OBJECTIVE: This study assessed the anterior-posterior positioning of the upper and lower first molars, and the degree of rotation of the upper first molars in individuals with Class II, division 1, malocclusion. METHODS: Asymmetry I, an accurate device, was used to assess sixty sets of dental casts from 27 females and 33 males, aged between 12 and 21 years old, with bilateral Class II, division 1. The sagittal position of the molars was determined by positioning the casts onto the device, considering the midpalatal suture as a symmetry reference, and then measuring the distance between the mesial marginal ridge of the most distal molar and the mesial marginal ridge of its counterpart. With regard to the degree of rotation of the upper molar, the distance between landmarks on the mesial marginal ridge was measured. Chi-square test with a 5% significance level was used to verify the variation in molars position. Student's t test at 5% significance was used for statistical analysis. RESULTS: A great number of lower molars mesially positioned was registered, and the comparison between the right and left sides also demonstrated a higher number of mesially positioned molars on the right side of both arches. The average rotation of the molars was found to be 0.76 mm and 0.93 mm for the right and left sides, respectively. CONCLUSION: No statistically significant difference was detected between the mean values of molars mesialization regardless of the side and arch. Molars rotation, measured in millimeters, represented ¼ of Class II.

OBJECTIVE: The purpose of the study was to evaluate the influence of the skeletal maturation in the mandibular and dentoalveolar growth and development during the Class II, division 1, malocclusion correction with Balters bionator. METHODS: Three groups of children with Class II, division 1, malocclusion were evaluated. Two of them were treated for one year with the bionator of Balters appliance in different skeletal ages (Group 1: 6 children, 7 to 8 years old and Group 2: 10 children, 9 to 10 years old) and the other one was followed without treatment (Control Group: 7 children, 8 to 9 years old). Lateral 45 degree cephalometric radiographs were used for the evaluation of the mandibular growth and dentoalveolar development. Tantalum metallic implants were used as fixed and stable references for radiograph superimposition and data acquisition. Student's t test was used in the statistical analysis of the displacement of the points in the condyle, ramus, mandibular base and dental points. Analysis of variance one-fixed criteria was used to evaluate group differences (95% of level of significance). RESULTS: The intragroup evaluation showed that all groups present significant skeletal growth for all points analyzed (1.2 to 3.7 mm), but in an intergroup comparison, the increment of the mandibular growth in the condyle, ramus and mandibular base were not statically different. For the dentoalveolar modifications, the less mature children showed greater labial inclination of the lower incisors (1.86 mm) and the most mature children showed greater first permanent molar extrusion (4.8 mm).

The aim of this study was to evaluate the facial profile changes due to natural growth and induced by Herbst appliance and Bionator in the treatment of Class II, division 1 malocclusion. In order to do that, we used a sample of 90 lateral radiographs of 45 individuals in pre-pubertal stage, divided up in two experimental groups and one control. The first group, composed of 15 brazilian individuals, with initial mean age of 9.4 years, was treated with the Herbst appliance for a period of seven months. The second experimental group consisting of 15 brazilian individuals, initial mean age of 9.9 years has gone through bionator therapy for an average period of 21 months. The control group of 15 individuals, who were not treated orthodontically, comes up from the Burlington Growth Centre, University of Toronto, Canada. The intragroup comparison was performed using the Student t test and intergroup comparisons by ANOVA complemented by the Bonferroni test. The results have shown that only the group treated with the Herbst appliance presented significant changes in facial profile with improvement of its convexity and lower lip protrusion.

A má oclusão de Classe II divisão 2 é caracterizada pela verticalização dos incisivos superiores. Nesse tipo de má oclusão, é comum a presença de sobremordida profunda e curva de Spee inferior acentuada. O presente artigo ilustra que o tratamento dessa má oclusão pode ser facilitado com o emprego dos arcos seccionados, como por exemplo, o arco utilidade, segundo a técnica Bioprogressiva de Ricketts.

Because it still brings polemic among the professionals, it was mention once more, in this literature review, the theme that aims to define the appropriate timing to approach the bad occlusions of Class II, and also when it would the right opportunity to treat them, in way that would convey to the patient consistent, stable results in a long term. These are important aspects to be taken into consideration by orthodontists, either by those who defend the early treatment or by those who defend the late treatment. Therefore, the present study aims to aid the clarification of daily doubts regarding this aspect.

Introduction: This retrospective cephalometric study analyzed the influence of intentional ankylosis of deciduous canines in patients with Class III malocclusion and anterior crossbite, in the deciduous and early mixed dentition stages, treated by orthopedic maxillary expansion followed by maxillary protraction. Methods: Lateral cephalograms of 40 patients were used, divided in 2 groups paired for age and gender. The Ankylosis Group was composed of 20 patients (10 boys and 10 girls) treated with induced ankylosis and presenting initial and final mean ages of 7 years 4 months and 8 years 3 months, respectively, with a mean period of maxillary protraction of 11 months. The Control Group comprised 20 patients (10 boys and 10 girls) treated without induced ankylosis, with initial and final mean ages of 7 years 8 months and 8 years 7 months, respectively, with a mean period of maxillary protraction of 11 months. Two-way analysis of variance and covariance analysis were applied to compare the initial and final cephalometric variables and the treatment changes between groups. Results: According to the results, the variables evidencing the significant treatment changes between groups confirmed that the intentional ankylosis enhanced the sagittal response of the apical bases (Pg-NPerp) and increased the facial convexity angles (NAP and ANB). Conclusions: The protocol involving intentional ankylosis of deciduous canines enhanced the sagittal response of the apical bases.

OBJECTIVE: This study aimed to evaluate the cephalometric effects promoted by the orthodontic treatment of Class II malocclusion patients with the use of the 10-Hour Force Theory, that consists in the use of fixed appliances with 8 hours a day using a cervical headgear appliance and 16 hours a day using Class II elastics, 8 hours on the first mandibular molar and 8 hours in the second mandibular molar. METHODS: Sample comprised 31 patients with mean initial age of 14.90 years, final mean age of 17.25 years and mean treatment time of 2.35 years. The lateral cephalograms in pre-treatment and post-treatment stages were evaluated. Evaluation of cephalometric changes between initial and final treatment phases was performed by paired t test. RESULTS: The cases treated with the 10-Hour Force Theory presented a slight restriction of anterior displacement of the maxilla, increase in the effective length of the mandible, significant improvement of the maxillomandibular relationship, significant increase in anterior lower face height, distal tipping of the maxillary premolar crowns, extrusion and distal tipping of the roots of maxillary molars, significant proclination and protrusion of mandibular incisors, significant extrusion and mesialization of mandibular molars, besides a significant correction of the molar relationship, overjet and overbite. CONCLUSION: The use of the 10-Hour Force Theory in treatment of Class II malocclusion provided satisfactory results.

A assimetria das arcadas dentárias constitui um assunto de grande importância estudado por profissionais de Ortodontia na elaboração de um diagnóstico correto, planejamento e execução de um tratamento ortodôntico bem sucedido. O objetivo deste estudo foi avaliar o grau de assimetria das arcadas dentárias em indivíduos com oclusão normal e má oclusão de Classe II, divisão 1 e 2, bem como o dimorfismo sexual existente. Foram avaliados 180 pares de modelos de estudo de indivíduos do sexo masculino e feminino, na faixa etária de 12 a 21 anos, divididos em 3 grupos de 60 pares de modelos, de acordo com a má oclusão. Os grupos foram classificados em: Grupo 1 - indivíduos com oclusão normal, Grupo 2 - indivíduos com má oclusão de Classe II divisão 1 (Cl II 1), e Grupo 3 - indivíduos com má oclusão de Classe II divisão 2 (Cl II 2). Os modelos foram medidos utilizando-se um aparelho inédito, idealizado e fabricado exclusivamente para a análise de assimetria das arcadas dentárias. Para a análise de assimetria foram realizadas 2 medições angulares desvio de linha média (DLM), posicionamento dos caninos (PC) e 3 lineares distância dos caninos à rafe palatina (DC), distância inter-caninos (DIC), posicionamento dos primeiros molares no sentido ântero-posterior (PM). Concluiu-se que a ocorrência de assimetria nas arcadas dentárias independe da má oclusão. O Grupo 1 apresentou um menor grau de assimetria nas arcadas dentárias em relação aos grupos 2 e 3, os quais apresentaram um grau de assimetria semelhante. O grau de assimetria nas arcadas dentárias inferiores, nos 3 grupos, foi maior do que nas arcadas dentárias superiores. A direção do desvio da linha média apresentou uma correlação inversamente proporcional do lado em que o molar se apresentava mesializado, nas arcadas superior e inferior dos três grupos, com exceção da arcada superior no Grupo 2 (Classe II divisão 1). Houve dimorfismo sexual estatisticamente significante para algumas medidas, porém é importante considerar os baixos valores e a disposição, destas diferenças, entre as medidas realizadas, a qual revela ter se tratado de dados obtidos ao acaso.
