups (different age classes) were often involved in socialization, displaying a broader behavioral repertoire. Foraging and resting behavior presented higher stability (continuous duration in minutes) than the other behaviors. The analysis of breathing intervals in synchronized groups showed significant differences depending on type of behavior, composition and area preference. During resting, breathing intervals were of longer duration, and groups with calves showed shorter breathing intervals than groups without calves. Lone individuals also preferred areas called corral , often used for the entrapment of fishes. The Markov chain analysis revealed behavioral changes in the presence of boats, according to the type of group composition. Groups composed of adults presented decreased resting and increased in traveling during the presence of boats. Groups of adults and juveniles showed a massive reduction of socialization, while the behavior transition probability traveling-traveling was higher in groups of adults and calves. In the presence of the boats, stability of resting was reduced by one third of its original duration and traveling more than doubled. The behavioral patterns analyzed are discussed in light of socio-ecological models concerning costs and benefits of proximity between individuals and behavioral optimization. Furthermore, significant changes in behavioral patterns indicate that Guiana dolphins, at Pipa Beach, have suffered the effects of tourism as a result of violation of rules of conduct established for the study area

OBJETIVO: Analisar o padrão de crescimento de prematuros de extremo baixo peso (EBP) até 24 meses de idade corrigida, a influência da displasia broncopulmonar (DBP) e os fatores de risco para falha de crescimento. MÉTODOS: Coorte de prematuros <1.000g de gestação única, nascidos e acompanhados em um centro terciário. O crescimento foi avaliado por meio de escores-z para peso, comprimento e perímetro cefálico ao nascimento, com 40 semanas, aos 3, 6, 12, 18 e 24 meses de idade corrigida. Dentre 81 sobreviventes, 70 foram estudados e estratificados em dois grupos: DBP (n=41) e sem DBP (n=29). Foi realizada análise bivariada com teste t ou Mann-Whitney, qui-quadrado ou Exato de Fisher, e análise multivariada com regressão logística. RESULTADOS: em ambos os grupos, o escore-z de peso diminuiu significantemente entre o nascimento e 40 semanas. Houve um pico de incremento nos escores-z de peso, comprimento e perímetro cefálico entre 40 semanas e três meses. No grupo sem DBP, os escores-z atingiram a faixa normal a partir dos seis meses e assim permaneceram até 24 meses de idade corrigida. Crianças com DBP tiveram menores escores-z de peso e perímetro cefálico no primeiro ano, mas equipararam-se às sem DBP no segundo ano de vida. A regressão logística mostrou que catch-down no escore-z de peso com 40 semanas foi fator de risco para falha de crescimento. CONCLUSÕES: Prematuros EBP apresentam catch-up precoce do crescimento nos primeiros dois anos. Crianças com DBP têm pior crescimento ponderal. A restrição do crescimento pós-natal prediz a falha de crescimento nos primeiros anos.

OBJETIVO: A sensibilidade de indicadores antropométricos e laboratoriais, na detecção da desnutrição protéico-energética de cirróticos foi estudada em 117 adultos, com gravidades (Child-Pugh), Child A (18), B (42) e C (57). RESULTADOS: Os indicadores antropométricos mais alterados foram: a circunferência braquial 61%, a prega cutânea tricipital 55% e a prega cutânea subescapular 53%, e os laboratoriais: albumina 93% e hemoglobina 90%. Com a combinação de indicadores circunferência braquial + prega cutânea subescapular + albumina ou hemoglobina, alcançaram-se 29% de déficit, valor idêntico ao da associação apenas de circunferência braquial + prega cutânea subescapular. Essa combinação (circunferência braquial e prega cutânea subescapular) detectou 63% de desnutrição protéico-energética, com predominância das formas moderada e grave em todos os graus Child. CONCLUSÃO: A desnutrição protéico-energética na cirrose hepática é predominantemente protéica, podendo ser caracterizada por indicadores laboratoriais (mais sensíveis) ou, com maior especificidade, pela circunferência braquial, embora inespecífica à discriminação da gravidade da desnutrição protéico-energética entre os grupos Child B e C.

Body builders have as their training goals the maximum muscle hypertrophy with minimum adiposity. However, the scarcity of specific standards implies often in framing wrongly those athletes either as overweight (by their BMI) or energy malnourished (by their fat stores). The objective of this study was to compare the body composition of body builders with population standards. Thirty-six adults, 26 male (27.2 ± 7.2 years) and 10 female (30 ± 6.1 years) nationwide competitive body builders, were assessed considering weight, height, body mass index, adiposity, arm and leg circumferences and skinfolds. The data were referred either as percentile or standard deviations (Z score) of population standards. Body weight and height were among the closest values from the populational mean whereas upper arm muscle circumference (for men) and body adiposity (for women) were the farterest. By using fat parameters as indicators of their protein-energy status, the undernourishment was found in 88.5% of men and 100.0% of women. Thus, it seems that body builders deserve their own anthropometric standards to avoid nutritional status misplacements.

The porpoise of this study was to evaluate the effects of extracorporeal shock waves in third metacarpus bone from healthy horses by determination of bone elasticity. It were used 20 Thoroughbred horses, male and female, with two years old, on top of training and selected as the state healthy. At the beginning of the experiment (D0), all animals were submitted for evaluation of bone elasticity held in the third metacarpus bone. The animals were divided into two groups (Control Group - CG and Experimental Group - EG). The application of extracorporeal shock wave therapy (ESWT) was performed on the right forelimb of the animals in the experimental group in the same place evaluated for bone elasticity and was used apparatus for extracorporeal therapy of waves with 0.15 mJ/mm2 energy flux density and 2000 pulses with E6R20 probe, with focus feature of the shock wave of 20 mm. The ESWT were repeated every 21 days, a total of three sessions (D0, D21 and D42). The analysis of bone elasticity determination was realized at D21, D42 and D72. The average of speed ultrasound differed between groups at D21, D42 and D72, and the animals from EG had lower bone mineral density after applications of ESWT. There was also difference in the analysis of bone mass (Z-Score) between the groups at D21 and D42, which animals from EG showed a significant decrease in bone mass. The risk of fracture was higher in animals from experimental group at D21. It was concluded that ESWT is able to promote change in bone mineral density.

Objetivo: Identificar os determinantes da desnutrição infantil em crianças menores de dois anos de idade em populações ribeirinhas do Pará. Métodos: Estudo transversal foi desenvolvido com 203 crianças residentes em quatro comunidades ribeirinhas: Aveiro (região Sudoeste), Barcarena (região Metropolitana), Cametá (região Nordeste) e Santarém (região do Baixo Amazonas) por meio de entrevista junto ao responsável pela criança. A variável dependente foi desnutrição, considerada presente para índice estatura para idade < -1 escores z de acordo com a referência atual da Organização Mundial da Saúde. As variáveis independentes foram características: de moradia, do chefe da família, da mãe da criança, do pré-natal, do padrão alimentar da família, do nascimento da criança, dos cuidados maternos e demográficos da criança. A prevalência da desnutrição foi calculada conforme indicadores básicos, subjacentes e imediatas, considerando-se a distância entre as variáveis que compõem os indicadores e o desfecho. Análise multinível foi realizada por regressão logística tendo em conta a hierarquia das relações entre os indicadores e a desnutrição, considerando-se p<0,005. Resultados: A prevalência de desnutrição atingiu 35,0 % das crianças estudadas, variando de 28,6% em Aveiro a 43,1% em Barcarena. As variáveis que se associaram com desnutrição foram baixo peso ao nascer e maior idade da criança. A idade da criança foi o preditor da desnutrição: a chance de uma criança entre 12 e 17 meses de idade apresentar desnutrição foi 3,4 vezes maior do que a de uma criança com menos de seis meses, aumentando em cinco vezes para as crianças entre 18 e 23 meses. Conclusão: Em populações ribeirinhas, a desnutrição em menores de dois anos mostra-se ainda como um grave problema de saúde pública, possivelmente pelo maior tempo de exposição aos fatores de risco ambientais.

O termo Habilidades Sociais (HS) é usado para designar um conjunto de repertórios comportamentais que envolvem interações sociais satisfatórias. A maioria dos estudos publicados sobre essa temática inclui a participação de crianças e adolescentes, abordando situações em ambiente escolar. Poucos são os estudos voltados para a área de saúde, e, até o momento, não foram localizadas pesquisas sobre HS realizadas com indivíduos portadores de Anomalia da Diferenciação Sexual (ADS). Esta anomalia se caracteriza pela malformação na genitália e/ou pela disfunção das gônadas, ocasionando características sexuais secundárias não correspondentes ao sexo de criação. Estudos clínicos realizados com indivíduos portadores de ADS têm destacado a ocorrência de déficits em habilidades sociais nesses indivíduos, caracterizados pela fuga-esquiva de situações sociais aversivas. Neste trabalho, pretendeu-se caracterizar o repertório comportamental correspondente a HS em indivíduos com ADS atendidos no ambulatório de um programa especializado de um hospital da rede pública de Belém, por meio de dois estudos complementares. No primeiro, foi realizado um estudo com delineamento transversal, com o objetivo de caracterizar comportamentos correspondentes a HS em indivíduos com ADS. Participaram 9 adultos com mais de seis meses em tratamento. Foram utilizados: Roteiro de Entrevista, Protocolo para análise de prontuário e Inventário de Habilidades Sociais - IHS. Os resultados sugerem que os participantes apresentam déficit de habilidades sociais em todos os fatores do IHS em menor ou maior grau, de maneira que poderiam se beneficiar com um treinamento de habilidades sociais como parte do tratamento, por se tratar de uma técnica que visa à superação e/ou redução dos déficits por eles apresentados. No segundo, foi realizado um estudo com delineamento de sujeito único, com o objetivo de verificar os efeitos do uso de treino em automonitoramento na instalação de comportamentos correspondentes a HS. Participou uma adulta com diagnóstico de ADS selecionada dentre os que participaram do Estudo 1. O procedimento de intervenção ocorreu por meio de entrevistas semanais, de acordo com as seguintes etapas: (1) Contrato: assinatura do Termo de Consentimento Livre e Esclarecido e agendamento de entrevista; (2) Avaliação: levantamento da linha de base dos comportamentos correspondentes à HS, elaboração da hierarquia dos comportamentos que indicaram déficit segundo as normas de análise do IHS e treino em registro de automonitoração; (3) Intervenção: leitura de um texto sobre habilidades sociais, apresentação da lista de direitos humanos básicos, aplicação do Questionário Construcional de Goldiamond adaptado e treino em registro de automonitoramento; (4) Re-avaliação das HS: reaplicação do IHS; (5) Follow-up: avaliação da manutenção dos ganhos obtidos com o estudo; e (6) Encerramento: entrevista devolutiva para explanação dos resultados do estudo ao participante. Os resultados sugerem que a intervenção promoveu o desenvolvimento de HS na participante, haja vista a interpretação da evolução da participante apresentada de acordo com a variação do Escore Z e da representação gráfica de significância clínica e mudança confiável.

Introduction: childhood obesity is a major public health problem, affecting children even at early ages. Objective: to assess the impact of dietary intervention and relatively simple physical activity on the nutritional state of preschoolers. Method: this was an intervention study in public daycare centers targeting children from 2 to 4 years of age, in the State of São Paulo. The sample consisted of 9 daycare centers covering 578 children, with 264 children in the intervention group (IG) and 314 in the comparison group (CG). Intervention was focused on modifications in diet and physical activity, avoiding overloading the routine of daycare centers, for duration of one year. A zBMI score > 1 (zBMI) and < 2 was considered risk of being overweight, and > 2 zBMI was considered excess weight (overweight and obese). Analysis was done by frequency calculations, comparisons of proportions by χ2, mean comparisons by t-student and calculations according to Pearson’s correlation coefficient. Results: IG showed an inverse correlation between the initial zBMI of the children and the difference with the zBMI at the end of the intervention (rP = -0.39, p <0.0001). The mean difference of zBMI of the overweight children in IG between the beginning and the end of the study period was negative (-0.46 z score), indicating weight reduction, while the children in the CG was positive (+0. 17 z score) (p = 0.0037). Conclusion: intervention in diet and physical activity in overweight preschool children in daycare centers could have a favorable impact on the evolution of their nutritional state.

Currently , as a result of the significant increase in the number of elderly, one can observe an increase in the number of chronic diseases , among them Alzheimer's disease (AD) , which affects both patients and their caregivers , that due care with the patient , just overwhelmed , anxious and depressed. Therefore, this study aims to draw a profile of caregivers of Alzheimer's patients, correlating the physical activity of patients with levels of overload, anxiety, depression caregivers. For both sample consisted of 40 patients with AD to characterize the physical activity levels and perform activities of daily living. In addition, we evaluated 40 caregivers. Patient assessment was made through a medical history, beyond the score Clinical Dementia Rating (CDR), Mini - Mental State Examination (MMSE), Modified Baecke Questionnaire for Older Adults (MBQ) , Functional Activities Questionnaire PFEFFER (QAFP) , and the Self Perception Performance in Activities of Daily Living (EAPAVD). For assessing the caregiver was also used an interview, then the Neuropsychiatric Inventory (NPI), anxiety and depression scale (HAD) Scale and the Zarit caregiver burden (Zarit). The data were processed using descriptive procedures for the analysis of characterization of samples, such as cognitive screening and physical activity level and profile of caregivers. There was a normal distribution of the data using the Shapiro Wilk, For data with normal distribution were used parametric descriptive procedures using One Way ANOVA to compare groups and applying a post hoc Bonferroni. As for the data that showed the destruction not normal was used to standardize the test by Z -score , and then treated by means of parametric statistical procedures , as presented earlier . The Pearson correlation was used to identify possible associations between variables. It was assumed significance level of 5 % (p ≤ 0.05) for all analyzes. Given these results, we conclude that...

Objective The purpose of the present study was to analyze the effects of a 20-week concurrent training (20 WCT) intervention program on gender-specific body composition and metabolic variables in obese adolescents.Subjects and methods Sample was composed of twenty-five obese adolescents, aged between 12 and 15 (13.4 ± 0.96) years. Fat-free mass (FFM), percentage trunk fat mass (TFM%) and percentage fat mass (%FM) were evaluated through dual-energy X-ray absorptiometry (DXA). Measurement of intra-abdominal adiposity (IAAT) was performed using ultrasound. Blood pressure was measured and blood samples analyzed for total cholesterol (TC), high-density lipoprotein cholesterol (HDL-c), low-density lipoprotein cholesterol (LDL-c), triglycerides (TG) and plasma glucose. All participants performed the concurrent training (combination of weight training and aerobic training) three times per week, one hour per day, for 20 weeks. Descriptive analysis and analysis of variance (ANOVA) for repeated measures were used to compare baseline, 10 week and 20 week moments using the Bonferroni post-hoc test. Statistical significance was set at p < 0.05. Significant decrease in TC, LDL-c and TFM% were verified in both genders after the 10 initial weeks of concurrent training.Results A significant increase in height was found in both the male and female groups (p = 0.001 and p = 0.047, respectively), after 20 weeks of concurrent training. In addition, several modifications were observed in body composition and metabolic variables, with a significant decrease in BMI (p = 0.002 and p = 0.017), BMI z-score (p = 0.033 and p = 0.004), FM% (p = 0.002 and p = 0.002), TFM% (p = 0.009 and p = 0.018), TC (p = 0.042 and p = 0.001) and LDL-c (p = 0.006 and p = 0.001) in the male and female groups, respectively, after 20 weeks of intervention when compared with baseline.Conclusion Our results identified that concurrent training was an effective intervention for treating metabolic variable and body composition disorders, in both genders, by decreasing adiposity with consequent improvement in BMI and BMI z-scores, and enhancement in lipid profile variables.
