Background Twin and family studies have shown that genetic effects explain a relatively high amount of the phenotypic variation in blood pressure. However, many studies have not been able to replicate findings of association between specific polymorphisms and diastolic and systolic blood pressure. Methods In a structural equation-modelling framework the authors investigated longitudinal changes in repeated measures of blood pressures in a sample of 298 like-sexed twin pairs from the population-based Swedish Twin Registry. Also examined was the association between blood pressure and polymorphisms in the angiotensin-I converting enzyme and the angiotensin 11 receptor type 1 with the 'Fulker' test Both linkage and association were tested simultaneously revealing whether the polymorphism is a Quantitative Trait Locus (QTL) or in linkage disequilibrium with the QTL. Results Genetic influences explained up to 46% of the phenotypic variance in diastolic and 63% of the phenotypic variance in systolic blood pressure. Genetic influences were stable over time and contributed up to 78% of the phenotypic correlation in both diastolic and systolic blood pressure. Non-shared environmental effects were characterised by time specific influences and little transmission from one time point to the next. There was no significant linkage and association between the polymorphisms and blood pressure. Conclusions There is a considerable genetic stability in both diastolic and systolic blood pressure for a 6-year period of time in adult life. Non-shared environmental influences have a small long-term effect Although associations with the polymorphisms could not be replicated, results should be interpreted with caution due to power considerations. Objective : To report the history of the Royal Alexandra Hospital for Children (RAHC) Papua New Guinea (PNG) cardiac surgical programme and describe the selection, preoperative clinical features and postoperative outcome of children with congenital heart disease managed by the programme. Methods : Details for each of the PNG cardiac patients admitted to RAHC following selection by visiting cardiologists between 1978 and 1994 were entered into a database, and analysed and interpreted. Results : A congenital heart defect was confirmed in 165 of the 170 children selected. The male to female ratio was 1:1 and the mean age on admission to RAHC was 5.5 years. Almost all of the children for whom data were available (98%) had a weight for age and 41% had a height for age less than the 3rd centile. One-sixth had delayed milestones. A large number were tachypnoeic, in heart failure, or had pulmonary hypertension on admission. Ventricular septal defect and tetralogy of Fallot were the commonest defects, and lesions such as aortic stenosis, coarctation of the aorta and transposition of the great arteries were absent or rare. Thirty-one (19%) of the children selected initially did not receive surgery because of pulmonary hypertension, or because the lesions did not fall within the programme guidelines for operation. One hundred and twenty-nine children had corrective and four had palliative procedures. Half of the operated children had postoperative complications. Eight children died, all following open-heart procedures, giving a case fatality rate of 6%. Preoperative tachypnoea, hepatomegaly, cardiac failure and pulmonary hypertension were strongly associated with poor outcome. Conclusions : The programme was an arduous exercise for all organizations concerned, but achieved comparatively good short-term outcomes. The experience gained should assist in planning for similar programmes.

Os cem anos do fim da gestão do Barão do Rio Branco no Ministério das Relações Exteriores do Brasil coincidem com um momento politicamente propício para se revisitar o legado do patrono da diplomacia brasileira, em especial o referente aos Estados Unidos e ao Prata, os dois principais eixos das relações externas do País. Examinam-se o teor das relações com a potência hegemônica do hemisfério e o caráter oscilante daquelas com a Argentina. A partir de fontes primárias e bibliográficas, o autor procura demonstrar que Rio Branco, além do fechamento dos limites do território nacional, firmou tendências e procedimentos que se incorporaram à tradição da diplomacia brasileira, analisando, para isso, a função do alinhamento Rio de Janeiro-Washington no conjunto da política externa do chanceler e o padrão das relações com o governo argentino, fatores que influenciaram seus movimentos no entorno geográfico. A política de prestígio desenvolvida por Rio Branco decorreu da aspiração em diferenciar seu país do conjunto de nações do segmento sul do hemisfério, identificadas com convulsões políticas e insolvência financeira.

Foi realizado estudo epidemiológico da encefalite por arbovirus na região do Vale do Ribeira, S. Paulo, Brasil. Uma epidemia da moléstia ocorreu em 1975, 1976 e 1977, com picos nas épocas de maior temperatura e pluviosidade. A partir de 1978 a moléstia manteve-se em níveis baixos numa presumível ende-micidade. A epidemia se deslocou em onda em direção leste-oeste e leste-sudoeste para a região litorânea vizinha. A cadeia montanhosa situada ao norte e noroeste atuou como barreira à propagação da moléstia. Considerou-se a hipótese que o agente etiológico, arbovirus Rocio, deva ter começado a infectar a população humana recentemente, tendo sido veiculado ao homem de reservatórios silvestres, aves e pequenos mamíferos, por culicídeos silvestres. Discutiu-se também prováveis formas de transmissão domiciliar. Verificou-se que os grupos populacionais que apresentaram as formas mais graves foram os de idades extremas e os que apresentavam piores condições de vida. Considerou-se que a perspectiva epidemiológica desta arbovirose é que ela persista na região, uma vez que a mesma apresenta condições ótimas para o desenvolvimento do agente etiológico, dos reservatórios e dos vetores biológicos, além de receber um contínuo afluxo de população suscetível, migrantes ou turistas.
