A Hipertensão Arterial Sistêmica é um grave problema de saúde pública no Brasil e no mundo. Sua prevalência no Brasil varia entre 22% e 44% para adultos (32% em média), chegando a mais de 50% para indivíduos com 60 a 69 anos e 75% em indivíduos com mais de 70 anos. Já a prevalência de Diabetes Mellitus nos países da América Central e do Sul foi estimada em 26,4 milhões de pessoas e projetada para 40 milhões, em 2030. Por conta disso, minha equipe da Unidade Básica de Saúde Salem Duarte optou por realizar a intervenção com o foco nesses temas, pois são doenças que tem uma alta incidência e prevalência e muita demanda de usuários na UBS. O objetivo da intervenção foi melhorar a atenção à pessoa com hipertensão e/ou diabetes Mellitus na UBS Salem Duarte, Mossoró/RN. Foi desenvolvido no período de quatro meses (março, abril, maio, junho) e participaram da intervenção todos os hipertensos e diabéticos de nossa área de trabalho da UBS. As ações realizadas na intervenção foram baseadas nos Cadernos de Atenção Básica n. 36 e 37 (2013), que apontam estratégias para o cuidado das pessoas com essas doenças crônicas. Foram definidos sete objetivos e estabelecidos metas e ações para a melhoria do acesso e qualidade em quatro eixos (organização e gestão do serviço, monitoramento e avaliação, engajamento público e qualificação da prática clínica). Trabalhamos com uma estimativa de 624 usuários hipertensos e 162 diabéticos (segum dados do caderno de ação programática). Nos quatro meses foram alcançados uma cobertura de 84,1% (525) e 100% (162) respectivamente. Do ponto de vista quantitativo, foi importante a cobertura alcançada para os usuários diabéticos, maior da planejada inicialmente (95%). Do ponto de vista qualitativo os melhores resultados aconteceram com a orientação nutricional sobre alimentação saudável, prática de atividade física regular, orientação sobre risco do tabaquismo, sobre higiene bucal, além de estratificação de risco cardiovascular, exame complementares em dia e adequado registro na ficha de acompanhamento. Todos esses indicadores foram alcançados em 100% tanto com os hipertensos, quanto com os diabéticos. As atividades com a comunidade foram satisfatórias, pois muitos usuários puderam aprimorar seus conhecimentos sobre a existência do Programa de Atenção à Hipertensão Arterial e à Diabetes Mellitus, compreendendo melhor sobre características, complicações e prevenção das suas doenças, além de conhecerem os direitos em relação à manutenção de seus registros de saúde. O vínculo entre profissionais – usuários – família foi maior. A intervenção exigiu uma maior preparação para a equipe e promoveu o trabalho em equipe. A UBS teve uma organização mais detalhada dos diferentes arquivos de atendimento dos usuários, assim como maior qualidade do acolhimento e otimização da agenda para a atenção.

The aim of this study was to investigate the association between bottle feeding and prevalence rates of increased overjet and Class 2 primary canine relationship. The sample consisted of 911 children (461 boys, 450 girls) aged 3 (13.9%), 4 (40.8%), 5 (34%) and 6 (11.3%) years, with complete primary dentition. Information about nutritive and nonnutritive (pacifier and/or digit) sucking habits was collected through questionnaires. Three calibrated dentists (κ: 0.9-1.0 and Rs > 0.90) performed the clinical assessments. The children were divided into four groups: G1 - not bottle-fed; G2 - exclusively bottle-fed; G3 - breast- and bottle-fed, bottle feeding ceased before 3 years of age; and G4 - breast- and bottle-fed, bottle feeding ceased between 3 and 4 years of age. Associations between nutritive and nonnutritive sucking behaviors and the malocclusions studied were analyzed by multiple binary logistic regression (α= 0.05). The frequencies of increased overjet were: 25.3% (G1), 38.8% (G2), 39.2% (G3) and 47.8% (G4). The percentages of Class 2 canine relationship were: 27.9% (G1), 48.8% (G2), 43.4% (G3) and 43% (G4). No significant effect of bottle feeding was found. The chances of diagnosing increased overjet (O.R. = 4.42, p < 0.001) and Class 2 canine relationship (O.R. = 4.02, p < 0.001) were greater for children with pacifier and/or digit-sucking habits, compared to those without a history of nonnutritive sucking behavior. It may be suggested that bottle feeding alone is not directly associated with higher prevalence rates of increased overjet and Class 2 canine relationship in the primary dentition.

Parte-se da hipótese de que tanto os modos de constituição da subjetividade, quanto as teses epistemológicas sobre a formação do eu têm sofrido importantes mudanças nas últimas décadas. No plano das identidades pessoais, tal fato se expressa no atenuamento do ideário intimista, tradicionalmente centrado na idéia da vida mental como espaço privado e interior. No plano epistemológico, diferentes saberes vêm questionando a equivalência entre vida mental e interioridade psicológica e propondo descrições da origem do eu que incluam a dimensão da corporeidade e da ação. Considerando as noções de representação e de ação como eixos fundamentais para diferentes concepções da subjetividade, este artigo examina um modelo internalista do psiquismo, contrastando-o com outro que toma o eu como agente corporificado.

Cyclic pseudo-galactooligosaccharides were synthesized by cyclooligomerisation of isomeric azido-alkyne derivatives of beta-D-galactopyranose under Cu(I)-catalysed azide-alkyne 1,3-dipolar cycloaddition reaction conditions. The principal products isolated were cyclic dimers and trimers, with lower amounts of cyclic tetramer and pentamer also evident in some cases. Molecular mechanics calculations suggest very compact but flexible structures for the cyclic trimers, with secondary OH groups exposed outside the macrocycle and available for enzymatic glycosylation. The cyclic dimers and trimers represent a new type of acceptor substrate for Trypanosoma cruzi trans-sialidase, giving rise to doubly and triply sialylated glycomacrocycles, respectively.

The aim of the present work is the presentation of a quantification methodology for the control of the amount of superparamagnetic iron oxide nanoparticles (SPIONs) administered in biological materials by means of the ferromagnetic resonance technique (FMR) applied to studies both in vivo and in vitro. The in vivo study consisted in the analysis of the elimination and biodistribution kinetics of SPIONs after intravenous administration in Wistar rats. The results were corroborated by X-ray fluorescence. For the in vitro study, a quantitative analysis of the concentration of SPIONs bound to the specific AC133 monoclonal antibodies was carried out in order to detect the expression of the antigenic epitopes (CD133) in stem cells from human umbilical cord blood. In both studies FMR has proven to be an efficient technique for the SPIONs quantification per volume unit (in vivo) or per labeled cell (in vitro).

Despite recent advances, patients with malignant brain tumors still have a poor prognosis. Glioblastoma (WHO grade 4 astrocytoma), the most malignant brain tumor, represents 50% of all astrocytomas, with a median survival rate of <1 year. It is, therefore, extremely important to search for new diagnostic and therapeutic approaches for patients with glioblastoma. This study describes the application of superparamagnetic nano-particles of iron oxide, as well as monoclonal antibodies, of immunophenotypic significance, conjoined to quantum dots for the ultrastructural assessment of glioblastoma cells. For this proposal, an immunophenotypic study by flow cytometry was carried out, followed by transmission electron microscopy analysis. The process of tumor cell labeling using nanoparticles can successfully contribute to the identification of tumorigenic cells and consequently for better understanding of glioblastoma genesis and recurrence. In addition, this method may help further studies in tumor imaging, diagnosis, and prognostic markers detection.

In this study, we evaluated the biodistribution and the elimination kinetics of a biocompatible magnetic fluid, Endorem (TM), based on dextrancoated Fe(3)O(4) nanoparticles endovenously injected into Winstar rats. The iron content in blood and liver samples was recorded using electron paramagnetic resonance (EPR) and X-ray fluorescence (XRF) techniques. The EPR line intensity at g=2.1 was found to be proportional to the concentration of magnetic nanoparticles and the best temperature for spectra acquisition was 298 K. Both EPR and XRF analysis indicated that the maximum concentration of iron in the liver occurred 95 min after the ferrofluid administration. The half-life of the magnetic nanoparticles (MNP) in the blood was (11.6 +/- 0.6) min measured by EPR and (12.6 +/- 0.6) min determined by XRF. These results indicate that both EPR and XRF are very useful and appropriate techniques for the study of kinetics of ferrofluid elimination and biodistribution after its administration into the organism. Adherent umbilical cord blood stromal cells (AUCBSCs) are multipotent cells with differentiation capacities. Therefore, these cells have been investigated for their potential in cell-based therapies. Quantum Dots (QDs) are an alternative to organic dyes and fluorescent proteins because of their long-term photostability. In this study we determined the effects of the cell passage on AUCBSCs morphology, phenotype, and differentiation potential. QDs labeled AUCBSCs in the fourth cell passage were differentiated in the three mesodermal lineages and were evaluated using cytochemical methods and transmission electron microscopy (TEM). Gene and protein expression of the AUCBSCs immunophenotypic markers were also evaluated in the labeled cells by real-time quantitative PCR and flow cytometry. In this study we were able to define the best cellular passage to work with AUCBSCs and we also demonstrated that the use of fluorescent QDs can be an efficient nano-biotechnological tool in differentiation studies because labeled cells do not have their characteristics compromised.

BACKGROUND The genetic analysis of human primary immunodeficiencies has defined the contribution of specific cell populations and molecular pathways in the host defense against infection. Disseminated infection caused by bacille Calmette-Guerin (BCG) vaccines is an early manifestation of primary immunodeficiencies, such as severe combined immunodeficiency. In many affected persons, the cause of disseminated BCG disease is unexplained. METHODS We evaluated an infant presenting with features of severe immunodeficiency, including early-onset disseminated BCG disease, who required hematopoietic stem-cell transplantation. We also studied two otherwise healthy subjects with a history of disseminated but curable BCG disease in childhood. We characterized the monocyte and dendritic-cell compartments in these three subjects and sequenced candidate genes in which mutations could plausibly confer susceptibility to BCG disease. RESULTS We detected two distinct disease-causing mutations affecting interferon regulatory factor 8 (IRF8). Both K108E and T80A mutations impair IRF8 transcriptional activity by disrupting the interaction between IRF8 and DNA. The K108E variant was associated with an autosomal recessive severe immunodeficiency with a complete lack of circulating monocytes and dendritic cells. The T80A variant was associated with an autosomal dominant, milder immunodeficiency and a selective depletion of CD11c+CD1c+ circulating dendritic cells. CONCLUSIONS These findings define a class of human primary immunodeficiencies that affect the differentiation of mononuclear phagocytes. They also show that human IRF8 is critical for the development of monocytes and dendritic cells and for antimycobacterial immunity. (Funded by the Medical Research Council and others.)

To evaluate the timing of mutations in BRAF (v-raf murine sarcoma viral oncogene homolog B1) during melanocytic neoplasia, we carried out mutation analysis on microdissected melanoma and nevi samples. We observed mutations resulting in the V599E amino-acid substitution in 41 of 60 (68%) melanoma metastases, 4 of 5 (80%) primary melanomas and, unexpectedly, in 63 of 77 (82%) nevi. These data suggest that mutational activation of the RAS/RAF/MAPK pathway in nevi is a critical step in the initiation of melanocytic neoplasia but alone is insufficient for melanoma tumorigenesis.
