À la suite d’un crime, les victimes vont ressentir différents besoins et celui d’être informé serait fondamental (Baril, 1984). Les recherches ont permis d’établir que les policiers ont un rôle important à jouer dans la transmission des informations aux victimes puisqu’ils sont les premiers acteurs du système pénal avec lesquels celles-ci sont en contact (Laxminarayan, 2013). De plus, la perception des victimes quant à la façon dont elles ont été traitées par les policiers peut avoir un impact significatif sur leur rétablissement psychologique. Cette étude a pour but de mesurer l’effet thérapeutique des interactions entre les victimes et les policiers ainsi que de la transmission des informations aux victimes à la suite d’un crime sur l’état de stress post-traumatique (ÉSPT). Pour ce faire, l’instrument qui a été administré aux participants (n=188) est l’Échelle modifiée des symptômes du trouble de stress post-traumatique. Cet instrument est une version adaptée et validée en français (Guay, Marchand, Iucci et Martin, 2002) de l’échelle MPSS-SR élaborée par Falsetti, Resnick, Resick et Kilpatrick (1993). Cette échelle permet d’évaluer la présence des symptômes de stress post-traumatique en terme de fréquence et de sévérité. Les résultats de l’étude montrent que l’information est un déterminant important d’un traitement juste et la qualité de ce traitement à un effet thérapeutique sur les symptômes de stress post-traumatique. Une discussion des résultats dans une perspective plus globale concernant les théories en victimologie sur la perception de justice, la problématique de la victimisation secondaire et les implications pour les politiques en matière de droits des victimes sera présentée à la fin de cette recherche.

Threatening intrusive images are central to posttraumatic stress disorder. It has been suggested that intrusive imagery in the context of a sense of threat leads to the development and persistence of posttraumatic stress symptoms. This study investigates London school children's (N = 76; age 10-11 years) self-reported posttraumatic stress symptoms in response to viewing the attacks of September 11, 2001 on television. Assessments were made at two time points. A minority of participants reported moderate-severe symptoms with functional impairment at 2 months (14.5%) and 6 months (9.2%) after viewing the September 11events. After controlling for symptom stability, persistent symptoms were associated with peri-traumatic factors, notably perceiving that one's life was in danger. The combined effect of intrusive imagery and peri-traumatic life threat was associated with symptom persistence. Assessments of intrusive image content via checklist and free-report indicated that the images were directly related to September 11 and were fairly stable over time. Objectives This study aimed to investigate post-traumatic stress symptoms (PTSS) in childhood brain tumour survivors and their parents. A further aim was to explore the relationship between objective illness parameters, parent–child interactions, coping styles and PTSS. Methods A cross-sectional correlational design was employed. Fifty-two childhood brain tumour survivors, aged 8–16, and 52 parents completed a battery of questionnaires designed to assess quality of parent–child interactions, monitoring and blunting attentional coping styles and PTSS. Results Over one-third (35%) of survivors and 29% of their parents reported severe levels of PTSS (suggestive of post-traumatic stress disorder ‘caseness’). Increased parent–child conflict resolution for survivors and number of tumour recurrences for parents independently predicted the variance in PTSS. Conclusions For a substantial proportion of brain tumour survivors and their parents the process of survivorship is a considerably distressing experience.

The purpose of the study is to investigate social workers experiences of compassion fatigue as well as their thoughts about health and risk factors in the area. The method used is a qualitative interview study in which eight social workers, investigating child welfare matters, were individually interviewed. The study shows that most of the social workers describe their own experiences of burnout but not of secondary traumatic stress. The most important support for not suffer from compassion fatigue is to be supported and the possibility to ventilate with colleagues and managers. However, the social workers consider that the risk of burnout is primarily due to a heavy workload in terms of the number of cases and high staff turnover. Finally, the study indicates that social workers do not have experience of secondary traumatic stress because they are using tools that prevent this. However, the risk of experiencing burnout is high since they have not found strategies for managing workplace stress.

BACKGROUND Posttraumatic Stress Disorder (PTSD) may occur in patients after exposure to a life-threatening illness. About one out of six patients develop clinically relevant levels of PTSD symptoms after acute myocardial infarction (MI). Symptoms of PTSD are associated with impaired quality of life and increase the risk of recurrent cardiovascular events. The main hypothesis of the MI-SPRINT study is that trauma-focused psychological counseling is more effective than non-trauma focused counseling in preventing posttraumatic stress after acute MI. METHODS/DESIGN The study is a single-center, randomized controlled psychological trial with two active intervention arms. The sample consists of 426 patients aged 18 years or older who are at 'high risk' to develop clinically relevant posttraumatic stress symptoms. 'High risk' patients are identified with three single-item questions with a numeric rating scale (0 to 10) asking about 'pain during MI', 'fear of dying until admission' and/or 'worrying and feeling helpless when being told about having MI'. Exclusion criteria are emergency heart surgery, severe comorbidities, current severe depression, disorientation, cognitive impairment and suicidal ideation. Patients will be randomly allocated to a single 45-minute counseling session targeting either specific MI-triggered traumatic reactions (that is, the verum intervention) or the general role of psychosocial stress in coronary heart disease (that is, the control intervention). The session will take place in the coronary care unit within 48 hours, by the bedside, after patients have reached stable circulatory conditions. Each patient will additionally receive an illustrated information booklet as study material. Sociodemographic factors, psychosocial and medical data, and cardiometabolic risk factors will be assessed during hospitalization. The primary outcome is the interviewer-rated posttraumatic stress level at three-month follow-up, which is hypothesized to be at least 20% lower in the verum group than in the control group using the t-test. Secondary outcomes are posttraumatic stress levels at 12-month follow-up, and psychosocial functioning and cardiometabolic risk factors at both follow-up assessments. DISCUSSION If the verum intervention proves to be effective, the study will be the first to show that a brief trauma-focused psychological intervention delivered within a somatic health care setting can reduce the incidence of posttraumatic stress in acute MI patients. Personnel involved in natural or man-made disaster response and recovery efforts may be exposed to a wide variety of physical and mental stressors that can exhibit long-lasting and detrimental psychopathological outcomes. In a disaster situation, huge numbers of "secondary" responders can be involved in contaminant clean-up and debris removal and can be at risk of developing stress-related mental health outcomes. The Occupational Safety and Health Administration (OSHA) worker training hierarchy typically required for response workers, known as "Hazardous Waste Operations and Emergency Response" (HAZWOPER), does not address the mental health and safety concerns of workers. This study focused on the prevalence of traumatic stress experienced by secondary responders that had received or expressed interest in receiving HAZWOPER training through the National Institute of Environmental Health Sciences Worker Education and Training Program (NIEHS WETP). ^ The study involved the modification of two preexisting and validated survey tools to assess secondary responder awareness of physical, mental, and traumatic stressors on mental health and sought to determine if a need existed to include traumatic stress-related mental health education in the current HAZWOPER training regimen. The study evaluated post-traumatic stress disorder (PTSD), resiliency, mental distress, and negative effects within a secondary responder population of 176 respondents. Elevated PTSD levels were seen in the study population as compared to a general responder population (32.9% positive vs. 8%-22.5% positive). Results indicated that HAZWOPER-trained disaster responders were likely to test positive for PTSD, whereas, untrained responders with no disaster experience and responders who possessed either training or disaster experience only were likely to test PTSD negative. A majority (68.75%) of the population tested below the mean resiliency to cope score (80.4) of the average worker population. Results indicated that those who were trained only or who possessed both training and disaster work experience were more likely to have lower resiliency scores than those with no training or experience. There were direct correlations between being PTSD positive and having worked at a disaster site and experiencing mental distress and negative effects. However, HAZWOPER training status does not significantly correlate with mental distress or negative effect. ^ The survey indicated clear support (91% of respondents) for mental health education. The development of a pre- and post-deployment training module is recommended. Such training could provide responders with the necessary knowledge and skills to recognize the symptomology of PTSD, mental stressors, and physical and traumatic stressors, thus empowering them to employ protective strategies or seek professional help if needed. It is further recommended that pre-deployment mental health education be included in the current HAZWOPER 24- and 40-hour course curriculums, as well as, consideration be given towards integrating a stand-alone post-deployment mental health education training course into the current HAZWOPER hierarchy.^

Bakgrund: Sjuksköterskan har stor del i den nära patientvården och förväntas vara en god lyssnare genom sitt personcentrerade vårdgivande. Händelser som är traumatiska drabbar förutom den direkt berörda patienten även sjuksköterskan genom sekundär exponering. Syfte: Syftet med studien var att belysa förekomst av STS hos sjuksköterskor samt beskriva hur det påverkar vårdkvalitet och personcentrerad vård. Metod: Litteraturstudie med en översikt av relevanta artiklar i databaserna PubMed och CINAHL. Resultatet baseras på 14 artiklar som uppfyllde inklusionsoch kvalitetskrav. Resultat: Sjuksköterskan påverkas av STS på flera plan, både professionellt och personligt. Vårdkvaliteten blir lidande om STS påvisas. Den personcentrerade vården utgör grunden i sjuksköterskans yrkesutövande men om medvetenhet saknas för STS finns risken att sjuksköterskan påverkas så till den grad att hen väljer att sjukskriva sig och/eller byta arbetsplats. Slutsats: STS är ett relativt okänt begrepp och det kan vara svårt att se tecken på begynnande symtom. Sjuksköterskan kan utsättas för STS oberoende arbetsplats. Detta kan sänka effekten i yrkesutövandet genom personliga symtom som; irritabilitet, trötthet, minskad empati, depression, återupplevelser av tidigare upplevda traumatiska händelser med flera. Arbetsplatsens ledarskap inverkar på identifieringen av sjuksköterskans utveckling/hanterande av STS.

OBJETIVO: Esta revisão da literatura tem como objetivo verificar a prevalência e os fatores de risco do parto traumático e do transtorno de estresse pós-traumático (TEPT) relacionado com o parto. MÉTODOS: Foi realizada uma pesquisa nos bancos de dados PubMed e BIREME, com as expressões "traumatic birth", "traumatic delivery", "postpartum posttraumatic stress disorder", "childbirth", "stress disorder" e avaliados estudos de 1994 a 2009. RESULTADOS: Três estudos qualitativos e quatro quantitativos sobre o parto traumático mostram que sua prevalência varia de 21,4% a 34% e que a mulher apresenta, durante o trabalho de parto ou o parto, medo intenso de sua morte ou do bebê, além de impotência, desamparo e horror. O parto traumático está relacionado a partos dolorosos, com procedimentos obstétricos de urgência e com assistência inadequada da equipe de saúde. Quanto ao TEPT relacionado com o parto, foram encontrados um estudo qualitativo e doze quantitativos e sua prevalência variou de 1,3% a 5,9%. Mulheres que apresentaram sintomas dissociativos ou emoções negativas no parto, que tiveram eventos traumáticos prévios, depressão na gestação e que tiveram pouco suporte social e pouco apoio da equipe de saúde são as mais vulneráveis para TEPT pós-parto. CONCLUSÃO: O parto traumático, apesar de pouco conhecido, não é um evento raro e traz consequências negativas para a vida da mulher, podendo inclusive ser sucedido de TEPT. A equipe de saúde que assiste mulheres no periparto deve estar preparada para prevenir e identificar esses casos.

OBJECTIVE: Previous studies have shown that premature birth and the immaturity of the child can affect the quality of the parent-child relationship. The present study examines the relationship between maternal and infant interactional behavior over time and infant perinatal risk factors as well as maternal perinatal recollected traumatic experience. Few studies have explored the relationship between maternal stress and the quality of parent-infant interaction. DESIGN: Mother-child interaction was recorded at 6 and 18 months of infant's age, in a population of 47 preterm infants (GA<34 weeks) and 25 full-term infants, born in 1998, during a play interaction. According to the Care Index, sensitivity, control and unresponsiveness have been used to code maternal interactional characteristics, and cooperation, compliance-compulsiveness, difficulty and passivity have been used to code the infant's interactional characteristics. The level of maternal stress was evaluated with the Perinatal Posttraumatic Stress Disorder Questionnaire (PPQ), and the infant's perinatal risk factors were assessed with the Perinatal Risk Inventory (PERI). RESULTS: Mothers of high-risk infants, as well as mothers that had experienced traumatic stress in the perinatal period, were less sensitive and more controlling at 6 months. The interactional behavior of the preterm infant was different from that of the full-term infant at 18 months of age, and was correlated with maternal traumatic stress but not with perinatal risk factors. CONCLUSION: These results underline the importance of maternal traumatic experience related to premature birth and its potential long lasting influence on mother-child interactional behavior.

This study examined the role of posttraumatic stress disorder (PTSD) symptoms of re-experience, avoidance, and hyperarousal in the relationship between different types of trauma and alcohol use disorders (AUD). We used data from 731 trauma-exposed individuals who participated in the first wave of the PsyCoLaus-study. Trauma characteristics were assessed relatively to the occurrence of lifetime PTSD symptoms and AUD. The results suggest that lifetime and childhood sexual abuse as well as overall childhood trauma were directly linked to AUD and PTSD symptoms, in particular to avoidance symptoms. From single symptom clusters PTSD avoidance was found to specifically mediate the trauma-AUD pathway. Both childhood and sexual trauma strongly contribute to the comorbidity of PTSD and AUD and avoidance-type symptoms appear to play a central role in maintaining this association. Hence, the alleviation of avoidance symptoms might be an important target for therapeutic intervention among victims of sexual abuse before specific addiction treatment is initiated.

La présente thèse porte sur l’évaluation de l’efficacité d’un nouveau traitement pour le trouble de stress post-traumatique (TSPT). Le traitement a été développé selon les prémisses de la théorie de la reconsolidation des souvenirs. Il consiste en six courtes séances de remémoration de l’événement traumatique réalisées sous l’effet du propranolol, un bêtabloquant. La population de l’étude est constituée de patients souffrant d’un TSPT chronique. La thèse comporte cinq chapitres. Le premier chapitre est l’introduction, on y retrouve une description du TSPT, des traitements validés empiriquement, de diverses théories de la mémoire, d’un modèle étiologique du TSPT, d’études sur la consolidation et la reconsolidation, de la pharmacocinétique et du mécanisme d’action du propranolol,ainsi que des objectifs de la thèse. Le second chapitre est une revue critique de littérature sur la théorie de la reconsolidation. Comme l’étude du phénomène de la reconsolidation est récente, nous tentons de faire le point sur l’état des connaissances dans le domaine, dans un effort de réflexion sur la validité de la théorie. Nous proposons une série de critères permettant de différencier la reconsolidation d’autres processus connexes. Nous concluons que la théorie paraît valide, bien que d’autres études soient nécessaires afin de rendre compte de résultats négatifs publiés par le passé. Le troisième chapitre est un essai ouvert, et vise à évaluer l’efficacité d’un traitement basé sur la reconsolidation à diminuer la sévérité et l’incidence du TSPT, auprès de 42 patients souffrant d’un TSPT chronique. Le traitement consiste en six séances de remémoration de l’événement traumatique sous propranolol. Lors d’un suivi à trois mois, nous rapportons une diminution des symptômes de TSPT de 41%-56%, ainsi qu’une diminution de l’incidence du TSPT de 74%. En comparaison, seulement 2/25 patients du groupe contrôle (ayant participé uniquement aux évaluations) ne souffrent plus d’un TSPT. Dans le groupe traitement, les tailles d’effet (d de Cohen)varient entre 1.32-2.19. Le quatrième chapitre a comme objectif d’identifier des caractéristiques des patients prédisant l’efficacité du traitement, et d’explorer s’ils s’améliorent dans des domaines de santé autres que le TSPT. Nous rapportons que les femmes s’améliorent davantage que les hommes, mais que d’autres facteurs, tels que la sévérité des traits de personnalité borderline ou le type de trauma (enfance versus adulte), n’influent pas sur l’efficacité. Également, les patients s’améliorent dans les domaines de santé suivants : la qualité de vie, la symptomatologie dépressive, l’intensité des émotions négatives au rappel de l’événement traumatique et dans la vie courante. Le cinquième chapitre contient la discussion générale de la thèse. Nous effectuons une synthèse et interprétation des résultats, nous examinons les hypothèses alternatives à l’amélioration clinique et abordons des pistes de recherches futures. Nous concluons que le traitement à l’étude a été efficace dans notre échantillon de patients souffrant d’un TSPT chronique. Étant donné la méthodologie employée (essai ouvert), nous ne pouvons statuer sur le mécanisme d’action du traitement, à savoir si l’amélioration clinique a été réellement causée par un blocage de la reconsolidation des souvenirs.

Cette recherche vise à explorer la satisfaction à l’égard des services reçus par les militaires diagnostiqués d’un trouble de stress post-traumatique (TSPT) et leur conjointe, ainsi qu’à comprendre leurs besoins et le sens donné au TSPT. Nous utilisons l’analyse qualitative exploratoire comme méthode d’analyse de données. Nous découvrons d’une part que les militaires et les conjointes rencontrées souhaitent recevoir des services adaptés à leur situation particulière (cas par cas) et d’autre part, que tous les répondants ont le besoin commun que l’on reconnaisse leurs sacrifices. Pour les militaires, une manière de reconnaitre ces sacrifices serait de respecter leurs souhaits et leurs demandes en ce qui concerne les soins. Une prise en charge complète de l’aspect administratif du rétablissement et de la libération serait également souhaitable lorsqu’indiquée. Nous trouvons également des différences entre les militaires ayant reçu un diagnostic de TSPT il y a plus de 5 ans et ceux qui l’ont reçu récemment. Nous proposons des hypothèses liées à ces différences. Finalement, les lacunes importantes en ce qui concerne les services offerts aux conjointes sont le manque de connaissance de ces dits services par les conjointes, la stigmatisation et la perception négative qu’ont les conjointes à l’égard du principal prestataire de service, les centres de ressources pour les familles des militaires. Cette étude nous permet de faire des recommandations en lien avec les résultats trouvés.

Posttraumatic stress disorder (PTSD) is an incapacitating syndrome that follows a traumatic experience. Predator exposure promotes long-lasting anxiogenic effect in rodents, an effect related to symptoms found in PTSD patients. Cannabidiol (CBD) is a non-psychotomimetic component of Cannabis sativa with anxiolytic effects. The present study investigated the anti-anxiety actions of CBD administration in a model of PTSD. Male Wistar rats exposed to a predator (cat) received, 1 h later, singled or repeated i.p. administration of vehicle or CBD. Seven days after the stress animals were submitted to the elevated plus maze. To investigate the involvement of 5HT1A receptors in CBD effects animals were pre-treated with WAY100635, a 5HT1A receptor antagonist. To explore possible neurobiological mechanisms involved in these effects, 5HT1A receptor mRNA and BDNF protein expression were measured in the hippocampus, frontal cortex, amygdaloid complex and dorsal periaqueductal gray. Repeated administration of CBD prevented long-lasting anxiogenic effects promoted by a single predator exposure. Pretreatment with WAY100635 attenuated CBD effects. Seven days after predator exposure 5HT1A mRNA expression was up regulated in the frontal cortex and hippocampus. CBD and paroxetine failed to prevent this effect. No change in BDNF expression was found. In conclusion, predator exposure promotes long-lasting up-regulation of 5HT1A receptor gene expression in the hippocampus and frontal cortex. Repeated CBD administration prevents the long-lasting anxiogenic effects observed after predator exposure probably by facilitating 5HT1A receptors neurotransmission. Our results suggest that CBD has beneficial potential for PTSD treatment and that 5HT1A receptors could be a therapeutic target in this disorder. (C) 2012 Elsevier Ltd. All rights reserved.
