Depuis les dernières années, la prévalence de personnes souffrant de dégénérescence des cartilages articulaires, communément appelée ostéoarthrite (OA), ne cesse d’augmenter. Les douleurs articulaires et les raideurs musculaires associées à cette pathologie mènent à des limitations des capacités fonctionnelles, à une perte de mobilité et d’autonomie affectant grandement la qualité de vie de ces personnes. Afin de soulager les personnes souffrant de cette pathologie, l’arthroplastie de la hanche est une procédure chirurgicale fréquemment utilisée. À la suite de cette chirurgie, une amélioration de la qualité de vie et une reprise des capacités fonctionnelles sont souvent observées. Cependant, comparativement à des sujets sains, la vitesse de marche est diminuée, une faiblesse des muscles abducteurs de la hanche est constatée et des mouvements compensatoires au niveau du tronc sont persistants. L’objectif de cette thèse est d’évaluer le patron locomoteur chez des patients qui subiront une arthroplastie de la hanche. Plus spécifiquement, les adaptations locomotrices pré et post-opératoires seront quantifiées dans le but d’apporter des modifications aux programmes de réhabilitation pour ainsi favoriser un patron locomoteur sans déficit. Afin de répondre à cet objectif, trois études distinctes ont été effectuées. Dans le cadre de la première étude, l’impact de l’implantation d’une prothèse totale de la hanche avec une tête fémorale de large diamètre et une prothèse de resurfaçage a été évalué par rapport aux sujets sains lors de la locomotion. Au cours de cette étude, le contrôle du tronc a été analysé en utilisant la distance entre le centre de masse corporel et le centre articulaire de la hanche opérée. Suite aux résultats obtenus, aucune différence majeure n’existe entre les deux types de prothèses en ce qui a trait au contrôle du tronc et ce, à un an post-opératoire. Lors de la deuxième étude, la symétrie des paramètres biomécaniques des membres inférieurs lors de la locomotion chez des patients ayant bénéficié de l’implantation d’une prothèse de la hanche a été caractérisée suite à un programme d’exercices péri-opératoires (pré et post-opératoire). Lors de cette étude, le programme d’exercices péri-opératoires était complémentaire au protocole de réadaptation du centre hospitalier. D’après les résultats obtenus lors de cette étude exploratoire, ce programme d’exercices péri-opératoires semble permettre d’améliorer la symétrie de la puissance et du travail musculaire au niveau de la hanche, du genou et de la cheville favorisant ainsi un patron de marche avec de minimes compensations. Finalement, dans le cadre de la troisième étude, l’approche prédictive et l’approche fonctionnelle, utilisées pour localiser le centre articulaire de la hanche, ont été comparées aux mesures radiographiques, chez des patients à la suite d’un remplacement articulaire de la hanche. À la suite de cette étude, les résultats démontrent que l’utilisation de l’approche fonctionnelle est plus appropriée chez des patients ayant bénéficié d’une arthroplastie de la hanche. En effet, cette approche individualisée est plus précise ce qui, par conséquent, permettra d’obtenir des résultats de plus grande qualité lors d’analyses biomécaniques de la locomotion.

El síndrome de pinzamiento femoroacetabular es una entidad reciente secundaria al “desacoplamiento” de la articulación coxofemoral, por alteración en la morfología de la cabeza femoral o del acetábulo, lo cual puede ocasionar osteoartrosis a temprana edad. El propósito del estudio es describir los signos clínicos más frecuentes y los hallazgos imagenológicos del síndrome de pinzamiento femoroacetabular. Metodologia: se realizó un estudio retrospectivo descriptivo de la frecuencia de las manifestaciones clínicas del síndrome de pinzamiento femoroacetabular y hallazgos en artroresonancia magnética entre los meses de Enero de 2008 a junio de 2009. Se seleccionaron treinta y dos pacientes en la institución, y se evaluaron sus manifestaciones clínicas, examen físico e imágenes de artroresonancia magnética. Resultados: todos los pacientes presentaron dolor inguinal en el momento de la consulta, con presencia de test de pinzamiento positivo para todos, y el signo de la C en el 90%. El subtipo más frecuente fue PINCER 46.6 % seguido por el pinzamiento MIXTO 39.3%. El signo de crossover estuvo presente en el 100% de los pacientes con retroversión acetabular (12). El resultado de incapacidad funcional fue WOMAC 48.44 ±14.79 (IC 95% 43.1-53.77), nunca fue mayor a 50 y el dolor tuvo un promedio de 11 / 20. Discusión: la artroresonancia magnética es el examen de elección, cuyos hallazgos permiten comprender las manifestaciones clínicas. El ángulo alfa y la versión femoral constituyeron los signos más significativos, estos hallazgos son equiparables a los obtenidos en estudios donde la mayor parte de la población son mujeres de edad media.

Introducción. Las imágenes obtenidas mediante rayos X, determinan una conducta clínica en ortopedia y son analizadas por parte del cirujano en el momento previo a realizar un acto quirúrgico. El planeamiento pre quirúrgico basado en radiografías de cadera, permite predecir el tamaño de los componentes protésicos a utilizar en un reemplazo de cadera. Con el advenimiento de las radiografías digitales, existe la falsa percepción de que estas tienen corregido el factor de magnificación. La corrección de dicho factor requiere un protocolo de calibración de imágenes, aún no implementado en la Fundación Santa Fe de Bogotá (FSFB). Como consecuencia, las radiografías de cadera actualmente resultan magnificadas. Materiales y métodos. Fueron seleccionados 73 pacientes con reemplazo articular total de la cadera intervenidos en la FSFB. Para cada paciente, se estableció la dimensión de la cabeza protésica en la radiografía de cadera obtenida mediante el sistema de radiología digital (PACS-IMPAX) y su tamaño fue comparado con el de la cabeza femoral implantada. Resultados. La concordancia entre los dos observadores al medir la dimensión radiológica de los componentes protésicos fue excelente y el coeficiente de magnificación promedio de 1.2 (20%). Este será introducido al PACS-IMPAX para ajustar el tamaño definitivo de la radiografía. Conclusión. El ajuste del PACS-IMPAX permite obtener radiografías en las cuales se refleja con mayor precisión el tamaño de los segmentos anatómicos y de los componentes protésicos.

Background:  An audit performed in the use of non-irradiated femoral head bone graft at the Geelong Hospital over a 10-year period. While it is thought the non-irradiated bone graft provides a better structural construct there is theoretical increased risk of infection transmission.

Methods:  We performed a retrospective review of prospectively collected data in the use of non-irradiated bone allograft used from the Geelong Hospital Douglas Hosking Research Institute bone bank over a 10-year period. The review was performed using data collected from the bone bank and correlating it with the patient’s medical record. All complications, including infections, related to the use of the allograft were recorded.

Results:  We found that over the 10 years to 2004 that 811 femoral heads were donated, with 555 being used over 362 procedures in 316 patients. We identified a total of nine deep infections, of which seven were in joint replacements. Overall this was a 2.5% deep infection rate, which was lowered to 1.4% if the previously infected joints that were operated on were excluded.

Conclusion:  The use of non-irradiated femoral head bone graft was safe in a regional setting.

Titanium alloy (Ti-6Al-4V) has a wide range of application in various fields of engineering. Titanium is mainly used to manufacture aerospace components like landing gear, fuselage, wings, engines etc. and biomedical components like hip joint, knee joint, dental implants etc. Titanium has outstanding material properties such as corrosion resistance, fatigue strength, tensile strength and a very good biocompatibility which makes this material very alluring for biomedical applications. Contrary, the machinability of the material is problematic because of the phase transformations and thus, titanium alloy is a challenge for machining operation. This research is a comparative analysis between the implants manufactured by traditional method of casting and machining. The femoral stem of the hip joint replacement is designed and the component is machined using a five-axis CNC machine.The machined component was subjected to surface roughness testing, tensile testing and bulk hardness testing. Avaliou-se a progressão dos sinais radiográficos de cães com displasia coxofemoral ou potencialmente displásicos tratados pela técnica da sinfisiodese púbica. Foram utilizados 14 cães, seis machos e oito fêmeas, com idades entre quatro e seis meses. Após exame físico, procedeu-se ao exame radiográfico para avaliação da articulação coxofemoral, segundo padrão estabelecido pelo Colégio Brasileiro de Radiologia Veterinária. Três deles foram também avaliados pelo método PennHIP. Pelo histórico clínico, nove (64%) animais foram trazidos por manifestarem sinais de dificuldade locomotora com os membros pélvicos, manifestada especialmente por claudicação unilateral ou bilateral e dificuldade de se levantar. Cinco (36%) cães não tinham qualquer sinal clínico, e em quatro (29%) realizou-se o exame radiográfico como prevenção. O sinal físico mais evidente foi dor à palpação da articulação coxofemoral, e dois (14%) cães apresentaram sinal de Ortolani positivo. Com exceção de um cão que apresentou infecção nos pontos cutâneos, todos os demais evoluíram sem complicações no período pós-operatório. Radiograficamente houve progressão das lesões articulares em seis (43%) animais e oito (57%) mantiveram a classificação inicial. Concluiu-se que cães tratados pela sinfisiodese púbica não apresentam melhora dos sinais radiográficos iniciais.

Background: Canine hip dysplasia (HD) is characterized by hip joint laxity and subluxation. It is the most common cause of osteoarthritis in dogs, especially in larger breeds. Its management includes nutritional supplements, nonsteroidal anti-inflammatory drugs, physical therapy, acupuncture or surgical procedures. Implantation of gold beads in acupuncture points and trigger points around a joint has been used in the treatment of osteoarthritis in dogs for at least 30 years. Gold bead implants(GBI) acts as continuous acupuncture stimulation and trigger point treatment in canine HD with long lasting results. Electrophysiological investigations of trigger points reveal dysfunctional muscle spindles which indicate that the electrical activity of active loci arises from extrafusal motor endplates.Case: This is a report on the use of acupuncture and GBI for bilateral HD in a nine year old female German Shepherd. The patient has a HD non-responsive to anti-inflammatory drugs and was unable to stand up or walk by its own. Radiographs showed marked dysplasia, significant subluxation with the femoral head partly out of a shallow acetabulum and massive secondary arthritic bone changes, mainly on the right side. The animal was submitted to eight acupuncture sessions with seven days interval. After the first acupuncture session the use of NSAID was interrupted. After eight weeks the dog was considered rehabilitated and underwent GBI in acupoints and trigger points as maintenance treatment. During the one-year follow-up period the improvement remained unchanged with no need of analgesics.Discussion: It has been suggested that acupuncture or GBI can treat the chronic pain resulting from osteoarthritis induced by HD. According to AP theory, GBI is permanent and long-lasting acupoint stimulation. Moreover, the method is inexpensive, quick and easy to perform, with no postoperative pain or need of exercise restriction. Although gold is extremely corrosion-resistant, the surface of the gold implants stimulates a reaction from the immune system causing an oxidative liberation of gold ions with anti-inflammatory actions. It is well known that gold ions are effective inhibitors of the respiratory burst of neutrophils and monocytes and the proliferation of lymphocytes. These findings suggest that gold implantation, on a local scale, mimics the anti-inflammatory and pain-relieving effect of drugs with chemically bound gold ions. The relatively slow speed of the process results in a limited liberation of gold ions securing that they are taken up almost exclusively by cells close to the implant. The nine year old female German shepherd had a positive response to acupuncture with pain relieve and locomotor rehabilitation. For the nine year old female German shepherd previous acupuncture sessions to GBI resulted in no post-implant worsening period. Indeed, the association acupuncture/GBI does not have the anti-inflammatory drugs undesirable effects and brings long lasting results. In conclusion, GBI therefore should be considered for canine HD when conservative or medical treatments fail to give the desired effect.

It was evaluated movements of lower limb in the double pulley system equipment on ten male volunteers during contraction of gastrocnemius (caput laterale ) and gluteus maximus muscles in the following movements: 1) hip extension with extended knee and erect trunk, 2) hip extension with flexed knee and erect trunk, 3) hip extension with flexed knee and erect trunk, 3) hip extension with extended knee and inclined trunk, 5) hip abduction along the midline, 7) hip abduction with extension beyond the midline, 8) adduction with hip flexion beyond the midline, 8) adduction with hip flexion beyond the midline, and 9) adduction with hip extension beyond the midline. Myoelectric signals were taken up by Lec Tec surface electrodes connected to a 6-channel Lynx electromyographic signal amplifier coupled with a computer equipped with a model CAD 10/26 analogue digital conversion board and with a specific software for signal recording and analysis. We observed weak gastrocnemius muscle activity for all movements studied. In the case of gluteus maximus, the most important potentials were observed for movement 2, while for the remaining movements the actions were of reasonable intensity. Compared to gluteus, gastrocnemius was less required for all movements.

STUDY DESIGN. Observational cohort study. OBJECTIVE. To investigate spinal coordination during preferred and fast speed walking in pain-free subjects with and without a history of recurrent low back pain (LBP). SUMMARY OF BACKGROUND DATA. Dynamic motion of the spine during walking is compromised in the presence of back pain (LBP), but its analysis often presents some challenges. The coexistence of significant symptoms may change gait because of pain or adaptation of the musculoskeletal structures or both. A history of LBP without the overlay of a current symptomatic episode allows a better model in which to explore the impact on spinal coordination during walking. METHODS. Spinal and lower limb segmental motions were tracked using electromagnetic sensors. Analyses were conducted to explore the synchrony and spatial coordination of the segments and to compare the control and subjects with LBP. RESULTS. We found no apparent differences between the groups for either overall amplitude of motion or most indicators of coordination in the lumbar region; however, there were significant postural differences in the mid-stance phase and other indicators of less phase locking in controls compared with subjects with LBP. The lower thoracic spinal segment was more affected by the history of back pain than the lumbar segment. CONCLUSION. Although small, there were indicators that alterations in spinal movement and coordination in subjects with recurrent LBP were due to adaptive changes rather than the presence of pain. © 2013, Lippincott Williams & Wilkins.

The stride before landing may be important during stepping down. The aim of this study was to analyze variability of the kinematics and muscle activity in the final stride before stepping down a curb, with and without ankle and knee muscle fatigue. Ten young participants walked at self-selected speed and stepped down a height difference (10-cm) in ongoing gait. Five trials were performed before and after a muscle fatigue protocol (one day: ankle muscle fatigue, another day: knee muscle fatigue). The analysis focused on the trailing leg during the last but one and the last step on the higher level. Kinematics and muscle activity were recorded. Fatigue increased variability of foot-step horizontal distance in the last step on the higher level of the trailing limb, as well as in the first steps on the lower level for both limbs. This appeared due to an increase in the range of motion of the knee joint after both fatigue protocols. Participants additionally showed an increased ankle and hip ROM and decreased knee ROM. Our results suggest a loss of control under fatigue reflected in a higher variability of trailing and leading limb-step horizontal distances, with compensatory changes to limit fatigue effects, such as a redistribution of movement over joints. © 2012 Elsevier B.V.
