Dysmenorrhea is common in adolescent years, especially after the onset of ovulatory cycles, usually 2 to 3 years after menarche. Pain and symptoms are responsible for school absenteeism and interruption of sports and social activities. OBJECTIVES: This study aims to measure the prevalence of severe dysmenorrhea and its consequences on adolescent girls in Switzerland. Treatment of dysmenorrhea is discussed and recommendations for clinical practice are given. STUDY DESIGN: Cross sectional survey (SMASH 02) on a nationally representative sample of adolescents (n=7548; 3340 females), aged 16 to 20 years who attended post-mandatory education. A self-administered questionnaire was used to assess the severity of dysmenorrhea and its consequences on daily life pursuit of medical help and medications used. RESULTS: Among 3340 girls, 86.6% suffered from dysmenorrhea-related symptoms: 12.4% described having severe dysmenorrhea and 74.2% moderate dysmenorrhea. Girls with severe dysmenorrhea described heavier consequences on daily activities compared with girls without dysmenorrhea: 47.8% of girls with severe dysmenorrhea reported staying at home and 66.5% declared reducing their sportive activities. Yet, fewer than half have consulted a physician for this complaint and even fewer were treated properly. RECOMMENDATION: The pediatrician has a pivotal role in screening young patients for dysmenorrhea, as well as, educating and effectively treating adolescent girls with menstruation-associated symptoms. Non-steroidal anti-inflammatory drugs are considered the first-line of treatment for dysmenorrhea, and adolescents with symptoms that do not respond to this treatment for 3 menstrual periods should be offered combined oestroprogestative contraception and must be followed up, as non-responders may have an underlying organic pathology. CONCLUSION: Dysmenorrhea is a frequent health problem in adolescent years and adolescent care providers should be able to care for these patients in an efficient way.

We have recently reported that the inhibition of endothelial cell COX-2 by non-steroidal anti-inflammatory drugs suppresses alpha(V)beta(3)- (but not alpha(5)beta(1)-) dependent Rac activation, endothelial cell spreading, migration, and angiogenesis (Dormond, O., Foletti, A., Paroz, C., and Ruegg, C. (2001) Nat. Med. 7, 1041-1047). Here we investigated the role of the COX-2 metabolites PGE(2) and TXA2 in regulating human umbilical vein endothelial cell (HUVEC) adhesion and spreading. We report that PGE(2) accelerated alpha(V)beta(3)-mediated HUVEC adhesion and promoted Rac activation and cell spreading, whereas the TXA2 agonist retarded adhesion and inhibited spreading. We show that the cAMP level and the cAMP-regulated protein kinase A (PKA) activity are critical mediators of these PGE(2) effects. alpha(V)beta(3)-mediated adhesion induced a transient COX-2-dependent rise in cAMP levels, whereas the cell-permeable cAMP analogue 8-brcAMP accelerated adhesion, promoted Rac activation, and cell spreading in the presence of the COX-2 inhibitor NS-398. Pharmacological inhibition of PKA completely blocked alpha(V)beta(3)-mediated adhesion. A constitutively active Rac mutant (L61Rac) rescued alpha(V)beta(3)-dependent spreading in the presence of NS398 or, but did not accelerate adhesion, whereas a dominant negative Rac mutant (N17Rac) suppressed spreading without affecting adhesion. alpha(5)beta(1)-mediated HUVEC adhesion, Rac activation, and spreading were not affected by PGE(2), 8-brcAMP, or the inhibition of PKA. In conclusion, these results demonstrate that PGE(2) accelerates alpha(V)beta(3)-mediated endothelial cell adhesion through cAMP-dependent PKA activation and induces alpha(V)beta(3)-dependent spreading via cAMP- and PKA-dependent Rac activation and may contribute to the further understanding of the regulation of vascular integrins alpha(V)beta(3) by COX-2/PGE(2) during tumor angiogenesis and inflammation.

Background: Adverse drug reactions (ADRs) are a threat to patients' health and quality of life, and can generate significant expenses. They are generally underreported, with different rates in different health care systems. Methods: We conducted a 6-month survey of all primary admissions to the medical emergency department of a university hospital and assessed the rate, characteristics, avoidability, and marginal costs of ADRs. Results: A total of 7% of all admissions were mainly caused by ADRs. The most frequent were gastrointestinal bleeding (22.3%) and febrile neutropenia (14.4%). Anticancer drugs were involved in 22.7% of the cases, and anticoagulants, analgesics, and non-steroidal anti-inflammatory drugs in 8% each. Physicians had prescribed 70% of these drugs. Patients were predominantly treated in intermediate care units and ordinary wards. The mean cost per case amounted to CHF 3586+/-342, or a total of CHF 821204 over the 6-month-period (1 CHF=0.56 US$=0.87 Euro). A total of 67% were considered definitely imputable to drug effects and 32% were retrospectively regarded as avoidable. Conclusions: Interventions aimed at reducing the incidence of ADRs should be directed towards both patient education and physician training. This could save hospitals admissions and money, and could be used as an indicator of prescription quality.

Experimental and clinical evidence indicates that non-steroidal anti-inflammatory drugs and cyclooxygenase-2 inhibitors may have anti-cancer activities. Here we report on a patient with a metastatic melanoma of the leg who experienced a complete and sustained regression of skin metastases upon continuous single treatment with the cyclooxygenase-2 inhibitor rofecoxib. Our observations indicate that the inhibition of cyclooxygenase-2 can lead to the regression of disseminated skin melanoma metastases, even after failure of chemotherapy.

Heterotopic ossification is a common complication following total hip arthroplasty and surgery following acetabular trauma. It is associated with pain and a decreased range of movement. Prophylaxis is achieved by either non-steroidal anti-inflammatory drug treatment or localised irradiation therapy. The objective of this study was to evaluate the evidence for pharmacological agents used for the prophylaxis of heterotopic ossification following hip and acetabular surgery. The study used a comprehensive literature search to identify all major clinical studies investigating the pharmacological agents used in the prophylaxis of heterotopic ossification following hip and acetabular surgery. It was concluded that indometacin remains the 'gold standard' for heterotopic ossification prophylaxis following total hip arthroplasty and is the only drug proven to be effective against heterotopic ossification following acetabular surgery. Following total hip arthroplasty, other non-steroidal anti-inflammatory drugs, including naproxen and diclofenac, are equally as effective as indometacin and can be considered as alternative first-line treatments. Celecoxib is also of equal efficacy to indometacin and is associated with significantly fewer gastrointestinal side effects. However, serious concerns were raised over the safety of selective cyclooxygenase-2 inhibitors for the cardiovascular system and these should be used cautiously.

BACKGROUND: Non-steroidal anti-inflammatory drugs are known to promote sodium retention and to blunt the blood pressure lowering effects of several classes of antihypertensive agents including beta-blockers, diuretics and angiotensin converting enzyme (ACE) inhibitors. The purpose of the present study was to investigate the acute and sustained effects of indomethacin on the renal response to the angiotensin II receptor antagonist valsartan and to the ACE inhibitor enalapril. METHODS: Twenty normotensive subjects maintained on fixed sodium intake (100 mmol sodium/day) were randomized to receive for one week: valsartan 80 mg o.d., enalapril 20 mg o.d., valsartan 80 mg o.d. + indomethacin 50 mg bid and enalapril 20 mg o.d. + indomethacin 50 mg bid. This single-blind study was designed as a parallel (valsartan vs. enalapril) and cross-over trial (valsartan or enalapril vs. valsartan + indomethacin or enalapril + indomethacin). Renal hemodynamics and urinary electrolyte excretion were measured for six hours after the first and seventh administration of each treatment regimen. RESULTS: The results show that valsartan and enalapril have comparable renal effects characterized by no change in glomerular filtration rate and significant increases in renal plasma flow and sodium excretion. The valsartan- and enalapril-induced renal vasodilation is not significantly blunted by indomethacin. However, indomethacin similarly abolishes the natriuresis induced by the angiotensin II antagonist and the ACE inhibitor. CONCLUSIONS: This observation suggests that although angiotensin receptor antagonists do not affect prostaglandin metabolism, the administration of a non-steroidal anti-inflammatory drug blunts the natriuretic response to angiotensin receptor blockade.

Molecular chaperones are central to cellular protein homeostasis. In mammals, protein misfolding diseases and aging cause inflammation and progressive tissue loss, in correlation with the accumulation of toxic protein aggregates and the defective expression of chaperone genes. Bacteria and non-diseased, non-aged eukaryotic cells effectively respond to heat shock by inducing the accumulation of heat-shock proteins (HSPs), many of which molecular chaperones involved in protein homeostasis, in reducing stress damages and promoting cellular recovery and thermotolerance. We performed a meta-analysis of published microarray data and compared expression profiles of HSP genes from mammalian and plant cells in response to heat or isothermal treatments with drugs. The differences and overlaps between HSP and chaperone genes were analyzed, and expression patterns were clustered and organized in a network. HSPs and chaperones only partly overlapped. Heat-shock induced a subset of chaperones primarily targeted to the cytoplasm and organelles but not to the endoplasmic reticulum, which organized into a network with a central core of Hsp90s, Hsp70s, and sHSPs. Heat was best mimicked by isothermal treatments with Hsp90 inhibitors, whereas less toxic drugs, some of which non-steroidal anti-inflammatory drugs, weakly expressed different subsets of Hsp chaperones. This type of analysis may uncover new HSP-inducing drugs to improve protein homeostasis in misfolding and aging diseases.

Introduction: Use of paracetamol has been associated with an increased risk of asthma in several epidemiological studies. In contrast, it has been suggested that non-steroidal anti-inflammatory drugs (NSAIDs) might be protective (Kanabar, Clin Ther 2007), but data relating to these drugs are scarce. Methods: Prevalence of asthma and intake of analgesics in the past 2 years were assessed by questionnaire in 2008 in young adults (≥;16 years) diagnosed with cancer between 1976 and 2003 (Swiss Childhood Cancer Survivor Study). In a multivariate logistic regression we analysed the association between asthma and intake of paracetamol only, NSAIDs only or their combination, adjusting for age, sex, cancer diagnosis, cancer therapy and time since diagnosis. Results: Of the 1293 participants (response rate 68%), 83 (6%) reported asthma and 845 (65%) intake of analgesics in the past 2 years. Of these, 257 (29%) took paracetamol only, 224 (25%) NSAIDs only, 312 (35%) a combination of both and 52 (6%) other analgesics. Adjusted Odds ratios for asthma were 2.2 (95% CI 1.0-4.7; p = 0.04), 1.9 (0.9-4.3; p = 0.12) and 2.9 (1.4-6.1; p <0.01) in those using paracetamol only, NSAIDs only or their combination respectively. Conclusion: These cross-sectional data in a selected population do not support a protective effect of NSAIDs against asthma, neither taken alone nor in combination with paracetamol. All analgesics were positively associated with reported asthma episodes in the past two years. This can be explained by reverse causation, with intake of analgesics being a result rather than a cause of asthma events. Randomised controlled trials in unselected populations are needed to clarify the direction of causation.

Les maladies rhumatismales sont fréquemment observées chez les personnes âgées et ont un impact considérable sur la qualité de vie des personnes en souffrant. Peu d'études suisses sur la situation épidémiologique et sur l'impact de ce type de maladies sur la consommation des services de santé existent. Cette thèse a pour but d'étudier les connaissances actuelles à partir de la littérature suisse et étrangère et d'établir un bilan en Suisse au moyen d'une enquête de population effectuée en 1997. Une revue systématique de littérature a été effectuée. En dépit d'une grande variété des approches méthodologiques rendant délicates les comparaisons géographiques et temporelles, la prévalence des maladies rhumatismales chez les personnes de 65 ans et plus semble être homogène entre pays et stable temporellement. particulièrement dès 1980. Celle-ci est élevée et augmente rapidement avec le très grand âge. De plus, les femmes sont plus fréquemment atteintes que les hommes. Notre revue montre que le recours aux soins ambulatoires (médecins, chiropraticiens, traitements ambulatoires des hôpitaux) lié aux maladies rhumatismales est important. L'impact de ces maladies sur les hospitalisations est, par contre, moins clairement établi. Les nouvelles générations de personnes en souffrant semblent avoir plus recours aux services de santé que les précédentes. Ces maladies sont aussi à l'origine d'une forte consommation d'anti-inflammatoires non-stéroïdiens avec comme conséquence une multitude de complications. En dépit de son efficacité et de son utilité, le recours à l'arthroplastie est sous- utilisé. Notre analyse se base sur une enquête réalisée auprès d'un échantillon représentatif des individus âgés de 15 ans et plus résidant de manière permanente en Suisse en 1997 : la prévalence des maladies rhumatismales en Suisse s'élève à 41 % chez les personnes âgées de 65 ans et plus, dont 48 % chez les femmes et 31 % chez les hommes. Ces prévalences sont inférieures à celles relevées dans la littérature probablement en raison de notre définition relativement restrictive des maladies rhumatismales. Ces dernières augmentent de 50 % le nombre attendu de consultations chez un médecin ou un chiropraticien et de 30 % le nombre attendu d'hospitalisations. Les personnes souffrantes ont. en outre, une probabilité de recours aux services de Soins à domicile 1,7 fois plus élevé que les autres. Aucun impact sur le nombre de traitements ambulatoires en milieu hospitalier n'a été trouvé. Nos résultats sont comparables à ceux relevés dans la littérature internationale et suisse, sauf pour les traitements ambulatoires des hôpitaux. En 1990, sur les 983'400 personnes de 65 ans et plus (recensement fédéral de la population de 1990), 403'200 personnes souffraient de maladies rhumatismales. Quelque 5'334'900 consultations chez un médecin ou un chiropraticien, 4'959'300 consultations chez un médecin et 216'800 hospitalisations étaient imputables aux personnes de 65 ans et plus toutes causes de consultations confondues, dont 1'008'000 consultations chez un médecin/chiropraticien, 927'300 chez un médecin et 98'500 hospitalisations imputables aux maladies rhumatismales. Selon ie scénario (( tendance )) des projections démographiques publiées par l'Office Fédéral de la Statistique. d'ici 2040, le nombre de personnes souffrant de maladies rhumatismales en Suisse risque d'augmenter de 80 % (en supposant que la prévalence reste stable), affectant 726'500 sur 1'772'000 personnes de 65 ans et plus. Cette augmentation est la conséquence de l'accroissement prévu de la population de 65 ans et plus dans la population générale. Le nombre global de consultatiordhospitalisations risque d'augmenter dans les mêmes proportions si le recours aux services de santé reste stable. En effet. en 2040, quelque 9'613'100 consultations chez un médecinichiropraticien, 8'936'200 consultations chez un médecin et 390'700 hospitalisations pourraient être imputables aux personnes de 65 ans et plus. dont 1'8 16'300 consultations chez un médecin/chiropraticien, 1'67 1'000 consultations chez un médecin et 1 90'600 hospitalisations en raison de maladies rhumatismales. Une légère diminution du nombre de personnes atteintes de maladies rhumatismales. ainsi que du recours aux services de santé engendré par ces maladies. est attendue dès 3040. Le nombre de personnes souffrant de maladies rhumatismales et le nombre de consultations/ hospitalisations associées risquant d'augmenter de façon considérable, il est nécessaire de freiner cette progression. Des mesures préventives primaires, secondaires ou tertiaires peuvent diminuer la prévalence des maladies rhumatismales et l'impact de celles-ci sur la consommation des services de santé.<br/><br/>Rheumatic diseases are frequently observed in elderly people and have an important impact on tlieir life qurlity. This thesis aims at studying the current knowledge based on Swiss and international literature and at establishing the situation in Switzerland from a population survey conducted in 1997. A systeinatic literature review lias been carried out. Despite a large range of methods making a comparisoii diffcult, the prevalence of rheumatic diseases seems to be homogeneous in different countries and stable. especially since 1980. It is high and increases rapidly with age. Furthermore, \niorneil suffer more frequently thaii men. Our review shows that the use of ambulatory care linked to rheumatic diseases is important. On the contrary, the impact of such diseases on hospitalization is less clearly established. New generations seem to consult more. Rheumatic diseases are also at the origin of a strong consumptioii of non-steroidal anti- inflammatory drugs \vitIl potential severe consequences. Despite its effectiveness and efficiency, arthroplasty is underused. Our analysis is based 011 a survey of Swiss permanent residents aged 15 or more in 1997. Based on Our analysis, the prevalence of rheumatic diseases in Switzerland is 41 % for elderly people (48 96 for women and 31 % for men). Theses prevalences are smaller than those found in the literature because of our relatively strict definition of rheumatic diseases. The latter diseases increase of about 50 o/o the expected number of consultations (chiropractor included or not) and of about 30 960 the expected number of hospitalizations. The affected persons have a probability of home care use 1.7 times higlier than the others. No impact on the number of outpatient care provided by hospitals has been found. Our results are comparable to those found in the international and Swiss literature, except for hospital outpatient care. In 1990, of 983,400 perçons aged 65 and older, 403,200 persons suffered from rheumatic diseases. 5,334,900 consultations by a physician or a chiropractor, 4,959,300 consultations by a physician and 2 16,800 hospitalizations were attributed to the elderly whatever, the reason of consultation, of which 1,008,000 consultations by a physicianlchiropractor, 927,300 by a physician, and 98,500 hospitalizations are due to rheumatic diseases. According to the "tendance" scenario of demographic projections published by the Swiss Federal Office of Statistics, until 2040 the number of persons suffering from rheumatic diseases will increase of 80 % if the prevalence stays stable, affecting 736,500 of 1,772,000 perçons of 65 and older. This increase is due to the increase of the percentage of persons 65 and older in the population. The global number of consultationshospitalizations will increase similarly if the use of health services stays stable. In 2040, 9,613,l 00 consultations by a physiciaidchiropractor, 8,936,200 Consultations by a physician and 390,700 hospitalizations could be attributed to the persons aged 65 and older, of which 1,816,300 consultations by a physician, 1,671,000 consultations by a physician/chiropractor and 109,600 hospitalizations will be due to the rheumatic diseases. However a small decrease of the number of affected perçons and of the subsequent use of health services is expected after 2040. The number of affected elderly people and the volume of conçultations/hospitalizations are expected to increase and it ir necessx-y to slow down this progression. Preventive interventions, primary, secondary or tertiary, can decrease the prevalence of rheumatic diseases and the impaci on the consumption of health services.

OBJECTIVES: Non-steroidal anti-inflammatory drugs (NSAIDs) may cause kidney damage. This study assessed the impact of prolonged NSAID exposure on renal function in a large rheumatoid arthritis (RA) patient cohort. METHODS: Renal function was prospectively followed between 1996 and 2007 in 4101 RA patients with multilevel mixed models for longitudinal data over a mean period of 3.2 years. Among the 2739 'NSAID users' were 1290 patients treated with cyclooxygenase type 2 selective NSAIDs, while 1362 subjects were 'NSAID naive'. Primary outcome was the estimated glomerular filtration rate according to the Cockroft-Gault formula (eGFRCG), and secondary the Modification of Diet in Renal Disease and Chronic Kidney Disease Epidemiology Collaboration formula equations and serum creatinine concentrations. In sensitivity analyses, NSAID dosing effects were compared for patients with NSAID registration in ≤/>50%, ≤/>80% or ≤/>90% of assessments. FINDINGS: In patients with baseline eGFRCG >30 mL/min, eGFRCG evolved without significant differences over time between 'NSAID users' (mean change in eGFRCG -0.87 mL/min/year, 95% CI -1.15 to -0.59) and 'NSAID naive' (-0.67 mL/min/year, 95% CI -1.26 to -0.09, p=0.63). In a multivariate Cox regression analysis adjusted for significant confounders age, sex, body mass index, arterial hypertension, heart disease and for other insignificant factors, NSAIDs were an independent predictor for accelerated renal function decline only in patients with advanced baseline renal impairment (eGFRCG <30 mL/min). Analyses with secondary outcomes and sensitivity analyses confirmed these results. CONCLUSIONS: NSAIDs had no negative impact on renal function estimates but in patients with advanced renal impairment.

Psoriatic arthritis is a chronic inflammatory disease. It affects up to 40% of patients suffe- ring from skin psoriasis. Joint involvement is relatively heterogeneous. Some clinical manifestations are similar to those of rheumatoid arthritis, others are close to spondylarthritis manifestations and are therefore considered as part of this entity. Treatment depends on initial presentation (peripheral or axial) but often begins with non-steroidal anti-inflammatory drugs and methotrexate, followed by anti-TNFalpha if needed. New therapeutic op- tions are available or under evaluation, parti- cularly targeting cytokines involved in psoriatic arthritis (IL-12/IL-23 and IL-17).

Mondor's disease is a rare entity characterized by sclerosing thrombophlebitis classically involving one or more of the subcutaneous veins of the breast and anterior chest wall. It is usually a self-limited, benign condition, despite of rare cases of association to cancer. We present the case of a 32 year-old female, breast-feeding, who went to emergency due to left mastalgia for the past week. She was taking antibiotics and non-steroidal anti-inflammatory drugs, previously prescribed for suspicious of mastitis, for three days, with no clinical improvement. Physical examination showed an enlarged left breast, an axillary lump and a painful cord-like structure in the upper outer quadrant of the same breast. Ultrasound scan showed a markedly dilated superficial vein in the upper outer quadrant of left breast. The patient was given a ventropic therapy and was kept in anti-inflammatory, with progressive pain improvement. Ultrasound control was performed after four weeks, showing reperfusion.

L’arthrite septique chez les bovins est une affection le plus souvent d’origine bactérienne qui est une cause de boiterie fréquente associée à des pertes économiques importantes. Le traitement, qui doit être initié le plus rapidement possible, s’appuie sur l’utilisation combinée d’anti-inflammatoires non stéroïdiens, d’un ou de plusieurs lavages articulaires ainsi que d’antibiotiques administrés de façon systémique pour un minimum de 3 semaines. Cette durée d’administration constitue une difficulté pour les élevages dits biologiques pour lesquels un cahier des charges strict limite un recours prolongé aux antibiotiques. Ainsi le traitement efficace de diverses conditions infectieuses et celui de l’arthrite septique en particulier dans ces élevages peut être compromis. L’objectif de ce travail est de s’appuyer sur les propriétés antimicrobiennes des solutions de saline hypertonique (SSH) pour limiter l’utilisation des antibiotiques dans les cas d’arthrite septique chez les veaux en intégrant ces solutions pour le lavage des articulations infectées. Notre première étude a consisté à déterminer la sécurité d’emploi de deux concentrations de SSH (une commerciale à 7.2% et une maison à 15%) dans des articulations supposées saines chez le veau. Une synovite sévère associée à des signes cliniques caractéristiques d’atteintes articulaires a été observé lors de l’utilisation de SSH 15%. Son utilisation clinique comme solution de lavage articulaire est par conséquent déconseillée. Concernant la SSH 7.2%, malgré une synovite d’intensité variable, nous n’avons pas noté des signes cliniques caractéristiques d’atteintes articulaires. Son utilisation dans un contexte expérimental d’infection articulaire nous a paru réaliste. Notre seconde étude a permis d’évaluer les effets du lavage articulaire avec de la SSH 7.2% dans un modèle expérimental d’infection à Escherichia coli. Trois groupes de traitement ont été constitués. Dans le premier groupe (traitement standard), un lavage était effectué avec du lactate de Ringer (LRS) et les veaux recevaient une administration biquotidienne de ceftiofur par voie intraveineuse pour 21 jours. Dans le deuxième groupe (LRS), un lavage était effectué avec du LRS et aucun antibiotique n’était administré. Enfin dans le troisième groupe (SSH), un lavage était effectué avec de la SSH 7.2% sans qu’aucun antibiotique ne soit administré. Tous les veaux ont reçu du kétoprofen quotidiennement pendant 3 jours. L’inoculation s’est fait au jour 1 et les traitements ont débuté au jour 2. L’efficacité des traitements a été établie sur des critères cliniques, bactériologiques et cytologiques. Dans le modèle que nous avons utilisé, les trois groupes de traitements ont conduits à une guérison clinique et bactériologique satisfaisante (absence de boiterie, de douleur et de croissance bactérienne dans le liquide articulaire en fin d’étude). La guérison cytologique n’a quant à elle pas été jugée satisfaisante avec des moyennes de comptage et de pourcentage en neutrophiles tout groupe confondu bien supérieures aux valeurs normales (11.39x109/l de neutrophiles représentant 74.73% des leucocytes en fin d’étude). Le traitement avec la SSH 7.2% s’est révélé être significativement plus traumatique et pas plus efficace dans l’élimination de l’infection comparativement au traitement LRS. D’autre part, le lavage articulaire au LRS s’est révélé être aussi efficace et sécuritaire que le traitement standard de l’arthrite septique qui incluait 21 jours de ceftiofur administré par voie intraveineuse. Ainsi, bien que le lavage avec de la SSH 7.2% n’est pas démontré de résultats satisfaisants qui permettrait de limiter le recours aux antibiotiques dans les filières biologiques, l’association d’un lavage au LRS avec le ketoprofen s’est révélée être une alternative possible. De nouvelles études sont requises pour confirmer ses résultats et pour déterminer le rôle respectif du lavage articulaire et du ketoprofen dans la guérison lors d’arthrite septique.

El objetivo fue evaluar la intervención de las alertas en la prescripción de diclofenaco. Estudio observacional, comparativo, post intervención, de un antes después, en pacientes con prescripción de diclofenaco. Se evaluó la intervención de las alertas restrictivas antes y después de su implementación en los pacientes prescritos con diclofenaco y que tenían asociado un diagnóstico de riesgo cardiovascular según CIE 10 o eran mayores de 65 años. Un total de 315.135 transacciones con prescripción de diclofenaco, en 49.355 pacientes promedio mes. El 94,8% (298.674) de las transacciones fueron prescritas por médicos generales.
