It is understood by Chronic Renal Failure (CRF) or Stage IV Chronic Kidney Disease (CKD) the morbid state in which there is a substantial and irreversible loss of functional capacity, metabolic and endocrine function in both kidneys, leading to a framework of dehydration, azotemia, electrolyte imbalance, anemia, uremic syndrome, secondary hyperparathyroidism, among other conditions that make the patient's life without treatment unfeasible. An adequate nutritional support to patients with CKD at any stage aims to maintain a good clinical condition and an attempt to delay the progression of the disease. The basis of a diet for patients with CKD is the restriction in this protein being of high biological value, low-phosphorus, high energy density, fiber and antioxidants combined with a good dietary practices that allow the patient a good quality of life

Objective: To analyze and correlate the physical activity level (PAL) and the quality of life (QL) in patients with chronic kidney disease (CKD) undergone hemodialyses and to verify in practitioner of physical activity (PPA) if they follow rules and recommendations on their own practice. Methods: The QL (KDQOL-SFTM questionnaire), PAL (IPAQ questionnaire) and the evaluation of rules and recomendations to practice PA were analyzed in one hundred patients. Results: It was observed commitment on different dimensions of QL, higher values to sexual function and social support (88,6) and lower ones to professional paper (25,2) and physical function (47,7). Most patients shows low values of PAL and most PPA implement the exercises incorrectly. It was observed differences between PAL and some areas of QL. Conclusion: Patients with CKD undergone hemodialysis presented low values to PAL, commitment on QL, and most of the patients that realized PA did not implement rules and recomendations on their own practice.

PURPOSE: Chronic renal insufficiency (CRI) is the last stage of a chronic renal condition in which the kidney loses its filtration and endocrine functions. Chronic endocrine hypofunction causes generalized damage to the body known as Uremic Syndrome, which affects the central nervous system as well as the cardiovascular, hematologic, dermatologic, ophthalmic, endocrine, respiratory, gastrointestinal and skeletal systems. The present study reports the case of a female patient with CRI who presented facial osteodystrophy of the osteitis fibrosa type, and highlights the main features of this condition. CASE DESCRIPTION: A 24-year old, female, Caucasian patient presented chronic glomerulonephritis recurrence and lost the transplanted kidney five years before, undergoing arteriovenous fistula hemodialysis three times a week. She presented swelling of the left masseter area with a hard consistency on palpation, covered by intact skin, swelling at the bottom of the left atrium, with a hard consistency on palpation, a mucosa-like color and absence of inflammation signs, suggesting expansive bone lesions on the face. These features were compatible with hyperparathyroidism brown tumor and/or osteodystrophy. The CT scan showed expansive bone lesions of heterogeneous appearance on the left jaw, maxilla/nasal floor, and right frontotemporal suture areas. The clinical and histopathological characteristics of the lesion, in association with PHT hormone high serum levels led to renal osteodystrophy diagnosis. The patient was referred to the nephrology services. CONCLUSION: Osteodystrophic bone alterations have a high prevalence in renal disease patients, and the dentist must take these alterations into consideration in bone lesion diagnosis for this specific group of patients.

Renovascular hypertension induced by 2 Kidney-1 Clip (2K-1C) is a renin-angiotensin-system (RAS)-dependent model, leading to renal vascular rarefaction and renal failure. RAS inhibitors are not able to reduce arterial pressure (AP) and/or preserve the renal function, and thus, alternative therapies are needed. Three weeks after left renal artery occlusion, fluorescently tagged mesenchymal stem cells (MSC) (210(5) cells/animal) were injected weekly into the tail vein in 2K-1C hypertensive rats. Flow cytometry showed labeled MSC in the cortex and medulla of the clipped kidney. MSC prevented a further increase in the AP, significantly reduced proteinuria and decreased sympathetic hyperactivity in 2K-1C rats. Renal function parameters were unchanged, except for an increase in urinary volume observed in 2K-1C rats, which was not corrected by MSC. The treatment improved the morphology and decreased the fibrotic areas in the clipped kidney and also significantly reduced renal vascular rarefaction typical of 2K-1C model. Expression levels of IL-1, TNF- angiotensinogen, ACE, and Ang II receptor AT1 were elevated, whereas AT2 levels were decreased in the medulla of the clipped kidney. MSC normalized these expression levels. In conclusion, MSC therapy in the 2K-1C model (i) prevented the progressive increase of AP, (ii) improved renal morphology and microvascular rarefaction, (iii) reduced fibrosis, proteinuria and inflammatory cytokines, (iv) suppressed the intrarenal RAS, iv) decreased sympathetic hyperactivity in anesthetized animals and v) MSC were detected at the CNS suggesting that the cells crossed the blood-brain barrier. This therapy may be a promising strategy to treat renovascular hypertension and its renal consequences in the near future.

Introduo: Osndrome de encefalopatia posterior reversvel, ou PRES, uma entidade clinico-radiolgica associada a uma grande variedade de contextos clnicos, nomeadamente em idade peditrica, com padres imagiolgicos mais tpicos com envolvimento dos lobos parietais e occipitais mas tambm com outras formas imagiolgicas, mais ou menos frequentes dependendo dos locais envolvidos (frontais e temporais, tronco cerebral, ganglios da base, cerebelo...). Objectivos: Apresentar formas de PRES em idade peditrica com caractersticas imagiolgicas infrequentes. Casos Clnicos: Apresentam-se os casos em duas jovens de sexo feminino, ambas com HTA grave e insuficiência renal, que se apresentaram com cefaleias intensas e uma delas com crises convulsivas, alterao do estado conscincia e hemiparsia esquerda. Num dos casos a RM demonstrou um envolvimento extenso da medula e bulbo, apenas com uma pequena leso parietal direita, com total reverso destas alteraes em estudos de controle. No outro caso, a TC e a RM evidenciaram inmeras leses de edema com envolvimento multifocal de todos os lobos cerebrais e com mltiplas reas hemorrgicas igualmente dispersas, desde micro-hemorragias a um hematoma mais volumoso, tendo-se constatado nos estudos de controle uma regresso parcial destas alteraes. Concluso: Apesar do envolvimento mais tpico das regies parietais e occipitais no PRES por leses de edema vasognico, padres atpicos e infrequentes esto documentados e devem ser reconhecidos, incluindo-se nestes padres o envolvimento predominantemente medular e a presena de mltiplas hemorragias.
